Letter from Schweiger Dermatology Group’s Founder The summer months are finally upon us and we can feel the excitement in our offices. As the sun comes out and many people head to the beaches, I think it is the perfect time to remind people of sun safety and the importance of self-monitoring. As we tell all of our patients, if you see a suspicious spot on

your skin, get it checked out immediately. One in five people will develop skin cancer in their lifetime. Early detection is the best way to prevent skin cancer from becoming serious, and vigilant sun protection is the most effective method of preventing skin cancer in the first place. In this issue of The SkinNY, our providers share their best sun-safe secrets. We have many new faces at Schweiger Dermatology Group. With the recent addition of our Smithtown, Long Island and Midtown West offices as well plastic surgeon Dr. Joshua Zuckerman, our company is growing to meet the needs of patients all over the Metro

Avoid sun exposure during the peak hours of 10am - 3:00pm.

Play in the shade: Under trees, under an umbrella at the beach or bring popup tents.

Wear sun-protective hats to shield against ultraviolet radiation to the face, head and neck.

4.

Wear sun-protective clothing: long sleeves (breathable cotton) rash guard swimsuits with built in sun protection (My faves are uvskinz.com and coolibar.com).

5.

Wear sunglasses protection.

6.

Wear broad spectrum sunscreen (protects against UVA & UVB radiation) SPF 30 or higher even on cloudy or overcast days. UV rays can still penetrate clouds.

7.

Apply sunscreen 30 minutes before planned sun exposure.

8.

Re-apply SPF every two hours and immediately after if the child has been sweating or swimming.

9.

Use water resistant sunscreens when the kids are going to be in the water.

10. Apply SPF generously, recommendation is one ounce (enough to fill a shot glass) to cover the exposed areas of the body. 11. 13. Teach children how to apply SPF themselves. The solid sticks are great for this—they’re very easy for them to use on face and super-portable.

20. Make applying SPF part of your morning routine, just like brushing their teeth, and they won’t think twice about doing it.

14. Check your children’s medications. Many antibiotics and some supplements are photosensitizers and will make the child more sensitive to sun. If they are going to be exposed, make sure to avoid direct sunlight and they should be in protective clothing and a hat with a wide brim.

“If you’re not so fond of your eyelids anymore, but don’t want surgery, you can get a combo treatment of Thermage and Botox. 5. STIMULATE HAIR REGROWTH While balding is typically seen as a man’s issue, thinning hair is becoming more of an issue for women, too. Daily stress, which New Yorkers are all too familiar with, can contribute to hair loss. There’s a new way to treat hair loss in the dermatologist’s office that doesn’t rely on time-consuming or painful methods. In Platelet-Rich Therapy (PRP), growth factors and other beneficial cells found in the patient’s own blood are reintroduced back into the scalp via injection to promote hair regrowth where hair has been lost or is thinning. PRP is an autologous treatment, meaning that the blood cells used to stimulate hair regrowth are the patients’ own and are not taken from a foreign substance. The preparation of the platelets is performed using a special centrifuge system that collects the growth factors and active proteins. You may recognize this treatment under a different name, the “vampire facial,” which became famous thanks to Kim Kardashian. 6. REMOVE YOUR DOUBLE CHIN A saggy chin and neck area is probably one of the most loathed trouble spots. This area (medically referred to as submental fat) has been impossible to treat without surgery until now. Enter Kybella, the first non-surgical treatment that can treat your turkey neck. Kybella uses a nonanimal/non-human substance called deoxycholic acid, which is naturally found in our bodies. When injected into the subcutaneous fat under our chins, deoxycholic acid destroys the fat cells…for good. In other words, once your double chin has disappeared after Kybella injections, it won’t return. Genius.

It is a minimally invasive procedure, with only very small incisions for both fat harvest and grafting. Fat grafting can be performed on its own, or in conjunction with another procedure such as a facelift, to provide a natural restoration of youthful attractiveness. WHAT’S A MOMMY MAKEOVER? A mommy makeover is the simultaneous performance of both breast and abdominal cosmetic surgery. This usually involves a tummy tuck (abdominoplasty) with a breast augmentation and/or lift (mastopexy). Your hands and fingernails take center stage in the summer. From healthy nail care to hand rejuvenation, Anita Fox PA-C explains how best to care for your digits.

What do healthy nails look like? Fingernails are composed of laminated layers of a protein called keratin and grow from the area at the base of the nail under your cuticle. Healthy fingernails are considered to be smooth, without pits or grooves. They’re uniform in color and consistency and free of spots or discoloration. Sometimes fingernails develop harmless vertical ridges that run from the cuticle to the tip of the nail. Vertical ridges tend to become more prominent with age. Fingernails can also develop white lines or spots due to injury, but these eventually grow out with the nail. What is not normal on a nail?

What are your tips on keeping fingernails looking their best? •

Changes in nail color, such as discoloration of the entire nail or a dark streak under the nail

Changes in nail shape, such as curled nails

Thinning or thickening of the nails

Separation of the nail from the surrounding skin

Bleeding around the nails

Swelling or pain around the nails

Dark streaks on the nail

Keep fingernails dry and clean. This prevents bacteria from growing under your fingernails. Repeated or prolonged contact with water can contribute to split fingernails. Wear cotton-lined rubber gloves when washing dishes, cleaning or using harsh chemicals.

Practice good nail sharp manicure clippers. Trim your across, then round gentle curve.

Use moisturizer. When you use hand lotion, rub the lotion into your fingernails and cuticles, too.

Ask your doctor about biotin. Some research suggests that the nutritional supplement biotin might help strengthen weak or brittle fingernails

Don’t bite your fingernails or pick at your cuticles. These habits can damage the nail bed. Even a minor cut alongside your fingernail can allow bacteria or fungi to enter and cause an infection.

Don’t pull off hangnails. You might rip live tissue along with the hangnail. Instead, carefully clip off hangnails.

Don’t ignore problems. If you have a nail problem that doesn’t seem to go away on its own or is associated with other signs and symptoms, consult your dermatologist for an evaluation.

Don’t use harsh nail care products. Limit your use of nail polish remover. When using nail polish remover, opt for an acetone-free formula.

What products do you recommend to keep nails their healthiest? There are tons of over the counter nail products out there but I like Just Natural skin care’s cuticle and nail serum. This formula is specially formulated to nourish, protect, and strengthen nails and cuticles with a special blend of antioxidant and antibacterial all nutritive ingredients: Super plant oil sea buckthorn and kukui nut and more are loaded with vitamins and nutrients that help refresh and restore your cuticles. Jojoba, castor and Manuka oils help to soften, soothe and protect as this formula absorbs quickly. Pure Vitamin E moisturizes and replenishes for healthy nails and cuticles. Another OTC product I like is Barielle’s intensive nail renewal oil. It contains sesame, safflower, sunflower and coconut oil, keratin, vitamin E and calcium. It replicates the body’s natural oils and lipids to restore cuticles and reduce further nail breakage and it contains no formaldehyde or dibutyl phthalate, ingredients which can be drying and damaging to nails.


find out if the salon uses a UVB device to harden the gel or if they use a LCD powered device. If it’s UVB, you should make sure to apply at least a 30 SPF to your hands prior to your appointment to prevent potential sun damage. (Don’t forget, UV radiation is considered the main cause of non-melanoma skin cancers, including basal cell carcinoma and squamous cell carcinoma.

As for prescribed treatments for the nails, I recommend Genadur by Medimetriks Pharmaceuticles. It’s a hydrosoluble nail lacquer that can be used under nail polish to help dry, brittle, splitting nails. It also comes in a kit that contains a 90 day supply of Biotin, a dietary supplement that promotes healthy skin, hair and nails. What are your thoughts about getting manis/pedis at a salon? Is it risky? It seems like everyone gets their nails professionally done these days and I think it fine as long as you take a few precautions beforehand: •

Stick to salons that display a current state license, and work only with technicians also licensed by the state board.

Don’t have your cuticles removed — it can lead to nail infection.

Make sure your nail technician properly sterilizes all tools used during your procedure to prevent the spread of infection.

Ask how the foot baths are cleaned. Ideally, a bleach solution is used between clients and the filters are cleaned regularly.

Don’t be afraid to ask, “Can you show me how you disinfect your pedicure chairs? Can you show me your cleaning log? Are you using single-use files on me?”

A good technician always wears gloves which can help reduce the transfer of bacteria to a client.

Also, never shave your legs before having a pedicure as it creates a portal of entry for bacteria to get into the skin.

Find out if they use a new pumice for each pedicure and new nail files for each new client. Alternatively, you could bring your own files and tools for them to use on you during your appointment.

If you’re getting a gel manicure,

also have common warts, while the bottoms can have plantar warts, which grow inward due to constant pressure between the foot and the ground. All of these are contagious. Warts can be spread if a salon worker uses the same pumice stone or nail file for multiple clients. 2.

Fungus: Tinea pedis (fungus of the skin on the feet) and tinea unguium (fungus of the nails) are fungal infections that require a moist, confined environment to take root and spread. Unfortunately, the pedicure baths of a salon provide a breeding ground. If a spa doesn’t regularly clean its foot tubs between each client, the odds of leaving the spa with a fungal infection you didn’t walk in with increase. Also, fungus isn’t so easily removed from the surfaces it grows on, so a light cleaning may not rid a tub of its presence. Frequent use of an anti-fungal cleaning agent is the best way to prevent spa clients from getting athlete’s foot from a foot tub.

MRSA: Though not common, MRSA is known to spread at nail salons, leading to infections on hands and beneath fingernails. Symptoms usually appear within 24 hours -- you’ll likely experience pain in your hands and be unable to bend or use your fingers with any degree of comfort. The swollen, red skin around the infected area will crack open and ooze pus. It’s hard to miss. MRSA can be spread through the sharing of unsanitized nail files or other nail implements.

Mycobacterium fortuitum: You can get Mycobacterium fortuitum from foot baths and the risk is greatly increased when a soak is preceded by leg shaving. The bacteria causes large boils on the toe, foot or leg. These boils may be surrounded by smaller bumps. Sometimes they heal on their own, but they can linger and even turn into open sores. M. fortuitum boils

It’s easy to overlook these precautions, but taking some basic steps can keep your skin and nails healthy and strong. What infections have you seen people contract from nail salons? One thing available at nail salons that’s not posted on any service menu is a contagious skin disease. With all the hands and feet passing through a salon each day, the odds are solid that some of those diseases could be passed on to you. Here are a few of them: 1.

Warts: Warts are caused by a contagious virus called human papillomavirus (HPV). There are many different strains of the virus (such as the type that can cause cervical cancer), but only a few different kinds spur the overproduction of skin cells that results in warts. Warts spread through person-to-person contact when HPV makes contact with a break in the skin. Hands play host to both common warts and palmer warts. The tops of feet may

can be lanced by a medical professional and treated with extremely potent antibiotics, but unfortunately, these boils and sores can cause heavy scarring. Remember that if a salon is properly cleaning and sanitizing their equipment and if you follow the precautions above, you should be fine. Is it safe to have your cuticles cut? Trimming or cutting cuticles during a manicure is purely cosmetic and doesn’t benefit the nail in any way. In fact, it’s actually harmful to your nails. The small area of skin is there to protect your nails from infection. When this skin is removed, your nail is left unprotected from bacteria and fungus

What treatments do you love for hand rejuvenation? There are several techniques available for hand rejuvenation, some are more invasive than others. These can be used alone or in combination to enhance the overall results. After having a consultation, you will be recommended an approach based upon your treatment needs and aesthetic goals: 1.

Fillers: Dermal fillers, such as polylactic acid, calcium hydroxylapatite or hyaluronic acid, can be used to add volume to your hands and fill in around protruding bones and veins.

Resurfacing: Chemical peels, intense pulsed light (IPL), or laser skin resurfacing may be used to treat fine wrinkles, skin discoloration, sun damage or age spots.

PDT: Photodynamic therapy (PDT) uses a solution with IPL or blue light for aggressive treatment of irregular pigmentation or brown spots.

Pregnancy is a very exciting and anxietyprovoking time in any woman’s life. When you are pregnant, or even trying to get pregnant, there are many changes that take place, especially with your skin care. While there are guidelines from the FDA regarding pregnancy safety categories for ingredients, there are still very few studies evaluating the safety of many skin care products and procedures. For this reason advice may vary from one physician to the next. Most providers believe in a conservative approach to avoid any unnecessary products and procedures during pregnancy as to avoid any complications or feelings of guilt if there are unforeseen issues later in pregnancy. However, in certain situations such as threatening maternal health conditions, the benefits may outweigh the risk and it is best to discuss your options with your physician.

WHAT PRODUCTS ARE SAFE IN PREGNANCY?

Pregnancy Friendly Skin Care

Generally speaking, FDA category A and B ingredients are considered safe in pregnancy. Over the counter products and “organic” products should not be assumed safe just because they do not require a prescription. Herbs and supplements are not regulated by the FDA and should be avoided unless approved by your doctor.

Acne is a very common complaint in pregnant women, and unfortunately many of the effective treatments commonly used are prohibited in pregnancy. Low strength glycolic acid and alpha hydroxy acids are considered safe in pregnancy, however salicylic acid and benzoyl peroxide, which are the most commonly used over-the-counter ingredients are class C, which generally should be avoided. Clindamycin, metronidazole, erythromycin , and azelaic acid are topical prescription medications which are FDA class B and largely considered safe. Blue and Red light therapy is a popular treatment for pregnant woman due to its efficacy, safety, and tolerability. Unfortunately, photodynamic therapy is not advised during pregnancy.

ANTI-AGING RECOMMENDATIONS DURING PREGNANCY Sun protection remains the most important part of any woman’s regimen, pregnant or not. During pregnancy physical blockers such

as zinc and titanium sunscreens are recommended. Neurotoxins, fillers, and laser procedures have not been studied in pregnancy and are best to be avoided. Vitamin C serums, low dose glycolic and alpha hydroxyl acids and microdermabrasion can be continued during pregnancy.

MEL ASMA IN PREGNANCY Pigmentation is a common complaint during pregnancy, especially in the summer. Sunscreens become even more imperative, but hormones themselves are usually the cause and the pigmentation usually lightens on its own after delivery. Glycolic acid topicals can help but it is best to treat the pigment after pregnancy since it may resolve and there are more effective ways to lighten postpartum.

GROWTHS There may be an increased number of benign growths or changes in moles during pregnancy so it is best to get new spots evaluated by a dermatologist. Skin tags, blood vessel growths, and moles can become more numerous or enlarge. Warts are also more common since pregnancy slightly lowers the immune system. If any moles are

significantly darker or abnormal they will be biopsied just as they would be in a non-pregnant woman. Stretch marks are also unfortunately common from the stretching of the skin and are usually more common in women who are genetically prone to them. There are many creams promising to prevent or reduce them, however the jury is still out as to if they actually work. Most dermatologists will advocate moisturizing and massage twice daily, even if only to prevent dry skin that commonly occurs and can lead to itching.

RASHES Aside from the same ailments that plague the general population, there are some rashes that are specific to pregnancy, but also some common rashes that may be harmful in pregnancy when they would otherwise would be considered benign. Chicken pox, parvo virus, and pityriasis rosea are examples of otherwise usually benign rashes that require extra attention in pregnancy. In general, any rash should be evaluated by a dermatologist as to prevent any complications. Common rashes may also worsen, such as dandruff, and your provider may change the regimen to safer products so it is always worth discussing all oral and topical medications with your OB/ GYN at your prenatal visit.

I WAS THERE TO GET COOLSCULPTING DONE. I was a little nervous, but everyone at the office made me feel comfortable. CoolSculpting allows you to remove fat from specific areas on your body. You know those stubborn areas like stomach bulge, love handles and double chin? Those are perfect for CoolSculpting! During my CoolSculpting, a gel pad was placed over the treatment area. The machine then sucked in the fat, almost like a vacuum. The fat cells of the targeted area then freeze and eventually die. Your body naturally gets rid of the dead fat cells over time. My provider explained everything prior and made sure I was comfortable the entire time I was bring treated. The procedure

was done by the amazing Todd Newman, PA-C. Todd did a great job and helped me understand what was happening each step of the way. I had CoolSculpting done on my lower stomach and love handles. These two areas are the ones that have given me

the most trouble. No matter how much I would slim down, there was always a little belly bulge. The first ten minutes of the procedure are the toughest, because it’s a feeling you’ve never felt before. There is a

strong suction, but soon after the area goes numb and you don’t feel anything else. After that each time they placed the machine on a new area, it didn’t bother me at all. Each area where CoolSculpting is performed takes about one hour. It sounds like a long time, but it actually flies by. You can nap, read a book or play on your phone. It takes 2-4 months to see the full results from CoolSculpting. At my first followup appointment one month after the treatment, we compared photos and there is a slight difference already.
