San Joaquin County’s only FULL-TIME PRACTICE SPECIALIZING IN
HIGH RISK PREGNANCIES High-Risk Pregnancy Care
Having a baby is an exciting, life-changing event for many couples. For those with pregnancy complications or medical problems, it also can be a stressful time. Three-fourths of problems in pregnancy can be predicted from a patient’s medical history and physical exam. However, problems can arise in even the healthiest patients. That’s why it’s helpful for expectant parents to have experts on their team to help guide them through the ups and downs of their 9-month journey.
What is a Perinatolog Perinatologist?
A perinatologist or maternal-fetal medicine (MFM) physician is an obstetrician with special training devoted to caring for pregnant women with conditions that put the mother or the developing fetus, or both, at increased risk for complications during or after pregnancy and birth. Perinatologists are capable of making difficult diagnoses and detailed care plans, and performing specialized procedures such as amniocentesis and Doppler ultrasound examination.
Where can I find a Perinatologist in Stockton?
Located in Stockton and serving San Joaquin County, Valley Perinatal & Genetics Diagnostic Center with Andrew Chao, MD is San Joaquin County’s only full-time practice specializing in caring for pregnant women who are high risk for reasons that may include: • Chronic medical conditions such as heart disease or diabetes • Family history of genetic disorders • Problems with previous pregnancies such as preterm labor or miscarriage • Pregnancy with multiples (twins, triplets) • Over age 35 at the time of conception • Structural abnormalities (birth defects) • Genetic disorders such as Down syndrome • Restricted growth in the womb We work hand-in-hand with patients and their obstetricians to monitor health concerns and review prenatal tests and ultrasounds. Before scheduling an MFM consultation or prenatal test, you should ask your primary caregiver for a referral. Please call (209) 933-9900 for more information.

Who Will Handle Your Affairs If You’re Unable? Scott G. Beattie, JD, LL.M (Tax)
Many people use living trusts as the cornerstone of their estate plan. It is common knowledge that Trusts (if properly established and funded) help families avoid the expense and delay of probate court when settling the affairs of a deceased family member. Similar problems exist during lifetime for people who become incapacitated. When a person is living, but incapacitated, families often have to go to court to establish a conservatorship so they can handle financial and health care decisions. Like a probate, conservatorships can be expensive and time consuming cases, involving complicated procedural requirements that are aggravating and costly for families. Conservatorships are used to appoint an agent (the Conservator) to handle the financial and medical care decisions for the incapacitated person. Like probates, conservatorships can be expensive and they present many challenges. There are two estate planning documents designed to avoid conservatorships. These are Durable Powers of Attorney for financial matters (Financial Powers) and Advance Health Care Directives for health care matters (Health Directives). California law allows individuals using these documents to appoint an agent to handle their affairs if the need arises without having to go to court. Agents acting for an incapacitated person under a Financial Power can sign contracts (to rent or sell property), sign medical and health care contracts, handle tax returns, hire household employees, pay bills using the incapacitated person’s bank accounts, make contributions to or withdrawals from IRAs and 401k plans, and generally take care of all financial matters in the same way the incapacitated person would do for him or herself if they were still able to act. Advance Health Care Directives are legal documents used to appoint an agent to handle health care matters for those unable to act on their own. In some states these documents are called “Living Wills” because they deal with lifetime care issues. Most people think of Health Directives when it comes to life ending medical decisions because such Directives provide an agent with the power to withdraw medical treatment (“pull the plug”) in certain circumstances. But health care agents can also decide on medical treatments, living facilities (assisted living or skilled nursing), and handle other important health related matters.
Durable Powers and Health Care Directives can be as important as Living Trusts to preserving your estate.
When setting up Health Directives and Financial Powers you should give your designated agent guidance about your personal values for these decisions. Would you want to stay in your home as long as possible? Would you want to end treatment if you are in a permanent coma? These documents authorize your agent to implement these decisions for you.
By providing instructions in advance, the person who signs Financial Powers and Health Directives can address foreseeable problems and provide your agent with guidance on difficult decisions. For instance, you may want to direct, in the event of a prolonged illness, whether your agent should try to provide for your care (as much as possible) in your home or whether the agent should instead have the ability to move you to an assisted living or skilled nursing facility, as necessary for your care.

Save taxes, preserve wealth, and secure your family’s future. continued from previous page
If a person wants his agent to “pull the plug” in the event of an irreversible coma or other terminal conditions, then the agent can do so in California, but only if the document provides clearly written authority using a properly witnessed or notarized document. Without the correct documentation, Hospitals will generally not allow an agent to withdraw medical treatments such as artificial hydration, respiration, or nutrition. Such powers can help avoid extraordinary costs of care during a person’s final days. Without Health Directives and Financial Powers, your family may have to go to court to get a conservator appointed to handle health and financial matters. Even worse, in California there isn’t any authority to withdraw life support in the absence of a written instruction in the appropriate document. Because conservatorships can last for years and be extremely expensive, the use of documents such as Durable Powers and Health Care Directives are often as important as Living Trusts to preserving your estate.
When setting up Health Directives and Financial Powers you should give your designated agent guidance about your personal values for these decisions.
My recommendation to clients is to take great care to discuss and think through the issues before signing these documents. Don’t do it yourself, see a professional. If you take the time to do it right, it will save money for you and your family in the future.

MEDICAL GUIDE | Surgery in the 21st Century
By Nissa Halquist
Surgery in the 21st Century
Now, this is more like it. With childhood visions of flying cars and dinners in pill form dashed, the 21st century has still been able to give us this: robotic surgery. To clarify, C3PO isn’t scrubbing up and taking out your spleen. Rather, it involves a human surgeon directing the movements of robotic arms. It is the latest surgical technology out there, and Lodi Health is the first medical facility in the San Joaquin area to have it. It is known as a daVinci system, and is currently being used by Lodi Health physicians to make certain complex surgical procedures as minimally invasive as possible. It can be used for a variety of delicate operations, including prostatectomy, hysterectomy, myomectomy, coronary artery bypass, mitral valve repair, colorectal surgery
The main benefits of the daVinci system include: • Enhanced 3D, high-definition vision of the operative field with up to 10x magnification • Optional dual console that allows a second surgeon to provide assistance • Superior visual clarity of tissue and anatomy • Surgical dexterity and precision far greater than even the human hand • An updated and simplified user interface to enhance operating-room efficiency • New ergonomic settings for greater surgeon comfort
In other words, by combining the surgeon’s skill and know-how together with robotic technology, this system brings new meaning to the term “surgical precision.” How does it work exactly? Basically, via a series of computerized controls, cameras, screens and a customizable arrangement of robotic arms--each equipped with specialized surgical tools--a surgeon actually performs surgery several feet away from the patient. To date, the outcomes from robotic surgeries have been altogether successful. The surgery itself is made faster and more efficient, while patient recovery times are shorter and less painful. Now, back to those flying cars...
and more.

MEDICAL GUIDE | Safe Sex, Safe Cervix
Safe Sex, Safe Cervix By Nissa Hallquist
In the U.S., an estimated 12,000 women contract cervical cancer every year. No woman is immune, but there are certain steps she can take to prevent it.
20
SAN JOAQUIN HEALTH
& MEDICAL GUIDE 2013
According to Dr. Param Gill of P. Gill Obstetrics and Gynecology, the majority of the risk factors for cervical cancer can be summed up in one word: sex. The main cause of cervical cancer is Human papillomavirus (HPV), a sexually transmitted virus that, depending on the actual type, has also been shown to cause genital warts. Other risk factors include a history of early intercourse, multiple partners, other STDs like Chlamydia and male partners who themselves have had multiple partners. Non-sexually based factors include smoking and a history of immunosuppression. HPV types 16 and 18 are the ones that can cause cervical cancer. Therefore, one of the best ways to prevent cervical cancer is to get HPV completely out of your system. Most younger women who contract HPV are able to naturally clear it in two years. However, the ideal situation is not getting it in your system at all. HPV vaccines have been recently licensed by the FDA as safe and effective for females age 9 through 26. They are given as a series of three doses and,
while considered most effective before any possible exposure to HPV, can be administered at any time in that age range. Alternately, the best course of action is safe sex, preventing the contraction of an STD with proper condom use. Whatever the cause, the really good news is that, of all the reproductive cancers, cervical cancer is the easiest to prevent through proper screening. That means, yes, the dreaded Pap smear. In order to catch and treat the condition while still in its pre-cancerous state, Dr. Gill recommends starting screening at age 21 and continuing every three years (as long as the results stay normal) until age 65. Sure, Pap smears may be uncomfortable. But before you “forget” to have one, consider the alternative.

One day, you’re going to have to admit it. You can’t see what that street sign up ahead says, the print in that book isn’t so easy to read anymore. It may even be hard for you to read this article. You’re going to have to admit that your eyes are aging...and not aging well. Generalized blurriness is the most common symptom associated with macular degeneration. There are different forms of the condition, but the great majority of cases are due to age-related macular degeneration (AMD). “Age-related macular degeneration is the leading cause of vision loss in adults 65 and older,” confirms Jerome Burke, O.D. of Burke Optometry. “And it currently affects more than two million Americans over the age of 50.”

MEDICAL GUIDE | Gracefully Aging Eyes
There are actually two forms of AMD:
Dry AMD (early or non-neovascular) and wet AMD (advanced or neovascular). Dry AMD is typically slow-developing, which is what may make it hard to notice at first. This is due to macular drusen (yellowish deposits of metabolic waste from photoreceptors) building up gradually behind the macula, instead of getting flushed away like normal. It is characterized by difficulty seeing fine print and adjusting to different light levels. Dry AMD (early or non-neovascular)
(if you’re curious to try this, you can go to AMD.org for a full-sized copy of the Amsler grid that you can print up and use at home). Another thing you can consider is a diet supplement specifically formulated to help with vision, one with large doses of zinc and antioxidants like A, C and E. Funded by the National Institutes of Health and the National Eye Institute, the Age-Related Eye Disease Study (AREDS) was a major clinical study designed to learn more about risk factors. AREDS concluded in 2001, after following about 3,600 patients for 12 years. It clinically proved that use of a supplement with high levels of the aforementioned minerals did in fact reduce the chances of dry AMD turning into wet AMD by 25% (this blend is commonly referred to as the AREDS formulation). A second study, AREDS2, was launched in 2006 to further study the effects of antioxidants as well as fish oil on the progression of AMD. Results are expected to be released sometime in 2013. If you do happen to develop wet AMD, there’s no need to panic. You won’t inevitably go blind. Researchers have determined that the chemical responsible for the formation of the blood vessels is vascular endothelial growth factor
Wet AMD (advanced or neovascular) Wet AMD is quite different. It’s fast-acting and can cause serious and permanent loss of vision. The name comes from when abnormal blood vessels grow beneath the macula and then start to leak or bleed. It only takes weeks or months for distorted vision and blind spots to become noticeable. There’s no complete cure for AMD but, according to Dr. Burke, it can at least be slowed down. Wet AMD always progresses from dry. To limit this progression, the best things you can do are stop smoking, control high blood pressure, use magnifying aids (such as glasses) and proper lighting to reduce eye strain, monitor your vision at home with an Amsler grid and, if you’re at risk for AMD, have your eyesight frequently monitored by an eye doctor.
Risk Factors for AMD
• Over 55 • Of northern or western European descent • Smoking • Uncontrolled high blood pressure
(VEGF). With that discovery, they have been able to develop drugs to counter the effects. They are usually given intravenously (Avastin) or injected directly into the eye (Lucentis) to limit blood vessel development at the source, near the macula. Right now, the main benefits from these medications seem to be stopping wet AMD from damaging eyesight any further. However, some patients that receive this treatment have actually found their eyesight to be improved. So while there’s still no cure for AMD—at least not yet—you can still rest assured knowing that your vision need not go gentle into that good night.
With the Amsler grid test (right), you stare at the dot at the center of a grid with each eye separately. If the grid has straight lines, your vision can still be considered relatively normal. But if the lines become wavy or otherwise distorted in spots, it could be a sign of macular degeneration

Are you one of the thousands of adults who dream about having straight teeth, but don’t do anything about it? Many have worn, chipped or crowded teeth and often suffer from bite problems as a consequence. Fortunately for adults, getting teeth straightened is not just for kids anymore. “There are some really exciting techniques we use today that help patients get the straight smile they want,” says Reconstruction and Cosmetic Dentist Todd Franklin, D.D.S. “I find that many of our adult patients are far more interested once

MEDICAL GUIDE | Straight Smiles 101
The Consequences of a Bad Bite It’s not just cosmetic. Teeth that aren’t in alignment can actually cause further reaching health problems. • Tooth decay and gum disease. Misaligned teeth can be harder to clean, potentially resulting in cavities or gingivitis. • TMJD. Malocclusions can put strain on the muscles in the face, jaw and neck. This can result in temporomandibular joint disorder (TMJD), with symptoms that include headaches, pain in the face and neck, and clicking sounds when the jaw moves. • Respiratory issues. Problems like snoring and sleep apnea have been found to be the result of improper jaw alignment. • Speech disorders. The alignment of teeth with the tongue is important for proper pronunciation. • Digestive issues. If you can’t chew properly, then food won’t be processed the way it should be before reaching the digestive tract.
they learn that they do not have to go through the traditional techniques.” He says the most important part of treatment is to properly identify the problem and determine how it happened. There is always a cause and effect relationship. Was the patient born with the condition? Have they received treatment that resulted in a problem? Has it been a slow progression over the years? “Patients all have one common desire: to have as little dentistry done while achieving a beautiful smile. They all have time concerns, budgets to work with, and question how they will look while trying to maintain what God gave them,” says Dr. Franklin. For kids, the primary concern is compliance (not interrupting the treatment). That’s why young adolescents and teenagers are best served by the traditional orthodontic approach, braces. Their misalignment and bite problems can be easily corrected with the traditional approach, and then they are finished. Since they have not had the time to wear down their teeth, usually no additional

& MEDICAL GUIDE 2013
treatment is necessary. Adults, on the other hand, have had years to wear down their teeth due to bad bites. Once their teeth are straighter, many still need to have cosmetic work to improve the worn problem areas. The advantage of straightening the teeth first is that any corrective cosmetic procedure needed to be done afterward can be dramatically more conservative. Treatment for adults usually falls into three different options. They can have traditional braces, tray aligners or a new technique called Cfast. Traditional braces give more defined control but can be expensive, have limited aesthetic options and may require more time in treatment. This is the proper course of treatment for a more advanced set of crowding and bite-related issues. Tray aligners, such as Invisalign, provide many options for tooth movement. They may or may not be as expensive as a traditional approach, but patients like the fact they can take them out if needed. While convenient in this aspect, their removability can lead to compliance problems with
this type of treatment. A technique called Cfast is a new approach that can finish a case in less than six months. Using clear brackets along with a white titanium wire will move teeth in the aesthetic area while maintaining the patient’s bite. The cost is usually less than other methods, it’s nearly invisible, and it’s usually a case for minimal tooth movement without compliance issues. With all the options available today, a full comprehensive evaluation remains the best way to determine the right approach to getting that beautiful smile. This is especially true in instances where a bite is so severely misaligned that standard orthodontics might not be sufficient. Or maybe a patient just doesn’t want to wait several months for a new smile to emerge. “We live in a very fast-paced society, and many times patients want that immediate new smile,” says Dr. Franklin. “In these instances, we discuss the option of veneers or hybrid porcelain restorations to correct smile and bite-related issues.”

MEDICAL GUIDE | San Joaquin 911
San Joaquin By Nissa Halquist | PHOTOS By Kristoffer Heacox
When you’re the victim of a medical emergency, there is nothing scarier. You’re in pain, afraid and lonelier than you’ve ever felt before in your whole life. You need help. So you call 911.And with that call begins a beautifully choreographed chain of events designed to get you away from that place of fear and to the help you need.
So, let’s say you feel chest pains. You make the call, and then... You are immediately connected to your local Public Safety Answering Point (PSAP). Depending on where you are, your PSAP is one of three law enforcement agencies: city police, the sheriff for rural situations, or the highway patrol. If you’re on a cell phone, your location is determined by GPS. The screener asks you a few questions to determine your exact address and situation (location is always first, since being able to find you is the most important thing). You tell the screener your address and that you’re having chest pains. With that, the call is rerouted to medical dispatch, which will send the appropriate emergency personnel to your location.

911 In most cases, ground ambulance services are more than sufficient for the assessment, treatment and transport of a patient to a hospital. Teams of Emergency Medical Technicians (EMTs) and paramedics can provide a broad range of care. Both are trained to provide basic life support (BLS), which includes CPR, the use of an automated external defibrillator (AED), maintaining an open airway and providing artificial respiration. A paramedic is also trained in more advanced directives. Their advanced life support (ALS) protocols include intubation, the administration of medications, inserting IVs, and so forth. But if the situation is particularly dire, and your location is far from a hospital, ground services might not be fast enough. After all, the goal is to stabilize you and keep any time out of hospital to a minimum. In such cases, an air ambulance might be called in for speedy transport. As their services are reserved for the most critical situations (as well as for interfacility transports of critical patients and pediatric cases), air ambulances are equipped with the most advanced life-saving equipment. And their crews always include at least one flight nurse, a fully-trained RN who works under a supervisory doctor’s medical license.
So, back to you and your heart attack. You’re in town, only a couple of miles from the nearest hospital. The emergency crew arrives and determines that you are indeed having a heart attack and need to be transported to the nearest cardiac care center as soon as possible. At this point, you’re having trouble breathing, so they give you oxygen and strap on a 12-lead EKG that electronically transmits your heart rate and pattern to the receiving emergency room doctor. The crew loads you into the ambulance, the siren begins to wail and you’re off. During the ride to the hospital, the www.sanjoaquinmagazine.com

What kind of training does it entail? You need to have at least three years ICU experience before you can apply to air transport. I worked in an ICU full-time, was part of an ambulance crew part-time (for UCD and Stanford) and was a volunteer firefighter before I got accepted at REACH.
What kind of calls do you most often get? Interfacility and pediatric transport and “at scene” (911) calls for medical emergencies and trauma, usually for auto accident injuries and recreational injuries from things like hiking or offroading accidents.
What’s your favorite part of the job? The autonomy of it. Rather than focus on a single specialty, you get to see a lot of different things and use a variety of clinical approaches. Second favorite? The flying!

Emergency Department Doctor, Co-Director of Stroke Intervention team, Chief of Staff (elect); St. Joseph’s Medical Center How did you make the decision to get into emergency medicine? I’d originally graduated as an RN in 1981. I worked for many years as an emergency room nurse before deciding to get my medical degree. And I ended up in emergency medicine because there was nowhere else I wanted to be. What’s your favorite part of the job? There’s a few things. There’s the variety of the cases, the adrenaline, the thinking on your feet. And you always get to treat; you don’t worry about getting billing information first.

How many visitors does the St. Joseph’s Emergency Department get? About 48,000 to 50,000 each year. And now you’re starting to get more into the administrative side of things. What drove that decision? I want to see the other side of things and be a part of how the hospital is run. As Chief of Staff, I’ll get to help create policy and quality measures.
crew continues to monitor your vital signs and overall condition. At the emergency room, the crew stays with you until you are formally admitted. That is how smooth the system can be, and just the kind of scenario for which it was designed. However, sometimes complications like a fire or a violent crime can slow down the process--the scene needs to be safe and secure before medical personnel can approach--but directives are in place to minimize the delay in care. Fire department and law enforcement personnel are all BLS-certified. Unfortunately, there are the occasional abuses of the system, often due to a caller’s misunderstanding of how to use it and why. This is usually a result of the subjectivity of an “emergency.” The system defines an emergency as potential loss of life or limb. However, not everyone defines it that way and will call 911 for whatever they deem an emergency: a twisted ankle, a finger whacked by a hammer, etc. Then there’s the insistence for ambulance transport. The dispatch of an emergency crew does not always mean that an ambulance ride to the hospital is needed. Unless the condition is life-threatening, insisting upon ambulance transport simply ties up a crew and keeps them from being available for other calls. Some people are under the misconception that just because they arrive in an ambulance means that they’ll be seen first once they get to the hospital. But that’s not always the case. The emergency department triage nurse will determine speed of admission based on the condition, not on the mode of transport. So, if someone else is in need of more immediate care, you will have to wait and, due to patient abandonment laws, the crew that brought you will have to wait as well. Urgent Care centers were developed specifically for care that is needed quickly, but is not necessarily life-threatening. This is where you can go for that twisted ankle, that rash, that lingering fever. At urgent care facilities, while you may have to drive yourself, you’re likely to be seen by a doctor more quickly than an emergency room. That being said, many of the people in the emergency services industry thrive on any and all emergency situation. Each call requires a different set of skills, a new problem to solve. And it’s more than just technical skills, more than just giving CPR or setting a splint. Emergency personnel know that, many times, the most important part of their job is simply to be there, to provide comfort as well as care.
My knowledge of the emergency services system in the San Joaquin area came chiefly from my discussions with those that work in it. They are great people who love their work, and I would like you to meet them.

Dennis Bitters | Ripon District Fire Chief and Certified EMT What first made you decide to become a firefighter and certified EMT? What do you feel is essential for someone to enjoy & be successful in that line of work? You have to like people. So much of the job is psychological, establishing trust and providing reassurance. You have to be able to talk to people, to bring order to chaos. You also have to be confident. You don’t wish for bad, but if something does happen you want it to be on your shift.
Do you get to go on many emergency runs anymore? Not many since I became Fire Chief (in 2003). But I’m always on call if everyone else on a shift is already out. How many calls did your department go on last year? 1789. 1102 were medical, and 270 were traffic accidents with medical care needed.
What would you say to someone who has questions about the system? Please come in and ask us. And if you want to see how things work firsthand, we encourage you to come and ride with us.

MEDICAL GUIDE | Combating Back Pain
How to Stop Back Pain At its best, chronic back pain is a frustrating nuisance. At its worst, it’s debilitating. Too often, people ignore it until it’s started to really take a toll on their daily lives. As Dr. Coykendall puts it, “People take better care of their cars than they do themselves.” According to the doctors, the mindset one should take to stop back pain can be summed up in one word: balance. For every bad thing you do to your body during the day--hunching over your computer, drinking a soda--you should counter it with something beneficial:
• Get up from your desk and take a walk • Counter the dehydrating effects of soda or coffee with a glass of water. • Take a good multi-vitamin, preferably one with omega-3s that will minimize inflammation. • If you have to stay at your desk, sit up straight. Keep your shoulders down and your neck in-line with the rest of your body. • If lifting something heavy, maintain the same in-line posture and use your legs. • Exercise. Spend just a few minutes a day stretching to build up strength and flexibility in the back. That being said, you may still overdo it every once in a while. For at-home relief of the occasional aching back, heat is your best friend. It loosens up muscle tissue and relaxes it. Spend twenty minutes with a heating pad or--better yet--the moist, penetrating heat of a bath, and you’ll feel tense muscles start to uncoil (and it’s a great way to

MEDICAL GUIDE | Goodbye Gluten
Goodbye, Gluten By Nissa Halquist
Gluten sensitivity. Up until a few years ago, you hardly ever heard about it. But it has swiftly turned into something of an epidemic. More and more people are giving up bread, pasta and cereal in the name of gluten sensitivity. Why does it seem like it came out of nowhere? According to Sarah Kay Hoffman, a Holistic Health Coach specializing in IBS (Irritable Bowel Syndrome) and IBD (Inflammatory
38
SAN JOAQUIN HEALTH
& MEDICAL GUIDE 2013
Bowel Disease) in women, the reason is twofold. First, the condition has been simply misunderstood and misdiagnosed up until recently. Second, there’s the problem with our food itself. “Our food is not the same as it was hundreds of years ago,” Hoffman says. “We have manipulated everything that goes into our food supply. Over time, this has caused the body to have to work really hard to process and digest the incoming food. The food we consume today is much more irritating to the gut and is wreaking havoc on our digestive system.”
For those with the condition, it’s hard to dismiss the physical symptoms that crop up with gluten intolerance, or the relief that occurs when the foods that contain gluten-usually wheat, rye, barley or oats--are no longer consumed. We’ve all felt a little bloated and tired after eating a big plate (or more) of pasta. But for someone with sensitivity to gluten, the reaction is more severe, with much less indulging. According to Hoffman, not everyone displays the same symptoms across the board. But they are most likely to include one or more of the following: bloat, www.sanjoaquinmagazine.com
excessive gas, fatigue, eczema, brain fog, foul stool, diarrhea, failure to thrive (in children), constipation, joint pain, infertility or asthma. It is this wide range of symptoms that leaves many people unaware of their condition and, even when it is suspected, often leads to misdiagnoses. According to a 2012 study conducted by the American Journal of Gastroenterology, about 1.8 million Americans have celiac disease. However, of these 1.8 million, 1.4 million of them are unaware of it. Meanwhile, 80% of the 1.6 million people who choose to have gluten-free diets do so with no diagnosis of celiac disease at all. There are different levels of gluten sensitivity. When the symptoms first start, it could be categorized as simple gluten intolerance. Even if not super-severe, recognizing the symptoms and changing your diet accordingly will prevent further damage to the digestive system. Take that intolerance a little further, and you have what’s called celiac disease. It’s a form of IBD where gluten damages the lining of the small intestine and stops it from processing food properly. While the exact cause is still unknown, in Hoffman’s experience it seems to be triggered by a traumatic event or illness, such as after giving birth or contracting an infection while traveling abroad.

In rare cases, there could be a full-blown gluten allergy. These are the instances where the reaction is acute and life-threatening. Like with seafood or peanuts, a gluten allergy results in itching, hives, wheezing and/or swelling of the throat. For all of these conditions, the only cure for symptoms is to stay away from gluten completely. And even if you don’t have a wheat allergy or gluten intolerance, there are still benefits to taking at least some gluten out of your diet. Cutting out even just a little and replacing it with whole foods and natural alternatives to wheat can give your digestive system a much needed break. In fact, there are so many good alternatives out there, you probably wouldn’t ever miss it. For more information on how to live well without gluten and a great selection of gluten-free recipes, see Sarah Kay Hoffman’s healthy lifestyle blog at www.agutsygirl.com.

Dameron Hospital
525 West Acacia Street, Stockton (209) 944-5550 www.dameronhospital.org
Dameron Hospital is a full-service, 202 bed notfor-profit communityowned hospital. 2012 celebrated its 100 years of independent service to the Stockton community. Today, under the leadership of current President and CEO Lorraine P. Auerbach, Dameron Hospital is still located where Dr. John Dameron first started construction in 1912: on the corner of Acacia and Lincoln Streets in central Stockton. The hospital campus now includes several surrounding blocks and is the county’s third largest private employer. Last year, HealthGrades, a national hospital ranking service, gave Dameron Hospital a “Five Star Rating” in the Stockton region for joint replacements, placing them in the top 10% of hospitals nationwide for clinical performance. Such a distinction points to Dameron’s position as the county’s leading orthopedic hospital. And, with its busy Start of Life Center, Dameron is also Stockton’s leading birthing hospital. The San Joaquin County Emergency Medical Services Agency has named Dameron a designated STEMI (ST segment Elevated Myocardial Infarction) receiving center in San Joaquin County. Because of Dameron’s LifeLine technology and cardiac-prepared ER staff, emergency response teams now automatically take patients suffering a STEMI to Dameron. Dameron Hospital has an active Cardiac Catheterization Lab and is a certified cardiac care hospital and cardiothoracic surgery center. Over the past decade, Dameron has transformed itself by upgrading its equipment and facilities from the inside out. Just some of those upgrades include new:
• Critical-care patient monitors in Emergency, Surgery and ICU/CCU departments • Patient Telemetry Monitoring System on medical/surgical floors

Doctors Hospital
1205 East North St., Manteca (209) 823-3111 www.doctorsmanteca.com
Doctors Hospital of Manteca first opened its doors with 28 beds, 41 employees and nine physicians. Since then, like the community in which it resides, Doctors has never stopped growing. Today, Doctors Hospital of Manteca has more than 400 employees and a medical staff of 157 physicians. Recent building additions and upgrades have brought the bed count up past 70. And, after making another investment in state-of-the-art imaging facilities, last year Doctors Hospital opened its new Outpatient Diagnostic Imaging Center. When it comes to personal care, Doctors Hospital still considers itself a small town hospital at heart, now with the tools to take that care to an even higher level. In addition to the diagnostic superiority offered by the new imaging center and equipment, Doctors Hospital offers many other advanced services and accommodations to its patients, including luxurious private rooms, fast emergency care, pharmacy, ICU/CCU and obstetrical services and weight loss surgeries. Doctors Hospital has been named a Certified Quality Breast Center www.sanjoaquinmagazine.com
of Excellence by the National Quality Measures for Breast Centers (NQMBC) program. This is due to the exceptional level of care and technological achievements of its Women’s Imaging Center. Imaging techniques there include Digital Mammography, Breast MRI, Breast Ultrasound/Breast Ultrasound Biopsy, Stereotactic Core Biopsy and MRI Guided Breast Biopsy. In 2012, Doctors Hospital received the Get With The GuidelinesHeart Failure Gold Plus Quality Achievement Award from the American Heart Association for its excellence in the treatment of patients with heart failure. Doctors also received the Environmental Excellence Award from Sterilmed, Inc., for reducing environmental impact and improving health care sustainability through their recycling of single-use medical devices.
Kaiser Permanente, Manteca Medical Center 1777 W. Yosemite Ave., Manteca (209) 825-3700 www.kp.org/centralvalley
The Kaiser Permanente Manteca Medical Center is a non-profit, privately owned acute care hospital. It was established when the hospital that formerly occupied the site— CHW/St. Dominic’s Hospital—was purchased by Kaiser Permanente in 2005. It currently provides 99 licensed beds and has been expanded to approximately 81,390 square feet. The Kaiser Permanente Manteca Medical Center specializes in general surgery and emergency services. It also has a well-regarded Skilled Nursing facility that provides round the clock care of the elderly as well as younger adults with physical or mental disabilities. Dedicated to providing quality service to both their members and others in the Manteca community, Kaiser offers a variety of classes and other programs to keep people well-informed and educated about their health. Just some of these classes include how to manage diabetes, weight control, family planning, living with COPD and smoking cessation. Together with its counterpart in Modesto, the Kaiser Permanente Manteca Medical Center was given The Joint Commission Gold Seal Accreditation in 2010 (valid for three years). And again in 2012, The Joint Commission named Kaiser Permanente Manteca Medical Center a Top Performer (in the top 18% of TJC accredited facilities). It also earned an “A” rating for safety by The Leapfrog Group. The medical center was also a recent recipient of the HIMSS (Health Information Management Systems Society) Stage 7 Award for their advanced directives in the area of information technology. This is due to Kaiser’s formation of an integrated delivery system for smooth communication between departments, paperless files and online access to medical records.

Kaiser Permanente, Stockton Medical Offices 7373 West Lane, Stockton (209) 476-2000 www.kp.org/centralvalley
The Stockton Medical Offices supports the Manteca Medical Center—and its members in the Stockton area--with a comprehensive array of outpatient medical care and specialty departments. When Kaiser Permanente first began providing services in Stockton in 1985, they did so with five physicians and 4,000 members. Today, the Stockton Medical Offices has more than 160 physicians and 900 support staff caring for nearly 120,000 local members. Their offerings run the gamut of outpatient health care from A to... well, W, starting with adult medicine and an allergy clinic through women’s health and wound care, and covering just about every health issue in between. The medical offices also offer a wide range of additional services and amenities, including an advice nurse, continuing care outreach program and work site wellness programs. The Stockton Medical Offices is proud to be a member of the Stockton community. That’s why, in addition to their health care offerings, they look to sustain a further-reaching sense of wellness by providing donations and volunteers, and otherwise collaborating with a variety of non-profit organizations, including the 9 to 90 Reading Program, Boys and Girls Clubs of Stockton, the Child Abuse Prevention Council, the Healthier Communities Coalition and the award-winning Educational Theatre Program. The breadth of services and the associated quality of care has not gone unrecognized. In January of 2013, the Stockton Medical Offices received The Joint Commission Gold Seal accreditation for Ambulatory Care.
Lodi Health Lodi Memorial Hospital 975 S. Fairmont Ave., Lodi (209) 334-3411 lodihealth.org
In 2012, Lodi Memorial Hospital celebrated its 60th birthday. After the inception of Lodi Memorial Hospital Association, Inc., in 1945, it built a hospital that opened in 1952. Since then, it’s started a home-health agency, a durable medical equipment company, an adult daycare center, a fitness center, a childcare center and 14 medical practices, and built two outpatientsurgery centers. In all, there are 58 separate programs and services operated. By 2012, the moniker “hospital” no longer described Lodi Memorial Hospital’s services. It had evolved into a health care “system.” So at the start of 2013 the name was changed to Lodi Health, which is

& MEDICAL GUIDE 2013

now the parent company for all the health businesses in the system. The hospital remains Lodi Memorial Hospital, but the name Lodi Health is associated with all its health endeavors. Lodi Health currently operates a 214-bed acute-care hospital and 14 medical offices throughout Lodi, Stockton, Galt and Ione. It offers a full range of inpatient services, including maternity, medical/surgical, ICU, telemetry and acute-physical rehabilitation. Meanwhile, Lodi Health and its various physician practices—family care, prenatal care, pediatric care, occupational medicine, wound care and specialty care—provide a wide range of outpatient services, such as physical, speech and occupational therapy, laboratory and diagnostic imaging. In addition to the breadth of services offered, Lodi Health is also dedicated to keeping such services and associated departments as up-to-date as possible. Just some of their recent advancements include:
• Bringing additional physicians to the area. • Collaborating with physicians and other providers to form an accountable-care organization (ACO).
• Working with physicians to form a non-profit medical
foundation to purchase and manage physician practices.
• Achieving “Stage One Meaningful Use” status for
use of certified electronic-health record (EHR) technology.
• Initiating a telemedicine program for stroke and pediatric
emergency patients in partnership with UC Davis, providing 24-hour access to specialist care. • Installing new video-interpreting program for hearing-impaired patients to provide timely access to interpreters. • Purchasing a $1.8 million surgical robot in 2012. Robot-assisted surgeries are more precise and less invasive, with less pain, scarring and blood loss. • They also create better surgical outcomes and quicker recoveries.
San Joaquin General Hospital
500 West Hospital Road, French Camp (209) 468-6000 www.sjgeneralhospital.org
Established in 1857 by San Joaquin County, San Joaquin General Hospital (SJGH) relocated to its current location in French Camp in 1895. Since then, SJGH has continually expanded with the construction of a new inpatient tower in 1996, additional outpatient clinic space in 2001 and a new laboratory building in 2006. Under the current leadership of CEO David Culberson, SJGH continues to stay up-to-date with the latest technology, techniques and facilities. For example, its digital radiographic services include CT Scans, nuclear medicine, a cardiac catheterization lab, ultrasound and general radiography and, as of 2011, a digital stereotactic mammography suite. The hospital currently provides 196 licensed beds, including 16 in Intensive Care, 26 in the Perinatal unit and 25 in the Intensive Care Newborn Nursery. SJGH is particularly proud of its labor and delivery section, with comprehensive obstetrical service (including an in-house obstetrician available 24/7), certified nurse midwives, a dedicated

& MEDICAL GUIDE 2013
obstetrical anesthesia service and a Neonatal Intensive Care Unit with a 24/7 transport team. The hospital’s presence extends far into the community with a variety of clinics in French Camp and Stockton. The numerous specialties of these clinics range from primary care to pediatrics and Ob/Gyn, as well as extensive outpatient services like diagnostic imaging, laboratory services, physical, occupational and speech therapies and outpatient dialysis. SJGH also excels as a teaching hospital, with Resident Physician programs in General Surgery, Internal Medicine and Family Medicine. The General Surgery resident physician training program at SJGH was recognized as one of only two programs in the U. S. that have had 100% of graduates pass the American College of Surgery Board Certification exam each year over the past ten years. Many of the community physicians in San Joaquin County trained and completed their residency programs at SJGH. Development continues in the Primary Care Clinics and Patient Centered Medical Homes, which will offer more coordinated and focused access and improved services to patients with diabetes. This Medical Home model will be administered by attending and resident physicians, psychiatrists, licensed social workers, registered dieticians, nurses and other specially trained staff. In August 2012, the Primary Care physicians at SJGH received recognition by the National Committee for Quality Assurance (NCQA) for its diabetes program.
St. Joseph’s Medical Center
1800 N. California Street, Stockton, CA 95204 (209) 943-2000 www.StJosephsCares.org
St. Joseph’s Medical Center is a not-forprofit, fully accredited hospital with 366 beds, a physician staff of over 400 and more than 2,400 employees. It is the largest hospital, as well as the largest private employer, in Stockton. It has consistently been chosen as the Consumer’s Choice “Most Preferred Hospital” in studies by the National Research Corporation. New technologies and techniques like minimally invasive surgeries and video-assisted procedures put St. Joseph’s at the forefront of cardiac care. This is why St. Joseph’s was the first to be designated a receiving center for STEMI (heart attack) patients. St. Joseph’s Heart Center features three state-of-the-art catheterization labs, two specially designed heart surgery suites, a complete diagnostic program and cardiac navigator services to answer patients’ and families’ heart-related questions. In 2013, CareChex, one of the nation’s largest healthcare information services companies, ranked St. Joseph’s in the Top 10% Nationwide for Interventional Coronary Care, as well as one of the top 100 hospitals nationwide for major cardiac surgery. HealthGrades concurred, also ranking St. Joseph’s as one of America’s 100 Best Hospitals for Cardiac Care. Since 1988, St. Joseph’s Regional Cancer Center (SJRCC) is the only cancer center in San Joaquin County accredited by the American College of Surgeons Commission on Cancer. This is due to cutting edgetechnology like the new $4 million Varian Trilogy linear accelerator, and services that include Intensity Modulated Radiation Therapy www.sanjoaquinmagazine.com

There are also a Cancer Resource Center, Cancer Navigator and a full spectrum of patient and family support services. Access to clinical trials has been bolstered by collaboration with the UCSF Helen Diller Family Comprehensive Cancer Center. Other recent developments include the acquisition of a state-of-the-art MRI, making St. Joseph’s the only hospital in the area with an on-site MRI. The hospital is also undergoing construction for a state-mandated seismic retrofit. It will be completed in November 2013, making the hospital perfectly safe in the event of an earthquake.

Sutter Tracy Community Hospital 1420 N. Tracy Blvd., Tracy (209) 835-1500 www.suttertracy.org
It was through the generous donations and support of Tracy community businesses and private citizens that Sutter Tracy Community Hospital was established in 1948. Originally called Tracy Community Memorial Hospital, the hospital became affiliated with Sutter Health in 1993 and changed its name to what it is today, Sutter Tracy. Sutter Tracy continues to maintain Sutter’s high standards of care, an effort that has met with notable success. In recognition of the hospital’s focus on excellence, The Joint Commission named Sutter Tracy a Top Performer on Key Quality Measures in 2012, specifically in the areas of pneumonia treatment and surgical care. The hospital was also a top performer in the Patient Safety First program, reducing two instances of hospital-acquired infection to zero. And Sutter Tracy was given an “A” Hospital Safety Score by The Leapfrog Group. Sutter Tracy currently provides 82 beds in its fully accredited, not-for-profit, full-service medical center, and is able to boast state-of-the-art technology and highly skilled physicians representing nearly every medical specialty. In 2004, a major expansion project remodeled and enlarged what are now Sutter Tracy’s pride and joy: their Emergency Department and Family Birthing Center.

Medical Director
Physician Advisor
“I enjoy working with community physicians, specialists and hospitals to decrease avoidable hospital readmissions.” Dr. Golden believes that Arbor’s new Congestive Heart Failure Rehabilitation program will be a great asset for the community, community physicians, facilities and congestive heart failure (CHF) patients.”
My main goal in practicing cardiology is to help my patients keep their hearts healthy. I believe this starts with understanding and controlling risk factors for heart disease, in addition to making lifestyle changes as necessary. I want my patients to enjoy healthy and happy lives with the fewest possible compromises. To achieve this, I pay careful attention to the specific needs of each person that comes into my care. I believe in creating a shared atmosphere where patients are encouraged to ask questions, and in return, given easy-tounderstand explanations of their condition and treatment options. I look forward to treating my patients through education, prevention and utilization of advanced treatments for effective disease management, including cardiac rehabilitation. My family is my greatest achievement, but after that, I’m honored that I can help motivate patients toward better cardiac health, and save lives.
From our family to yours, Happy Mother’s Day 56

STOCKTON CARDIOLOGY MEDICAL GROUP was established in the mid-1950s. Our philosophy remains steadfast in that “it is a privilege to care for the people of the San Joaquin, Calaveras and Tuolumne communities.” Our relationships with our patients continue to enrich our lives. Today, the pace of medicine is so fast that there are multiple interventional options to prevent heart attacks, even before they happen. Stockton Cardiology offers a comprehensive heart and vascular program that includes cardiac risk factor assessment for the prevention of coronary artery disease (CAD), invasive and noninvasive cardiovascular evaluation, intervention for congenital heart disease, intervention for coronary artery, electrophysiology service and peripheral vascular intervention. Complex coronary and peripheral endovascular interventions, including carotid stenting and limb salvage procedures are areas of special interest for the physicians of Stockton Cardiology.

Salida Surgery Center Dental Group Children’s Dental Care While They Sleep 5712 Pirrone Road Salida, (209) 543-9299 www.SalidaSurgeryCenter.com Salida Surgery Center, Dental Group is dedicated to the well-being of oral health in children. We understand that it is very difficult for some parents to obtain quality dental care for their young ones, especially when they are uncooperative and fearful of the dentist. We are here to give parents an alternative other than strapping their child to a papoose board. Dental treatment is usually completed in just one visit! Our dentists believe in the prevention of severe dental caries and that starting at a young age with good oral hygiene habits will continue throughout their lifetime. We like to educate our parents on how to establish good oral habits and we hand-out educational materials for reading and future reference. Oral health is a very important factor in a child’s development and should be taken seriously. Each of our staff members is an important essential part to our specialized team and we are pleased to have them as a part of our family. Our staff promotes a welcoming environment where children and their families feel at ease. We find that many of our parents have been greatly assisted by our bi-lingual staff members. When you call we will be happy to answer all of your questions and make your appointment. Please visit our website at: www.SalidaSurgeryCenter.com to meet our Doctors, staff, and read our valuable dental information.

Kids Care Dental Group Children’s Dentistry
Stockton Office (209) 957-4386 3485 Brookside Rd., Suite 101 Outpatient General Anesthesia is recommended for apprehensive children, very young children, and children with special needs that would not work well under conscious sedation or I.V. sedation. General anesthesia renders your child completely asleep. This would be the same as if he/she was having their tonsils removed, ear tubes, or hernia repaired. This is performed in a hospital or outpatient setting only. While the assumed risks are greater than that of other treatment options, if this is suggested for your child, the benefits of treatment this way have been deemed to outweigh the risks. Most pediatric medical literature places the risk of a serious reaction in the range of 1 in 25,000 to 1 in 200,000, far better than the assumed risk of even driving a car daily. The inherent risks if this is not chosen are multiple appointments, potential for physical restraint to complete treatment, and possible emotional and/ or physical injury to your child in order to complete their dental treatment. The risks of NO treatment include tooth pain, infection, swelling, the spread of new decay, damage to their developing adult teeth, and possible life threatening hospitalization from a dental infection.

Do you want to restore that youthful smile or replace missing teeth? Do you want to prevent dental problems? Do you have a toothache? We all have different priorities and reasons for going to the dentist, but one thing is certain: We don’t want to spend a lot of time there. Our goal is to care for your dental health while making everything as convenient, comfortable, and economical as possible. Today’s high technology allows dental work to be done in much less time and with minimal expense, but still provides long lasting, esthetic restorations. Experience for yourself our family-like atmosphere and personal service. Priscilla from Modesto says, “I couldn’t believe I got my cosmetic work done in one appointment, with no temporaries.” John from Stockton says, “I’ve had root canals and crowns done before, but I’ve never had a root canal and crown done in one visit. I’ll never go back to the old way of fixing teeth.” Brian from Missouri says, “I was in Stockton with a dental emergency and Dr. Noriesta fixed my tooth in one appointment. I couldn’t find a dentist like Dr. Noriesta in Missouri, so I drove back to Stockton and got the rest of my crowns done in one appointment.” To find out more, contact us at (209) 472-7500 or visit us online at www.oneappointment.com. www.sanjoaquinmagazine.com
