Q. You’ve been a plastic surgeon for many years. What have been some of the biggest changes in the field changed over the course of your career? A. Industry and plastic surgeons have helped develop many new products and techniques to help slow down the signs of aging in our patients. Q. Who is a good candidate for bariatric surgery? A. When it comes to who qualifies for bariatric surgery, it’s based on the National Institute of Health’s criteria from 1991, and it essentially boils downs to your body mass index (BMI) and the presence or absence of diabetes, high blood pressure or sleep apnea. For instance, an ideal range for BMI is 20-25. Overweight is considered 25-30, type 1 obesity is 30-35, type 2 obesity is 35-40 and morbid obesity is 40 and higher. Once a patient’s BMI is in the range between 35-40, if they have either diabetes, high blood pressure or sleep apnea, that’s when they first become a candidate for bariatric surgery. If a patient’s BMI is 40 or higher, they are a candidate regardless of whether or not they’ve developed any of those three medical problems or not. The ideal candidate is someone who is personally motivated and ready to make a change. Bariatric surgery is the single most effective tool we have to get a significant amount of weight off and keep it off, but there’s a lot more to long-term success than the operation. When you need us: The good news is the majority of medical situations most people face every day are not life threatening. And while sprains, strains, minor breaks, cuts, common colds and flu are no fun, they are all treatable at an urgent care. The even better news is that by appropriately choosing urgent care over an ER, you’re going to save significant time and money. There are some medical conditions that are considered emergencies because they can require rapid or advanced treatments (such as surgery) that are only available in a hospital setting. For instance, if you’re experiencing chest pain or difficulty breathing, severe abdominal pain, weakness or numbness on one side, slurred speech, fainting/change in mental state, vaginal bleeding with pregnancy or if you have sustained serious burns or a severe head or eye injury, it’s best to head to the ER. How insurance works: Urgent care centers can be in-network with your insurance company and often are, but not always. Urgent care centers, like ExperCARE, who are in-network with major insurance companies, should file the claim for you with your carrier, just like you are used to when going to the doctor. The reimbursement for that claim is subject to your particular plan and the benefits associated with it, just like a visit to any in-network provider. Urgent care centers are a great point of access to healthcare for those without insurance as well; with affordable cash pay rates, a wide-range of services offered and transparency regarding pricing and options, this is a great choice of place to get the care you need without breaking the bank.

Q. There are so many kinds of professionals who provide counseling. What’s the difference? A. Several different types of professionals provide counseling and psychotherapy (terms which are often used interchangeably). The most common are psychiatrists, psychologists and social workers. All of these professionals are trained to provide psychotherapy; the differences lie in their educational requirements, extent of their training and licensure requirements.
Psychologists have attended graduate school in psychology for four to six years of academics followed by one to two years of full-time supervision, working with clients during residency. Successful passing of oral and written state and national board examinations is then required to practice in each state. Some states have
Social workers attend graduate school typically for two years of coursework and supervised training working with clients in a variety of community and hospital settings. They too must pass exams in order to obtain a license to practice within each state.” As to whom to call to make that first appointment, it depends most on what you’re looking for. Male or female? Specialist or generalist? Or do you think you have a specific problem such as Obsessive-Compulsive Disorder or Attention-Deficit Hyperactivity Disorder or an addiction for which you need to be evaluated? Are you having family or relationship problems? Are you dealing with anxiety, depression, health problems or are you looking to find a therapist to help you maximize your potentialities? Whomever you choose for that first consultation appointment, remember to go prepared. Take a list of questions and be sure to ask about the therapist’s orientation and experience. And finally, you need to feel like it will be a "good fit" between you and the therapist — regardless of their skill and education. In fact, research has repeatedly demonstrated that the relationship between therapist and client is the single most important factor in successful therapy outcomes.

Q. What kinds of new technologies are making heart and lung surgery easier on your patients? A. Transcatheter Aortic Valve
Replacement (TAVR): This alternative to open-heart surgery uses a minimally invasive procedure to replace the aortic valve. We pass a catheter with an attached valve through the artery in the groin and then thread the catheter all the way up to the heart. There we to replace the aortic valve right at the top of the heart. Some patients go home one or two days after the surgery

Impella Heart Pump: This is a heart-assist device for patients who have had a heart attack and their heart muscles are too weak to pump well. Again, we insert the pump in the groin and up through the aorta but then we go into the left ventricle — the main pumping chamber of the heart. The Impella pump is very small — the size of a capsule — and it sucks the blood from the left ventricle and ejects it into the aorta above the aortic valve, making it easier for the heart to pump. Additionally, the device can remain in place for a couple of days until the heart recovers. Extracorporeal Membrane Oxygenation (ECMO): “Even if a patient is on a ventilator, they may not be getting the oxygen they need. To combat this, newer techniques with smaller catheters and much more efficient pumps have been developed, such as ECMO, which temporarily takes the place of the lung. A catheter goes in the neck and then down towards the heart where it sucks unoxygenated blood out. That blood goes through an external pump about the size of an oil filter that oxygenates the blood and pumps it back in. ECMO is great because it allows time for the lungs to recover from the flu or other conditions.
