Bucket List?
Emerging from the movie
“Bucket List” I commented to my husband that I did not like that movie! He chuckled and responded he was certain of that going in and now, he was laughing as he heard my response.
Recently, I read an article that said “80 is the new 40.” I believe that. We are living longer and better due in part to taking better care of our bodies, getting regular check-ups and seeing that we watch our diet and exercise more.
Countless people have chosen to accomplish amazing things in what was once called the sunset years of life. Mary Armstrong went sky-diving to celebrate her 90th birthday. Sam McMillian began his career as a dot painter at age 60. Nola Ochs became the oldest person to graduate from college at age 95. People have been known to take up water-skiing at 100 and run marathons in their 80’s.
But the day came, I retired! I found myself with too much time on my hands and no plan. Then it happened. All kinds of aches and pains began to surface and I began to wonder which would be my demise. I found myself crying for no reason, unable to sleep, or sleeping too much. I wondered if I should make one of those “bucket lists,” got out a notebook and set my hand to the task:
• Flying in a hot-air balloon
• Seeing the Northern Lights again
• Visiting a real castle
• Creating my dream room
• Living in a foreign country for six months
• Traveling to as many states as I can that I haven’t already seen
• Being in the same room with ALL of my children and grandchildren
I retreated to my favorite spot, my swing and with my list in hand, had a nice pity party. I have never been one to give into depression but felt myself teetering on the edge and rapidly becoming a basket case. Suddenly, these words filled my empty heart: “give of your best to your Master, give of the strength of your youth…” I curtly responded out loud that my “youth” was gone! Immediately, I heard it a second time! (This is because I wasn’t a first time responder!) That ended my short pity party attended only by myself and my heavenly Father!
I have thrown away my Bucket List. I’ve rolled up my sleeves and am diving into all kinds of interesting things. In these “golden years” we have a chance to live life to the fullest. The rat race is behind us and we no longer exist under the pressure of the corporate world to succeed. We have earned the right to walk barefoot in the grass and trample all our regrets under our feet.
Psalm 92:14 says, “they shall still bear fruit in old age; they shall be fresh and flourishing.” Go swimming! Drag out those scraps and find a quilting group, read to someone, master the art of raising an herb garden, scrapbook with all those old photos, write a journal of memories for each of your grandchildren; You can do it!

Alzheimer’s Impact Movement (AIM) Advances Cause Through Political Process
By Vanessa OrrAlzheimer’s disease is not just an aging issue—it’s a public health crisis. More than 6 million Americans of all ages have Alzheimer’s, and in 2023, an estimated 6.7 million Americans age 65 and older were living with the disease.
Alzheimer’s affects not only individuals, but families, businesses and communities. And while there is good news—with new diagnostic tests making it easier to identify Alzheimer’s and new medications coming on line to help delay the progression of the disease—this new era of treatment is already facing hurdles when it comes to providing Alzheimer’s patients with access to the care they need.
AIM (Alzheimer’s Impact Movement) is working to change this. As the advocacy arm of the Alzheimer’s Association, AIM fights to support Alzheimer’s and dementia research funding and to advance bipartisan legislation that will have a positive and meaningful impact on people living with Alzheimer’s and their caregivers today and in the future.
“AIM works to ensure that Alzheimer’s remains a leading priority on Capitol Hill by participating in the political process,” said Jen Ebersol, director of state government affairs, Alzheimer’s Association of Greater Pennsylvania. “We work to educate candidates of both parties to make sure that Alzheimer’s and dementia are part of the national plan. Our voices make sure that Alzheimer’s remains a national priority.”
Working collaboratively, AIM members target numerous issues on both a state and national level that affect those with the disease. These include the creation of Alzheimer’s-related public health infrastructure, pushing for more federal funding for research, and fighting the Centers for Medicare & Medicaid Services (CMS) for its decision to block access to FDAapproved Alzheimer’s treatments.
“When we talk about the need to move our mission forward and to change the trajectory of the disease, work in policy and advocacy is instrumental in that,” said Ebersol. “The impact of what we do is big, because changing the laws changes the lives of so many people.”

A life care plan is a living, breathing, document that provides for you as you age. It is a tool that helps you plan for your future. In my opinion, it is never too early to have a Life Care Plan because it can always be changed and updated as time passes. Such plans are not a one-time, static document but provide an opportunity to be shaped and changed as your health, life situation, and circumstances change. SOUNDS WORSE THAN IT IS
We recently received a distressed call from a husband that we represented about a year ago on a long-term Medicaid application for his wife who was a local nursing facility resident. We had done an asset protection plan for the husband, preserving approximately $300,000 more than he had expected, plus the house, the couple’s car, and his retirement account.
He was tickled pink about the result that we received. Unfortunately, his wife recently died, and he received a letter in the mail, coming into my office extremely upset. The letter was from the Estate Recovery Department office in Harrisburg, requesting information about the deceased, including the cause of death, how much money she had, and how the assets were titled.
The husband had made comments throughout the original planning process that it all seemed “too good to be true”. He was extremely pleased but remained cautious. When he entered my office with the letter in hand, he commented, “See, I knew it. We’re in big trouble.” I looked at him and asked why he felt that way. I reviewed the form and explained that it was nothing more than an information/fact-gathering sheet and did not mean that the answers should be interpreted as negative.
Estate recovery in the Commonwealth of Pennsylvania only pursues the probate estate of a Medicaid recipient if their assets are over $2,400. In a situation where we qualify someone for Medicaid, we make sure there is no more than $2,400 in assets for the client so that we know the individual is always below the amount the Commonwealth will go after in estate recovery.
Furthermore, we always make sure that the couple’s house is in the community spouse’s name alone so that there is no situation wherein if the spouse predeceases the Medicaid recipient, the house ends up back in their name and is at risk for estate recovery. It sounds complicated, but for a firm that understands the rules, our team makes sure that everything is done appropriately – that all the i’s are dotted, and all the t’s are crossed. I assisted the gentleman with the form, which he submitted that afternoon. He couldn’t thank us enough for our advice and assistance.

a professional, please do not talk to outside people who are not involved in the situation. This even means other professionals, because it is very difficult to understand everything that is going on unless you are involved in it.
Oftentimes, people think that they’re helping by providing advice, but because they don’t know the context, sometimes they mislead an individual and it causes a lot of stress.
In this case, the gentleman had received bad advice from a friend, but once we were able to clear it up and explain to him why there was nothing that they could go after, everything was taken care of, and everything was right in the world. If done properly, estate recovery is never an issue and only becomes a problem when people do not know how to properly plan.

For those caring for loved ones with disabilities, planning for the future can seem overwhelming, but PA ABLE can help bring peace of mind.
The PA ABLE (Achieving a Better Life Experience) Savings Program, administered by the Pennsylvania Treasury, is a tool for Pennsylvanians with disabilities and their families to save money without affecting eligibility for important benefits like Medicaid and SSI.
The first PA ABLE account was opened in 2017. Now, just six years later, there are NEARLY 7,500 ACCOUNTS WITH ALMOST $90 MILLION SAVED.
PA ABLE accounts are exempt from state inheritance tax. There’s no federal or state income tax on savings, and no taxes owed on account withdrawals for qualified expenses. Up to $17,000 in contributions can be deducted annually from Pennsylvania income taxes. Accounts are also protected from creditors of the account owner, beneficiary or contributor – and it’s not subject to Medicaid repayment.
You can open a PA ABLE account for your loved one with as little as $25, choosing from six investment options or an interest-bearing checking account.
Which option is right for you depends on your plans for the account. Some people see PA ABLE as a way to save for more immediate expenses, which might make the checking account option a convenient choice. Others may be planning for a young child’s future care, so an investment account

where the money has more time to grow may be the best option. No matter what your goals are, there’s a PA ABLE account option that will work.
PA ABLE is open to anyone whose qualifying disability onset is prior to their 26th birthday.
Starting in 2026, the eligibility age will rise to 46 – expanding access to ABLE programs nation-wide to six million more people, including one million veterans – thanks to the ABLE Age Adjustment Act that Congress passed last December. Pennsylvania was instrumental in getting that change across the finish line. As chair of the national ABLE Savings Plan Network, I was a strong advocate for these changes, and I’m excited that so many more people will have the opportunity to save.
Savings in PA ABLE accounts can be used for a wide range of qualifying disability-related expenses. These include everyday things like groceries or transportation and larger, more long-term expenses, like education, healthcare, assistive technology or vehicle adaptations.
Another great part of PA ABLE is that anyone, including friends and family, can contribute to an account anytime.

HEARING AIDS AND SMART PHONE (IN) COMPATIBILITY
Hearing aid technology is continuing to advance at rapid speeds. These advances allow wearers to hear better in many situations where listening was previously difficult.
Advancements have also been made in the compatibility of hearing aids and smart phones. Many hearing aids can be linked with Apple and Android devices in order to give the wearer direct streaming between the aids and the phone. It’s important to remember though that the primary purpose of hearing aids is to assist you in hearing speech and other sounds in the environment that you are in. The priority is not the streaming of audio from your phone. While this capability is there, it is still far from perfected.
There are many variables which can interfere with the connection between the aids and the phone. One such interference is software updates. Almost every time you take a software update on your cell phone, the pairing between the phone and the aids will be disconnected. You will be required to re-pair the aids to the phone in this instance. Other interferences can be caused by cell service interruptions, wi-fi interruptions, and even other Bluetooth devices that may be nearby.
In almost every situation, connectivity issues are caused by smart phones and not by hearing aids. Hearing aids are always and will always be first and foremost medical devices and not just another cell phone accessory.

Help Reduce Falls at Home
Some injuries from falling can be serious. They can also make it hard to stay independent. But if you take the right actions, you can help reduce falls. Here are some simple ways you can help lower the danger of falls at home.
CLOSET/ATTIC
• Do not climb or reach for high shelves
• Only use a stepladder if it has a handrail on top
LIVING ROOM
• Remove furniture from common walkways
• Keep floors clutter-free
• Do not use throw rugs
• Be sure to secure carpets to the floor

WHAT WOULD IT LOOK LIFE IF AGING WASN’T BOUND BY
AGEISM?
Cathy Bollinger, managing director, Embracing Aging/ executive director, TroveStreetEnrolled in the AARP Network of Age-Friendly States and Communities, York County, PA is committed to helping our residents have opportunities and accessibility to the people, places, and experiences we need as we grow older.
In Spring 2023, we convened our county commissioners and community partners to discuss barriers hindering progress to this goal and identify solutions to breaking through them.
The stigma of aging was a common barrier theme. This did not surprise us because ageism is still a tolerated form of social prejudice in our country. Ageism affects everyone and impacts everything.
Solutions brainstormed by the participants included increasing education on how age bias hurts a community, promoting real life aging experiences, and hosting conversations that reframe the narrative of aging.
One of the most powerful solutions identified was summed up in the form of a simple question, “does it work for people of all ages?” Applying this lens to anything and everything improves livability because if something works for someone age 8 and age 80, it will most likely work for everyone.
A community unbound by ageism means:
• Vibrant opportunities for all ages (cultural, educational, employment, etc.)
• Variety of options across all price points that align with residents’ needs and wants (housing, transportation, services, etc.)
• Residents know what’s available to assist them.
• Simplified process and access to services.
• The community has an intergenerational focus and sense of belonging for all ages.
• Safe, walkable sidewalks and outdoor spaces; accessible parking.
• Existing homes are modified to support safety and comfort as we age.
• Increased communication and collaboration on everything.
• First responders and frontline workers are trained in the impact of ageism and dementia.
With people living longer, we are doing and spending more, and reimagining life beyond the traditional retirement age. Without changes to our community’s physical and organizational structures, and without attitudes and culture that embraces aging, the quality of life for people of all ages is compromised.
This is why York County is working to spread the word that an Age-Friendly plan benefits everyone. We want more stakeholders across all sectors sharing their expertise and perspectives in creating a community that is both a great place to grow up and grow older. A community where age-equitable thinking, decisionmaking, and choices are the norm. A community where people age with dignity, health, and resources to help them thrive.

INVESTING IN THE FUTURE OF OUR CHILDREN
lucky to have that connection right here in York County through Community Progress Council’s Foster Grandparent Program.
More than two dozen volunteers are placed right now in classrooms throughout York County, working with children to help them develop basic learning skills. Relying on their life experiences, Foster Grandparents provide emotional support and one-on-one attention in the areas of reading, writing and math, all to prepare youth for independent living. Volunteer sites include public schools, Head Start centers and non-profit daycare centers.
It’s a win-win for volunteers, who can also remain physically and mentally active through community service. In fact, studies have shown that volunteering and giving back improves overall health and happiness. Volunteers through Foster Grandparent programs and other senior volunteer programs nationwide reported feeling happier and healthier while making a difference in their communities.
“I volunteer because it keeps me active and feeling useful. I also enjoy investing in the future of our children,” says Grandma Sandy.
Grandma Sis agrees: “I love working in the classroom with the children and helping the teacher. It’s satisfying knowing that I helped the children learn something new.”

Keeping Your Loved One Engaged When They DementiaHave
You’ve had a long day at work and you’re tired. But, you need to go home and care for your loved one who has Dementia. Take a deep breath and know there are many ways to have a conversation or keep them engaged:
• Use familiar words and phrases when speaking or asking questions
• Use humor if appropriate but remember to only laugh with them and not at them
• Use objects or props especially items that are familiar to your loved one
• Look interested and mirror their expressions
• Don’t argue – just listen and go along with them
• You may have to repeat yourself – don’t get angry or aggravated because it’s not their fault they can’t remember they just asked you the same question a few times. BE PATIENT!
• Pick some activities to do on a daily basis – things you know they used to do or have always loved doing:

❍ Do arts and crafts – create something together (you’ll always treasure it).

❍ Play music or sing together – music is a great form of therapy and very calming.
❍ Watch old family movies.
❍ Work in the garden or flower beds if they are able.
❍ Do puzzles.
❍ Take a walk as long as it’s safe.
If they still have some cognitive skills, help them do some of the things they love like writing, photography, crocheting or any other familiar hobby.
Don’t worry if they resist or don’t want to do an activity. Take a break to help them relax. You may see your loved one getting frustrated. In order to avoid it escalating, step away or have someone else step in to help calm the situation.
Always concentrate on the activity and not the outcome. Be positive – they’ll remember the time with you
and not the activity.
Your loved one deserves to enjoy each day like they did for years. You deserve to enjoy time with your loved one and interact with them as much as possible. Don’t forget to take care of yourself too and don’t get overwhelmed with stress while you’re being a caregiver. There are many resources out there including support groups to help you through difficult times.

The surgeon inserts a tube into the opening to bypass an obstructionallow air to get to the lungsor remove secretions.
WOUND CARE – Refers to services for providing diagnosis and treatment for ulcerspressure sores and other wounds. Treatments may include but is not limited to irrigationsdressingand wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

SERVICES PROVIDED:
RN-REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family education as needed
SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, and provides counseling and support to meet identified needs, and makes referrals to other providers based on family and
patient’s needs
CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.
VOLUNTEERS – Provides companionship and needed non-medical services for patients and their families, offers support at time of death and during bereavement.
LEVELS OF CARE:
RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in managing the individual’s disease symptoms. Care is provided at the individual’s private residence or in a healthcare facility.
RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a skilled nursing facility. Hospice is responsible for financial arrangements with the facility.
GIP – GENERAL IN-PATIENT CARE – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This higher level
of care is due to symptoms which are out of control and more intensive nursing services are required. The room and board are covered by hospice during this period.
CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided to the patient at their private residence or in a personal care home or assisted living facility. The criteria for continuous care may include: Severe pain or symptoms which are out
of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for crises that require a constant presence by a nurse or care team.
THERAPY SERVICES PROVIDED:
AR-AROMA THERAPY – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

Hospice Care Definitions
MA-MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to. Massage therapy has become essential in hospice care. This is not reimbursed by Medicare.
MU-MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.
P-PET THERAPY – There’s nothing like a visit from a fourlegged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits
of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.
24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.
GRIEF SUPPORT
GROUPS
–
Grief
is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many hospice companies provide Grief Groups that meet and address issues of grief in a supportive group setting.
HOME HEALTH – Indicates
whether the hospice agency has a sister home health agency that will provide assistance with medically based home health needs such as nursing services, home health
aide services, physical therapy, occupational therapy, respiratory therapy or speech therapy.
INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.
PAIN PUMPS AVAILABLE –
Many patients have their symptoms controlled through oral medications. Some situations require a
pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.
PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.
Senior Housing Definitions
ACTIVITIES OF DAILY LIVING (ADLs) – Activities involving personal care and mobility such as bathing, dressing, toileting, walking and meal preparation.
AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.
ASSISTED LIVING RESIDENCES (AL) – Typically apartment-style with kitchenettes and private baths. Assisted living services and personal assistance with ADLs are provided. In PA there is a licensure distinction between Assisted Living Residences and Personal Care Homes. An Assisted Living Residence in PA is “a significant long-term care alternative to allow individuals to age in place… and [to] develop and maintain maximum independence, exercise decision-making and personal choice.” according to PA Code, Ch 2800.
ADULT DAY SERVICES – Most providers can elect to provide day/ night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day.
CONTINUING CARE RETIREMENT COMMUNITY (“CCRC”) – There are many different types of CCRCs. There are Pay for Fee Communities (Type C), Life Plan Communities
(Type A) and even some rental models (Type D) inside CCRCs, but the unifying factor Is that they provide all levels of care - Independent Living, Assisted Living/Personal Care, Memory Care and Skilled Nursing. Many offer many additional amenities, such as Rehabilitation Facilities, meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces.
CCRC DEFINITIONS:
BENEVOLENT PROGRAM – A program created to maintain a resident’s lifestyle when they run out of resources through no fault of their own.
FEE FOR SERVICE COMMUNITIES (TYPE C) – Includes housing, residential services and offers different levels of healthcare, but not at the same monthly cost. Where you are on the continuum of care dictates what you pay for the monthly fee. There is typically an up-front fee that varies based on the type residential amenities and other factors.
LIFE CARE COMMUNITIES (TYPE A) – Includes housing, residential services and unlimited use of healthcare services with little or no increase in monthly fees. Your monthly fees are not dependent on the level of care that you need, which is why a Life Care Community may charge a larger up-front fee, as the Life Care Community is
absorbing the risk on the resident’s level of care needed.
RENTAL AGREEMENT (TYPE D)Typically a small amount of rental properties are available which are not subsidized. Does not participate in the buy-in fee, nor has access to the continuum of care benefits.
INDEPENDENT LIVING COMMUNITIES – Apartments and communities for adults 55+.
INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADLs) – Activities involving more complex functioning such as grocery shopping, writing checks, housework, cooking, travel-
ing, and managing money.
INTERMEDIATE CARE – 24-hour inpatient care for residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive medications from a nurse following a doctor’s orders and may receive outpatient rehab services.
MEMORY CARE RESIDENCES (MC)
– Licensed Assisted Living or Personal Care Homes that specialize in delivering care to patients with Alzheimer’s and other dementias. Commu-

PERSON CENTERED COUNSELING (PCC)
PA Link has implemented PCC to assist consumers who are inquiring about publicly and privately financed Long Term Services and Supports (LTSS). PCC is defined as an interactive, decision-support process that helps people assess and understand their needs, goals and preferences regarding LTSS services. PCC is a voluntary program provided to consumers free of charge and represents a new way of supporting Pennsylvanians with LTSS access. PCC
will be available without regard to income or disability to prospective LTSS users and the family members and significant others who support them.
PCC is an interactive, decisionsupport process that helps people assess and understand their needs, goals and preferences regarding LTSS services. Person-Centered Counseling focuses on: Interaction, Facilitation, Evaluation of options, Decision support, and Follow-up.
ALPHA NEEDS YOU
Do you know a senior that needs support? Perhaps they could use assistance with their daily activities, or they spend too much time alone? Personal Care Homes (PCHs) and Assisted Living Residence (ALRs) exist to help fill this gap. PCHs provide a supportive atmosphere but sometimes their monthly cost is out of reach for a low-income Pennsylvanian. PA has a program built to help very low-income individuals needing care. Using a combination of income, federal and state dollars, there is a limited payment (“SSI Supplement”) available for PCHs or ALRs. Unfortunately, the SSI Supplement amounts to only $46 per day for the PCH/ALR and this rate has only increased by the Federal COLA increase and $7 since 2006. This payment is less than 2/3’s of the market rate for PCHs and ALRs. In fact, we have lost more than 53% of the SSI providers since 2008: How could we be surprised that very few providers are able to provide care for this rate, especially since the inflation rate since 2006 has increased by more than 50%? Individuals that qualify for this assistance find it nearly impossible to find a room.

WHAT IS ? ?
I have a very important question to ask you. What does the phrase “Living Well” mean to you? In other words, if you were living life to the fullest or are having a good day, what would you do that day? For me, it would mean that I could get out of bed when I wake up – not with an alarm.
Next, I would have the opportunity to go and water my flowers, pull a few weeds in my garden or harvest something yummy out of the garden. Then I would have a chance to play a game or two with my son and husband or perhaps to take a walk with my dad and talk to my mom, daughter and friends. It would also mean that I might have the opportunity to kayak, go to the beach or cook a meal for friends. That would be a truly great day! Whatever a “Living Well” day looks like to you, all I ask is that you think about it in enough detail that you could write it down.
Now that you have that perfect day in mind, I need to ask you to think about something not so wonderful. What would happen if you were in a car accident or suddenly became very ill? Let’s suppose that you were so hurt or ill that you couldn’t speak. What would your loved ones do? How would they know what treatment options and medical decisions to make for you? For that matter of fact, who would you want making those decisions? What personal values or beliefs do you have that you would want considered when making these types of decisions?
Take a deep breath. I know that this can be a hard subject, but you are not alone in trying to figure this out! This process that I’ve begun to outline is called Advanced Care Planning or
ACP for short. ACP involves several issues and is for all adults over the age of 18. It means that you think about and plan for future healthcare decisions if you had sudden healthcare event, as I outlined above. There are many different type of individuals who help you to identify and document your decisions such as Social Workers, Care Managers and Advanced Care Planning Facilitators. Why should you care? Simply put if you care about the burden your family and/or loved ones would be under if you were taken ill or injured or if you care about your wishes being heard, then please look at the Advanced Care Planning process.
