Answers to the questions on these two pages will help you evaluate an assisted living community. It is always a good idea to take a copy of these pages with you when visiting an assisted living community. PERSONAL CARE NEEDS: YES NO Is there a written plan for the care of each resident? r r Does the initial needs assessment include the resident, family, and facility staff, along with a physician? r r Is there an ongoing process for assessing a resident’s need for services? How often? r r How does the facility accommodate residents with changing needs? r r Is staff available to provide 24-hour assistance with activites of daily living (ADLs)? r r Does the residence have programs for Alzheimer’s, dementia, and other special needs? r r How often does a staff member check on a resident’s whereabouts and well-being? r r If a resident’s behavior changes and becomes verbally or physically abusive, what is the facility’s course of action? r r HEALTH CARE NEEDS: Does a physician/nurse make regular checkups? To what extent is medical care available? r r Will the staff set up medical appointments and transportation for each resident? r r Does the residence’s pharmacy provide delivery, consultation, and review of medicines? r r Does staff supervise/assist residents in taking medicine? q q May residents take their own medications? q q What health care services are available at the facility? Social services? q q Does the residence have a clearly stated procedure for responding to medical emergencies? q q Is there an arrangement with a nearby hospital? q q Under what circumstances does the facility call the family, or the resident’s doctor? q q RESIDENT SERVICES: What resident services are available onsite? r r Are housekeeping, linen service, and personal laundry included in the fees, or are they available at an additional charge? r r Are on-site laundry facilities available and convenient? r r Does the residence provide transportation to doctors’ offices, the hairdresser, shopping, and other activities, and can it be arranged on short notice? r r LOCATION: Do you like the facility’s outward appearance? r r Is the facility convenient for frequent visits by family and friends? r r Is the facility near shopping centers and other entertainment? r r Can the resident access a medical complex easily? r r Is public transportation available and accessible? r r LIVING ENVIRONMENT: Is the decor attractive and homey? r r Is the floor plan well designed and easy to follow? r r Are doorways, hallways, and rooms accommodating to wheelchairs and walkers? r r Are elevators available for those unable to use stairs? r r Are hand rails available to aid in walking? r r Are floor coverings made of a nonskid material? r r Is the residence clean, free of odors, and appropriately heated/cooled? r r Does the residence have good lighting, sprinklers, and clearly marked exits? r r What is the facility’s means of security if a resident wanders? r r Is there an emergency evacuation plan? r r Does the facility hold fire drills? r r

HEARING AID 4CHECKLIST

If you need a hearing aid, or think you do, you can avoid problems by checking this list to guide you in the process. You may check some boxes yourself or take this checklist along on an appointment. TESTING:

Were you given a hearing screening or a full hearing exam? Did they explain the affect your loss of hearing has on understanding? Were you given a full explanation of all tests? Were you charged for the examination? Were you told what type of hearing loss you have? Did your audiologist discuss this with you? What your audiogram means? Why you sometimes can “hear” but not “understand” What a hearing aid can do for you and what it can’t do for you? Were you asked about the effect of hearing loss on your life at home, work, school, using the phone, etc. Did a “Significant other” have an opportunity to discuss the effect of your hearing loss on them? DISPENSING: Do you know why a particular type of hearing aid was recommended? Behind-the-ear, in-the-ear, in-the-canal, completely-in-the-canal, open fitting? Were your personal preferences considered regarding style, cost, or remote control?. Were hearing aid features explained to you? Directional microphones, number of microphones, self-adjusting volume control, etc? Information about the telecoil and its uses? Do the aids fit comfortably? Were you able to insert the hearing aids in your ears by yourself? Was information concerning batteries explained to you? How to put batteries in the hearing aid--can you put them in by yourself? Where you can buy batteries, how much they cost, keeping spare batteries handy? Were all the controls explained to you? Can you operate all the programs yourself? Do you have the dexterity to operate the controls for your hearing aid? Do you have the vision to operate the controls and put in batteries? Was care of the hearing aids and earmolds explained to you? How to keep earwax out of the sound bore of the hearing aid? Using a dehumidifier box for night storage? How often should you return for regular checkups and cleaning? Did you discuss the use and care of your hearing aids? Did you receive written material on the information you received? Was the following covered at the time of your first follow up visit? Your listening experiences with hearing aids (in noise, feelings of loudness, discomfort, etc) Fit of earmolds (comfort, “whistling” noises)? How to troubleshoot problems? Did you receive information about using telephones and assistive listening and alerting devices with your hearing aids? M/T Ratings for hearing aids, cell phones; labeling for cordless phones? HEART PROBLEMS 4CHECKLIST

Heart attack and stroke are life-and-death emergencies so every second counts. The following questions will help you spot symptoms. Immediately call 9-1-1 for help. Don’t delay...get help right away! SIGNS OF A HEART ATTACK: YES NO SIGNS FOR BOTH MEN & WOMEN Does the person feel discomfort in the center of the chest?   Does the pressure go away and come back?   Is the feeling of pressure, squeezing, tightness, fullness or pain occur intermittently?   Does the person feel sudden pains, discomfort or uneasiness in different parts of their body?   Does the person have shortness of breath? (May occur without pain)   Does the person have an irregular heartbeat?   Does the person feel fatigued or extremely tired for no reason?   SIGNS FOR WOMEN: Does the person have nausea, light-headedness?   Has the person broken out into a cold sweat?   Does she have dizziness? (May cause women to black out.)   Is she experiencing nausea: (May be mistaken for stomach or gastric upset.)   Is she vomiting?   Does she have feelings of nervousness and anxiety? She might have a rapid heartbeat and palpitations.   Note: Men may also experience some of these signs but they are more common with women. SIGNS OF A STROKE: YES NO Does one side of the face droop or is it numb? (Ask the person to smile)   Is one arm weak or numb?   Does one arm drift downward when you ask the person to raise both arms?   Is speech slurred?   Are they unable to speak?   Are they hard to understand? (Ask a person to repeat a simple sentence.)   Is the sentence repeated correctly?   Does the person have trouble seeing in one or both eyes?   Does the person have trouble walking, dizziness, or loss of balance or coordination?   Does the person have sudden severe headache with no known cause?   Did you make a note of when the first symptoms appeared?   If the person shows any of these symptoms, even if they go away, call 9-1-1 and get them to a hospital immediately. CARDIAC ARREST WARNINGS: YES NO Is there a sudden loss of responsiveness?   Is there no response to tapping on shoulders?   Does the person not respond when asked if they are okay?   Is there no normal breathing?   Does the person take a normal breath when you tilt the head up?   If these signs of cardiac arrest are present call 9-1-1 for emergency services, get an automated external defibrillator (AED) if available, and begin CPR immediately until emergency help arrives. Use this checklist to help evaluate hospice services to determine which one will work best for you and your family. Hospice services can be a great relief for both a patient and family members. GENERAL INFORMATION: What services are provided? What kind of support is available to the family and caregiver? What roles do the attending physician and hospice play? What does the hospice volunteer do? How does hospice keep the patient comfortable? How are services provided outside of business hours? How and where does hospice provide short-term inpatient care? With which nursing homes or long-term care facilities does the hospice work? Can this service be brought into a nursing home or other living facility? How long does it typically take the hospice to enroll someone once the request for services is made? How does the physician work with the family and patient? Is the hospice licensed or certified? If so, for what types of services? Are services offered in the home? Is the service insured against liability? Is a written contract provided regarding eligibility, payment and staff training? Is the care plan developed in consultation with your loved one’s physician? Are family members, including myself, included in reviewing and contributing to the care plan?
