This section offers three different comparison grids; the first features Skilled Home Healthcare; the second contains Non-Medical and Personal Assistance at home, and the third grid showcases Hospice Services for end of life care. The number of Americans living past age 65 has exponentially increased over the past few decades and will continue to grow over the next few years. According to the Centers for Disease Control and Prevention (CDC), there will be more than 80 million seniors in the U.S. by 2040. This increase requires more people to pay close attention to the need for longterm care planning.
An elder law attorney can help seniors create a plan to pay for their care needs for the rest of their lives. Many seniors may worry about a lack of financial resources to pay for food, rent, medical care, and transportation in the years to come. Others hope to stay in their homes as they age but aren’t sure what the options may be for
in-home care if it becomes necessary. An elder law attorney can help you create a customized plan for your needs and assist you in allocating money to pay for the essentials as you age. In addition, with expertise specific to elder law at the federal level and in your state, they can aid you in determining what public benefits you could qualify for, such as Medicaid and Medicare, and help you successfully apply for them.
Many older adults report wanting to age in place in communities. Elder law attorneys help their clients achieve this objective by representing them in landlord-tenant and other housing disputes.
For instance, an elder law attorney may help a homeowner with limited mobility who has been unable to maintain their lawn respond to a city ordinance violation. A related piece of the puzzle is incapacity planning. This could mean having an elder law attorney advise you on documenting your wishes for care at the end of your life, in the wake of a disability, or after a diagnosis of dementia.
An elder law attorney can assist seniors and their families as they try and protect the senior’s financial and physical well-being as their condition progresses.
Elder law attorneys help family members and older adults with guardianship.
This may include a situation in which an older adult has dementia or another condition affecting decisionmaking. In such a case, an elder law attorney can help a concerned family member obtain guardianship. This includes securing a physician’s report, petitioning the court, and attending a hearing.
An older adult might sometimes dispute guardianship, asserting
continued decision-making capacity and wishing to maintain autonomy. The attorney may also help support an older adult’s autonomy by recommending less restrictive alternatives to guardianship. Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

If you notice changes in friends, family or others close to you and are concerned for their health — particularly when it involves changes in memory, thinking or behavior — it can be di cult to know what to do or say.
Although it’s natural to be uncertain or nervous about how to offer support, these changes could be a sign of a significant health concern. Use the guide below or print out the PDF to help you feel more confident and prepared as you assess the situation and take action.
Assess the Situation
1. What changes in memory, thinking or behavior do you see?
What’s the person doing — or not doing — that’s out of the ordinary and causing concern?
2. What else is going on?
Various conditions can cause changes in memory, thinking and behavior. What health or lifestyle issues could be a factor? E.g., family stress or health issues like diabetes or depression.
3. Learn about the signs of Alzheimer’s and other dementias and the benefits of an early diagnosis. Visit alz.org/10signs to educate yourself on 10 common warning signs of Alzheimer’s and why it’s
important to know if dementia is causing the changes. Do you notice any of the signs in the person you’re concerned about?
4. Has anyone else noticed the change(s)?
Find out if friends and family have seen changes. What are they?

5. Who should have the conversation to discuss concerns?
It could be you, a trusted family member or friend, or a combination. It’s usually best to speak one-on-one so that the person doesn’t feel threatened by a group, but use your best judgment to determine what will likely be most comfortable for the individual.
6. What is the best time and place to have the conversation?
Have the conversation as soon as possible. In addition to choosing a date and time, consider where the person will feel most comfortable.
7. What will you or the person having the conversation say?
Try the following: • I’ve noticed [change] in you, and I’m concerned. Have you noticed it? Are you worried?
• How have you been feeling lately? You haven’t seemed like yourself.
• I noticed you [specific example] and it worried me. Has anything else like that happened?
8. Offer to go with the person to the doctor.
Ask the person if he or she will see a doctor and show your support by offering to go to the appointment.
Some words of encouragement may include:
• There are lots of things that could be causing this, and dementia may or may not be one of them. Let’s see if the doctor can help us figure out what’s going on.
• The sooner we know what’s causing these problems, the sooner we can address it.
• I think it would give us both peace of mind if we talked with a doctor.
9. If needed, have multiple conversations.
The first conversation may not be successful. Write down some notes about the experience to help plan for the next conversation.

10. Turn to the Alzheimer’s Association for information and support.

Navigating Parkinson’s disease (PD) can come with many surprises for the person living with the disease and those who love them. Since Parkinson’s progresses, symptoms often begin to complicate daily activities as a person progresses through the stages of PD.
Learning how to manage Parkinson’s is a constantly evolving learning curve that should always be modified to best suit both the needs of the person with Parkinson’s and the care partner. In this article, we highlight tips that can help your loved one adjust to these changes.
Movement changes are common in Parkinson’s and can become more difficult to manage in later PD stages. As a care partner, you can help your loved one move safely.
• Check that your loved one’s feet are placed firmly underneath before standing. You may need to help with proper foot placement.
• Remind your loved one to take big steps. People with PD often need “cues” to take long steps as automatic motions become more difficult to perform. Keep cues short and simple, for example by saying, “Big steps.”
• Find an occupational therapist. These rehabilitative experts can help your loved one maintain independence and ways care partners can assist.
People with Parkinson’s sometimes experience freezing episodes, which is the temporary and involuntary inability to move. Freezing is a significant cause of falls, as it creates a feeling like your feet are glued to the floor.
• Avoid tight turns when possible. Instruct your loved one to make wider turns as freezing often happens while turning around in close quarters.
• Make a plan. Consider how you’ll access help in the event of a fall, such as calling a neighbor or friend.
Leaving the home can be difficult, but outings to a doctor’s office or physical therapist are often necessary. Consider methods that promote safety and decrease care partner stress.
• Plan appointments around your loved one’s daily routine. Try to schedule appointments while the person with Parkinson’s is rested and PD medications are working well.
• Maximize your time. If endurance allows, schedule a few appointments on the same day, especially if they are in the same clinic or medical complex.
Advanced Parkinson’s can cause difficulty with eating and drinking because of movement and swallowing problems. Making adjustments to
mealtimes can help your loved one get the nutrition they need.
• Serve foods that are easy to eat. Avoid tough, dry, or crumbly textures that might be difficult to swallow.
• Focus on hydration. To ensure your loved one is drinking enough fluids, encourage sips of liquid between solid foods.
Parkinson’s symptoms change over time, as will your role as a care partner. The most important thing to remember is that if you do not take care of yourself, you will not be able to take care of your loved one.
Caregiver burnout is a state of physical, emotional and mental exhaustion. Burnout can occur when caregivers don’t get the help they need, or if they try to do more than they are able, physically or financially. Caregiver burnout symptoms can include:
• Extreme fatigue unrelated to sleep
• Unusual frustration and anger
• Feeling “cloudy” or “foggy.” Often dismissed as age-related change, this can occur as a result of carrying too much responsibility and/or anxiety
These tips can help you navigate your own wellbeing as a care partner:
• Manage stress. Identify what triggers your stress and find ways to control your emotions, like writing in a journal or going for a walk.
• Seek connection. Connect with a Parkinson’s Foundation chapter near you to find local resources and events.
• Accept help. Make a short list of
specific tasks that typically help you care for your loved one, so family and friends know what they can do to help when you need a break.
When thinking changes are mild, these symptoms often do not impact everyday life. But as Parkinson’s advances, more profound changes in thinking can occur, including dementia. Dementia is when someone experiences problems with memory and thinking that are advanced and interfere with daily activities and quality of life.
To improve communication, try these strategies:
• Ask one question at a time. Slowing down and asking one thing at a time can be helpful.
• Give hints and cues. Use short and simple phrases to provide cues. Give a short hint if your loved one has difficulty finding a word or loses their train of thought.
• Resist the urge to argue or correct. If your loved one is experiencing a delusion, try to find ways around the situation instead of contradicting them. It can be helpful to keep in mind ‘it is the disease making these accusations, not my loved one.’
• Look for community resources BEFORE you need them. Explore resources in your community, such as the Area Agency on Aging.

A stroke happens when blood flow to the brain is blocked. Stroke is the number five cause of death and a leading cause of disability in the United States.
Use the acronym F.A.S.T to recognize the signs of a stroke:
F = Face Drooping – Does one side of the face droop or is it numb? Ask the person to smile. Is the person’s smile uneven?
A = Arm Weakness – Is one arm weak or numb? Ask the person to raise both arms. Does one arm drift downward?
S = Speech Difficulty – Is speech slurred?
T = Time to call 911
There is no time limit on the recovery journey for stroke survivors and many may find that their goals extend into the chronic period of recovery (i.e., at least six months after the initial stroke event). Continuing to practice what was taught in the hospital and rehabilitation is important, but sometimes the focus is primarily on physical function. Here are some things that are important to discuss with your medical providers as soon as you are medically stable:
1. Sleep: Ask about how sleep can be impacted, how to get in and out of bed, and how to find a comfortable position to support your affected side.
2. Pain management: Ask about what kind of pain can be expected after a stroke, side effects of pain medication, and lifestyle changes you can make.
3. Physical movement: Ask about how you should be exercising and moving your body, how to exercise in a wheelchair or using one hand, and how to find an activity level that does not make the bad kind of pain worse.
4. Nutrition: Ask about changes in taste and smell, how to make sure you are getting enough nutrients, and how to manage weight changes after stroke.
5. Resiliency: Getting better can feel lonely for both the stroke survivor and the caregiver(s). Ask for information on support groups and community wellness programs in your area.

While both Home Care and Home Health provide services in the home aimed at helping patients achieve health and independence, knowing the di erence between them can help you quickly and e ectively navigate care needs in a family crisis.
Home Health is a clinical or skilled care model that provides medical care by licensed healthcare workers, like nurses, physical therapists, and social workers. Home Health requires a doctor’s order, and patients must meet certain eligibility criteria to receive it. It’s usually prescribed when a patient is discharged from the hospital or rehab facility and sent home but needs additional medical attention to fully rehabilitate. Home Health services help with things like wound care, speech therapy, physical or occupation therapy, IV Therapy and is overseen by a registered nurse. Visits are a few hours a week and are short, generally an hour or less depending on the patient’s needs. It is usually paid for by private insurance, or Medicare - for those over 65 years of age. It typically lasts a few weeks but can be extended if the ordering physician deems it necessary.
Home Care provides nonclinical or non-medical care by professional caregivers, usually CNA’s (Certified Nurse’s Aide) or Home Health Aides. These aides are professionally trained in assisting with ADL’s, or “Activities of Daily Living.” The six most common ADL’s in home care are: Mobility, Bathing, Toileting, Continence Mgmt., Dressing and Feeding. They also assist with IADL’s or “Instrumental Activities of Daily Living” like light housekeeping, laundry, dishes, and medication reminders. Full-Service Home Care agencies may carry an additional insurance policy that allows their caregivers to run errands or provide transportation assistance for things like doctors’ appointments, groceries, and other needs outside of the home. Home Care does not require a doctor’s
order and is usually arranged privately and paid for out of pocket, however, depending on the agency you work with, some will accept Long Term Care Insurance, Veteran Benefits or Medicaid as a form of payment. Unlike Home Health, Home Care is arranged for longer periods of time. Most agencies have minimums and will visit anywhere for a few hours a day, up to around-the-clock 24/7 care. In Home Care, it’s not uncommon for care to continue for months or even years - as long as the patient requires someone to be with them so that they can remain safe at home, they will provide services for them.
Often times, Home Care and Home Health will be providing
services at the same time –having home health manage the medical needs, while home care stays with the patient to manage their personal care needs and keep them safe. When evaluating options for either Home Health or Home Care, the Senior Blue Book provides an index of local agencies in your area, as well as a search tool online where you can review what services they provide, read customer reviews, and contact them directly to ask questions or request an in-home consultation.

24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.
CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.
DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ.
ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.
HOUR MINIMUM – The least number of hours a Non-Medical
Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.
HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided.

Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.
– Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.
PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services
related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.
SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.
(ST, PT, OT, RT) –
Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.
Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.
Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help
clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.
Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.
TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.
CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

After a hospitalization, don’t go home alone. HOSPICE - Hospices provide individuals facing a terminal illness with care in a private residence or in a healthcare facility. The goal is to provide quality of life and to assist patients to live the last stages of their lives with dignity. The focus is on comfort rather than curative treatment of the terminal illness.
SERVICES PROVIDED:
RN-REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family education as needed
SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, and provides counseling and support to meet identified needs, and makes referrals to other providers based on family and patient’s needs
CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.
VOLUNTEERS – Provides companionship and needed non-medical services for patients and their families, offers support at time of death and during bereavement.
RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in managing the individual’s disease symptoms. Care is provided at the individual’s private residence or in a healthcare facility.
RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a skilled nursing facility. Hospice is responsible for financial arrangements with the facility.
GIP – GENERAL IN-PATIENT
CARE – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This higher level of care is due to symptoms which are out of control and more intensive nursing services are required. The room
and board are covered by hospice during this period.
CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided to the patient at their private residence or in a personal care home or assisted living facility. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for crises that require a constant presence by a nurse or care team.
THERAPY SERVICES PROVIDED:
AR-AROMA THERAPY – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.
MA-MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication
or prefer not to. Massage therapy has become essential in hospice care. This is not reimbursed by Medicare.
Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.
P-PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.
24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays. Continued on next page 110

Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many hospice companies provide Grief Groups that meet and address issues of grief in a supportive group setting.
– Indicates whether the hospice agency has a sister home health agency that will provide assistance with medically based home health needs such as nursing services, home health aide services, physical therapy, occupational therapy, respiratory therapy or speech therapy.
– Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at
the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.
Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.
– Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Specifically trained staff usually serves the special needs of this population. Amenities vary for each community.
Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.
SKILLED
– 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources.
TRACH CARE - Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.
VENTILATOR - Moves breatheable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable to breathe, or is breathing insufficiently.
WANDERGUARD - Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering. ~

Health and safety is no laughing matter. Chronic and painful conditions can limit and isolate a person from healthy interactions and medical care. Especially when seniors live alone, the risks are too great for accidents. Assisted living
communities are equipped to provide the necessary support and care to ensure every senior’s safety and well-being.
Loneliness and social isolation significantly impact seniors’ mental and emotional well-being. If a senior finds themself alone, and craving social interaction and companionship, they may be ready for a change. Senior living communities offer so many different ways to expand a senior’s friend circle and make lasting connections. Enjoying a sense of community is important for overall happiness and quality of life.
Homes hold a special place in our hearts. They’re a chest of memories, laughs, and tears. But with all the good comes a lot of work. There comes a time when maintaining a home is too much work and increasingly difficult to manage. Between cleaning, yard work, home repairs, and maintenance, the tasks are overwhelming and stressful. Assisted living communities ease the burden and take care of all those responsibilities so that your loved one can enjoy life and pursue other interests.

Before most seniors enter an assisted living community, their family is normally their caregiver. As wonderful and supportive as this is, it’s incredibly stressful for the family caregiver. A caregiver plays a major role in every aspect of life for the senior. And to fulfill those needs, they need to make sacrifices in their own life, which often means spending less time with their own family. This can be physically and emotionally draining. Switching to a senior living community can be just the change everyone involved needs to balance life and get the necessary support and care.
There’s no time like the present to make a change in your senior’s living situation. We always recommend making the change sooner rather than later so that you can do as much research as possible without feeling rushed. This decision is hard. There’s so much involved in making the right choice. But if your loved one is struggling with any of the challenges listed above, the right time to switch to an assisted living community is now.
