PALLIATIVE CARE 4 CHECKLIST The differences between hospice and palliative care.

Hospice care and palliative care are very similar when it comes to the most important issue for dying people: care. Most people have heard of hospice care and have a general idea of what services hospice provides. What they don’t know or what may become confusing is that hospice provides “palliative care,” and that palliative care is both a method of administering “comfort” care and increasingly, an administered system of palliative care offered most prevalently by hospitals. As an adjunct or supplement to some of the more “traditional” care options, both hospice and palliative care protocols call for patients to receive a combined approach where medications, day-to-day care, equipment, bereavement counseling, and symptom treatment are administered through a single program. Where palliative care programs and hospice care programs differ greatly is in the care location, timing, payment, and eligibility for services. Hospice: Hospice programs far outnumber palliative care programs. Generally, once enrolled through a referral from the primary care physician, a patient’s hospice care program, which is overseen by a team of hospice professionals, is administered in the home. Hospice often relies upon the family caregiver, as well as a visiting hospice nurse. Palliative Care: Palliative care teams are made up of doctors, nurses, and other professional medical caregivers, often at the facility where a patient will first receive treatment. These individuals will administer or oversee most of the ongoing comfort-care patients receive. While palliative care can be administered in the home, it is most common to receive palliative care in an institution such as a hospital, extended care facility, or nursing home. Timing Hospice: You must generally be considered to be terminal or within six months of death to be eligible for most hospice programs or to receive hospice benefits from your insurance. Timing Palliative Care: There are no time restrictions. Palliative care can be received by patients at any time, at any stage of illness whether it be terminal or not. Payment Hospice: Before considering hospice, it is important to check on policy limits for payment. While hospice can be considered an all-inclusive treatment in terms of payment (hospice programs cover almost all expenses) insurance coverage for hospice can vary. Many hospice programs are covered under Medicare. Payment Palliative Care: Since this service will generally be administered through your hospital or regular medical provider, it is likely that it is covered by your regular medical insurance. It is important to note, however, that each item will be billed separately, just as they are with regular hospital and doctor visits. If you receive outpatient palliative care, prescriptions will be billed separately and are only covered as provided by your regular insurance. In-patient care however, often does cover prescription charges. For more details, check with your insurance company, doctor, or hospital administration. Treatment Hospice: Most programs concentrate on comfort rather than aggressive disease abatement. By electing to forego extensive life-prolonging treatment, hospice patients can concentrate on getting the most out of the time they have left, without some of the negative side-effects that life prolonging treatments can have. Most hospice patients can achieve a level of comfort that allows them to concentrate on the emotional and practical issues of dying. Treatment Palliative Care: Since there are no time limits on when you can receive palliative care, it acts to fill the gap for patients who want and need comfort at any stage of any disease, whether terminal or chronic. In a palliative care program, there is no expectation that life-prolonging therapies will be avoided. It is important to note, however, that there will be exceptions to the general precepts outlined. There are some hospice programs that will provide life-prolonging treatments, and there are some palliative care programs that concentrate mostly on end-of-life care. The second list are questions you should ask your care agency before hiring them. (Loose wires, slippery floors, rugs)? q q Called pharmacist in advance for any necessary prescriptions? q q Made a list of important telephone numbers to call in an emergency? q q Made a backup list of people in case a caregiver does not show up? q q Arranged transportation to hospital or physical therapy? q q Can the patient drive himself if the car is properly equipped? q q Placed a calendar near the telephone for easy and quick reference? q q Are there working smoke detectors on every floor? q q Keep electric fans out of reach. Do you have a list of your medical providers? You should include the name and contact information for your current primary care provider and any specialists managing your care. Do you have an Advanced Directive or will? Make sure that your wishes are known in case of any scenario. Advanced Directives ensure you are prepared for any outcome. Do you have a list of current medications? Make a list of all medications that you are currently taking and include the name, dosage, frequency, and the prescribing provider. Have you included your medical insurance information? Bring your current insurance information (primary and secondary if applicable). Have you brought entertainment? Long stretches during the day can seem longer without positive distractions. Remember to bring things that you enjoy (books, music, movies, puzzles, electronic devices). ADULT FAMILY HOME 4 CHECKLIST

Carry this checklist with you when you visit an Adult Family Home. The checklist is designed to help you know what to look for and to remember what you saw. After visiting the homes, use the checklist to easily compare facilities. WHO IS SERVED: Can the home provide the level of assistance you require given your current condition? Are there medical conditions the home will not accept? If yes, what are these conditions? Are there medical conditions or other criteria which might require you to leave at some point? Are pets allowed? Are you comfortable with the rules and regulations stipulated in the residency agreement? SERVICES: Are meals provided at convenient times? Are special diets taken into consideration when meals are prepared? Are snacks available between meals? Can medication reminders be given if needed? Is there a hospital close by? How far to the nearest hospital___? Is assistance provided with eating, bathing, dressing, toileting or other personal tasks if needed? Does the home offer social and recreational activities that you enjoy? Are there health promotion and exercise programs? Are residents actively involved in planning activities and events? Is transportation available to community activities that you enjoy Are other services available? If yes, what are they? PHYSICAL ENVIRONMENT: Does the home have private rooms? Can you furnish the room with your own furniture? Is the room large enough that you feel comfortable? Are the common areas (family room kitchen, etc,) sufficiently large and comfortably furnished? Are there outside areas that residents may use? Are there sufficient handrails and grab bars, especially in the bathroom? Is there adequate security? FINANCIAL CONSIDERATIONS: Is a written fee schedule provided? Do you know what the basic rate does and does not include? Are these fees refundable (at least partially) in the event that you need to leave the facility? Are there fees for add-ons, such as cable, telephone, Internet, etc.? What procedures are in place if you can no longer afford the fees? LIVING ENVIRONMENT: Is the decor attractive and homey? Is the floor plan well designed and easy to follow? Are doorways, hallways, and rooms accommodating to wheelchairs and walkers? Are hand rails available to aid in walking? Are floor coverings made of a nonskid material? Is the residence clean, free of odors, and appropriately heated/cooled? Does the residence have good lighting, sprinklers, and clearly marked exits? What is the facility’s means of security if a resident wanders? Is there an emergency evacuation plan? Does the facility hold fire drills?
