Estate Plan Check-Ups

Effective estate planning is personal, and it’s more than just deciding who to leave your assets to once you die.

Effective estate planning is a comprehensive process that encompasses pre-need planning: health care decisions, financial management, and maintaining a delicate balance between independence and security. Like your preventive doctor visits, you should regularly check in on your estate plan to ensure it fits your current needs, considers and plans for potential future care needs, and will give effect to your wishes now and in the future.

Generally, estate planning involves creating a last will and testament, possibly a revocable trust, possibly an asset protection trust or a supplemental needs trust for a loved one who is unable to manage finances or may be vulnerable to abuse or exploitation. Estate planning also involves important advanced directives, such as a durable financial power of attorney, a health care power of attorney, and a living will. Creating an estate plan, or “getting my affairs in order,” tends to be an item on our to-do lists, for us to get done and move on to the next thing. However, while it may not be something you have to look at every month, or even every year, once your initial estate plan is completed, it is

something that needs to be reviewed with some regularity.

Most people get an annual physical when they are healthy, not when they are sick. They do this because they want to proactively spot any issues that could cause them to become ill in the future. The same concept can and should be applied when it comes to reviewing and updating your estate plan. Your estate plan may be “healthy” now, but you want to make sure that it stays that way by checking it regularly, to ensure it fits your needs and family circumstances, protects and provides for you now, and accomplishes your goals and wishes in the future. ~

Who Is Mandy’s Cleaning Service, and

Mandy will tell you that two conversations greatly influenced how she runs her business.

In the early years a customer said, “The little things you do make the difference.” This customer appreciated Mandy for going above and beyond general cleaning. Years earlier, when Mandy was baby- sitting, her Mom encouraged her to also clean the dishes and pick up around the house, that doing more than expected would set her apart.

These two conversations greatly shaped who Mandy would become as a person. They also set the standard for how she would one day run her business—with great attention to the little things.

Who Is Mandy?

Five years later, Mandy had a team of employees, a website, and a lot of business. Today, Mandy believes it’s about relationships and treating every customer like they are the only one. Communication ranks high among her priorities, which means text messaging reminders before customer cleanings and following up with them to make sure they’re pleased.

It’s humbling to see how far the business has come since 2009. In the days ahead, Mandy’s Cleaning Service is looking to expand outside of Mobile. She shared, “I want people to feel relief, that they can enjoy friends and family without being concerned about the house.”

Fifteen years ago, Mandy chose to stay home with two small kids, yet she wanted to contribute financially to meet the needs of the home. A few friends suggested starting a cleaning service and committed to helping her find a few clients. She took her friends up on the offer and began cleaning. It was just a few homes at first, and things grew exclusively by word of mouth. Mandy recalls business picking up in year two, requiring her to hire help.

As the business reaches outside of Mobile, the mission will always be the same. “The little things still matter,” Mandy will tell you. “It’s still about going above and beyond and doing those special things for every customer.”

It really is more than cleaning your home. It’s about caring for you. ~

Medicare 2024 “In A Nutshell” (Parts A, B & Medigap Plans)

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Medicare Part A Hospital Insurance

Generally no cost

(If you paid Medicare taxes while working)

$1,632 deductible (per benefit period)

It is possible to pay the $1632 deductible and other daily charges for multiple stays & extended care.

Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.

Providers are paid by Medicare assignment.

No coverage outside USA.

No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Medicare Part B Medical Insurance

Most people pay monthly $174.90 (Premium could be higher based on income)

$240 annual deductible. (this can increase each year)

80/20 after deductible.

Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits

Most doctors accept Medicare.

No coverage outside the USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

A & B are designed for people over age 65 and those with certain disabilities

Supplemental Policy Plans A - N Medigap

Premium varies by company, age, location & other factors.

Guarantee Issue at initial enrollment

(Other special enrollment periods may be avalable)

These plans generally pay the 20% not covered by Part B & other Medicare eligible benefits

When this coordinates with Medicare, many benefits are 100% covered

Can provide benefits outside USA

Plans are available through insurance companies.

Important to add Part D plan to cover prescription drugs & avoid penalties.

Must have Part A & B to enroll.

There is NO additional cost to use the services of a full time insurance professional. Please call us or a broker in your local area for additional assistance.

Bone Health - Understanding

An estimated 54 million Americans are affected by osteoporosis, a condition that weakens bones, making them more prone to fractures from minor incidents. An estimated 50% of women and 20% of men will break a bone from osteoporosis.

The trained specialists at The Orthopaedic Group, P.C., emphasize the importance of bone health, especially among seniors, to manage and prevent this condition.

Identifying Risks and Signs

Osteoporosis often goes unnoticed until a fracture occurs. Risk factors include a history of falls, being over 65 (men) or 50 (women), a family history of the condition, and lifestyle habits like smoking or alcohol use. Certain medical conditions and treatments also increase susceptibility.

Diagnosis and Screening Healthcare providers use DEXA Scan (dual-energy X-ray absorptiometry) to diagnose osteoporosis and assess fracture risk. This scan is recommended for adults over 65 and those with risk factors. It’s a quick, painless procedure using minimal x-rays.

Prevention and Treatment

and Managing Osteoporosis

Exercise, including weight-bearing and balance activities, is vital for enhancing bone density and preventing deterioration.

Medication and Personal Care

For those at high risk, medications may be prescribed to mitigate bone loss or increase bone mass. Oral health care is critical due to the potential risk of osteonecrosis of the jaw from osteoporosis medications. The Orthopaedic Group offers a comprehensive orthopedic bone health care and education program through its Fracture Liaison Service (FLS), which provides tailored treatment plans to reduce fracture risk and improve quality of life.

The Orthopaedic Group, P.C., advocates for proactive screening, preventive nutrition and exercise, and informed treatment decisions to address osteoporosis. Our specialists focus on maintaining independence and well-being so you can live the life you love!

Diet plays a key role, with a focus on high-calcium foods (preferably Calcium Citrate) and Vitamin D supplementation for absorption.

Exploring the Advantages of Reverse Mortgages

Reverse mortgages have gained popularity as a financial tool for retirees looking to access their home equity without selling their homes.

These unique financial products offer several advantages to eligible homeowners:

1 Stay in Your Home:

One of the most significant advantages of a reverse mortgage is the ability to stay in your home for as long as you live. As long as you maintain the property, pay property taxes, and keep up with homeowners insurance, you can continue to reside in your home.

2 No Monthly Mortgage Payments:

Unlike traditional mortgages, reverse mortgages do not require monthly payments. Instead, the loan is repaid when the homeowner sells the home, moves out, or passes away. This feature can ease financial burdens for retirees on fixed incomes, allowing them to remain in their homes without worrying about ongoing mortgage payments.

3 Supplement Retirement Income:

One of the primary benefits of a reverse mortgage is the ability to convert home equity into cash, providing a valuable source of supplemental income during retirement. This can be especially beneficial for individuals with limited savings or those looking to enhance their lifestyle without taking on additional debt.

4Flexibility in Accessing Funds:

Reverse mortgages offer flexibility in how homeowners can access their funds. Borrowers can choose to receive a lump sum, a line of credit, regular monthly payments, or a combination of these options.

5No Repayment Until You Leave the Home: With a reverse mortgage, the loan does not need to be repaid until the last borrower permanently leaves the home. This provides peace of mind for borrowers, knowing that they can access their home equity without immediate repayment obligations.

Reverse mortgages offer a valuable option for retirees seeking to unlock the wealth tied up in their homes while maintaining the comfort and security of home ownership. ~

Definitions

24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

ALZHEIMER’S/DEMENTIA CARE

– Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

COMPANION/HOMEMAKER

SERVICES – Refers to services provided by a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine household activities – cooking, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to physician’s offices,

grocery and clothing shopping as well as recreational activities.

Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing, dressing, toileting, ambulating nor dispense medications for their clients.

INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button.

MEDICATION REMINDER –

Assistance with reminding an individual about medications but not dispensing medications.

TRANSPORTATION – Rides to shop-ping, appointments and other outings. ~

Hospice Care Definitions

HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

Services Provided:

SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs

HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

Levels of Care:

RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility.

GIP – General In-Patient Care –

Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.

HOSPICE CARE DEFINITIONS ALABAMA GULF COAST – SPRING – SUMMER 2024 84

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
