The spotlight of ESC Congress is “Innovation and the Heart”, and this includes cutting edge innovations in basic and clinical science, innovations in clinical applications, in education and in quality improvement. We have more Hot Lines than ever before and 5 important new ESC Guidelines will be presented. Come and see how it can affect real clinical cases. If you are in training or in research you cannot miss the ESC Cardiologists and Scientists of Tomorrow sessions. Basic science has a strong profile, its own village and linked translational sessions. Remember the exhibition, the satellites and new developments from industry partners. ESC Congress is designed to be a resource for the whole year: key sessions will be shown live from Barcelona on the website. On ESC Congress 365, you will find free resources, all year long! The Inaugural Session on Saturday 30 August is a key introduction to the congress and it has some surprises in store – don’t miss it! We are delighted that you have chosen to be part of ESC Congress and together we can help shape the future of cardiology! We know you will enjoy the legendary hospitality of the City of Barcelona and will be part of a great ESC Congress!

Risk estimation using SCORE: Qualifiers
can be estimated using the electronic version of SCORE, HeartScore, which has been updated to include HDL cholesterol level.
4. Establish the total 10 year risk for fatal CVD.
Another approach to explaining risk to younger persons is to use cardiovascular risk age. For example, in the high risk chart, a 40 year old male hypertensive smoker has a risk of 4%, which is the same as a 65 year old with no risk factors, so that his risk age is 65. This can be reduced by reducing his risk factors.

s 4HE CHARTS SHOULD BE USED IN THE LIGHT OF THE CLINICIAN S KNOWLEDGE AND judgement, especially with regard to local conditions. s !s with all risk estimation systems, risk will be over-estimated in countries with a falling CVD mortality rate, and under estimated if it is rising. s !T ANy given age, risk appears lower for women than men. However, inspection of the charts shows that their risk is merely deferred by 10 years, with a 60 year old woman resembling a 50 year old man in terms of risk. s 2ISK may be higher than indicated in the chart in: - Sedentary or obese subjects, especially those with central obesity - Those with a strong family history of premature CVD - Socially deprived individuals and those from some ethnic minorities - Individuals with diabetes- the SCORE charts should only be used in those with type 1 diabetes without target-organ damage; other diabetic subjects are already at very high risk. - Those with low HDL cholesterol* or increased triglyceride, fibrinogen, opoB, Lp(a) levels and perhaps increased high-sensitivity CRP. - Asymptomatic subjects with evidence of pre-clinical atherosclerosis, for example plaque on ultrasonography. - Those with moderate to severe chronic kidney disease (GFR <60 mL/min/1.73 m2) *Note that HDL cholesterol impacts on risk in both sexes, at all ages, and at all level of risk. This effect can be estimated using the electronic version of SCORE, HeartScore, which has been updated to include HDL cholesterol level.

ESC WORKING GROUPS
16 ESC Working Groups for 16 areas of expertise in Cardiology Working Groups are the scientific backbone of the ESC and have been created to provide scientific expertise by excellence in a specific field. They contribute to the Mission of the ESC which is “To reduce the burden of cardiovascular disease in Europe”. Among many initiatives such as study groups, publications, registries, position papers and ESC Clinical Practice Guidelines, the main function of a Working Group is to “contribute scientifically to the ESC Congress and other educational and scientific initiatives”. From research to clinical applications, ESC Working Groups stimulate and disseminate scientific knowledge through various educational programmes. They encourage and ease interactivity and exchange at a European level.

ESC CLINICAL PRACTICE GUIDELINES
Supporting the dissemination of knowledge on treament of cardiovascular diseases is a major responsability of the European Society of Cardiology. In order to improve clinical pratice, the Committee for Practice Guidelines (CPG) brings together groups of European experts with the task of developing recommendations and guidelines for clinical practice. These recommendations and guidelines clarify areas of consensus and disagreement, allowing distribution of the best possible guidance to practicing physicians. Guidelines aim to present all the relevant evidence on a particular clinical issue in order to help physicians to weigh the benefits and risks of a particular diagnostic or therapeutic procedure. They should be helpful in everyday medical decision-making.

EUROBSERVATIONAL RESEARCH PROGRAMME
A broad range of Registries and Surveys across ESC Countries and the World
Aim of the programme: to provide a better understanding of medical practice, based on observational/real life data. Data collected through a web-based eCRF from a network of volunteer centres in Europe and around the world. Extended statistical analyses performed in view of publications of main results/ sub-group analyses. EORP has been developed in cooperation with the healthcare Industry, while retaining its full independence. Database management, analysis and the EORP publication policy are run by the ESC.
General : To assess the characteristics and management of diseases, the incidence of which have a major epidemiological impact Special : To assess orphan and rare or highlydemanding diseases (i.e. bad outcome, high costs, complex management). Sentinel : To assess the development and impact of new tools and therapeutic and diagnostic procedures. Prevention : To assess cardiovascular risk factor epidemiology and prevention measures.
