arty Ryan, the former owner of M Marty’s Sneaker World in Honesdale, is grateful he has both his legs and

that he’s still in the shoe business after 35 years. Wayne Memorial Hospital staff recently surprised Ryan with a cake at his current shoe store in Waymart to congratulate him on his anniversary and his success at battling a debilitating disease – lymphedema. Ryan, 67, was quick to thank them for much more than the cake. “I wouldn’t have my leg here,” he said, pointing to his right leg, which was wrapped in compression bandages, “and maybe both of them if it hadn’t been for these folks. I owe the doctors and nurses who worked so hard to heal my many wounds and these therapists a lot!” The praise was reciprocated by Amy Krempasky, RN and clinical coordinator of the hospital’s Advanced Wound Care & Hyperbaric Center and Occupational Therapists Christine Carlozzi and Sean Phillips. “Marty would not be where he is today if he hadn’t shown up for his appointments and done the work,” said Krempasky. “He was vigilant and diligent and that’s why he still has his leg.” Ryan, a US. Army veteran who served in Korea during the Vietnam War period, admitted he was not an easy patient when he was first diagnosed in 2007.

“When the Wound Care Clinic staff wanted me to start therapy, I was definitely not interested,” he said, “I didn’t like change and I fought it all the way. But finally the nurses convinced me I had no other choice if I wanted my legs to get better. I had to give in, and now, I’m glad I did! Ryan undergoes treatment in the Wound Care Center with Edward (“Ted”) Murray, DPM, the Wound Care team and Carlozzi and Phillips. The two therapists underwent training supported by Wayne Memorial Hospital to become certified in Compete Decongestive Therapy (CDT) for lymphedema. A chronic condition in which excess protein-rich fluid collects in the tissues, lymphedema causes extreme swelling and possible skin changes in the body that don’t go away and may even get worse over time. Carlozzi and Phillips use the Vodder Technique which moves the lymph fluid through and out of the body through very gentle skin stretches along the normal pathways of the lymphatic system. This technique is combined with skin care, compression bandaging and specific exercises. .” “I’m really happy to share my own story if it can help even one other person out there who has lymphedema to seek help from these folks,” said Ryan. Wayne Memorial Rehabilitation Services, with two locations in Honesdale, offers physical, occupational and speech therapy, as well as specialty therapies for patients with lymphedema, Parkinson’s Disease and cancer. hannon Bruning felt the bite, swatting away the tiny dark insect from her lower right leg. She’d been standing amongst the tall grass of her Kauneonga Lake back yard, watching husband Butch test a new weedwhacker. But within seconds of the bite, “I had a blister bigger than a silver dollar,� she recalled. “And the itch was unbelievable.� Ice, peroxide, anti-itch cream – nothing salved it, and Shannon couldn’t resist scratching. That burst the blister, and suddenly she was confronted with a nasty open wound straight out of a horror movie. “The black and yellow are poison,� Shannon pointed out during a visit last week. She was changing the dressing on a painful, two-inch-long gash in her leg – four months after the initial bite. her it could take another eight months for the wound to heal. And she will likely never know exactly what bit her. Shannon thinks it could be a brown recluse spider, but their range doesn’t extend this far north. “They’re not common here,� affirmed Sullivan County Public Health Director Nancy McGraw. “So we really can’t say whether it was a brown recluse spider or not.� Allergic reactions, immune system problems and cardiovascular issues can all aggravate insect bites, she added, meaning a person’s particular reaction to a bite may be very different from another’s – whether it was a spider or something else. McGraw sympathized with the alarming nature of Shannon’s wound but said brown recluse spider bites are not a local problem. “From a public health perspective, it’s not something the average person should be concerned about,� she assured. Indeed, “if I were to caution anyone to be more vigilant about bugs,� said McGraw, “it would be ticks.� A bite from a tick may or may not be noticeable, but they can transmit Lyme Disease, which, left untreated, can have long-term health effects. Regardless, if you’re wearing clothing that exposes arms, legs or other parts of the body to vegetation, be ready for insect bites in Sullivan County – and to react swiftly. Shannon initially visited her doctor, who gave her antibiotics to stave off further complications. The team at Hudson Valley Hospital Center helped her recover from an infection in the wound, but she’s still got a long road to travel. “This is $274 a tube,� she remarked, holding up a container of Santyl,

which helps her clean out the dead tissue three times a day. “I’m going to go broke!� And Butch still has to take her down to Peekskill weekly for further cleaning and inspection of the bite area. Some of that’s covered by Medicare and private insurance, some not. The itch, thankfully, is gone, but so is Shannon’s independence. “I haven’t driven, but I can walk. Sometimes it’s very painful,� Shannon related – made more painful by the fact she broke her toe after stubbing it on furniture in her home. The grandmother of eight misses visits with family, though Butch and their dog Sam have been constant companions. Her pets, in fact, are what brought her from Westchester County to Sullivan County 14 years ago. “I wanted a place where I could have my animals,� she said with a smile. She didn’t count on the kind of critters that might feast on – or be irritated by – her presence, however. So she’s spreading the word to be careful. “I want people to be aware of this,� she affirmed, “that there’s something here that bites you and eats your flesh away.�

ack in the Fall of 1984, Dr. Rajan Gulati was the wellrespected Director of the Emergency Department at the nowclosed Horton Memorial Hospital in Middletown. After a decade in this position, he decided to branch out on his own and open “Immediate Medical Care,� walk-in Urgent Care services and the first of its kind in the region, at a small location on Ridge Street in Middletown, just down the road from Horton, and with just three employees. CRMC named designated Stroke Center

HARRIS — In recognition of its expertise in treating stroke patients, Catskill Regional Medical Center’s Harris Campus has been named a designated Stroke Center by the New York State Department of Health. According to New York State protocol, emergency responders should always transport stroke patients to the nearest designated stroke center when they present symptoms and signs of a stroke. As a designated Stroke Center, Catskill Regional has an expert Stroke Team comprised of highly trained physicians, nurses and technicians that can treat stroke quickly and effectively, using the latest medicines and techniques, to minimize brain damage. Having quick access to these types of services within the first three hours of showing symptoms of a stroke is essential to preventing long-term neurological

damage. “This designation signifies our ability to rapidly assess our patients and get them the treatment they need, states Chief Medical Officer and Chief Executive Officer, Gerard Galarneau, MD. “Now, people who show signs of a stroke can be cared for right in their own community, receiving quality stroke care and minimizing the affects that a stroke can cause. ” A stroke, or brain attack, occurs when a blood clot blocks a blood vessel or artery, or when a blood vessel breaks, interrupting blood flow to an area of the brain. When a brain attack occurs, it kills brain cells in the immediate area. Doctors call this area of dead cells an infarct. These cells usually die within minutes to a few hours after the stroke starts. “We want to remind area residents to call 911 immediately if they begin

to experience symptoms of a stroke — sudden numbness or weakness of the face, arm or leg, especially on one side of the body; sudden confusion; trouble speaking or understanding; sudden trouble seeing in one or both eyes; sudden trouble walking, dizziness, loss of balance or coordination; and/or sudden, severe headache with no known cause,” said Medical Director of the Stroke Center Dr. Carlos Holden. “In the case of a stroke, every minute counts.” Additionally, Catskill Regional’s Harris and Callicoon Campuses have constant contact with qualified specialists, regardless of the time of day or weather, through the use of “Doctor Polly” or a “Doc on Wheels.” Doctor Polly is state-of-the-art technology used to help diagnose stroke and other illnesses by remote consultation. ERH earns ‘Gold Seal’ for mammography ELLENVILLE — Ellenville Regional Hospital has been awarded a three-year term of accreditation in mammography as the result of a recent review by the American College of Radiology (ACR). Mammography is a specific type of imaging test that uses a low-dose x-ray system to examine breasts. A mammography exam, called a mammogram, is used to aid in the early detection and diagnosis of breast diseases in women. The ACR gold seal of accreditation represents the highest level of image quality and patient safety. It is awarded only to facilities meeting ACR Practice Guidelines and Technical Standards after a peerreview evaluation by board-certified physicians and medical physicists who are experts in the field. Image quality, personnel qualifications, adequacy of facility equipment, quality control procedures, 18817

and quality assurance programs are assessed. The findings are reported to the ACR Committee on Accreditation, which subsequently provides the practice with a comprehensive report they can use for continuous practice improvement. MIDDLETOWN — Middletown Medical announced the acquisition of George Giovannone Physical Therapy of Middletown, one of the region’s most successful such practices since 1992. T Stay fit when temperatures drop . . . winter is no time to stop exercise routines

M

any animals hibernate throughout the winter. Humans are afforded no such luxury. In fact, living a sedentary lifestyle during the colder months of the year can actually prove detrimental to human health. Exercise is beneficial any time of the year, but it can be especially so during the winter months when colder temperatures force many people inside, where they live more sedentary lifestyles than they do during the rest of the year. Adapting your exercise habits in the winter can help you make it through the colder months in great shape. There is no reason to stop exercising when the temperature drops. The American Heart Association says working out in the cold weather has distinct advantages over working out in hot and humid conditions. When the weather is cold, you may be able to work out longer and harder because the heat won't zap your energy levels, and exercising outdoors in the winter is a great way to get small doses of sunlight that can improve mood and help your body produce more vitamin D.

The Centers for Disease Control and Prevention says exercise can help boost your immune system, which can help you fight colds and flu symptoms. Just a few minutes of exercise each day can help prevent simple viral and bacterial infections as well. Working out in the winter may help you burn more calories than in warmer seasons. Research published in Medicine & Science in Sports & Exercise found that race times are faster in cold weather than in warmer temperatures. Quicker runs or walks can burn more calories. If exercising outdoors is too uncomfortable, break up your routine into smaller, more manageable sessions. Aim for 10 minute sessions several times per day. This quickly can add up to the 30 minutes of daily recommended exercise. Dress in layers so you can feel comfortable, adjusting your clothing as necessary. You don't want to be freezing, but you don't want to wear so many clothes that you start sweating and risk hypothermia. Consider less traditional exercises when winter sets in. These include shoveling snow, sledding, skating, skiing, and snowshoeing.

The relationship between obesity and type 2 diabetes ways and can put more pressure on the body's ability to properly control blood sugar and use insulin. Carbohydrate-rich diets full of simple sugars and refined foods can cause unhealthy spikes in blood-glucose readings. Individuals who overeat and lead sedentary lifestyles may not burn enough energy, and excess calories are then stored as fat, compounding the problem. The Obesity Society says type 2 diabetes is largely preventable, and research shows that lifestyle changes that include small amounts of weight loss (between 5 and 10 percent of body weight) can prevent or delay the onset of type 2 diabetes. In fact, managing weight is the best thing a person can do to reduce their risk for

diabetes. Losing as few as 10 pounds can markedly improve insulin resistance and make it much easier to manage blood sugar levels. In addition to losing weight, eating a healthy diet and exercising regularly can reduce a person's risk of developing diabetes. Lean proteins, leafy green vegetables and complex carbohydrates will not spike blood sugar levels. Exercise helps burn calories and boost metabolism, and may even help the body use insulin more effectively. Type 2 diabetes can be a chronic, debilitating condition that leads to other illnesses. But type 2 diabetes is largely preventable, and losing weight and maintaining a healthy weight are great ways to lower your risk of diabetes.
