In the Lord of the Rings by J. R. R. Tolkien, Frodo Baggins is the ring bearer, which means he has the responsibility of carrying the one ring to Middle Earth. At first, the one ring isn’t a heavy burden on Frodo. He’s able to manage the burden, walking along without a problem. But over time that burden becomes heavier. By the end of the story, the ring weighs so heavily on Frodo that he can no longer walk under his own strength, he collapses. Samwise Gamgee, Frodo’s best friend, must pick up Frodo and carry him the rest of the way.
Here’s another story that may be familiar to you. The Israelites had just been delivered from slavery in Egypt. It didn’t take very long for the 2 million Israelites to disagree with one another, which meant someone had to judge their cases, all 2 million of them — tough break Moses! “Moses sat to judge the people, and the people stood around Moses from morning till evening. When Moses’ father-inlaw saw all that he was doing for the people, he said, “What is this that you are doing for the people? Why do you sit alone, and all the people stand around you from morning till
evening?” And Moses said to his father-in-law, “Because the people come to me to inquire of God; when they have a dispute, they come to me and I decide between one person and another, and I make them know the statutes of God and his laws.” Moses’ father-in-law said to him, “What you are doing is not good. You and the people with you will certainly wear yourselves out, for the thing is too heavy for you. You are not able to do it alone (Exodus 18:13-18).
94% of Americans report experiencing stress. What stands out to me is that 6% of people report not experiencing stress! Stress is the most common disruptor of our mental health. Stress isn’t something that happens out in the world, per se. Rather, stress is a symptom of our response to what happens out in the world. Stress is a physiological or psychological response to an event that affects nearly every system of the body, influencing how we feel and behave. Here are a few signs that you may be experiencing stress: palpitations, sweating, dry mouth, shortness of breath, fidgeting, and accelerated speech.
Think back to the two stories that we just read. Two ideas stand out. Sometimes we get stressed out because we’re trying to carry too much. Other times, we get stressed out because we’ve isolated ourselves from others. Frodo tried to carry the load all by himself, refusing to accept the help of others. It cost him in the end. And Moses isolated himself from others which is why his father-in-law reminded him, “You are not able to do
As we all know, having a plan before starting a home improvement project makes all the difference. Maybe you’ve experienced stress. But now you’re asking, “Where do I start? Here are three things to consider as you begin to develop your mental health plan: Recognize, Address, Act.
Recognize the signs of stress. Sometimes the symptoms of stress appear right away, like those butterflies in your stomach. Other times, the symptoms of stress appear later, like a later afternoon headache or tightness in your neck and shoulders.
I’m going to address anxiety by _______. It’s up to you to fill in the blank. Every person is different and so what works for someone else might not work for you. But one thing that doesn’t change from person to person is intentionality. Be intentional about addressing stress by being specific. Here’s an example of how you can be specific. I’m going to address stress by taking 5 minutes to breathe deeply and redirect my focus. Again, the key idea is intentionality—know exactly what you’re going to do and when you’re going to do it. Once you’ve determined how you’re going to address stress, it’s time to move to the final step in your mental health plan.
This is the simplest and hardest step of your mental health plan. Sometimes, this is where we fall short. But consider this, you’ve already put a plan into action by developing your mental health plan. Way to go! You do have what it takes, and you do follow-through with a plan. Keep reminding yourself of that and you’ll find it much easier to keep putting your mental health plan into action.
Lastly, consider that both Frodo and Moses had a great friend come alongside each of them to carry the load. Friendship is truly a great gift. God doesn’t expect us to do life alone. Instead, he wants us to do life together. Allow others to carry your loads and help carry their loads. And remember, Jesus carried our sin on the cross!

Alzheimer’s disease is not just an aging issue — it’s a public health crisis. More than 6 million Americans of all ages have Alzheimer’s, and in 2023, an estimated 6.7 million Americans age 65 and older were living with the disease.
Alzheimer’s affects not only individuals, but families, businesses and communities. And while there is good news—with new diagnostic tests making it easier to identify Alzheimer’s and new medications coming on line to help delay the progression of the disease—this new era of treatment is already facing hurdles when it comes to providing Alzheimer’s patients with access to the care they need.

Our voices make sure that Alzheimer’s remains a national priority.
AIM (Alzheimer’s Impact Movement) is working to change this. As the advocacy arm of the Alzheimer’s Association, AIM fights to support Alzheimer’s and dementia research funding and to advance bipartisan legislation that will have a positive and meaningful impact on people living with Alzheimer’s and their caregivers today and in the future.
“AIM works to ensure that Alzheimer’s remains a leading priority on Capitol Hill by participating in the political process,” said Jason Gerard, director of state government affairs, Alzheimer’s Association of Greater Pennsylvania. “We work to educate candidates of both parties to make sure that Alzheimer’s and dementia are part of the national plan. Our voices make
sure that Alzheimer’s remains a national priority.”
Working collaboratively, AIM members target numerous issues on both a state and national level that affect those with the disease. These include the creation of Alzheimer’s-related public health infrastructure, pushing for more federal funding for research, and fighting the Centers for Medicare & Medicaid Services (CMS) for its decision to block access to FDA-approved Alzheimer’s treatments.
“When we talk about the need to move our mission forward and to change the trajectory of the disease, work in policy and advocacy is instrumental in that,” said Gerard. “The impact of what we do is big, because changing the laws changes the lives of so many people.”

After receiving Estate Planning documents, one of the most important steps to take is ensuring that all doctors and organizations making up your medical team have a copy of the Health Care Directive. This document serves to appoint an agent as your Health Care Power of Attorney and gives treatment instructions in the event of an end-stage medical condition, otherwise known as a “Living Will”. We stress the importance of having a Health Care Directive, but rarely do we stress the importance of making sure it is shared appropriately. Recently, a friend of mine shared a story of what happened with her family when they did not share their Healthcare Directive with their medical team. Her mother, for whom she was the health care agent, was struggling and ended up needing to be hospitalized. Having reached a point where decisions needed to be made on her mother’s behalf, the hospital requested a copy of her estate planning documents.
Neither my friend nor her stepfather had brought a copy with them and were unsure where the documents even were. The hospital then offered to provide a new set of documents for her mother, particularly the Health Care Directive in case anything was to go wrong. Rather than driving back home and attempting to find her original documents, my friend and her stepfather decided to go ahead and have the new ones made. When the nurse asked the mother who she would like to act as her agent, her daughter suggested appointing both herself and her stepfather. Her mom, however, immediately stated that her daughter was the only one she wanted as her agent and the only one to be making the decisions for her.
Ensure that all who make up your medical team have a copy of your Health Care Directive.
The situation was difficult and presented an uncomfortable moment for the daughter. She could tell that her stepfather, whom she was extremely close to, was upset and did not understand why his wife did not trust him to make such decisions for her. If this family had given the documents to their medical team right away, it perhaps would have saved them from a difficult moment, making an impossible situation even more stressful.

We will always provide extra copies of documents for children, relatives, or those you have appointed so they can have them on hand as well. It is not enough to simply have the documents, but make sure you have them where you need them. Knowing where your documents are and having peace of mind knowing all medical parties on your team have them could save your future self from unnecessary heartache.

If you’ve ever played the Pennsylvania Lottery, then you’ve probably heard their slogan: Benefits Older Pennsylvanians. Every Day. But have you ever wondered how?
One way is by partially subsidizing Shared Ride services across Pennsylvania. Lebanon Transit is one such operator of Shared Ride, which is a curb-to-curb, reservation-based transportation program servicing all of Lebanon County.
“Shared Ride” means that the service is open to the public but is not a taxi or Uber. The Shared Ride van will pick you up at your curb and transport you to your destination. You can travel to any destination in Lebanon County, including doctors, stores, and friends or family, plus the Hershey Medical Center campus. Since the service is shared there may be other people in the vehicle besides you, or the driver may need to pick up other passengers prior to dropping you off at your destination.
Anyone can utilize Shared Ride services. However, full fare may cost more than $20 per trip. But seniors may qualify for a reduced fare. To qualify, you must be at least 65 years old and supply one of the eligible proofs of age to Lebanon Transit. Your fare is then only $2.50 per trip regardless of the distance you travel. The Lottery pays the rest!
And when you are signed up for Shared Ride you also ride any fixed route bus for free!

Many seniors in Pennsylvania rely on Personal Care Homes (PCHs) and Assisted Living Residences (ALRs) for daily support and care. These residences are crucial for those who need assistance with daily activities. Many seniors that need this care face financial barriers due to their low income. Pennsylvania’s “SSI Supplement” program aims to help low-income individuals afford PCH care by providing limited financial assistance from federal and state funds. However, the current rate of $46 per day for care has seen minimal adjustments since 2006, despite a 55% rise in inflation. Consequently, over half of the providers who accept SSI residents have closed since 2008, making it increasingly difficult for eligible seniors to find affordable care.

Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise modifying risk factors to prevent further heart damageor psychological rehabilitation.
DIABETIC CARE – Refers to services including patient assessment specific diet information management of the disease process medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ.
ENTERAL
– Enteral feeding or tube feeding is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.
HOUR MINIMUM – The least number of hours a Non-Medical
Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.
HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is givenand sophisticated symptom relief provided.

Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies hydration therapy pain management chemotherapy and central line care and maintenance.
– Assistance with Activities of Daily Living (ADL’s)homemaking companionship and other nonmedical in home care. These services are available for hire after or in conjunction withMedicare certified services. Private payment Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.
Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech language cognitive skills or swallowing.
Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch strengthen facilitate muscles challenge balance test coordination abilities teach home exercise programs and enhance basic mobility skills.
Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental physical developmentalor emotional disabilities. Therapists also help them to developrecoveror maintain daily living and work skills.
Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases
and their equipment environmental control systems humidification aerosols ventilator support bronchi pulmonary drainage and exercise respiratory rehabilitation assistance with cardiopulmonary resuscitationand maintenance of natural artificial and mechanical airways.
TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstructionallow air to get to the lungsor remove secretions.
WOUND CARE – Refers to services for providing diagnosis and treatment for ulcerspressure sores and other wounds. Treatments may include but is not limited to irrigationsdressingand wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

Hospice care is a vital resource designed to provide compassionate support to individuals facing lifelimiting illnesses. However, despite its benefits, hospice remains significantly underutilized in many communities. This trend raises important questions about why individuals and families may not be accessing this valuable service when they need it most. One key factor contributing to the underutilization of hospice care is a lack of awareness and understanding. Many people are unfamiliar with hospice and may mistakenly believe that it is only appropriate for individuals in the final days or weeks of life. Hospice care is most effective when accessed earlier in the journey of a life-limiting illness, allowing patients and their families to benefit from comprehensive support, pain management, and emotional care for a longer duration.
Furthermore, cultural and societal attitudes toward death and dying play a significant role in shaping perceptions of hospice care. In some cultures, discussions about death may be considered taboo or uncomfortable, leading individuals to avoid conversations about end-of-life care until it becomes unavoidable.
This reluctance to confront mortality can prevent individuals from seeking hospice services until it is too late to fully benefit from them.
Financial concerns also contribute to the underutilization of hospice care. While hospice services are typically covered by Medicare, Medicaid, and most private insurance plans, some individuals may be unaware of their coverage options or may be reluctant to incur additional expenses related to end-of-life care. This financial barrier can prevent individuals from accessing hospice services until their illness has progressed to a point where the benefits of hospice may be limited.
Additionally, healthcare provider attitudes and practices can influence the utilization of hospice care. Some healthcare professionals may be hesitant to broach the topic of hospice with patients and their families, fearing that it may be perceived as giving up hope or admitting defeat. This reluctance to discuss end-oflife care options can delay access to hospice services and limit the ability of patients and their families to make informed decisions about their care. Addressing the underutilization of hospice care requires a multifaceted approach that involves

HOSPICE - Hospices provide individuals facing a terminal illness with care in a private residence or in a healthcare facility. The goal is to provide quality of life and to assist patients to live the last stages of their lives with dignity. The focus is on comfort rather than curative treatment of the terminal illness.
SERVICES PROVIDED:
RN-REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family education as needed
SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, and provides counseling and support to meet identified needs, and makes referrals to other providers based on family and patient’s needs
CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.
VOLUNTEERS – Provides companionship and needed non-medical services for patients and their families, offers support at time of death and during bereavement.
LEVELS OF CARE:
RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in managing the individual’s disease symptoms. Care is provided at the individual’s private residence or in a healthcare facility.
RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a skilled nursing facility. Hospice is responsible for financial arrangements with the facility.
GIP – GENERAL IN-PATIENT CARE
– Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This higher level of care is due to symptoms which are out of control and more intensive nursing services are required. The room and board are covered by hospice during this period.
CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This
level of care is usually provided to the patient at their private residence or in a personal care home or assisted living facility. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for crises that require a constant presence by a nurse or care team.
THERAPY SERVICES PROVIDED:
AR-AROMA THERAPY – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.
MA-MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to. Massage therapy has become essential in hospice care. This is not reimbursed by Medicare.
MU-MUSIC THERAPY – Music
Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist creates goals and objectives for their patients and uses music activities and music experienc-
es (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.
P-PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.
24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.
GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many hospice companies provide Grief Groups that meet and address issues of grief in a supportive group setting.
HOME HEALTH – Indicates whether the hospice agency has a sister home health agency that will provide

assistance with medically based home health needs such as nursing services, home health aide services, physical therapy, occupational therapy, respiratory therapy or speech therapy.
INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.
PAIN PUMPS AVAILABLE – Many patients have their symptoms con-
trolled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.
PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness –whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

(ADLs) – Activities involving personal care and mobility such as bathing, dressing, toileting, walking and meal preparation.
AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.
(AL) – Typically apartment-style with kitchenettes and private baths. Assisted living services and personal assistance with ADLs are provided. In PA there is a licensure distinction between Assisted Living Residences and Personal Care Homes. An Assisted Living Residence in PA is “a significant long-term care alternative to allow individuals to age in place… and [to] develop and maintain maximum independence, exercise decision-making and personal choice.” according to PA Code, Ch 2800.
SERVICES – Most providers can elect to provide day/ night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day.
COMMUNITY (“CCRC”) – There are many different types of CCRCs. There are Pay for Fee Communities (Type
C), Life Plan Communities (Type A) and even some rental models (Type D) inside CCRCs, but the unifying factor Is that they provide all levels of care - Independent Living, Assisted Living/Personal Care, Memory Care and Skilled Nursing. Many offer many additional amenities, such as Rehabilitation Facilities, meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces.
BENEVOLENT PROGRAM – A program created to maintain a resident’s lifestyle when they run out of resources through no fault of their own.
(TYPE C) – Includes housing, residential services and offers different levels of healthcare, but not at the same monthly cost. Where you are on the continuum of care dictates what you pay for the monthly fee. There is typically an up-front fee that varies based on the type residential amenities and other factors.
A) – Includes housing, residential services and unlimited use of healthcare services with little or no increase in monthly fees. Your monthly fees are not dependent on the level of care that you need, which
is why a Life Care Community may charge a larger up-front fee, as the Life Care Community is absorbing the risk on the resident’s level of care needed.
RENTAL AGREEMENT (TYPE D)
- Typically a small amount of rental properties are available which are not subsidized. Does not participate in the buy-in fee, nor has access to the continuum of care benefits.
INDEPENDENT LIVING COMMUNITIES – Apartments and communities for adults 55+.
INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADLs) – Activities involving more complex functioning such as grocery shopping, writing
checks, housework, cooking, traveling, and managing money.
INTERMEDIATE CARE – 24-hour inpatient care for residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive medications from a nurse following a doctor’s orders and may receive outpatient rehab services.
MEMORY CARE RESIDENCES (MC) – Licensed Assisted Living or Personal Care Homes that specialize in delivering care to patients with Alzheimer’s and other dementias. Communities are typically bedroom-

Moving can be stressful, physically demanding, mentally exhausting and full of unpredictable challenges. How do you best mitigate this uncomfortable reality? At Armstrong Relocation – Jack Treier Moving, we like to call it the M-OV-E plan.

All your service providers should provide you with Licenses, Training Certifications and Certificates of Insurance. Do they background check and drug test their employees? Get service commitments in writing and read the agreements and disclosures.
“E”
Whether you hire a move manager, get help from a friend or family, or go at it alone; stay engaged in the process.
Being satisfied with the results is the priority. Make sure success does not slip through your fingers! So, how do you best mitigate this uncomfortable reality?
1. Review Your Moving Company’s Relocation Policy: Carefully go over what is covered with your move.
2. Schedule Your Survey: Complete a virtual or in-home survey to get an estimate.
3. Submit All Cost of Move Estimates: Get your estimate approved by yourself or your employer.
4. Specialty Items: Armstrong works with a third party to handle specialty items, disconnect major appliances, and/or remove wallmounted TVs.
5. Start Packing: Your packers will put items in boxes. Double-check cabinets and closets for overlooked items.
6. Be Available During Loading: Armstrong will carefully load your belongings onto the truck. Make sure you sign off on all the paperwork.
7. During Transit: Keep your Armstrong contact information handy while your belongings are on the way.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy, a surgical procedure in which a cut or opening is made in the windpipe (trachea), to bypass an obstruction, allow air to get to the lungs, or remove secretions.
VENTILATOR – Moves breathable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable.
WANDERGUARD – Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects and alarms when a pendant moves near an exit.

WellSpan York Hospital, ALPHA addresses the critical gap in care for low-income individuals who require PCH essential services. These homes serve Pennsylvanians eligible for the Supplemental Security Income Supplement, crucial for extremely low-income residents.
PCH operators face substantial financial hurdles. While there was a recent increase to $51 per day in 2022 due in part to ALPHA’s advocacy efforts, Pennsylvania only supports this with $20.44 daily. Despite this improvement, residents receiving SSI only get $1,553 per month, with a mere $85 allocated for personal needs—far from adequate for both residents and PCH operations.
Beginning in York County, ALPHA now supports SSI PCHs statewide by advocating for increases in the SSI Supplement, cost-of-living adjustments, and personal needs allowances. We provide resources such as coaching, business training, grants, and community engagement to bolster these facilities’ operations and enhance resident care and quality of life.

In the early stages of dementia, a person may do well independently. As brain changes continue and cognitive abilities decline, living at home becomes impractical without the help of relatives or a home-care agency.

• Behavior and mood changes. Brain changes caused by dementia can affect your loved one’s personality, causing them to feel agitated, more anxious, or experience hallucinations. They may also withdraw from social activities and events they once enjoyed, which can increase the risk of depression.
• Caregiver burnout. Burnout is a state of physical, emotional, and mental exhaustion. Without intervention, burnout will cause you to lose interest in caring for yourself and your family member.

PCHs offering the SSI Supplement play a crucial role in healthcare, offering cost-effective care and assistance with activities of daily living for a diverse population, including seniors and individuals with disabilities. Upholding their integrity ensures these vulnerable populations receive essential services.
As of June 2024, Pennsylvania has seen a significant decline in SSI-accepting PCHs, dropping from 894 in 2008 to 391. Similarly, the number of residents benefiting from the SSI Supplement has decreased from 8,776 to 3,844 during the same period. This reduction reflects the pressing need for sustainable solutions to support low-income PCHs and their residents. Preserving low-income PCHs isn’t just a moral obligation but also a financial necessity for Pennsylvania. Collaboration between ALPHA, policymakers, and the community is crucial in developing sustainable strategies that safeguard these vital components of the long-term care system.
