DISSECTING ANESTHESIA COSMETIC SURGERIES AND SEDATION PROCEDURES According to the American Society of Anesthesiologists, more than 21 million patients receive general anesthesia in any given year. What’s more, is the fear or jitters that a majority experience before going in for simple cosmetic procedures that call for varying degrees of sedation. Whether it’s Botox, microneedling, scalp treatments, or liposuction, there are different types of anesthesia that are used to ensure the treatment fits the procedure. Dr. Kimberly Cockerham with Central Valley Eye Medical Group has been treating patients for more than 20 years and previously worked as a surgeon in the U.S. Army. She’s also the founder of the nonprofit Let’s Face It Together (LFIT) that provides medical care and surgical reconstruction for facial disfigurement and dysfunction for those in need. All donations to LFIT cover medical costs, supplies, prosthetics, and more for those who face challenges such as autoimmune disease, spousal abuse, accidents, and other tragedies. Dr. Cockerham shines light on the various forms of anesthesia and what procedures each is used for, and she also provides context to the day-of-surgery jitters to help all patients rest a little easier when it’s time to pay her a visit.
Lidocaine
Lidocaine is applied topically or administered through a needle injection to an area that needs attention. Procedures like Botox injection to tighten the skin and smooth wrinkles, and microneedling, which helps with everything from hyperpigmentation relief and scars to stretch marks and also cellulite, are two examples of topical lidocaine use.

“Lidocaine and similar longer acting local anesthetics like Marcaine are used as injections to numb the area of [treatment]. They can be used as eye drops, topical creams or gels or infused into body spaces during liposuction,” Dr. Cockerham says. “These do not alter the patients’ ability to think or communicate—they are used just locally in the region of the treatment.” If the patient wants to undergo an eye lift, skin or lesion removal, or scalp treatment, then an injection of lidocaine will numb the area, and the doctor will proceed.
Minimal Sedation
During minor facial surgeries, minimal sedation such as an injection of lidocaine paired with an oral sedative like valium is one combination that’s used for procedures like facial fillers, lip augmentation, and collagen stimulation. “If a patient wants minimal sedation in addition to the local anesthetic oral valium can be used to calm the patient and make them drowsy,” Dr. Cockerham says. “Extra strength Tylenol can also be used prior to the procedure and after the procedure to minimize discomfort.”
IV Sedation
With IV sedation, a board-certified anesthesiologist or nurse anesthetist oversees the sedation while the plastic surgeon focuses on a number of different cosmetic surgeries including liposuction. “A board-certified anesthesiologist starts an IV prior to the procedure and then delivers a variety of drugs through the IV to make the patient comfortable and also provide retrograde amnesia (i.e., doesn’t remember the procedure) during the injections with lidocaine and for the duration of the procedure,” Dr. Cockerham says.
General Anesthesia
According to Dr. Cockerham general anesthesia is used for patients who are sicker and for longer and major procedures like body lifts. “General anesthesia is performed in cases where the patient needs to be very deeply sedated and not move,” Dr. Cockerham says. “A breathing tube is placed to help the patient breath, and a muscle relaxant is used to keep the patient relaxed and still during the procedure.”

DENTAL IMPLANTS ARE SOMETHING TO SMILE ABOUT We all love a perfect smile. But, it can be very upsetting when you have missing or damaged teeth. Studies show there is an emotional impact with tooth loss. People are more likely to feel self-conscious, less willing to carry out everyday activities, and unhappy with the inevitable change in facial shape. Certainly, dentures offer a reasonable replacement for tooth loss. But there is a downside—slipping while talking or getting food caught underneath. That’s why many people choose dental implants as an alternative. “In the event of lost teeth, it is a fixed option,” says Dr. Caressa Louie, who practices cosmetic and reconstructive dentistry in Stockton, “They help you chew and replace the missing function.” A dental implant is a prosthesis and acts like a tooth’s root. Surgically inserted into the jawbone, the titanium post is attached to an abutment connecting it to the crown. Implants, closely resembling a screw, fuse to the jawbone through a process known as osseointegration. Implants can’t come loose like dentures and they don’t attach to other teeth like bridges.
SO, WHAT ARE THE PROS AND CONS?
The upside is that implants look real and function like actual teeth. Depending on your needs, you can get single implants or multiples as part of a greater tooth restoration plan.

Implants can get a little pricey, however. On average, you can expect a $3,000 to $5,000 price tag. Because they are permanent, your long-term dental care costs will be lower. Yet, some individuals may require a bone graft for the procedure to work properly. “Nobody likes to be told they can’t have implants but sometimes there are factors that are out of our control,” says Dr. Todd Franklin, a family and cosmetic dentist in Lodi. “Many times, patients have lost a tooth and many years have gone by and the bone levels are not where they need to be to place the implants.” Implants may take several weeks to heal and can cause some discomfort. The whole process from beginning to end takes about six months but can last up to a year.
WHAT MAKES A PERSON NOT A GOOD CANDIDATE FOR IMPLANTS?
Patients with certain health issues may be considered unsuitable for the procedure. As Dr.
Franklin explains, there is a “systemic link” between oral health and overall medical health. “Any patients who are medically compromised often have issues with their oral health,” he says. “Some may be able to overcome some acute problems. But if a patient suffers from a chronic illness, such as uncontrolled diabetes, they would not be a good candidate.” There is also the possibility of infection. It can happen shortly after the procedure or even some time later if your immune system is compromised by other health problems. “If you’re a smoker,” says Dr. Louie, “that is a horrible systemic condition in which you can lose teeth and bone to begin with.”
WHAT ABOUT IMPLANT CARE?
Dr. Louie also points out that the component can fail if the screw breaks, for example. There are a number of reasons behind tooth loss including age, congenital conditions, or even sports injuries. “Teeth are like God-made implants,” says Dr. Louie. “Your next best set is a man-made one.” The market for dental implants will continue to expand. According to a recent press release from the Global Dental Implants Market, the industry has been growing at a steady pace and is expected to continue with more than 200 million patients worldwide by 2030. But while implants may be a sturdier long-term option, Dr. Louie maintains that you still have to take care of your teeth and get regular check-ups. Dr. Franklin heartily agrees. “One may think that once an implant is placed that it can be ignored. Routine dental care is always a must for anyone who has implants,” he says. “A patient can still lose an implant if they start to have gum disease. Also, it is important for a trained dental professional to check the implants on a regular basis to make sure all aspects of the implant are in tip top shape.”

CAN YOU HEAR ME NOW? Nearly 15 percent of American adults (37.5 million) age 18 and over declare trouble hearing, and almost 29 million U.S. adults will benefit from hearing aids. Hearing loss is not an uncommon problem, and we are affected whether it is someone we know—or ourselves—who could benefit from a hearing evaluation conducted by a professional. Some common complaints include hearing but not understanding what is being said, increased difficulty following a conversation surrounded by outside noise, family or friends pointing out that the TV is too loud, or feeling the need to frequently ask, “What?” “A thorough hearing evaluation is the only way to verify a hearing loss, and the extent of the loss,” informs state licensed audiologist and hearing aid dispenser Lorie Frazee who owns Hearing Services Center in Lodi. “The evaluations are performed in an OSHA certified soundproof booth with the patient wearing headphones through much of the test.” Once the straightforward evaluation is complete a hearing specialist goes over the results with the patient. “Each individual has different communication demands and needs. Everyone has different tolerances and preferences for sound, different expectations,” explains Lorie. “My approach is to educate when needed, set realistic expectations, all while addressing the individual goals of the person sitting across from me.” A specialist identifies the person’s listening lifestyle, which is fundamental in guiding them to the technology that will best suit the acoustic environments where they live and work. “I compare hearing aid technology to a cell phone or computer technology. There are different levels of complexity and function. There are differ-

ent sizes and styles that house the technology. These options are determined primarily by the hearing evaluation results, then the listening lifestyle, and personal preference. As with most electronics, hearing aids are much smaller than they were a decade ago, so most styles are aesthetically pleasing,” informs Lorie. The fit of a hearing aid needs to be comfortable and appealing to the wearer regardless of style. With new software programs, fitting a hearing aid is simple with a trained audiologist. With the appropriate amplification set along with specific hear-
ing aid software, the patient will hear better. “Most hearing aids will automatically adjust to emphasize speech. The sensitivity of a hearing aid is dependent on the technology and how the audiologist programs it. Some have no manual adjustments, it depends on the individual needs of the person being fit. Many hearing aids now have connectivity to cell phones and other devices,” explains Lorie. If you are overweight, have flat feet or high arches, you can also be susceptible to the condition. “It can occur in anyone, but mostly it’s people who are active or on their feet all day,” says Dr. Alexander Curfman, of Lodi Podiatry Group. “Whether it’s runners or weekend warriors, truck drivers or factory workers, anyone can get an irritation or strain on the ligament in their feet.” The fascia is a fibrous, connective band that runs from the heel along the ball of the foot to the toes. When it becomes injured, it feels like a stabbing pain back near the heel. Often felt when rising in the morning or after standing for long periods of time, the pain can be debilitating. In 2015, then Denver Bronco’s Peyton Manning tore the fascia in his left foot and was unable to walk. At one time, surgery was recommended. Dr. Meredith Warner and Dr. Keith Wapner, both affiliated with the American Orthopedic Foot and Ankle Society, cautioned that it is a last resort. Dr. Warner says she disapproves of surgery because the fascia is cut to release the tension. Dr. Wapner

agrees, noting that he’s done very few surgeries. “In my 33 years of practice, I’ve only had to operate on two people,” says Dr. Wapner. “No surgery unless you absolutely have to.” Doctors warn against the frequent misconception that heel spurs are the cause of fascia pain. Spurs are a calcium deposit that create a bony protrusion on the underside of the heel. Like a hook pointing toward the toes, they develop where the muscles in the foot connect to the bone. But one doesn’t necessarily occur with the other. Only around half of those with fasciitis also have spurs. “They are related, but not causative,” says Dr. Warner. “Plenty of people walk around with a heel spur without any pain.” Both Dr. Curfman and Dr. Warner noted that stretching is still a proven method to alleviate pain. Using ice packs, gel shoe inserts, and physical therapy are also helpful. Kevin Lane from Pine Street Physical & Occupational Therapy in Stockton suggested manual manipulation and stretching, therapeutic exercise, and taping the foot helps it heal. “Physical therapy,” he says, “is one of the most effective treatment interventions for someone suffering from plantar fasciitis pain.” So, the next time you experience a little foot pain, talk to a podiatrist or a physical therapist. You’ll be back on your feet in no time.

TIRED? Ever snore so loud you woke yourself up? Do you get sleepy during the day? Do you sometimes wake up because you can’t breathe? These are all symptoms of sleep apnea. The condition happens when the upper airway gets blocked repeatedly while sleeping. It affects all ages and both genders, but the two main causes are obesity and genetics. “While there is a correlation between obesity and sleep apnea,” says Dr. Yusuke Suzuki, who practices dentistry in Lodi. “I’ve seen really skinny people have it.” There are three different types of apnea: obstructive, central, and complex. Obstructive blocks the airway when the soft palate muscles at the back of the throat relax. In central, your brain doesn’t send the necessary signals to the muscles that control your breathing. Complex apnea is a combination of both. Apnea can be a nuisance, but it can also be dangerous. Undiagnosed obstructive sleep apnea was to blame for two separate commuter train crashes in the New York metropolitan area last month, killing

one and injuring more than 200 others. “Job accidents are associated with sleep apnea,” says Dr. Suzuki. “When you don’t get enough sleep at night, you’re not attentive during the day.” The leading treatment is a Continuous Positive Airway Pressure (CPAP) machine. Through pressurized air in a face mask, it allows for better breathing but can be uncomfortable. Dental appliances are another option. Similar to a mouth guard or orthodontic retainer, this gets the tongue out of the way and pulls the jowl forward to create an easier breathing pattern during sleep. Dr. Ronald Kass, who heads Pacific Sleep Disorders Center in Stockton, says the dental options are a simple solution. “It only takes a few millimeters of advancement to control sleep apnea.” Left untreated, sleep apnea can be fatal. Carrie Fisher and Antonin Scalia died from it in 2016. NFL superstar Reggie White died from sleep apnea in 2004. His family founded the Reggie White Sleep Disorders Foundation, which provides CPAPs for those unable to afford them. Dr. Kass mentioned sleep apnea aggra-

But, the lack of oxygen from ineffective breathing can pose another longterm problem. “Sleep apnea causes dementia, specifically Alzheimer’s,” Dr. Kass replies. “Memory problems are common complaints even in people in their teens and 20s with sleep apnea.” Sometimes patients opt for surgery in a procedure known as Uvulopalatopharyngoplasty, or UPPP. IUDS
DEMYSTIFIED Rumors abound when it comes to intrauterine devices, or IUDs, but many of these are outdated misconceptions that can be traces back to the 1970s. In reality, IUDs are one of the most effective forms of birth control and can be used worry free for up to 12 years. As a one-and-done birth control option, IUDs are nearly foolproof and are 99% effective in preventing pregnancy. You can’t forget to take it like the pill, which is 91% effective taking into account forgotten doses or the interference of other medications. And there’s no chance of using it wrong like condoms, which are 85% effective. But if you decide you do want to get pregnant, IUDs are reversible, allowing you to conceive immediately after removal. There are two different types
of IUDs, and five brands available in the US. Let’s explore options and find the best choice for you.
Hormonal:
There are four brands of hormonal IUDs: Mirena, Kyleena, Liletta, and Skyla. They range from a three-year lifespan (Skyla) to six (Mirena). These use a progestin hormone called levonorgestrel to prevent pregnancy. Hormonal IUDs work in to two ways. First, they thicken the mucus on the cervix, which traps sperm before it enters the uterus. They also affect ovulation–this can mean lighter or no periods when using a hormonal IUD. These IUDs don’t immediately take effect, however, so you should also use condoms or another non-hormonal form of birth control the week after insertion.
Non-hormonal:
There is only one option for a non-hormonal based IUD: ParaGard. As the longest lasting IUD, it can prevent pregnancy for up to 12 years. This is a great birth control option for women whose medical history makes hormonal birth control options risky, such as those who are likely to have blood clots or who have had breast cancer. The ParaGard IUD is wrapped with small copper coils which is an extremely effective spermicide. Much like the hormonal IUDs, ParaGard also increases mucus on the cervix, ensuring any straggling sperm can’t make it into the uterus. Because ParaGard doesn’t use hormones, it is effective from day one–in fact, it can be used as emergency contraceptive up to five days after unprotected sex. However, your cycle is entirely natural and you may have heavier periods.
INSERTION DAY DOS AND DON’TS The insertion itself can range from uncomfortable to painful, but there are several ways to lessen the discomfort the day of: • Take ibuprofen before your scheduled insertion and have some on hand to take after. • Wear loose fitting pants to the appointment. You’ll likely feel bloated and experience cramping after the insertion and the last thing you want to contend with is your tightest skinny jeans. • Have someone pick you up. You will be safe to drive as you won’t be impaired by any medication, but you will be uncomfortable and it’s better just to sit this one out. • If you’re concerned about the pain of the insertion, ask your doctor for a paracervical block, a common gynecological antiesthetic procedure that may help lessen the pain of insertion.

TYPE 2 DIABETES Diabetes is one of the top ten serious diseases. The most common is type 2 diabetes, where the body cannot maintain enough insulin to keep glucose levels normal. To combat chances of becoming type 2 diabetic, you must embrace a lifestyle change. Such a switch will not happen overnight, but following the five recommended health hacks below will result in an improved you, with a better chance of outsmarting type 2 diabetes.

LOSE WEIGHT AND INCHES AROUND THE BELLY
Keeping the extra pounds off can improve your body’s blood sugar levels and help prevent or control type 2 diabetes. As you head toward your weight goal, you and your doctor will notice the difference. Studies from Centers for Disease Control and Prevention notes that if you lose just 5 to 7 percent of your body weight, you are helping to lower the possibility of developing type 2 diabetes. Take an individual whose body weighs 200 pounds, he sets a goal of losing roughly 10 to 14 pounds. It is not just the excess weight to be concerned about. The way your body distributes fat influences your probability and management of diabetes. Individuals who mainly support fat in their tummies are more likely to develop or have type 2 diabetes compared to those with fat focused in the thighs, hips, and buttocks.

DEVELOP A HEALTHY DIET
Developing a diet to naturally outsmart your type 2 diabetes is imperative. A healthy diet includes fresh fruits and vegetables, lean meats, beans, and low-fat or fat-free dairy. Avoid foods with a high glycemic index (GI) and glycemic load (GL)—mainly, processed foods. Processed foods—like fast food, frozen or packaged, and commercially prepared foods—contain more white sugar and flour than fresh foods. To integrate more whole foods into your diet, head to the produce section of the grocery store. Most fruits are low on

the GI except dried fruit and pineapple. In addition, whole-grain products, such as quinoa and brown rice are great choices, and their fiber helps control the body’s sugar levels.

MAKE EXERCISE PART OF YOUR ROUTINE
Exercise is part of any health plan and is most effective in preventing and gaining control over type 2 diabetes. Even if you are having a hard time keeping the extra pounds off your body, the more intense physical exercise you perform the healthier your body will be. Working toward a high-intensity interval training program (HIIT) is the best option for both weight loss and glucose control compared to other aerobic activities like jogging or cardio machines. HIIT program involves interchanging short spurts of exercise with intensity and changing to a lower intensity exercise. Indoor cycling at your fastest pace on hill climbs for two minutes then four-minute flat road cycling is a good example of HIIT. Work with a certified trainer, and together you can develop an ideal HIIT program.

TAKE CONTROL OF YOUR STRESS
Stress causes a tremendous amount of hardship on the body and makes it harder to control blood sugar levels. Learn
natural ways to relieve stress like changing your environment, avoiding stimulates and spicy food, scheduling a massage, taking therapeutic walks, listening to soothing music, and going to a movie you want to see. Relieving stress will help you sleep better, too. A good night’s sleep (7 to 8 hours) will lower the chances of type 2 diabetes based on a study reported in Diabetes Care.

REDUCE ALCOHOL INTACT
Avoiding alcohol is the best answer to reducing the risk, or managing, type 2 diabetes. In all, some agree a glass of red wine or a cold beer now and then is healthy and helps reduce the risk of diabetes. However, excessive drinking of alcohol is one of the principal causes of developing diabetes. Limit alcohol intact to weekends, with just a glass or two of the spirits. All around, it is healthier to hardly drink any alcohol. If you are at high risk, it’s best to completely remove the act from your lifestyle.

TRADING SHEEP for SLEEP It’s 2:00 AM and, by your count, you could open a shearing factory with all the sheep you’ve seen hopping over that imaginary fence. After all, four hours into what should have been a good night’s rest, and you still haven’t sawn a single log. But before you give up your lumberjack ambitions, we’ve got the 4-1-1 on sleep aids and techniques that will help you get the well-earned rest you need to start the following day off strong.
Exercise
Studies reveal that those who maintain an adequate exercise regimen in the morning or afternoon fall asleep much more easily, maintain longer restful sleep patterns, and wake up feeling more refreshed. Basic aerobic exercises work best at a minimum of 15 minutes per day. Avoid exercising in the evening or close to bedtime, which can worsen insomnia. The most you should be performing in these late hours are stretching techniques that can help you to unwind and relax.
Over the counter meds
While it’s never wise to form a habit of taking certain over the counter medications as a sleep aid, products such as Benadryl offer a sedating antihistamine known as Diphenhydramine that help you to become drowsy. This is also the key

ingredient in the medication known as ZzzQuil, a branding of Nyquil. Doxylamine is another antihistamine found in such brand names as Unisom that eases you into sleep. Follow any directions provided carefully to prevent any potential side-effects from developing, and never continue to take the product after the recommended amount of days unless otherwise directed by your doctor.
Natural remedies
“Although Melatonin is recommended as a sleep aid, it does not work for many people unless it is combined with some relaxing herbs,” comments Dr. Eunice Green of Green’s Nutrition. Dr. Green recommends chamomile, skullcap, lemon balm, valerian, and California Poppy in tandem with Melatonin doses. These herbs can be purchased in pill form, for ease of use. Essential oils are another calming component that can help lull you to sleep. “Lavender is always number one,” recommends Dr. Eunice. “Serenity by doTerra is wonderful—the gel caps [can be] taken internally, and the oil [can be applied to] the bottom of the feet.” Utilizing powdered magnesium in water after a long day is perfect for relaxing tired muscles and promoting the first steps toward a rewarding night’s rest.

Wine is a vice—often used as a treat at the end of a long day, or a splurge. But wine, especially red wine, could be keeping you healthy. Recently linked to impressive health benefits, the evidence keeps piling up. Too much, of course, is never good. However, too little doesn’t stack up either. According to science, moderate wine drinking is just what the doctor ordered.
It’s good for your heart. Wine
has cardioprotective benefits. A nightly glass of red won’t replace the need for a healthy diet and balanced exercise routine, but it could be the extra boost your heart needs. Moderate red wine intake has been linked to reduced mortality caused by heart health issues as well as limiting heart damage after a stroke. FOUR TRICKS to BOOST YOUR VISION at WORK It’s 3 pm on a Monday. You’re hunched over your desk, staring the same screen you’ve been staring at for the last six hours straight. Your head is buzzing, vision is blurring, and you’re starting to see double. If you were at home, you’d give yourself a break, but you’re not home—you’re at the office, and, unfortunately for you, there’s a mountain of tasks that still need to be done on the computer. This situation is all too familiar for those who work the classic 9 to 5 in the office. And, as inconvenient and annoying as this situation can be, it may actually indicate a more serious condition. Symptoms such as blurred, double vision, dry and/or red eyes, headaches, or neck and back pain could be your body’s way of telling you that you’re straining your eyes. And while there’s no evidence yet to suggest that working at a computer has long-term affects on one’s vision, if the eye-strain continues, it could cause some serious discomfort. Fortunately, there are ways you can minimize this strain, or at least provide your eyes with some relief, by following these four simple tricks.
The 20-20-20 Rule
As you switch windows, read text, and look up and down from your paperwork to your monitor, your eyes are constantly focusing and refocusing. This repetitive focus/refocus motion can cause damage to your eye muscles similar to the way a writer develops carpal tunnel. To avoid this, look away from your computer every 20 minutes, and for 20 seconds, stare at something 20 feet away. This breaks the repetition and gives your eyes a rest.
Palming
A technique originally invented by Tibetan Yogis, “palming” is an exercise that enve-

lopes your eyes in total darkness which soothes the optic nerve. To begin, rub your hands together to create heat in your palms. Then, close your eyes and place your hands over them, with your fingers brushing your hairline and your palms touching your cheekbones. Block out all light, but do not press directly on your eyes. Relax, visualize total blackness, and hold this position for as long as is comfortable.
Re-arrange Your Workstation
Eye strain is often caused by improper lighting. At your desk, your computer screen should be 18-30 inches from your eyes. Your monitor should never be directly in front of or facing a window; the
former causes a silhouette that makes it difficult to see your screen and the latter creates a glare. Avoid working under overhead fluorescent lights. Any lamps should be placed to the side of your computer, never above.
Blink!
The importance of blinking cannot be overstated—blinking naturally lubricates the eyes, and studies show that avid computer users blink about one-third as often as they would otherwise, and when they do blink, it’s usually only a partial lid-closure. Throughout the day, this can cause dry, irritated eyes. To remedy this, every 30 minutes, take a break to blink ten times, slowly and fully.

HEADACHE Headaches can be painful and debilitating. Understanding the cause may help ease your troubles and relieve some of your pain. There are several types of headaches, and a few may require a visit to your doctor. Let’s take a look at the different types, the causes, and some solutions that might ease your discomfort.
common than tension headaches and migraines. The best treatment is prevention by avoiding alcohol (red wine) and smoking. Poor posture and lack of hydration might be part of the cause as well. If you experience cluster headaches, talk to your doctor and try to determine what triggers the pain.
Tension Headaches
Caused by an infection in the sinus cavities, sinus headaches cause moderate to heavy pain around the forehead, cheeks, or bridge of the nose. The sinus infection usually results in a fever and requires a visit to the doctor who may prescribe antibiotics, antihistamines, or decongestants. Allergy testing is recommended as a preventive measure.
Tension headaches are caused by stress and are not chronic. They do fluctuate with moderate pain caused by constant pressure or ache surrounding the head at the temple to the back of the neck. It’s your body telling you to relax and find solace. Taking a nice quiet walk, scheduling a massage, or getting some exercise are excellent outlets in which to relieve stress. Pamper yourself! Feel the stress and the headache melt away.
Migraine Headaches
Said to be the worst form of headache, migraines can cause sensitivity to light, smells, noise, nausea, belly pain, and loss of appetite—lasting from a few hours to a few days. Studies indicate poor hydration and a deficiency in magnesium are behind dreadful headaches. It is recommended to drink plenty of water and eat high fiber foods and green leafy vegetables.
Cluster Headaches
These headaches are very severe and cycle with unbearable pain behind the eyes or on one side of the face with watery eyes, nasal congestion, and a runny nose. The good thing is they are less

Rebound Headaches
Overuse of medications like Tylenol, ibuprofen, and prescription painkillers are the cause of rebound headaches. Taking an excessive amount of medication, or withdrawing from it, can trigger these painful headaches. It is best to talk with your doctor and determine if you are experiencing rebound headaches. The solution is simple and easy to remedy. Experiencing a headache is not something to take lightly. You are miserable, and it’s a sign that something is not right with your body. Popping a Tylenol or ibuprofen may seem like the best solution at the time, but it is only treating the symptoms. It is best if you discover the cause of your headache and remedy it accordingly. After all, prevention is the best medicine.

VALVE TECHNOLOGY
New minimally invasive surgeries will soon be offered locally, allowing surgeons to improve quality of life without open-heart surgery. Transcatheter Mitral Valve Replacement, or TMVR, is currently in the research phase—and the Mitral Valve Clip Procedure is now available outside of our county—but Dr. Manshadi of Manshadi Heart Institute is eager to see these offerings available here at home.
There are four valves in the heart, the most important being the aortic and mitral valves because they are both involved and imperative to proper function of the left side of the heart—to pump oxygen and blood throughout the rest of the body. “Disease of these two valves are significant,” Dr. Manshadi notes. “And over time we have been working toward less invasive surgeries with better outcomes and less post-surgical complications.” TMVR is perfect for those who cannot have open-heart surgery due to age or health complications. As Dr. Manshadi explains, the most common type of heart disease is mitral, with over 4.1 million people suffering from mitral valve regurgitation—a condition in which your heart’s mitral valve doesn’t close effectively, allowing blood to flow backward in your heart. The prevalence increases with age, as Dr. Manshadi notes, stating, “0.5% age 18-44 have significant regurgitation, and 3.5% over 75 years old have it. When it’s serve, it can cause patients to be short of

breath, barely able to walk, and can lead to heart failure.” The mitral valve clip procedure which aids in regurgitation is now outside of research and available at UC Davis and Stanford. The valve is accessed through a small incision in the groin, as threads guide a wire and catheter through the femoral vein and toward the heart. Once the upper left chamber of the heart is reached, the guide wire is removed and a clip is applied to the leaflets of the mitral valve to create two smaller channels that prevent the backflow of blood. Patients feel instant relief, and are typically home in one to two days. Since TMVR’s debut in Denmark in 2017, 250 transcatheter mitral valve replacements have changed the lives of non-operable, high-risk and operable patients throughout the world. “Once you replace the valve, everything is 100 percent perfect.” As Dr. Manshadi notes, TMVR hasn’t ‘taken off’ like transcatheter aortic valve replacement (TAVR), which
is offered in our county, due to the complexity of the mitral valve when compared to the aortic. “ The structure, the physiology is different and much more complex.” In an effort to remain on the cutting edge of surgical options for his patients, Dr. Manshadi has been following and studying TMVR, as studies have shown that the procedure is as effective, but safer, than surgery and at 50 percent of the cost.

HEALTHY LEG VEINS and TREATMENTS THAT CLARIFY They emerge in colors like deep blue or dark purple and although they play a vital role in the human body’s circulatory system, often men and women would rather these vessels just stay beneath the surface. The culprits are varicose veins—the bulgy, twisted and enlarged vein that bubbles up to the skin’s surface. With more than 25 percent of Americans living with varicose veins daily, there are now several treatment options available and also new alternatives recently approved by the Federal Drug Administration that can clear the appearance of these blemishes up to 90 percent. Treatments include everything from sclerotherapy and verithena, both used to close the problematic veins but with different medications that are directly injected into the area. For more than 15 years, Dr. Stephanie Dentoni, from the California Vein & Vascular Institute, has worked with patients to give them both peace of mind and a boost of confidence when they come to her seeking answers. Dr. Dentoni answers the most commonly asked questions when it comes to vein health, the varieties of treatments, and also dispels myths.
What can people do to prevent varicose and spider veins?
“Walking is excellent because when you walk your calf muscles contract and that acts as a pump to help push

the blood back up to your heart. Compression socks, so there’s an external force compressing the veins. If you have a job where you stand for long periods of time without moving around then the compression socks help prevent it. Elevating your legs at night is very good or when you have the chance. Exercise is good. You want to stay at the best body mass index because obesity increases your risk.”
What’s the most common cause?
“Most of the time people have a genetic predisposition to getting varicose veins disease, so it runs in the family. Other factors to make them come on sooner and become more of a problem are standing for long periods of time, sitting for long periods of time, and heavy lifting. It’s usually more common in women although men get them too and it can occur at any age.”
What are the different types of treatments available and how successful are they?
“It depends on what you’re having done. If you’re doing the endothermal ablation where we go in with a catheter and heat-seal it closed, you usually see a difference within a couple of days as far as swelling and the bigger veins are going to shrink up about 50 to 75 percent and your legs feel better immediately.” “For sclerotherapy, it’s 80 to 90 per-
cent. It’s very, very good. You inject the medicine inside the veins with a little needle. The medicine causes those veins to seal off and then your body reabsorbs them overtime. With sclerotherapy, your legs usually feel better within a week or so, but you don’t see the results for several weeks to several months after that.” “In Varithena, the medication looks something like shaving cream so the medicine looks like these little, tiny micro-bubbles and those micro-bubbles have a greater surface area so they’re more effective. For the bigger veins, what happens is the procedure displaces the blood and it works well and it doesn’t get diluted with the blood like the liquid medication does.” “There’s also a recent procedure called VenaSeal that’s just been recently FDA approved and insurances are now starting to cover it where you put a catheter inside the bigger vein and what it does is it gives little amounts of medical glue down the vein so basically you glue the vein shut instead of heat-sealing it closed.”
What is the recovery time for such procedures?
“You don’t have to go under general anesthesia with surgery. You don’t have all the incisions on your legs. There’s no recovery time and you’re back up on your feet doing your normal activities right afterwards no matter what treatment you have done.

Bone loss isn’t something people feel so the first sign of osteoporosis is often a fracture. Doctors may scan for osteoporosis if the patient experienced a break after only mild to moderate impact, or if they reach the profile described above. Many women who have osteoporosis will experience a loss of height or adopt a stooped posture. Back pain can occur as a result of a fracture or a slipped vertebrae caused by bone loss. A bone density scan will be used to provide a diagnosis.

There is no cure for osteoporosis but a mix of lifestyle changes and medication can prevent, slow, or stop its progress. Those who receive an osteoporosis diagnosis will most likely be told to take dietary supplements, antacids, and, in some cases, hormones. Maintaining a healthy diet and physical exercise will also help.
THE BONE HEALTH DIET
To prevent or slow the progress of osteoporosis, commit to a bone health diet. The National Osteoporosis Foundation calls out certain foods known for strengthening bones. The list includes dairy products fortified with vitamin D, fish (such as salmon, sardines, tuna, and mackerel), plenty of fruits and vegetables, namely dark, leafy greens, and other foods fortified with calcium and vitamin D. In most cases calcium and vitamin D are the two nutrients those worried about developing osteoporosis want to keep up. Vitamins K and C as well as potassium (present in tomato products, raisins, potatoes, spinach, sweet potatoes, papaya, oranges, bananas, plantains, and prunes) are also important.

The future is bright—and robotic—at San Joaquin General Hospital. Just four months ago, the surgical center teamed with the minimally-invasive da Vinci Xi Surgical System, which offers a 3-D degree view, 10 times magnification, four arms that help with precise movement, and multiple tools that allow for many options. “It’s one of the most advanced technologies, where two surgeons including a trainee can operate together,” explains Dr. Vyas, chairman of the hospital’s general surgery residency program. From stomach and lung cancer surgeries to bypass, adrenal, hernia, prostate, and gynecological surgeries, the surgical system boasts less stress, a smaller incision, quicker healing time, and a lower mortality rate. With the ability to access the stomach and intestine without major cutting, the da Vinci Xi Surgical System allows surgeons to fix major complex cancer. As Dr. Vyas explains, major cutting can sometimes cause serious problems, as the incision takes time to heal, resulting in a longer hospital stay, and can require pain medications. 50

“Instead of having to stay for 10 days, patients can be out the next day, so the hospital can serve many more patients,” Dr. Vyas states. “It’s economical for the county, community, and the hospital as it can serve more people with the same capacity.” Perfect for aging or frail patients, robotic options mean life for those who can’t tolerate major surgery. “An 86 year old patient was not even considering her needed stomach cancer surgery because she didn’t believe she’d make it out of the hospital,” Dr. Vyas says. “But with the robotics option, she had the surgery and returned to my office stating it was the most amazing experience.” Alongside its services, SJGH boasts the strongest surgical education program in the nation, with cutting edge equipment, research, science, and technology at the forefront. Dr. Vyas notes that, “surgeons are provided with exceptional research and innovation experience, and in the last 6 months 3 PhD and additional health law and health economics personal. A team of this magnitude is unheard of in many large universities.” The hospital also participates in “Million Life Fighters”, a program designed to properly train a million surgeons and first responders in the next five years. “There is a huge unmet trauma care need around the world and in rural America,” Dr. Vyas reports. “So we are running one of the biggest pre surgical care training initiatives in the world.” To date, Dr. Vyas has trained 20,000. With hopes to make San Joaquin County the next big medical health hub, SJGH and Dr. Vyas work to bring technology and innovation from the Bay Area to here at home. “Our new research is calling for partnership!” Dr. Vyas says. A horrible mental image, but particles of what gets flushed do become airborne and find new lodgings in your toothbrush. And, eventually, your mouth. courtesy. It helps to keep those particles from becoming airborne. Shut the toilet lid before flushing. Do not use a plastic cap to cover your toothbrush, which can keep it from airing out and spur extra bacterial growth. Swap for a new toothbrush every month, or before the bristles begin to fray.

A HISTORY OF CARDIAC EXCELLENCE For nearly fifty years, St. Joseph’s Medical Center has been paving the path to heart wellness in San Joaquin County. Recognized as having one of the top 10 programs in California for heart surgery, St. Joseph’s has continued its tireless efforts to provide premier heart care, right here in Stockton. “St. Joseph’s has a long legacy of excellence in cardiac services,” says Don Wiley, Hospital President and CEO. «Right here at St. Joseph’s, our community has a high volume, high quality cardiac surgery program with a strong track record and a history of ‘firsts’ in advancements. We’re proud to be the hospital most doctors and patients in our community choose for heart surgery. But not only do patients in our community choose St. Joseph’s, over 30% of the patients that have open heart surgery at our hospital come from outside the community, and some even come from other countries.” St. Joseph’s performed the area’s first open heart surgery in the 1970s, and to date, has performed well over 13,000 open heart procedures. When it comes to cardiac surgery, research shows that having higher volumes increases patient safety, quality, and outcomes. With the 1990s, St. Joseph’s introduced a wave of new technology, tools, and procedures that included beating heart open heart surgery, rotoblation, and the use of carotid stents. In 1994, St. Joseph’s opened a new 30,000 square foot Heart & Vascular Institute that today houses its three cardiac catherization labs, two open heart surgery rooms, and a cardiac hybrid suite. From the year 2000 and beyond, St. Joseph’s debuted complex aortic surgeries, stent grafting, endoscopic vein harvests, minimally invasive thoracic surgery, and valve surgery. In 2016, St. Joseph’s opened the area’s first and only cardiac hybrid suite and began offering advanced procedures including transcatheter aortic valve replacement (TAVR), giving new hope to some individuals considered too risky for traditional valve replacement surgery. St. Joseph’s was also among the first in Northern California to offer the Micra TPS, also known as the world’s smallest pacemaker in early 2017. Their constant commitment to excellence is showcased in the many awards the Institute continues to garner, rated as one of Healthgrades America’s 50 Best Hospitals for Vascular Surgery, and a five-star recipient for carotid surgery seven-years running. As a result of St. Joseph’s distinction in the field, they are a designated STEMI Heart Attack & Stroke Receiving Center, and a Blue Distinction Center for cardiac care. St. Joseph’s is also a recipient of the American Heart Association’s achievement awards for both stroke and heart attack care. St. Joseph’s also has a robust Cardiac Research program, being involved in over 75 cardiac research studies 54

Do you see the glass half empty or half full? Perhaps a bit of both. Some days you have to work a bit little harder to see things in a positive light, and that’s perfectly okay. We spoke with Cathy Francis, PsyD, MFT and Social Services Director at St. Joseph’s Behavioral Health Center, to get an expert opinion on how to maintain a positive outlook for better mental health. Remaining positive is a challenge and something that needs to be practiced often — here are some simple tips:
fear of failure, which keeps us from seeing the positive side of things. “People either see their failure as a product of personal deficits beyond their control or mistakes that they can fix with effort,” says Cathy. Fear contributes to feelings of depression, anxiety, and negativity. Combat this by looking at mistakes as learning experiences. Pursue your passions and reveal in what makes you feel good. Your future positive self will thank you for trekking onwards.

have the power to make everything perfect. Sometimes, things will go wrong. But have you ever heard the saying, “don’t make a mountain out of a molehill?” As Cathy states, “when we encounter a stressful event our bodies go into a fight or flight mode. When we have a positive attitude, we are able to fight the stress hormones within our bodies.” Basically, if we perceive our situations in a positive manner, we are able to relax and solve the problem at hand. Try mediating or even listening to music to calm yourself for a renewed focus. Next time you’re feeling not-so-positive, remember to be patient with yourself and slowly try to implement these tips into your everyday habits. Even something as simple as smiling can help you feel happier and healthier. Sending positive vibes your way.
This won’t come easy, but with practice we can choose to see an optimistic view in even the most negative of situations. As Cathy states, “optimism is beneficial to our physical and psychological health,” contributing to lower levels of depression, negative thinking patterns, and an overall boost in happiness. When you think positive thoughts, you actually feel better. Mindset is everything. Next time, try challenging your negative thoughts— ask yourself, “how can I look at this in a positive light?” You may also find that helping others or even keeping a gratitude journal can help channel your optimistic side.

When a patient suffers from arthritis of the hip, the symptoms include pain, stiffness, and limitation of range of motion. These symptoms combine to interfere with all activities of daily life including walking, getting up out of a chair, going up or down stairs. The pain can be chronic or progressively more severe including sharp pains into the groin region. Dr. Tom McKenzie, a Lodi Sutter Orthopedic Surgeon who primarily admits patients to Adventist Health Lodi Memorial Hospital for total joint replacements, has spent years studying the hip and the various reconstructive surgeries to restore hip function and mobility. He began offering total hip replacement by the anterior incision approach in 2013, and is one of few to offer this technique to patients in San Joaquin County. “A very respected mentor, Dr. Joel Matta, has been advocating this procedure since the late 1990s. After several years of in depth study, multiple surgical labs, multiple site visits to learn the technique, I converted to the anterior incisional approach,” says Dr. McKenzie. The anterior incisional approach differs from the traditional posterior surgical approach by location of the incision, fewer postoperative restrictions for patients, increased mobility after surgery, less need for narcotic medication, and speedier return to normal life. The incision starts off the side of the lower pelvis and extends at most 10 cm along the lateral thigh. This small mini-incision provides adequate exposure for the entire surgery. No muscles are incised, but rather retracted out of way during the surgery. The exposure carefully goes in between all musculature and nerve planes. The exposure to the acetabulum, or hip socket, allows better visualization to ensure the components are more reliably placed. “The result is decreased patient pain to allow immediate ambulation with physical therapy the afternoon of surgery, 82

and 90% probability of discharge directly home the day following surgery,” explains Dr. McKenzie. The traditional posterior surgical approach requires a typically longer curved incision along the posterior and side of the hip, through the large and small posterior muscles to gain access to the hip. Although these muscles are usually repaired at closure, functional return is not uniformly reliable. “Those surgically violated and repaired muscles are critically important to the immediate and long-term rehabilitation of patients in their return to walking activities and the prevention of dislocations,” explains Dr. McKenzie. Dislocation precautions are consequently fewer and much easier for the anterior incision patient, as they can pretty much return to all daily activities with minimal restrictions. “The ability to do more is there,” adds Dr. McKenzie. “My outcome statistics document a significantly decreased hospital stay, accelerated improvement and return of ambulatory activity, decreased need for narcotic and nonnarcotic pain medication in comparison to the posterior incision surgery. Both my patients and I were quite satisfied with the outcomes documented with posterior incision surgery. The anterior incision outcomes are dramatically and clearly superior,” explains Dr. McKenzie.

They say breast is best. And, according to the Centers for Disease Control and Prevention, breastfeeding rates are up. Eightythree percent of babies breastfeed from birth, up 10% between 2004 and 2014. Babies are also breastfed for longer, lasting at least six months. We talked to Eugenia Hurlbut, DO, of Gill OBGYN Medical Group in Stockton for her feedback on the subject. If you do plan on breastfeeding your baby, here are some interesting facts you probably didn’t realize:
Breast milk comes in different colors depending on your diet

Breastfeeding reduces the risk of certain diseases.
Because there is a tendency to suppress ovulation during breastfeeding, it can reduce the risk of ovarian cancer. Dr. Hurlbut says decreased numbers of ovulations means less scarring of the ovary and less chance for precancerous, dysplastic cells. Studies also suggest a lower rate of heart disease in breastfeeding moms later in life, but more research is needed to determine why.
You should continue taking your prenatal vitamins.
Breast milk doesn’t look like a glass of milk you get at the store. The hue can run from white to yellowish to pink, blue, or even green. “The color can be influenced by diet,” Dr. Hurlbut says, “so if you eat a lot of red beets, the milk is likely to be pinkish!”
During the course of breastfeeding, mothers lose calcium in their bones due to the loss of estrogen. It also means a loss of calcium in the milk for the growing baby. Calcium stores return to normal levels within six months of weaning but continuing to take your vitamins is best for you and your baby’s health.
Colostrum is very nutritious and can provide immunity to your newborn.
Breast implants may pose a problem in breastfeeding.
Colostrum is the first milk that “lets down.” It too can be colorful, frequently dark yellow or orange from high levels of beta-carotene. When you breastfeed, you pass on your own antibodies to your baby keeping them immune from diseases. A Brigham Young-Stanford-Harvard research team identified a molecule in the mother’s breast milk that defends the baby against certain intestinal infections. Formula-fed infants have twice the incidence of those kind of illnesses as breastfed babies.
Dr. Hurlbut says breast implants and reductions can disrupt the flow of milk through the breast ducts and prevent breastfeeding or decrease milk production. It can also increase incidence of breast engorgement and mastitis. Other special conditions that may limit milk production are women with hypoplastic breasts (breasts that are underdeveloped) and women with Polycystic Ovarian Syndrome. These conditions don’t exclude breastfeeding, but these women may have issues with having enough supply and may need to supplement with formula.

Breast milk sometimes doesn’t come in right away. There is usually a bigger let down by the third day of breastfeeding and production will typically increase from there. Dr. Hurlbut says not to get discouraged, “So, stick with it!”

A 2008 Danish study showed the “slimming
effects” of breastfeeding. Breastfeeding was associated with a lower postpartum weight and body mass index. It could, in fact, eliminate weight retention by six months postpartum if the recommended breastfeeding regimen (up to one year) was followed.
The right breast produces more milk.
Some studies have shown that the right breast produces more milk than the left one. Milk production is a little over sixtyfive percent greater in the right breast and typically occurs early in lactation. Although, being a first-time mom versus a seasoned breastfeeder does make a difference. However, total milk output is not affected. When it comes to breastfeeding, there are plenty of resources available. “The bottom line is,” Dr. Hurlbut advises, “if you have questions or concerns about breastfeeding, ask your obstetrician, your pediatrician, or primary care physicians before or after birth!”

We combine that with 3-D direct visualization, providing the same benefits of robotic surgery, but with a greater connection between surgeon and patient and fewer incisions.” In this way, Dr. Keyser continues to maintain a personal touch with his patients. Dr. Keyser finds this aspect to be the most fascinating, as he witnesses patients who may not have done well with traditional techniques “recover as quickly as a healthy patient would have, or even faster.” But, it’s not all about surgery. “What I find to be the most important component is the attention to quality and outcome. As a registered dietitian and clinical diabetic educator at San Joaquin General Hospital, Dawn Siberts says the trick to glowing skin is no secret. Not only is fiber great for the body’s digestive system it also helps control blood sugar. High blood sugar can contribute to dry, flaky skin. According to Dawn, women should consume 25 grams per day and men should consume 38 grams per day. “High fiber helps to improve your glucose and your blood sugar control. High blood sugar makes your skin itchy and dry,” Dawn says. “By controlling blood sugars, your skin is able to heal faster and it’s able to be healthier.”

Five a day
The saying, “eat the rainbow” couldn’t ring truer. Dawn recommends that her patients each five fruits and vegetables per day to maintain a nutrition-rich diet that will have a positive affect on the skin.
Moderation
An alcoholic beverage every now and then is A-OK, but mass consumption of alcohol or tobacco will not only dehydrate skin, but it encourages wrinkles and even jaundice when the liver is unhealthy. “When your liver gets to a certain degree of not functioning correctly
your skin will become jaundice and turn yellow,” Dawn says. “Once you see it on somebody, you never forget that shade of yellow.”
Vitamins
Dawn recommends multivitamins and vitamin C to help obtain radiant skin and also to promote faster healing in cuts or scrapes. “For healthy skin and a healthy body, I also recommend getting plenty of sleep, regular exercise, eating healthy, and keeping your skin out of the sun,” Dawn says. “As a skin cancer survivor—I can’t recommend enough sun block, hats, long sleeves, and long pants.”
