Each time I write this letter it’s a memorable experience, as it signifies the progress in fulfilling our vision of “Improving the Human Condition.” There is much to celebrate about 2012 as it was another banner year for Sanford Health. I want to express our great appreciation to you—our donors—for your steadfast support and genuine desire to make a difference. The special features shared in this report are hallmarks of your generous spirit. I also want to recognize the philanthropic leadership provided by our lead benefactor, Denny Sanford. His gifts to us now total $600 million and forever distinguish him in the world of health care philanthropy as his magnificent generosity has transformed our organization, and in fact, given added leveraging impact to what he has so graciously given to the mission. This past year, we added the Bismarck region to the Sanford family with the merger of Medcenter One, a historic integrated health system. This enables us to serve the unique and increasing health care needs in western North Dakota. In July, we began the process for a new medical center campus in Fargo as we also plan for continued services downtown in conjunction with the Roger Maris Cancer Center. Across our system, we celebrated many noteworthy accomplishments. A new hospital was completed in Aberdeen and a new Sanford Heart Hospital opened in Sioux Falls. Additional leading physician groups and community health care organizations joined Sanford Health, enhancing our unified system approach to serving regional needs. Groundbreakings were held for a new hospital and clinic in Thief River Falls, Minn., a new clinic facility in Moorhead, Minn., a hospital expansion in Canton, S.D., and a new orthopedic/sports medicine and cardiovascular center in Bemidji, Minn. Through the Sanford Initiatives, there were significant advancements in development and research, including new milestones involving our Sanford Project to cure type 1 diabetes and Edith Sanford Breast Cancer. Our World Clinics program saw four clinics become operational in Ghana and a new location in Klamath Falls, Ore., opened while planning continues in Mexico, Israel and beyond. In all our communities, we focus on quality of life through health and wellness activities, especially those oriented around youth and family. Our Sanford PROMISE connects students and educators with research to inspire future interest in health and science, and a new Sanford Sports Complex is under way in Sioux Falls. This year will be another exciting one for Sanford Health as we remain, “Dedicated to the Work of Health and Healing,” and provide what we believe is a model for health care delivery, research and education in our nation. Your commitments to the Foundation for campaigns, special projects, endowments and legacy giving elevate us amongst other health care systems in America. On page four you’ll find an outline of how your gifts were used, and below are additional highlights from 2012: • Savor the Flavor raised more than $97,000 to benefit the Sanford Children’s Pediatric Rehabilitation Department in Bismarck. Funds will be directed toward expansion of the TheraTree, an interactive sculpture that improves a child’s level of independence. • The Meddy Bear Golf Classic, held in June, raised nearly $45,000 for the Dakota Children’s Advocacy Center. • Thanks to the generous support of our donors, the Sanford Health Foundation in Bismarck was able to distribute $395,385 to help improve the human condition throughout the Sanford Bismarck region. With the merger of these health systems, the operations and processes of each region’s foundations blended. What we’ve discovered is that the generosity of our donors knows no boundaries, literally! A growing number of individuals, corporations and private foundations are now giving in multiple Sanford regions. In an effort to provide consistency and maintain efficiency, beginning with all gifts made in 2013, we will implement changes to our recognition program to standardize the giving levels within the Bismarck Foundation. Making an impact on children’s lives Ranchers Aaron and Sheyna Strommen have learned to enjoy the little things in life. The couple and their three children make a living raising cattle and horses on a ranch 37 miles southwest of Mandan, N.D. “Aaron and I feel very blessed to be raising our children in the wide open spaces of rural North Dakota – where there’s room to explore, build forts and experience the beauty of nature,” Sheyna Strommen said. The couple was elated when their third child, Clint, made his debut in June 2011. The Strommens and their two other children, 6-year-old Cassidy and 5-year-old Cooper, waited patiently for Clint to come home from the hospital, but unexpected health complications and an unknown diagnosis kept him under constant medical watch until he was 4 weeks old. Minutes after birth, Sanford nurses noticed that Clint made an uncommon “gurgling” sound and had difficulty breathing. The nurses whisked him away to the Neonatal Intensive Care Unit where he stayed for the next week under the care of Sanford Neonatologist Rafael Ocejo, MD, and Pediatrician Michelle Seibel, MD. “Six days after Clint was born, Dr. Ocejo told us he thought Clint had a rare muscle disorder, a form of muscular dystrophy, and that we needed to go to another hospital for additional testing,” Sheyna said. Three months later, specialists confirmed Dr. Ocejo’s diagnosis for Clint: merosin-deficient congenital muscular dystrophy (CMD).
Although little things like waving his arms are more difficult for Clint than with other kids, joyous moments are intertwined with his ongoing battle with CMD. “We were overjoyed the first time we saw our toddler operate a powered wheelchair or when a swallow study proved to everyone that Clint was perfectly capable of chewing and swallowing food,” Sheyna said. For nearly two years, Clint has met with physical, occupational and speech therapists and continues to see Dr. Ocejo at Sanford Bismarck to improve his health, mobility and development. As a toddler, Clint can now feed himself, sit independently and talk continuously – accomplishments the Strommens credit to the physicians, nurses and specialists at Sanford Health. “The doctors had the perceptiveness to pick up on Clint’s signs of distress, understand the complexities of his condition and confidence to refer us to specialists,” Aaron said. Donors to Sanford Children’s help provide state-of-the-art equipment used by specially-trained medical professionals so they can identify health problems immediately, as in Clint’s case, and address them right away.

Making an impact on cancer patients Sue Sletto and her husband, Terry, were on the threshold of retirement in the summer of 2012. She just finished a career in education in South Dakota and they had just relocated to Mandan, N.D. “We were ready to move into our retirement home,” Sletto said. “We were ready to travel and take care of our grandkids.” They spent hours each day, from sunrise to sunset, working to get their new home organized. “One morning, I had trouble swallowing,” Sletto said. “I touched my hand to my throat and my thumb hit a large lump.” Sue Sletto
An ultrasound showed something serious, she was diagnosed with Hodgkin’s lymphoma by Sanford Bismarck surgeon, Douglas Berglund, MD, who also did a biopsy of a few smaller lymph nodes. “I have never really had any serious illness, so for me to get hit like this was strange,” she said. The medical team at Sanford Medical Center Bismarck stepped right in to start treatments. Sletto immediately met Oncologist Vijay Chaudhary, MD, to create a healing plan. Sletto said she was never fearful because all her questions were answered clearly and succinctly. “I am so thankful for efficiency of the Sanford system for reducing worries that many patients face,” Sletto said. “From the beginning, referrals were scheduled for the next specialist that was needed. The anxiety of scheduling and coordinating my own appointments for labs, doctors, tests and infusion center was relieved through management from the oncology department. It worked like clockwork.” Sletto said there was always overwhelming skill, compassion and concern for her. She completed 12 chemotherapy treatments with four drugs. Even when the treatments were suspended twice due to low white blood cell count and a severe drug side effect, Sletto said she felt confident in her doctors and the staff. “The staff at the infusion center was simply fabulous,” she said. “They are such a family and treat everyone with consideration, respect and dignity. They anticipated my needs. They became my friends.” Donors to Sanford Cancer help provide the latest equipment, specialized programming and advanced education for staff so they can offer the very best care to patients like Sletto. As of March, Sletto is in remission. She hopes to remain cancer free because she has lots of other plans, like gardening, spending time with family, taking piano lessons and teaching children to read.

the screening results were alarming. People with no heart disease have screening numbers of 100 or less. Dave’s number scored at 3,053. “Coronary artery disease is much more prevalent than we used to think, and even patients with no symptoms are at risk for significant narrowing of their arteries,” Dr. Iwamoto said. “That’s why these CT Cardiac Calcium Scoring Screenings are so important to show whether a patient has heart disease or is at risk for developing it.” Dr. Iwamoto asked Schmidt what kind of symptoms he was experiencing. “I told him none at all,” he said. “I’d just golfed 18 holes that day and walked the dog at least a mile.”
Dave and Darlene Schmidt
Dave Schmidt felt good and fortunate at the age of 70. So when he found himself hospitalized at Sanford Medical Center Bismarck for a major heart surgery, he was shocked that his life had taken such a turn. This episode all started in September, after attending a Sanford Health Foundation Premiere Physician Meet and Greet event in Watford City. Schmidt had received a gift certificate for a free CT cardiac calcium scoring screening, a new service offered by Sanford Health. A few weeks later, the couple from Alexander traveled to Bismarck to visit their daughter, Kristi Schmidt, who works for the Foundation as the Director of Development. She reminded her father about the gift certificate and encouraged him to schedule the screening. “So I did it and it didn’t take more than 15 minutes. No pain. No needles,” he said. “There was nothing to it.” However, Sanford Health Radiologist Matthew Iwamoto, MD, called Schmidt at home the following day to report
At Dr. Iwamoto’s urging, Schmidt agreed to return to Bismarck for an angiogram. Sanford Cardiologist Stephen Korte, MD, found his arteries were severely blocked. The following day, Cardiovascular Surgeon Sean Russell, MD, performed quadruple bypass heart surgery. Today, Schmidt is back in his normal routine of golfing, walking his dog and spending time with Darlene, his wife, of 50 years. “I got another chance at life,” he said.

Making an impact in orthopedics Brian Jorgenson started riding his bike to work with a coworker 22 years ago as a way to stay in shape. He continued to cover between 15 and 20 miles a day even after getting married and having two daughters. “It was a good way to get out with them,” he said. “I would hook up a child cart behind my bike and get up to four kids in it and go for rides. The kids loved it and it gave me a great work out.” Jorgenson often times bikes to the YMCA to exercise, just like he did on Oct. 2, 2012. That night he rode on the sidewalk and, as he approached a street intersection and turned his head left to check for any traffic, he lost control. In a blur, Jorgenson spotted the yellow handicap sidewalk section and gripped his brakes to avoid the curb, causing him to fall onto his right side. “I tried to break my fall with my arms, sliding on the sidewalk on my right side and the right side of my face,” Jorgenson said. Witnesses stopped and one of them drove him home where his wife, Paulette, took him to a Sanford Walk-In Clinic. The doctor thought he had shattered the bones surrounding his right eye and ordered him to go to the Sanford Emergency and Trauma Center. X-rays showed no broken bones in Jorgenson’s face, but he did have a broken left wrist in three places and broken right elbow and wrist. The right side of his face had several lacerations, including one on his right eye lid that swelled it shut. Jorgenson said the medical staff at both the clinic and the emergency center answered all his questions, described his injuries in detail and clearly explained his healing plan throughout his treatment. He especially appreciated Orthopedic Surgeon Laura Gehrig, MD. “Her goal was to get the injuries to heal by themselves so surgery wouldn’t be needed,” Jorgenson said. “I didn’t want surgery either, so our goals were the same, which was important to me.” Nonetheless, the healing process took time, patience and continuous family support in the months that followed. Jorgenson wore a cast on his left wrist for four weeks to help align the three breaks, followed by a splint for a total of three months. A brace from his right shoulder to hand locked his right arm for three months to heal a broken elbow and swollen wrist. “I never thought I would get hurt like I did that night from riding bike,” he said, adding he was wearing a helmet. “I was always careful when riding. I’ll just be a little more careful now.”

In October, the Sanford Health Foundation kicked off the Builders of Excellence Endowment Campaign, a $20 million matching gifts campaign designed to double donors’ investments while building a base of permanent support for programs throughout Sanford Bismarck.
“It will allow for continued growth of the Foundation while partnering with the people of the region as they contribute philanthropically to impact the lives of their neighbors, friends and family,” said Brian Mortenson, President of the Sanford Health Foundation.
A gift of $10,000 or more during the Builders of Excellence Endowment Campaign allows donors the opportunity to establish their own named endowment fund to support the area of health care that is nearest and dearest to them and their gifts qualify them for the dollar-for-dollar match from Sanford. The annual income generated perpetually from endowed funds will transform the future of health care in central and western North Dakota to unprecedented levels by providing resources to advance medical science and quality of care.
An endowment is a unique way to show a commitment to a specific cause while fulfilling a donor’s philanthropic spirit and leadership role in the community. Endowed gifts are especially beneficial in North Dakota after the state legislature approved charitable income tax credit in 2011 for taxpayers who make gifts to charitable endowments in the state.
“This is a historic time at the Foundation,” said Tom Petrik, Sanford Health Foundation Vice President of Development. “We’ve never had the opportunity before to maximize our donations with matching dollars.” Physicians and other donors have already taken advantage of the matching opportunity, pledging more than $2.8 million in new endowment funds.
“With the addition of the 40 percent North Dakota State Tax Credit, it’s a win-win situation for our donors and Sanford Health in Bismarck,” said Kristi Schmidt, Director of Development.

we’ve met so many families who can’t afford basic services because of their insurance, so we wanted to create an endowment fund to help those families.”
“We have a 10-year-old with autism who has benefited from speech and occupational therapy services, and we really believe in therapists and the work they do,” Carolyn Fogarty said.
They also believe mental health services and medical equipment assistance have not received sufficient funding in the past. Both Ted and Carolyn have family members who struggled with mental health issues and are motivated to support these programs at Sanford.
That’s why the Fogartys pledged $250,000 to the Sanford Health Foundation’s Builders of Excellence Endowment Campaign. This $20 million fundraising effort allows donors to create lasting legacies by establishing a named endowment with a minimum pledge of $10,000, which will be matched by Sanford. The Fogartys created four separate family-named endowments to support pediatric rehabilitation, patient assistance, mental health and purchase of medical equipment. “We want to help with pediatric rehabilitation in any way we can,” Fogarty said. “The patient assistance fund is also near and dear to our hearts because

The Fogartys believe the Builders of Excellence Endowment Campaign is an ideal way to maximize the impact of their heartfelt gift. “It is the biggest philanthropic bang for your buck you can find in the country now,” Dr. Fogarty said.

Making an impact on nursing students When Sanford College of Nursing senior BreAnn Schaffner was flying to a wedding in Wyoming, a flight attendant asked if anyone on the plane had a medical background. Schaffner bravely stepped forward, clarifying that she was not yet a registered nurse. The flight attendant welcomed her help. The passenger, clearly in distress, couldn’t answer Schaffner’s questions, but was able to relay he has an aspirin allergy. “That really limited our resources because aspirin or nitro tabs are recommended for chest pain,” Schaffner said. “I started mentally going through what I’d learned at the chest pain patient simulation I had done at the college, and remembered that oxygen can be used, too.”

So the flight attendant got an oxygen canister and put it on him. By the time they landed, the patient felt better due to the oxygen treatment. “I don’t think he’d have gotten oxygen if I hadn’t been there,” Schaffner said. A life was saved because Schaffner had been trained for this type of scenario during a recent cardiac assessment and treatment session utilizing the college’s patient simulator, which exists because of generous donors. Endowment funds ensure financial support is available forever to help improve and expand programs and educational services, such as the patient simulators.

It’s unique because professionals from social services, law enforcement, prosecution, victim advocacy and medical and mental health work together so a child only has to tell his or her story one time. “So it is a one-stop shop,” Condol said. “The children can come here and get all their needs met without having to tell their story over and over again and without having to meet several different people.” These same professionals met during therapy sessions with “Emily” and her family to learn how to cope with stress and anxiety. The multidisciplinary team supported the family and helped them through the system, which ultimately empowered them to tell their story in court. “Children come here to be taken care of and to start their road to healing,” Condol said. Staff at the Dakota Children’s Advocacy Center provides multidisciplinary services to assist victims of child and sexual abuse.
Making an impact on the Dakota Children’s Advocacy Center When 8-year-old “Emily” heard the staff from the Sanford Health Dakota Children’s Advocacy Center (DCAC) talk about safe and unsafe touches at her school, she realized what was happening to her wasn’t OK. “Emily” stepped forward and participated in an interview at the DCAC where she revealed she was being sexually abused. The DCAC team took immediate action to stop the abuse and address the consequences. The Sanford Health Dakota Children’s Advocacy Center is a community partnership program committed to improving the response to child abuse. It utilizes a comprehensive multidisciplinary team approach to investigate child abuse cases, which usually involve sexual abuse. “Our mission is to bring hope, help and healing to children who have been abused, so we want to make sure we provide wrap-around services,” said Paula Condol, Sanford Health DCAC Coordinator. 20 Donor Impact Report
The DCAC started as a task force to improve and create a coordinated response to child sexual abuse investigations in 1997, and there’s been tremendous growth since then. Statistics show one in four girls and one in six boys will be sexually abused before the age of 18. The program serves all counties in North Dakota, including the state’s four Native American reservations, and areas in northern South Dakota. The Dakota Children’s Advocacy Center served 372 children in 2012 and more than 1,700 children in the last five years. The program provides a child-friendly atmosphere, an unbiased third party to investigate, expert court testimony and specialized medical practitioners – all without cost to the family. The DCAC is fully funded through grants, donations and third party payers. Donors to the Sanford Health Dakota Children’s Advocacy Center dissolve the financial pressures in an already complicated scenario when there’s child sexual abuse in a household.

Jacobson said her menstrual cycles were always regular and predictable, but they were becoming more sporadic and frequent in the last two to three years. It interrupted her life. “I would think my period was done and then a couple days later it would start again,” she said. “I had to be prepared whenever and wherever I was. This was especially difficult when we would go someplace.” Jacobson said she felt comfortable choosing the surgery because of the da Vinci® Surgical System available at Sanford Bismarck. “I was excited about the use of the da Vinci® equipment for my surgery,” she said. “I had been reading about it on the Internet and was impressed with its capabilities. I also knew that Dr. Klemin had been trained on it and I trusted what he could do.” The da Vinci® Surgical System allows surgeons to perform complex procedures, such as a hysterectomy, in a minimally invasive fashion using tiny incisions instead of a larger one. Although it is a robot, surgeons control every movement of its enhanced features, including highdefinition 3-D vision, a magnified view via a small camera inserted into the body and complex movements using instruments a human hand or wrist cannot do. Patients benefit with significantly less pain, blood loss and scarring and a shorter recovery time. They often stay in the hospital for 24 hours and return to work in two weeks. Jacobson went home one day after the da Vinci® surgery in October 2012 and returned to work part time four weeks after her complex surgery, concluding a successful experience at Sanford Bismarck.

Making an impact on Women’s Late last year, Dr. Peter Klemin, Obstetrician and Gynecologist, presented two options to Sara Jacobson regarding the health of her uterus. She could wait two to five years to reach menopause or choose hysterectomy surgery. Tests revealed the lining in Jacobson’s uterus was thickening and causing excessive bleeding. It was also abnormally large and she had severe pelvic scar tissue. “I knew that I could not and would not wait for option one,” she said. “I jumped at the chance for the hysterectomy.”
“Someone was always with me during the preparation for my surgery, keeping me from getting too nervous,” Jacobson said. “Any time I needed anything, a nurse was there at a moment’s notice, and Dr. Klemin is absolutely wonderful! He always answered all my questions, giving me all the pertinent information, but giving me hope that I could and would be fixed.” Today, Jacobson said she feels like a different person. “I am not worrying about the things I was last year at this time,” she said. Making an impact on emergency and trauma One night in July 2011, Shawn Gieszler lay down in his bed and suddenly got an upset stomach. He stood up and the discomfort subsided only to return as soon as he went back to bed. But after a while, he also started sweating heavily and developing chest pain. Gieszler, 47 years old at the time, knew something was wrong. His wife, Jan, drove him to the Sanford Emergency and Trauma Center. “The faster you come in and get it taken care of, the less permanent damage there is to the heart,” said Richard Howard, MD, a Sanford Health cardiologist. Gieszler slipped into cardiac arrest when he arrived in the Sanford Emergency and Trauma Center. The medical staff shocked his heart back to life, stabilized him and activated the emergency heart team that’s on-call at all hours. In the cardiac catheterization lab, the Sanford Health team gently threaded a catheter – a long, flexible tube tipped with a balloon – through the blocked blood vessel that caused the heart attack. The balloon inflated inside the vessel, opening it up and allowing blood to flow. Gieszler said he doesn’t recall much of the experience. “I remember them getting me into one of the rooms, and I kept telling them, ‘You’ve gotta help me,’” he said. Dr. Howard said the emergency heart team must work together in order to save lives. “A phone call comes in and within 20 minutes the cath lab is open,” Dr. Howard said. “It’s a well-orchestrated event. 28 Donor Impact Report
It’s almost like a ballet. It’s all about how quickly you can get the patient in.” Gifts to emergency and trauma funds through the Sanford Health Foundation provide support for advanced training and certification for medical staff and ensure the very latest equipment and technology is available in this region. Donor gifts also help expand programs and services, such as Cardiac Rehabilitation, which Gieszler participated in following his procedure and recovery and where he learned about exercises and other lifestyle adjustments he could make to strengthen his heart and mitigate his risk of future heart disease. He started a regimen of cholesterollowering medications and daily aspirin under Dr. Howard’s guidance and also avoids fried and fast food. “He had a heart attack that could have killed him,” Dr. Howard said. “But by getting treated quickly and taking the right medications after, he has normal heart function.” Gieszler said he’s grateful for this second chance in life, feeling better than before his heart attack. “I’m just thankful they were there and could provide that service so quickly,” he said. “It’s out of your hands when you come in. To have people trained and knowledgeable, I owe them everything.”

Making an impact on hospice Don Matejcek and his wife, Kathleen, were married more than 50 years. Pictures on a bulletin board in their home show them smiling on their 50th wedding anniversary, during two weddings of their grandsons and at other family gatherings. Life changed once Kathleen was diagnosed with cancer. “She’d gone through chemotherapy, radiation, surgeries, and then when the cancer reappeared, she just didn’t want to do it anymore,” Matejcek said. It made sense to him that she would be cared for in their home, which eased their stress and her pain. “Without hospice, I don’t know how we would’ve done it,” Matejcek said. Don Matejcek
Sanford Hospice staff visited every morning and helped Kathleen by bathing her, dressing her and even fixing her hair. These services continued for 14 months until she passed away. “What a wonderful program,” Matejcek said. “These people that serve hospice, they’re not humans, they’re angels.” Sanford Hospice enables patients in Bismarck and the five-county region to live at home, pain-free, surrounded by the people they love. In 2012, more than 140 patients and families benefitted from the services of Sanford Hospice in the Bismarck-Mandan region. Donors to Sanford Hospice help ensure medical services, emotional support and spiritual resources are available for patients and their families who are in the last stages of a serious illness.

A Final Farewell It is with the very deepest regret and sorrow that I have retired from the chairmanship of the Sanford Health (Medcenter One) Foundation. I have served on the Foundation board for 22 years, 16 of those on the Corporate Board and eight years as a board member of CAPCARE, the first health maintenance organization in North Dakota. Thirty years is a long time to work as a volunteer in health care and I have enjoyed every minute of it. Each of you as donors to the Foundation have been the greatest joy of my career. Your dedication and support of the Foundation has been tremendous and much appreciated. Thank you for all you do to support the excellent care at Sanford Health.
