Let’s Talk About Heart Disease and High Blood Pressure

Nearly half of adults in the United States have high blood pressure, but only 1 in 4 adults have their condition under control. Your race and biological sex can play a role with your blood pressure control.*

Did you know? In Pennsylvania, Hypertension impacts about 20-40% of adult residents.

A few facts about Blood Pressure Control:

• Men tend to have higher or uncontrolled blood pressure*

• High blood pressure is more common in non-Hispanic black adults than in Hispanic, non-Hispanic white or Asian adults.*

• Blood pressure control is highest among non-Hispanic white adults than in nonHispanic black, non-Hispanic, Asian, or Hispanic adults.*

Some people use folk medicine to help improve their health. However, in folk medicine traditions, references to “High Blood” and “Low Blood” are NOT the same thing as High Blood Pressure. In folk medicine, “High Blood” conditions are treated with

eating lots of salt. This is just the opposite of what should be done to treat High Blood Pressure. Have an honest discussion so that your doctor can develop a plan that works best for you and your beliefs. Be sure to tell your doctor about any herbs and supplements you use. Add these to your medication list.

BBB Spotlights Riskiest Scams for Seniors

Better Business Bureau research has found that seniors have a lower susceptibility to falling for scams, but those 65+ report a higher median dollar loss to fraud.

Data used to generate the riskiest senior scams was determined by using the BBB Risk Index to analyze exposure, susceptibility and monetary loss of scams reported to BBB Scam Tracker (BBB.org/ScamTracker).

BBB Scam Tracker is a crowdsourced tool that enables consumers to report and track instances of fraud.

Riskiest Scam Types for Older Adults:

1. Home Improvement Scams: Door-to-door solicitors offer quick, low-cost repairs and then take payment without returning, do shoddy work or “find” issues that dramatically raise the price. These schemes often occur after a major storm or natural disaster.

2. Online Purchase Scams: These scams involve the purchase of products and/or services where the transaction occurs via a website or other online means. Scammers use technology to offer attractive deals, but once payment is made, no product or service

is delivered. In some cases, fraudsters send low-quality or counterfeit products.

3. Cryptocurrency Scams: These scams involve the purchase, trade or storage of digital assets. The situations often involve fraudulent Initial Coin Offerings (ICOs). Investors pay money or trade their digital assets even though the scammer has no intention of building a company. Cryptocurrency scams also involve scenarios in which investors store their cryptocurrencies with fraudulent exchanges. Scammers use a variety of tactics to perpetrate scams, including impersonation, the promise of an opportunity and threats to encourage engagement. Never share personally identifiable information with someone who has contacted you unsolicited and always use secure, traceable transactions when making payments.

Research companies through BBB to find out a company’s rating, complaint history and more before making a hiring or purchasing decision.

A New Role: Caregiver

A new dementia diagnosis for a loved one is a shock. It means your life will be different than what you thought it would be. Now, in addition to being a wife, husband, son or daughter, you are a caregiver for someone with an unpredictable, progressive disease.

Or maybe the diagnosis was made a while ago, but lately, it seems that they need more help from you than they did last year, or last week, or even yesterday.

Here are some strategies for managing the role of caregiver:

TAKE A BREAK FROM CAREGIVING

This isn’t selfish; it’s needed for your own well-being. You need to stay physically and mentally healthy to be an effective caregiver.

NOURISH YOUR RELATIONSHIPS

Take time to be the husband, the wife, the daughter, the son, the friend you always were. Maintain social connections for your wellbeing and the well-being of the person for whom you are caring. They still need you, they need emotional connections, they need to feel loved and know that they still matter.

CREATE A ROUTINE

As dementia progresses, the familiarity of a routine will help them to be as independent with

care as possible for as long as possible.

JOIN A SUPPORT GROUP

Caregiving can be isolating, and being with others in similar situations is a great way to receive support and validation, and to realize you are not alone.

OTHER THINGS TO REMEMBER

When you’re frustrated, step away for a moment, calm down and then try again.

Let people in your life know what you need from them—don’t make them guess, tell them.

Learn as much as you can about the disease, what to expect, and what your family member needs from you.

Life does not prepare anyone to be a dementia caregiver.

contribution allowances

For those age 60-63, the catchup contribution limit will increase to the greater of $10,000 or 150% of the regular catch-up amount for 401(k) and similar type plans. The higher allowance is effective starting in 2025.

Cost of living adjustments to IRA catch-up contributions

Individuals age 50+ can currently contribute an additional $1,000 to either a traditional or Roth IRA. Beginning in 2024, catch-up contributions will be indexed for inflation.

REQUIRED MINIMUM DISTRIBUTION (RMD) CHANGES

RMD age increased to 73, but individuals can still do Qualified Charitable Distributions (QCD) at 70 ½. Beginning in 2023, a one-time QCD of up to $50,000 can be directed towards a splitinterest entity such as a Charitable Remainder Annuity Trust (CRAT), Charitable Remainder Unitrust (CRUT) or Charitable Gift Annuity (CGA).

TRANSFER OF EXCESS 529 FUNDS TO A ROTH IRA

Effective in 2024, a beneficiary’s

unused 529 funds may be transferred to a Roth IRA in the name of the beneficiary, subject to several rules:

• The lifetime amount that can be transferred is $35,000 and is subject to the annual contribution limits.

• The 529 plan must be established for at least 15 years.

ROTH RULE CHANGES

Effective 2024, individuals will no longer be required to take distributions from Roth accounts from employer plans.

Roth-style version of SEP and SIMPLE IRA accounts

Effective in the 2023 tax year, you can now create SIMPLE Roth IRA and SEP Roth IRA accounts.

CATCH-UP CONTRIBUTIONS REQUIRED TO BE ROTH FOR HIGH WAGE EARNERS

• No taxes paid on withdrawals for qualified expenses; and

• Gift and inheritance tax benefits.

Also, PA 529 account assets don’t affect PA state financial aid eligibility.

Before opening an account, check with your grandchild’s parents to see if they already have a PA 529 account. If you do decide to open a PA 529 account, you’ll need some information like the child’s Social Security Number.

Seniors Staying Safe at Home with LIFE Pittsburgh

LIFE Pittsburgh, a nonprofit program that provides comprehensive and integrated care to seniors who may not otherwise be able to live independently at home, continues to provide care to their participants by keeping them safe in their homes during these uncertain times.

While adapting the process at their Adult Day Health Centers to ensure the health of current participants, LIFE Pittsburgh also left enrollment open for new participants who would benefit from access to their essential programs.

LIFE Pittsburgh has been serving the elderly population in Allegheny County for over 20 years with a comprehensive program of support services that provide seniors the ability to maintain their independence in their homes and communities. The program model, known nationally as PACE (Program of All-Inclusive Care for the Elderly), provides the participants, family, caregivers, and professional health care providers the flexibility to meet those health needs with integrative care.

Eligible participants must be 55 years old or older, live in the designated service area, meet the state’s medical requirements for nursing facility level of care, be able to live safely in the community, and be financially eligible as determined

by the Allegheny County Assistance Office or be able to privately pay. Individual care plans are coordinated for each participant to assist in medical, social and daily living activities while focusing on providing care with respect and dignity as outlined in the Balanced Budget act of 1997 set forth by the National PACE association.

A participant of LIFE Pittsburgh’s team may include a doctor, registered nurse, master’s-level social worker, licensed therapist, personal care aid, or registered dietician in addition to the center manager and staff. Care for participants can be daily vital checking, solution focused health routines, communicating with family members the participant’s ongoing needs, surveying home accessibility, or providing transportation to appointments with their LIFE Pittsburgh wheelchair accessible van fleet.

New 2023 Prescription Drug Law

A new prescription drug law that went into effect January 1, 2023 will help save money for people with Medicare. This law improves access to affordable treatments and strengthens the Medicare program.

In addition, several changes to improve the way Medicare beneficiaries can access behavioral health care will take effect in 2023. Changes could also be coming to telehealth.

Beginning in 2023, under a provision in the Inflation Reduction Act, Part D enrollees will pay no more than $35 per month for covered insulin products in all Part D plans, and will pay no cost sharing for adult vaccines covered under Part D.

Under the provision, copays for a 30-day supply of any insulin that a Medicare drug plan covers will be capped at $35. Enrollees won’t have to pay more than $35 even if they have not yet met their annual Part D deductible. Note that not every plan covers every type of insulin.

Also under the new law, vaccines recommended for adults which

are covered under Part B are already free to Medicare enrollees. Others are covered by Part D prescription drug plans and require beneficiaries to pay some of the cost. These include shots for the flu, pneumonia, COVID-19 (initial shots and boosters) and hepatitis B for some enrollees are free under Part B and that coverage will continue in 2023. Part B also covers vaccines needed to treat certain injuries or exposure to a disease. Those include hepatitis A, rabies, and tetanus.

Other vaccines, most notably the two-dose Shingrix which guards against shingles, are covered under Part D prescription drug plans but may require cost sharing depending on the plan. Shingrix, for example, can run as much as $200 a dose.

Another provision of the new law that will be fully effective in 2023 requires pharmaceutical manufacturers to pay a rebate to Medicare if they raise prices of their drugs more than the rate of general inflation.

Free Moving Service for Seniors

Most seniors as they age will need to change their residences as a result of medical, financial, or social factors.

In addition, most seniors require significant downsizing of possessions and memorabilia. Such relocation requires significant financial costs. Seniors who are economically disadvantaged in our region have very little access to resources that would assist them in moving to a new home. If the older adult is struggling with a physical disability and has an existing mental health diagnosis, this creates an even stronger need for the right

type of assistance - one with trained staff members who can assess a client’s needs, plan for those services, and provide a continuum of services.

Our mission at Safe Moves for Seniors is to provide free moving services to those seniors.

Make Giving Back Your Second Act

Older individuals are active more than ever before. It has been proven that they like to give back to their communities through volunteerism.

To connect with others and be part of something bigger has been reported to give individuals fewer feelings of isolation. 88% of volunteers experience decreased anxiety, depression, and loneliness. While 84% report stable or improved health after one year of service.

If you enjoy encouraging children to be their best self, and you are 55 or better and on a limited income, the Foster Grandparent Program could be what you are looking for.

Giving back by serving children and helping those in need gives you purpose and helps teach them empathy and compassion. Children need positive, caring individuals to influence them with social, emotional, educational, self-help and learning skills. It’s the little things that stand out to a child. Something as simple as being there on a consistent basis, listening, and providing extra one-on-one support can make an impact on a child’s life. These small acts help build children’s confidence, self-esteem and life skills while building them up for success.

There aren’t any educational requirements, just a passion to help children.

A Foster Grandparent volunteer provides 15 or more hours to support non-profit schools, Head Start

and Early Head Start classrooms, childcare and development centers and after-school programs. Through service, volunteers can earn a nontaxable, non-reportable stipend that does not affect current benefits or services one may already have or have in the future, there is also travel reimbursement, supplemental accidental and liability insurance, professional development and recognition.

Grandma Marlene says, “It’s a wonderful program, it gives you the opportunity to be compassionate and watch out for the well-being of children. Not to mention it keeps you active.” Why don’t you bring out the best in America by making giving back your second act?

24 HOUR CARE – Refers to “round the clock care” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

24 HOUR CONTACT – Refers to someone being available 24 hours a day to speak with you.

CERTIFIED NURSING ASSISTANT (CNA) – Provides personal care such as bathing, dressing, changing linens, transporting and other essential activities. CNAs are trained, tested, certified and work under the supervision of an RN or LPN.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companionship and other household duties.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button.

MEDICATION REMINDER –

Assistance with reminding an individual about medications but not dispensing medications.

MINIMUM HOURS – This refers to the minimum number of hours that an agency provides service on a per visit basis.

PERSONAL CARE – Non-medical assistance with bathing, dressing, hygiene, eating and other personal care issues.

TRANSPORTATION – Rides to shopping, appointments and other outings.

Initial Safety Assessments for Seniors

It is estimated that there are over 3.5 million potentially preventable adult hospitalizations every year in the US. These are defined as hospital stays of at least three nights and do not include short trips to the ER.

As verified by the Agency for Healthcare Research and Quality, most of these stays are related to seniors not properly managing their medical conditions! To prevent these hospitalizations, a Senior Care Agency that provides in-home care must perform an assessment of a senior’s living situation to understand what areas can be improved. This evaluation should focus on 5 key areas: Safety Risks, Medical Condition Monitoring, Independence, Quality of Life, and Caregiver Support.

The homecare agency should evaluate 13 key areas that a Senior must have their needs met to successfully “Age in Place”. These areas include mobility, activities of daily living, and the number one reason for hospitalizations: proper medical condition management. After the in-home assessment is complete, the homecare agency can analyze the findings and make recommendations for corrective action. This will greatly enhance the five key areas of focus that were originally identified. To illustrate, on average, most Seniors participate in only six – eight activities that

they enjoy each week. Tools such as a Quality-of-Life weekly Activity Planner can be utilized to track and increase participation in quality of life activities, leading to decreased depression, improved strength, and increased motivation. Safety hazards can be identified and mitigated. Furthermore, any additional gaps in the five key areas can be addressed and corrected. Some in-home care agencies can now do this electronically even generating reports that can be utilized cross functionally across the healthcare spectrum (Doctors, Social Workers, Care Managers, etc.). Proprietary tools such as LIFE Profile from Senior Helpers is one such example.

In short, a thorough and complete initial assessment performed by a senior care agency can have a significant positive impact for seniors, leading to better outcomes.

24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment, specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness.

Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

THERAPY SERVICES (ST, PT, OT, RT) –Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills.

Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but are not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

HOSPICE Hospices provide individuals facing a terminal illness with care in a private residence or in a healthcare facility. The goal is to provide quality of life and to assist patients to live the last stages of their lives with dignity. The focus is on comfort rather than curative treatment of the terminal illness.

SERVICES PROVIDED:

RN-REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family education as needed

SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, and provides counseling and support to meet identified needs, and makes referrals to other providers based on family and patient’s needs

CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

VOLUNTEERS – Provides compan-

ionship and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

LEVELS OF CARE:

RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in managing the individual’s disease symptoms. Care is provided at the individual’s private residence or in a healthcare facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a skilled nursing facility. Hospice is responsible for financial arrangements with the facility.

GIP – GENERAL IN-PATIENT CARE

– Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This higher level of care is due to symptoms which are out of control and more intensive nursing services are required. The room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided to the

patient at their private residence or in a personal care home or assisted living facility. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for crises that require a constant presence by a nurse or care team.

THERAPY SERVICES PROVIDED:

A-AROMA THERAPY – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

MA-MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to. Massage therapy has become essential in hospice care. This is not reimbursed by Medicare.

MU-MUSIC THERAPY – Music

Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals.

This is not reimbursed by Medicare. P-PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings, weekends, and holidays.

GRIEF SUPPORT

GROUPS

– Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many hospice companies provide Grief Groups that meet and address issues of grief in a supportive group setting.

HOME HEALTH – Indicates whether the hospice agency has a sister home health agency that will provide assistance with medically based home health needs such as nursing services, home health aide services, physical therapy, occupational therapy, respiratory therapy or speech therapy.

Knowing when to pursue hospice care can be a difficult decision. Whether you are a patient or a family member, having the correct information about hospice services can help you decide whether this care is appropriate.

Hospice is for people with advanced illness, no matter their age, culture, beliefs, or cause of illness. Hospice care supports people near the end-of-life by providing individualized plans focused on each patient’s wishes.

What to Know About Hospice Care

Hospice is for patients who have 6 months or less to live according to their physician. This can be extended when needed. Patients can leave or “revoke their hospice status” at any time. They can also come back to hospice at any time, as long as they meet hospice eligibility guidelines. There is no time limit a patient can remain on hospice.

Also, most hospice patients do not have any out-of-pocket expenses. Medicare Part A covers up to 100% of the cost of hospice care related to a hospice-eligible patient’s illness, with no deductible or copayment. Private or employerprovided health coverage can vary by provider. Medicaid provides hospice coverage, but it varies by state.

Why Hospice? Improved Quality of Life

Hospice is a service that comes to a patient in the place they prefer to be for end-of-life care. Patients at home feel better surrounded by people and things they love. To ease pain, hospice doctors try to give just enough medicine to manage the symptoms and comfort level of the patient. Hospice also provides home medical equipment and supplies related to the cause of the illness.

With support and expertise from the hospice team, family and friends are a hospice patient’s primary caregivers. For patients who live in assisted living communities and nursing homes, the hospice team works together with the facility staff.

For more than a year after a patient dies, hospice addresses the emotional and spiritual pain suffered by loved ones.

They provide bereavement support to help family members and friends cope with the loss and grief.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If

this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

It is important to think of hospice as a level of care designed for those with advanced illness towards the end of life, not as just a final step in the dying process. Patients also have the freedom to leave hospice care, known as revoking their hospice status, at any time should they decide to pursue more aggressive treatments. They can then return to hospice care at any time as long as they remain eligible. Unlike other healthcare

services, there is no specific time limit for how long a patient can receive hospice care. Hospice agencies are there for you at any time to discuss your options, talk about services, and answer any of your questions, you should feel free to call at any time. In conclusion, the hospice benefit provides an invaluable service that is tailored to the patient’s needs and provides immense support to them, their caregivers and loved ones.

SENIOR HOUSING DEFINITIONS

ACTIVITIES OF DAILY LIVING (ADLs) –

Activities involving personal care and mobility such as bathing, dressing, toileting, walking and meal preparation.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.

ASSISTED LIVING RESIDENCES (AL) –

Typically apartment-style with kitchenettes and private baths. Assisted living services and personal assistance with ADLs are provided. Rates vary from an all-inclusive to base rates with levels of care. Amenities vary for each residence. In PA there is a licensure distinction between Assisted Living Residences and Personal Care Homes. An Assisted Living Residence in PA is “a significant long-term care alternative to allow individuals to age in place…and [to] develop and maintain maximum independence, exercise decision-making and personal choice” according to PA Code, Ch 2800.

ADULT DAY SERVICES – Most providers can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT (HUD) – Helps apartment owners offer reduced rents to low-income tenants.

INDEPENDENT LIVING COMMUNITIES (IL) – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...).

INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADLs) – Activities involving more complex functioning such as grocery shopping, writing checks, housework, cooking, traveling, and managing money.

INTERMEDIATE CARE – 24-hour inpatient care for residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADLs and transfers. Residents receive medications from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as their consultant and the residents receive periodic assessments from a licensed practitioner.

MEMORY CARE RESIDENCES (MC) –

Licensed Assisted Living or Personal Care Homes that specialize in delivering care to patients of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Specifically trained staff usually serves the special needs of this population. Amenities vary for each community.

PA LICENSURE – In PA there is a licensure distinction between Assisted

Living Residences and Personal Care Homes.

PERSONAL CARE HOME (PCH) – Typically apartment-style with private baths. Personal assistance with ADLs is provided. Rates vary from all-inclusive to base rates with levels of care. Amenities vary for each home. In PA there is a licensure distinction between Personal Care Homes and Assisted Living Residences.

RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, PCH, AL, MC or SNF care can elect to provide fully furnished accommodations to a person for a short period of time, if space is available. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities.

RETIREMENT LIVING

COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services may include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly and typically based upon size/ location of apartment.

SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SKILLED CARE – 24 hour licensed nursing services. Patients may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physician’s orders.

SKILLED NURSING FACILITY (SNF) –

Provides 24/7 care for residents and offers full assistance with ADLs. Daily rates typically include a furnished bedroom, private or shared bath, meals, activities, transportation, phone service, cable TV and all personal/nursing care. Payor sources usually accepted: Medicare, Medicaid, VA, some insurance plans and private sources.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy, a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

VENTILATOR – Moves breathable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable.

WANDERGUARD – Offered at some facilities. A resident with memory loss or confusion wears a small discreet pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering.

Choose the Right Rehabilitative Care

After experiencing a life-changing stroke or other illness, your doctor may recommend rehabilitation as the next step in your journey to recovery – but what does that mean, and where should rehabilitation take place?

These settings – known as postacute care – include home health, inpatient rehabilitation, skilled nursing homes and long-term acute care hospitals. Should you need additional care, your care team could refer you to one of these settings based on your abilities and goals; but as a patient, you have the right to choose where you go for post-acute care.

Your doctor said you need rehabilitation. What next?

Independent studies show that inpatient rehabilitation hospitals are a better setting for stroke, hip fracture, and hip replacement patients. Unlike skilled nursing facilities, inpatient rehabilitation hospitals are required to provide at least three hours of therapy five days a week and will allow access to trained physicians and rehabilitation equipment and technology that may not be available elsewhere.

On average, inpatient rehabilitation hospitals have a lower length of stay and discharge more patients home than skilled nursing facilities. This means inpatient rehabilitation hospitals are able to help individuals return

to their families, their jobs and their communities more effectively and more quickly.

The services delivered at inpatient rehabilitation hospitals serve as a next step for many patients who are ready to leave the acute care hospital, but need extra rehabilitation and recovery before returning home.
