Email: head@oswaldtwistle.org REFERRALS TO OSWALDTWISTLE SCHOOL FOR YOUNG PEOPLE WITH ADDITIONAL MEDICAL NEEDS Pre-referral: We welcome non-prejudicial visits from parents, children and agency professionals at mutually convenient times. The visit will include an opportunity to meet the Headteacher, Medical Lead, Teachers, Teaching Assistants and Learning Mentors subject to availability. Visitors should feel free to ask any questions and/or raise concerns at any time during the visit. MAKING A REFERRAL • Requests for provision for students should be sent to school or emailed to: referrals@oswaldtwistle.org or, medical@oswaldtwistle.org • All relevant education and medical information must be appended. • Referrers should note that Oswaldtwistle School has a limited number of places available and will offer a place subject to an induction period to be agreed. ADMISSIONS MEETING • This will be arranged at the earliest opportunity and, at least, within 10 days of referral. It will be attended by a Core Group of representatives from; Oswaldtwistle School; referring school; referring professional(s); Carers/student. • • • • • •

An Individual Learning Plan will be drafted with agreed targets for attendance; academic progress; social and emotional development; agreed intervention. External agency support and completed support plans will be addressed. A date will be set for baseline assessment, if required. Transport will be arranged and agreed where required. A start date will be set for the induction period and an initial review date. Regular contact arrangements will be agreed.

REVIEW PROCESS AND CYCLE • Reviews will take place every half term to be attended by the Core Group. • The following will be reviewed; attendance, progress (social, emotional, educational and medical), intervention, barriers to learning, and reintegration. • Alterations will be made to provision as and where agreed. • Target attendance rate of above 90% (of allocated provision) is an expectation. • Where full time attendance is not deemed appropriate, there is an expectation that an agreed distance learning timetable will be undertaken by students.
