How to Use this Guide
GETTING STARTED IS EASY Our guide is divided into 3 distinct sections and divided by color tabs: 1. Community Resources and Services This section is alphabetical. You will find hundreds of different services and organizations that work with seniors and their families. This includes everything from networking to professional services, such as Adult Day, Care Management, Elder Law, Social Security, Senior Centers, etc… 2. Health at Home – Home Health & Personal Care Agencies This section offers four different comparison spreadsheet/grids; one for Homemaker Companion (nonmedical care and personal assistance at home) and two for (Medical Care) Private Pay Home Health Agencies and Medicare Home Health Agencies. First familiarize yourself with the different services that are offered by each agency. You will find definitions about each service in this section. Older adults are often targeted by thieves who think of them as “easy prey”. You can minimize the risk of becoming a victim. Consider the following tips:

1. Eat Healthy: As people age, it can become difficult to prepare nutritious meals. Meals on Wheels PLUS helps individuals maintain a healthy diet. Noontime meals are delivered Monday through Friday, and are specially designed by a dietician for adults over 60 years of age. 2. Stay Active: It is crucial to stay active in both mind and body. Whether it is attending line dancing classes at the Senior Enrichment Center, or volunteering, it is important to have hobbies and regular activities. 3. Don’t Be Afraid to Stop Driving: Giving up the car keys doesn’t necessarily mean giving up independence. Senior Wheels, a membership-based transportation service for Manatee County residents takes seniors to
appointments and errands. 4. Make Friends: As an adult, it can be difficult to meet new people, but having friends makes the aging process much easier. One opportunity is attending the Senior Lunch Club, which meets weekly in Lakewood Ranch and in Bradenton at Renaissance on 9th. The lunches offer interesting speakers, fellowship and a delicious meal. 5. Be Purposeful: Maintaining a positive outlook on life can be made easier if one has a sense of purpose. Be passionate about something—a hobby, cause, organization and enjoyable activities. Meals on Wheels PLUS offers a variety of volunteer opportunities. YOUR SURGICAL TEAM: The Unknown Force in the OR WHO ARE THEY? THEY ARE THE QUIET FORCE THAT IS CRITICAL for a positive outcome before, during and after the surgical process. Consisting of operating room nurses and surgical technicians, the team supports the operating surgeon and anesthesiologist. The Operating Room (OR) Nurse: The primary job of the OR nurse is patient advocate. From start to finish, throughout the entire surgical experience, until the patient leaves the OR and is handed over to another nurse in recovery, the OR nurse will follow her/his patient, making sure that safety checks are followed. He or she will interview, review, assess and learn everything necessary in order to support the patient. Identification, mental acuity, specific allergies, incapacity such as hearing, eyesight, prosthesis and ambulation are noted. Consents are carefully reviewed noted and checked. The surgical site is inspected for the surgeon’s mark. The OR nurse will then assist in the transfer of the patient into a warmed OR bed. The patient is secured and made comfortable. After transport to the OR and before anesthesia agents are administered, a time-out is called with patient participation. The entire team goes through a complete safety check identifying the patient, the procedure and the particulars, specific to that patient. Right patient, right surgeon, right site and side, allergies and med-
ications noted. Quiet is demanded during this important process. The Surgical (Scrub) Technician: The surgical technician works under the direction of an OR nurse or physician. He or she is primarily responsible for the sterile imperatives in the surgical field. They apply the sterile drapes to the surgical site, label medications passed from nurses to the site, assist nurses with patient care, scrub patients as needed, anticipate and assist surgeons with their every need. No two operations are exactly the same. Each patient has a different profile and history. Anticipating those variances while following protocol and safeguards is the role of the team. The nurse will order any blood products that might be needed and will make sure that specimens arrive safely at pathology. When the procedure is finished and final safety checks are done, the team assists with the transfer of the patient to their post-op bed. At that time, a detailed ‘hand-off ’ is given to the nurse taking over the patient’s care. The general patient population is often not aware of the superior care they are receiving by the surgical team. Communication between the surgeon and surgical staff is vital. The OR team is never in the limelight, but always behind the scenes. The mission of the OR is exceptional care for every patient. Every time. Excellence is the standard.”

The NICHE Program at Venice Regional Bayfront Health THIS YEAR, 80 PERCENT OF PATIENTS AT OUR HOSPITAL were 65 years of age or older? Remarkable, considering the average age of Florida hospital patients is 54. The acronym NICHE stands for Nurses Improving Care for Healthsystem Elders. The goal of NICHE is to ensure that hospitals throughout the country operate in a seniorfriendly manner. They set standards for nurse training, hospital’s environment and procedures to assure that geriatric patients’ needs are being served. Venice Regional meets all the standards. For most hospitals that have earned a NICHE designation, it is almost always for a single department within their hospital. In our case, the entire hospital is NICHEdesignated. To obtain a NICHE designation. Every policy and procedure has to be examined. For example, floors cannot be too shiny. An elderly patient sees a shiny floor and thinks it’s slippery. That can cause a fall because the floor isn’t slippery, which can cause a fall. Even if the floor is not slippery, the light reflected from a shiny floor can make it difficult for some elderly patients to see where they’re going.
Beds need to be equip-ped with weight-change alarms, to alert the nursing staff if a weak or disoriented patient, who shouldn’t be out of bed decides to try. Video monitors may be needed to keep track of patient movements to alert staff when assistance is needed. But being senior-friendly goes way beyond a hospital’s facilities. Venice Reg-ional Bayfront Health has 20 nurses who have been specially trained to be Geriatric Res-ource Nurses, helping other staff properly manage elders’ specific care needs. For example, elders may have agerelated changes in their skin, nutritional needs, sleep disorders, depression; and fall prevention is a vital concern. In addition to bed alarms and video monitors, many other steps must be taken to prevent falls. Nurses must make frequent rounds with patients if they have had surgery, have an IV, are using a cane or walker, are not alert or have a history of falling. They are assessed twice a day. It’s not easy being a senior, and it’s no piece of cake to take care of a senior either. When you have to go to a hospital, it brings peace of mind to choose one that has a NICHE for you.

Dr. Rozales Swanson regularly performs minimally invasive lung, esophagus, and mediastinal surgery, valve repair and replacement procedures, coronary artery bypass grafting, aneurysm, dissections, MAZE procedure for the treatment of atrial fibrillation, and carotid surgery. He also performs endobronchial ultrasound, navigational bronchoscopy, and bronchial valves and robotic lung surgery. Dr. Robert Hooker is experienced in implanting ventricular assist devices and thoracic transplantation. He specializes in thoracic aortic pathology, valve repair and replacement, and arrhythmia surgery, and has written multiple, reviewed journal articles. ACCORDING TO THE US CENSUS, there are roughly 40 million people aged 65 and over, and only 4 percent live in institutionalized settings. Thirty percent of people not living in a nursing home live alone. The most costly 5 percent of Medicare beneficiaries account for about half of Medicare expenditures. They have chronic illnesses, such as dementia, CHF, stroke, are symptomatic with functional impairment and at risk for ER visits and hospitalizations. They have multiple providers, lack of continuity of care, limited access to care and poor communication between settings. So what is the answer? Sarasota Manatee Geriatrics is a medical house calls practice. It was founded to meet
the growing needs of older adults in our community who have difficulty leaving their homes, assisted living or nursing facility. These patients are seen in their homes. House calls improve overall care, patient outcomes and lowers costs. Our first visit is more than 2 hours, including a thorough physical exam, review and refills of medications, communication and education for patients and families, safety, psychosocial and functional evaluation, advance care planning and arrangements for inhome lab, diagnostics and therapy when appropriate. We complete essential paperwork for moving into an ALF (1823 form), medical equipment and transportation forms.

Suncoast Center for Independent Living Empowers Seniors with Disabilities SUNCOAST CENTER FOR INDEPENDENT LIVING (CIL) IS a non profit organization serving people with disabilities within Sarasota and Manatee Counties. Our goal is to empower people with disabilities to take charge of their lives and guide them through their own destinies. Services are free of charge and provided to people with disabilities of all ages residing within Sarasota and Manatee counties. CIL staff, boards, and consumers work together to remove barriers and prejudices in society so that all individuals can live and work and enjoy all that their communities have to offer. Fiftyone percent of the staff and boards of CILs are persons with disabilities, which means that they play significant roles in the decision-making responsibilities of the Centers.Here is one of our many success stories. William's Story On his birthday, William received a gift that people often take for granted. No, it wasn't a new car, clothes, or even a vacation. He received the gift of independence through the installation of a new wheelchair ramp for his home. The lack of a wheelchair ramp resulted in multiple falls in front of his wife, whose petite frame prevent her from assisting him. With no other recourse she was forced to contact 911 each time for assistance. After the installation of
his new ramp William was excited to be able to exit his home for the first time in three years and enjoy his independence in the fresh air and Florida sunshine. SCIL provides information and assistance for people with disabilities and their families in promoting change when faced with barriers and discrimination and in accessing support systems, benefits and services such as applying for social security benefits. We help with appeals, discrimination, housing information, Medicaid benefits, ADA consultations and self advocacy. We offer a Peer Mentoring Program that offers opportunities to interact with peers. Sharing ideas, experiences, and difficulties while obtaining practical advice through one-on-one support group activities that promote positive living with a disability. SCIL provides comprehensive and currentinformation to Sarasota and Manatee residents on available resources for people with disabilities. It assists individuals by providing referrals for: SCIL programs and services, community resources, assistive technology and Federal, State and County regulations and housing operations. VOLUNTEERS IN THE SARASOTA COUNTY SCHOOL DISTRICT give children abundant measures of their time, talent, experience and heart. Still, volunteers always say that they get more from the experience than they give. Bunny has been volunteering at the same school for 20 years. She said, “I have always felt that I received much more from the students than I could ever give to them. Just being able to talk to them one-on-one, with whatever their problem was that day, was a good feeling.” A volunteer tutor for 11 years, Jack reports, “Being able to tutor students has been one of the most personally rewarding experiences of my life. We all hope to leave a legacy. I hope that many years from now some of my students will say, ‘Mr. Jack taught me this.’ What could be more satisfying?” Marge has been a volunteer reading tutor for 10 years. “Being a school volunteer has added great enrichment to my life,” she said. “My day at school is the best day of my week. I hope that I am making a difference in students’ lives. I know that they have made a difference in mine!” Murray is a retired superintendent of
schools and natural-born teacher. “Volunteering in the Sarasota County Schools gives you the wonderful opportunity to light up your life with the smile of success on a young child's face!” he said. “It is a very special sense of delight when the children I tutor “get it”! Multiply that good feeling by a week, a month, or a school year and you will understand the awesome feeling of positively and successfully touching the life of a child! I love it!” School volunteers help students master skills and experience the joy of learning. They share their life experiences, serve as mentors and role models, and let students know they care. For a little as an hour a week, you can join Bunny, Jack, Marge and Murray in making a difference in a student’s life and in your own. Volunteers with a wide range of skills and interests are needed throughout the school district at all grade levels. Free training is provided for tutors. You receive a free annual membership for yourself and one for a friend, and the quarterly newsletter. Contribute one afternoon a week or a few hours during the month. We will arrange your driving assignments and routes around your schedule. You can even pick up a rider on your way to work and begin your day with a good deed. All of us have a stake in helping to ensure that older adults and the visually impaired can remain active once they stop driving. Volunteers help older people stay connected to families, friends, and activities that bring meaning to their lives - and to yours.
Who we are-The Independent Transportation Network(r) (ITN) adds dignity to transporting seniors. Older adults who join ITNSarasota become dues-paying members of a non-profit organization committed to their independence and mobility. When they pick up the telephone to schedule a ride, they are not asking a favor. The transition from the driver’s seat to the passenger seat is never easy - for the drivers or for their families. However, it is infinitely easier when there is a viable transportation alternative that keeps the older adult in control. Our mission is to provide excellent customer service with the armthrough-arm, door-through-door standard that sets us apart from taxis. Like using one’s own automobile, the ITNSarasota service is available 24/7 and gives customers the flexibility to schedule their rides in advance or as needed, as well as the option to ride alone or with others, as they please, to destinations of their choice. ITN is a membership-based service for those over 60 years of age that provides affordable fares that are typically lower than a comparable taxi ride and Available 24/7 for any purpose. Discover Communication Freedom with Florida Relay 711 AN ESTIMATED THREE MILLION PEOPLE IN FLORIDA ARE DEAF or hard of hearing – nearly 16 percent of the population. Hearing, speech and visually-impaired individuals are at risk for isolation and missing, or misunderstanding, vital information. If you are concerned about a loved one who cannot hear well on the telephone or may not hear the phone ring at all, there is good news from Florida Telecommunications Relay, Inc. (FTRI). A private, non-profit 501(c) 3 organization, FTRI administers the Specialized Telecommunications Equipment Distribution Program for Florida residents who are deaf, hard of hearing, deaf/blind and speech impaired. FTRI also educates people about the specialized equipment and Florida Relay Service 711, so they understand exactly how to use it. To be eligible for specialized telephone equipment and ring signaling devices, you must be a Florida resident, at least three years of age, have a landline, and be certified as hard of hearing, deaf, deaf/blind or speech impaired. The equipment is loaned to all qualified permanent Florida residents for as long as it is needed, and there is no charge. Addressing the needs of hearing impaired individuals, Florida Legislature passed the Telecommunications Access System Act (TASA F. S. 427) in 1991. FTRI has contracted with 25 non-profit organizations
statewide to serve as regional distribution centers. There educational presentations are conducted, and the equipment is distributed – at no charge. Since 1991, more than 500,000 clients have been served and more than 1-million specialized telephones or ring signaling devices have been distributed through FTRI. During fiscal year 2013-14, FTRI served 13,671 new qualified clients, ranging from four to 80+ years of age. Loaning individuals this specialized equipment is made possible with funding through a TASA-mandated 11-cent surcharge on landline telephone service for Florida customers. The Florida Relay Service can be accessed by dialing the national relay number, 7-1-1. That reaches a confidential communications assistant service, or relay operator, who connects hearing, visual and speechimpaired telephone users to standard telephone users to facilitate clearer communication. Eligibility can be certified by a deaf service center director, audiologist, hearing healthcare professional, speech pathologist, licensed physician, certified teacher for hearing impaired or visually impaired, or a state or federal agency representative. PRIVATE PAY HOME HEALTH DEFINITIONS ACCREDITATION: The process of review that healthcare organizations participate in to demonstrate the ability to meet predetermined criteria and standards of accreditation established by a professional accrediting agency. Accreditation represents agencies as credible and reputable organizations dedicated to ongoing and continuous compliance with the highest standard of quality. ACHC collaborates with industry experts to create standards to ensure that quality is maintained throughout all aspects of the organization. COMPANION/HOMEMAKER SERVICES Refers to services provided by a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine household activities such as cooking, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to physician’s offices, grocery and clothing shopping as well as recreational activities. Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing, dressing, toileting, ambulating nor dispense medications for their clients. PRIVATE PAY HOME HEALTH AGENCY: An agency that provides services to patients in private homes, assisted living facilities, and adult family care homes. Some of the services include nursing care; physical, occupational, respiratory, and speech therapy; home health aides; homemaker and companions; and medical equipment and supplies. Along with services in the home, an agency can also provide staffing services in nursing homes and hospitals. Home health agencies are licensed and surveyed by the State of Florida. Their services are paid for by a variety of health insurance and Long Term Care Insurance Policies or on a fee for service basis. NURSE REGISTRY: An company that acts as employment agency between an individual patient and nurses, nursing assistants, home health aides, companions and homemakers for services in the patient’s home. Nurse registries also provide temporary staff for nursing homes, hospitals, and other businesses. Each individual health care worker is an independent contractor and is not a direct employee of the nurse registry. Nurse registries are licensed and surveyed by the State of Florida. Their services are paid for by a variety of health insurance and Long Term Care Insurance Policies or on a fee for service basis. MEDICARE HOME HEALTH AGENCY: If you have Medicare, you can use your home health benefits if you meet all the following conditions: You must be under the care of a doctor, and you must be getting services under a plan of care established and reviewed regularly by a doctor.. You must need, and a doctor must certify that you need, one or more of the following: Intermittent skilled nursing care, Physical therapy, Speechlanguage pathology services, Continued occupational therapy. The home health agency caring for you must be approved by Medicare (Medicare-certified). You must be homebound, and a doctor must certify that you’re homebound. A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services. HOME HEALTH AIDES/ASSISTANCE ADL’S: Refers to persons specifically trained to help individuals with their personal care needs and activities of daily living (ADL’s). Activities of Daily Living are activities that reflect the client’s ability to perform tasks that are essential for self-care such as bathing, feeding oneself, dressing, toileting, transferring from a bed to a chair, etc. Other tasks may be assigned to aides per individual agency and family negotiations/contract. Home Health Aides are available for just a few hours per week up to 24-hour care on a temporary or long term basis. SOCIAL WORKER: A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventative, developmental, or remedial in nature. MED-ALERT SYSTEMS: Personal medical alert products activated by pushing a button in case of emergency. There are several different manufacturers. MEDICATION ASSISTANCE: This means that an agency employee may assist the client with medication reminders and assistance. Only a licensed RN or LPN can administer medications in the home or Assisted Living setting depending on their licensing. SKILLED NURSING: Refers to services performed only by a licensed Registered Nurse or Licensed Practical Nurse that is registered with the State Board of Nursing in their respective State. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician. PHYSICAL THERAPY (PT): Refers to assessing and treating large motor function, utilizing rehabilitative techniques of exercise, gait training, prosthetics, and heat to restore the client to his/ her highest functional level of strength, range of motion and mobility. OCCUPATIONAL THERAPY (OT): Assesses and treats small motor function. Utilizes rehabilitative techniques of exercise, splinting and assistive devices to increase the client’s ability to carry out normal day to day activity such as feeding, dressing, grooming, and performing household tasks. Guides the client through specialized regimes and encourages sensory and muscle return. Exercises and activities are used to increase upper body extremity function. SPEECH THERAPY (ST): Assesses and treats speech and swallowing disorders. Provides individually designed treatment programs to maximize communicative effectiveness for the client, such as stroke, head injury, laryngectomy, and voice disorders, as well as cognitive deficits. TRANSPORTATION: Refers to assistance both locally and nationwide. It is very individualized by each Agency. Contact individual Agencies for details. BONDED: Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. INSURED: This refers to what happens if someone gets hurt on the job. If the company Isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. WORKMEN’S COMP: Insurance that covers medical costs, rehabilitation costs and lost wages for employees injured at work.

Senior Housing - Definitions
SENIOR HOUSING DEFINITIONS ACTIVITIES OF DAILY LIVING (ADLs): Define the personal care services available in assisted living and include bathing, dressing, grooming, bathroom assistance, eating and ambulation. Medication Administration and transferring are not ADLs but are factors in determining a person’s level of functioning. ADULT DAY STAY PROGRAMS: There are two general types of adult day care programs. One is based on a medical model and the other on a social model. The medical model provides comprehensive medical, therapeutic, and rehabilitation day treatment. The social model offers supervised activities, peer support, companionship, and recreation. Both models assist older adults and those with chronic conditions to remain as independent as possible, for as long as possible. ANYTIME DINING: Many CCRC’s and Independent Living communities offer anytime dining which gives you the freedom to enjoy your meals at times that are convenient for you. ASSISTED LIVING: Florida providers must be licensed with a Standard Assisted Living Facility License. Two additional licenses: the Limited Nursing Service (LNS) and the Extended Congregate Care (ECC) allow providers to deliver some skilled nursing services and more personal care. Communities are typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Amenities vary. Services include meal plans, cable television, local phone, housekeeping, laundry, maintenance and transportation. Also provided is personal assistance with Activities of Daily Living (ADLs) b Payor sources usually accepted are private pay, and some insurance.
LNS “LIMITED NURSING SERVICES”: Licensure that allows some nursing services to be administered to patients to maintain agin in place.
ECC “EXTENDED CONGREGATE CARE”: The purpose of such license is to enable residents to age in place in a residential environment despite mental or physical limitations that might otherwise disqualify them from residency in a facility licensed under this part. INDEPENDENT LIVING: Housing designed specifically for the elderly. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths. Services include meal plans, transportation, wellness clinics, housekeeping, laundry and maintenance. These communities are NOT licensed to provide personal/nursing care.
LIFECARE: Some CCRC’s offer Lifecare contracts that provide assurance that future health care needs will be met over one’s life-time at predictable and manageable costs. 106 • Please mention the Seniors Blue Book when you call our advertisers.
MANAGED CARE: Any system that manages health care delivery that acts as a gatekeeper for other services, such as specialized medical care, surgery, and physical therapy.
MEMORY CARE These communities are either licensed assisted living facilities that specialize in delivering care to victims of dementia or a unit of a skilled nursing facility. The same licensure rules apply. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Staff is usually specially trained for the special needs of this population. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care/point system for care.
MEDICAID WAIVER Waivers are state specific Medicaid programs that allow for services to be provided outside of nursing homes. A very important distinction between nursing home Medicaid and waivers is that nursing home Medicaid is considered an entitlement while waivers are not. Entitlement, in this situation means that if one meets the eligibility requirements, then they are entitled to receive benefits. With waivers, one might meet the eligibility requirements however the waivers may only be available to a certain number of individuals and therefore the person may be put on a waiting list to receive benefits. They are not immediately entitled to those benefits.
SHORT TERM/RESPITE CARE: Circumstances require a caregiver to seek temporary senior housing for your loved one. Knowing they can move seamlessly into an emergency care environment that supports the wellness needs of seniors allows peace of mind when you are called away for business or vacation, or if you just need a short break from caregiving
SKILLED NNURSING FACILITY (SNF) This level of care is typically found in a nursing center or a transitional care unit in a hospital. Daily rates typically include a furnished bedroom & a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans & private sources. STATEWIDE MEDICAID MANAGED CARE (SMMC): Long-term Care Program.
