If you would like to have your payments automatically withdrawn from your account, you will click on “I authorize”. Once you click on “I authorize” you will be prompted to fill out the portion of the form shown here. You will need to fill out the ABA number, account number, account type you would like it withdrawn from and draft date that you would like it to be withdrawn. It will also require your email address and phone number to be provided.
When you are done, you will be able to click finish.
