Circle of Friends Program Targets Loneliness and Loneliness has long been a serious problem for older adults. With the Social Isolation pandemic, health and mental health issues have been exacerbated by circumstances – home orders, closed senior centers and churches, fewer family gatherings, restrictions on hospital and nursing home visitation. All have brought a face to the problem of loneliness. Loneliness Linked to Serious Health Conditions Loneliness and social isolation in older adults are public health risks. Older adults are more likely to face living alone, loss of family, chronic illness and hearing loss. The AARP Foundation and the National Council on Aging, in extensive studies, found older adults who describe themselves as lonely have a 59% greater risk of functional decline and a 45% greater risk of death. Surveys reported 17% of adults aged 65 and older are isolated (nearly 1 in 5), with 46% of women aged 75 and older living alone. Isolation is harmful at any age but is especially dangerous for people over 50. The health risks of prolonged isolation have been found to be equivalent to smoking 15 cigarettes a day. Social isolation and loneliness are associated with higher blood pressure, increased susceptibility to the flu, greater risk of heart disease, the earlier

Some of the most prevalent causes of isolation reported by the AARP Foundation are: n n

So, what can help mitigate this? No matter the future, loneliness will continue to be a challenge for older adults. The Area Agency on Aging is increasing resources to address this issue by developing the Circle of Friends program. The Circle of Friends program will include:

Intervention by a social worker to address needs and provide home visitation when indicated.

Friendly Calling: The telephone reassurance program aimed at addressing feelings of social isolation and improving mental and emotional well-being through increased socialization and friendships. The Area Agency on Aging RSVP Volunteer program will be expanded to offer friendly phone visitation, and in-person visitation when indicated between matched “friends” utilizing volunteers who have been prescreened.

Social Isolation Resources: To improve social connectedness the Area Agency on Aging will provide the following resources: n
A home study toolkit and small groups calls for Chronic Disease Self-Management classes.

Feeling Good and Staying Connected workbooks to educate in habits to relieve loneliness, offers engagement activities and ways to improve connections. On-line resources with free access to many social, educational and health zoom classes through GetSetUP, Connect2Affect and Trualta.

Robotic pet dogs and cats for persons with dementia
The Area Agency on Aging will offer free GetSetUp virtual senior center classes to those able to benefit from live zoom social get-togethers, health, travel, educational and technology classes. Trualta classes will also be available free on-line for isolated caregivers. These resources help older adults stay mentally, spiritually, and physically fit, with activities that reduce isolation and associated negative health and mental health impacts. Circle of Friends is open to anyone age 60 or over in Mobile County who lives alone or is homebound, has little or no family or friend support, or simply would like someone to talk to on a regular basis. To volunteer for the friendly calling program, to sign up for virtual GetSetUp or Trualta classes or home wellness classes, to enroll in the Circle of Friends program or to obtain a robotic pet for a persons with dementia, contact the Area Agency on Aging at 251-706-4680. ~

Spring Cleaning Your Estate Plan Now that spring has sprung and COVID restrictions are not dominating our daily lives, we need to get back to the basics. Time to change your linens, clean those baseboards and review your documents! At a minimum, everyone needs a Durable Power of Attorney for financial matters and one for health care issues. It is surprising to me how few people execute this very basic estate document. If you don’t have one, and you have a massive stroke, your family will likely have to go to court in order to handle your most basic personal financial matters. This cost thousands of dollars and is a very difficult process on your loved ones. Next, everyone should execute an Advanced Directive for Health Care, which is also known as a living will. This document allows you to choose ahead of time whether you would want to have your life artificially extended by the administration of medicine or machines, and whether you wish to have food or water given to you through a tube or IV. If you do not have this document and catastrophe strikes, the hospital must take the maximum steps to

keep you alive, and often the client is then sent to a nursing home where they can live a very long time under these circumstances. Many do not want this to happen. If this is the case, execute this document. For Alabama, this document can be located for free on the Alabama Hospital Association’s website. Most hospitals, hospice companies and doctor’s offices can also provide the form. This document is often associated with a “DNR” or “Do Not Resuscitate” form. That form is initiated by your physician most often, and is available for you if you reach the stage of your life that you really have no quality of life. This is not a required document, even though a lot of facilities would prefer it and may infer that it is required. You are the master of your destiny and you get to choose how your life ends. The next, most common document is the Last Will and Testament. Everyone should have one, even if you put your children as beneficiary on your accounts and policies. There may be a reason to open an estate and it will be more expensive and time consuming

control over your situation after your death is to set up a trust, that will manage the property over time. If you have a set of documents that have been completed, make an appointment to determine if any changes should be made. Laws and circumstances change all the time, so it is important to keep up with those changes. Talk to your estate professional as soon as possible and get your spring cleaning under way! If you are concerned about getting out in person, call and set up a virtual appointment. ~

If you just want to say, “I want my children to have my stuff,” then the document will divide your “residuary” estate. A common problem I see is where someone has a child they have raised but never formally adopted, or a partner they didn’t marry, and those will be left out of any estate if you do not write a will. The last document you may want to consider is a Trust. We are living in more complicated times, and it is important to have a will, however, that will needs to be probated in court to take effect following your death. One way to exercise more

Are You One in If you’re a Caregiver, you’re one a Million? of the 1.8 million Caregivers in Alabama.
One in four Baby Boomers (ages 57-75) are caregivers, providing regular assistance to friends or family who have a disability or health problem. The burden of caring for a loved one can be time-consuming, expensive and can cause Caregivers to become depressed and feel guilty about their decisions. “Caring for the Caregiver” is Relatively New Until the late 70’s, Caregivers were an unseen, unheard and unrecognized part of the healthcare team. These families and friends were caring – primarily at home - for loved ones with cognitive disorders like Alzheimer’s disease, strokes and other age-related diseases. The first national caregiving organization was founded in 1977 – the National Caregiving Alliance – to raise awareness. This was followed by a 2016 reauthorization of the Older American Act of 1965 to assist Caregivers, the RAISE Family Caregivers Act of 2018 and the National Family Caregivers Support Act in 2000. Recently, the national

CARES Act (Coronavirus legislation) also includes caregiving support provisions. I Can’t Take Care of You, If I Don’t Take Care of Me Both the Caregivers and the cared for can experience similar emotions. As days, weeks and months continue with mounting responsibilities, Caregivers can become overwhelmed physically, emotionally, spiritually and develop significant health problems. They worry their compassion is faltering (compassion fatigue) and experience guilt (Caregiver Guilt Syndrome) they are not doing enough for their loved one. Caregiver assessments reports that 20% of family caregivers suffer from depression, twice the rate of the general population. Meanwhile, those in their charge also can experience anxiety, frustration and depression. Options for Caregivers and Their Charges As many Caregivers need to work or be away from home for most of

members and obligations. Other options for Caregiving respite include home health care, assisted living facilities and nursing homes. The best advice is to know your healthcare plan and options: Medicare now offers long-term home and community-based services, like adult day care, in-home personal care assistance, and respite care in certain conditions; Medicaid can pay 100% of nursing home care costs if you meet eligibility requirements; and private health insurance companies typically offer some respite care assistance. Local resources for Caregivers can be found on the Area Agency on Aging website listed under programs: Alabama Cares Caregiver Program.~

the day, AARP recommends adult daycare as a more affordable option (ranging from $50 - $70 per day) for Caregivers respite. When older loved ones are unable to structure one’s activities, feel isolated and lonely, have difficulty starting and focusing on activities and no longer seem safe on their own, adult daycare provides watchful care and socialization. An adult daycare is typically a non-residential facility that supports the health, nutritional, social and daily living needs of older adults. Caregiver support is built into the program, with counselling and other support services. Caregivers are provided daily respite to continue to live their own lives, work and take care of other family

Medicare 2022 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. Dora spent much of her day in bed – in a heavy, gray fog. Many days, she let the phone and doorbell ring without answering them. In the kitchen, dishes, mail, and trash piled up, but she was too weak to clean or care. Excessive sleep and a lack of energy are warning signs of a serious but treatable disease called depression. Older adults face increased risks for depression. Surprisingly this common illness frequently goes undetected and untreated in late life. Recognizing Depression “Coexisting medical conditions, complicated medication regimes, and painful losses sometimes mask the symptoms of depression in late life,” says Janet Linton, M.D, medical director of Senior Behavioral Health at Mobile Infirmary, located in Mobile, and North Baldwin Infirmary, located in Bay Minette. “Some of the most visible signs of depression,” Dr. Linton says, “are significant changes in appetite, sleep, and energy level.”These symptoms should always be reported to your physician. A checklist of other warning signs of depression follows. If someone experiences a number of these symptoms for more than two weeks in a row, it is time to see a doctor. Many people—especially those who grew up in times or places where mental health problems were viewed as personal, religious, or moral failings—have difficulty talking about the emotional symptoms of depression. “You can start by talking to your doctor about your physical symp-

Treating Depression Depression can be successfully treated at any age. The most effective treatments are medication and psychotherapy. Medications for depression are safe and effective, with very few side effects. They help improve mood, sleep, appetite, energy, and concentration. Psychotherapy is private counseling with a trained professional. It helps
you overcome the effects depression has on your moods, thought, and relationships. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

FALSE: Hospice is a place. TRUE: Hospice care takes place wherever the patient calls home, including long term and assisted living facilities. In fact, patients and families receive the greatest benefit when hospice care is started early. Each patient and his caregivers receive individualized care that meets their physical, emotional and spiritual needs. Hospice care focuses on caring, not curing, and neither hastens nor prolongs the dying process. FALSE: Hospice is only for cancer patients. TRUE: More than 50% of patients have a diagnosis other than cancer. A large number of our patients have end-stages of chronic diseases such as emphysema, COPD, Alzheimer’s and neuromuscular disease. FALSE: Hospice means giving up control. TRUE: The hospice team will sit down with the patient and family to create a care plan to address each patient’s needs. The control in almost all instances rests with the patient and the family.
FALSE: Hospice over medicates the patient which causes them to die sooner. TRUE: One of the primary goals of hospice care is to make the patient comfortable, pain free and as alert as possible. FALSE: Hospice is too expensive. I can’t afford it. TRUE: Hospice services are covered under the Medicare Part A, Medicaid and most private insurances. This benefit covers the hospice team’s services, medications, supplies and equipment related to a patient’s life-limiting illness. Hospice staff will work with patients, families, insurance providers and other resources to ensure the patient receives all the benefits available. It also is given when more help is needed to assist the individual and family in coping with the crisis. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

Rates may vary depending on the changing levels of care while one maintains the same physical residence. PUBLIC HOUSING (PH) – Affordable apartments for low-income families, the elderly and persons with disabilities. REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients. RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have
a small number of residents who live together and receive care from livein caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances.

RESPITE CARE / SHORT-TERM STAY – Otherwise known as a shortterm stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision. SHORT-TERM CARE – Acute care provided on a short term basis. THE RETREAT AT SAAD HOSPICE
The Making of the Retreat at When Saad set out to build The Saad Hospice Retreat, maintaining the connection to nature was imperative. The property sits on 7 acres of undisturbed woodland that contains a gentle slope to the east, providing a quality typically found in the Appalachian highlands. Walking through the property mature water oaks, red oaks, and pines sit densely among the terrain. Owls hoot, birds chirp, and the transcendental elements of an untouched landscape clear the mind and provide peace for the soul. The property is beautiful, so keeping the aspects of the land or incorporating them into design was paramount. Trees were marked for keeping, plans were changed to include some of the most mature trees, but many still needed to be removed for the building to begin. Saad made contact with tree removal companies, but rather than haul the timber away, it felt only right to use it in the facility. Several companies and contractors insisted the trees were pulpwood at best. Pulpwood is timber with the principle use of making paper products. There was something melancholy about turning

these trees into paper products, principally as a hospice company. Our stories live on with our loved ones and we felt these trees should be a part of The Retreat’s story. A sawyer was found who believed he could turn this timber into beautiful wood. Robert fit every aspect of the wonderfully simple definition of a sawyer: One who saws. Fueled by coffee and donuts, Robert diligently sawed trees down to usable lumber. Giant trees were cut for columns and beams in the lobby, smaller timber transformed into shiplap and trim for the hallways and patient rooms. The milling process took months to complete. The lumber after being cut was then transported to a drying facility before it could be sanded and stained for use. A defining feature of The Retreat is the wood elements incorporated throughout the property and it was made possible by Robert believing this lumber had more to give. ~ Editor’s Note: This article was submitted by The Retreat at Saad Hospice.
