The Blue Book will continue its valuable service to the senior citizens of the area. As always, our goal remains to improve dignity in aging by providing needed information for independence and improved quality of life. DEAR READERS OF ALABAMA My name is Jennifer Anne Stephenson but most people call me Jen. I am excited to be the new publisher of the Seniors Blue Book for the Alabama Gulf Coast. I feel very blessed that Thad and Carolan have chosen me and my family to carry on publishing the SBB. This guide is such a valuable resource for aging well in our community. I am a native of Southern California and attended San Diego State University, graduating with a BS in Business Management. I, as well as my parents, moved to Gulf Shores over 13 years ago. Many people ask why? I moved to the area to wed my husband, Jason Stephenson, an Alabama native, who was born in Troy, grew up in Florence, and attended Troy (State) University. Since then, we have grown our family, first with four legged children– we have 4 dogs and 6 cats. We welcomed our daughter, Davis Anne, to our family in 2008 and our son, Luke Hanlon, in 2011. I have enjoyed immersing myself and my children in the numerous Southern traditions and culture. My dad was born in the South so I grew up with my grandmothers making fried chicken and beignets. While this may not seem like a big deal when you grow-up in the South but for a kid from California, this was unique. We had football season tickets at my alma mater and I rarely missed a game. However, I quickly learned that there is nothing like the spirit of football in the South. I rapidly came to enjoy SEC football and know that I cannot plan events unless I check the football ALABAMA – SUMMER-WINTER 2015
schedule first! My mom, Fran, also known as “MeMa” lives with us in our home in Gulf Shores. She is a RN who continues to work in the senior care services arena on a part-time basis and lives a very active senior life. My husband’s business is in the financial and insurance services dedicated to the senior market. One of the main reasons that we decided to buy the Seniors Blue Book for this area was the success that he experienced by advertising in SBB. My goal is to continue to share the informational foundation of senior services that Thad as created. The Seniors Blue Book was founded on a core principle of integrity. This principle has served as its foundation and the SSB has never strayed from this practice. My plan is to continue these same high standards and values on which the Seniors Blue Book was founded. These ideals are the reason that Seniors Blue Book is the trusted name in Senior Resources across the country today. My hope is to provide relevant and interesting information for our clients and readers. We have passion for what we do at SBB. This is an amazing company and I feel so fortunate to be part of this passionate group of people that bring valuable information to families, individuals and professionals who are trying to provide for those in need. If there ever was a monument to freedom, strength of national commitment, and pride of citizenship, the National World War II Museum is that tribute. In spite of all French Quarter attractions in New Orleans, the National World War II Museum is the #1 Attraction reported by TripAdvisor, and the #1 Best Place to Learn U.S. Military History by USA Today. The Museum displays actual aircraft and vehicles of the era, and hundreds of exhibits of WW II events. But that’s not all; remember the entertainment? The Andrew Sisters, Bob Hope, big band music, patriotic songs are part of the Museum’s offering. There’s a little café with malts and burgers, and a terrific restaurant with a nationally-known chef in charge. The gift store is packed WW II memorabilia, and there’s hands-on activities for the grandchildren (and us), and the introductory 4-D film, Beyond all
Boundaries, is a terrific experience, not to be missed. Here’s a bonus particularly for men: As a boy born in August 1941, the soldiers, sailors and airmen of this war were my heroes. I loved the airplanes of WW II, and yearned to fly someday. Now, at age 74, with those dreams fulfilled, I can walk up to a Rolls Royce Merlin engine that powered the Mustang and Spitfire, and a Pratt and Whitney Twin Wasp that powered the mighty C-47. I can almost hear the roar, feel the power, smell the exhaust, and be in awe as I was years ago. Where else can anyone get such a sense of pride and respect for our Nation? I encourage you, the reader of this article, not to wait any longer to visit the Museum. I guarantee you will be glad you did. Bring the family, or better yet, insist that the family take you. The Museum earned the title “National” by being such a national treasure, discoverable by all.

Having Difficult Conversations We often hear the term “Care for With Your Parents the Caregiver” but most people do not understand the profound depth of what that means.
Nothing makes adult children more nervous than finding the right words to use when bringing up concerns about their loved one. Maybe you are worried because your dad is living alone and think he would be better off in an assisted living apartment, or you think that your mom should not be driving anymore. You might be struggling with how to bring up the conversation about end of life wishes. These conversations can be very challenging and frustrating. So how do you start to talk to your parents about the future? For most of us it would seem natural to start by making a “plan” or a list

While a plan is important, it is equally important to realize that often we may be so focused on what we want to say that the conversation becomes one-sided, and we may not take time to consider our parents’ feelings. you about moving?” You may even try a validation statement like, “You hate the idea of depending on others to drive you around. I would be upset too.” The more honest conversations you have, the more positive results you get when it is decision time. Remember we do not grow old in a day and likewise, the process of preparing for the future is not something that can be accomplished in one afternoon. So be prepared to start small. If you have a conversation on a sensitive subject, be ready for a “no.” This happens a lot. Do not become discouraged; give them time to think about it. Respect the fact that, “change” to many people, is a kin to “loss”. Letting go may take some time. There are times when you will not have the luxury of taking your time. This may be when your parent is unable to reason due to Dementia or

Social workers and case managers are readily available in most health care settings. Using the skills of a professional to act as a mediator, facilitator or advocate is wise and can help preserve precious family relationships. Home Health Care Definitions HoMe HealtH ageNCies – Refers to someone State licensed health care providers that offer both medical and non-medical assistance in the comfort of your home. Services include skilled nursing, occupational, physical and speech therapies, assistance with activities of daily living, homemaker services and transportation services, both local and long distance. peRsoNal CaRe seRViCes – Services provided by a companion/sitter to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine activities such as cooking, cleaning, laundry, errands etc. Many agencies provide transportation to doctor visits, shopping etc, as well as recreational activities. They may not dispense medications. HospiCe – a comprehensive program of comforting care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. Professional medical care is given, and sophisticated symptom relief provided. iNtRaVeNous tHeRapY (iV tHeRapY) – any medical treatments/medications that need to be introduced into the body via a patient’s veins including antibiotic and hydration therapy, pain management, chemotherapy, and cen-

tral line care. Numerous IV Therapies can be provided in a patient’s home under supervision of a M.D. Respite CaRe – care that is shortterm or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the patient. skilled NuRsiNg – a higher level of care where services are performed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver related to the disease process, treatment, health measures and medication management. soCial WoRkeR – a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Also included in this category are residential area homes with an average of 5-8 residents. Some offer companion accommodations. Amenities vary. Services include meal plans, cable television, local phone, housekeeping, laundry, maintenance and transportation. Also provided is personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. CCRC / liFe CaRe – Continuing Care Retirement Community otherwise known as Life Care, offers a campus-style environment that may include villas, apartments/condos, assisted living and skilled nursing services. An endowment fee may be required that entitles residents to use assisted or skilled services when needed throughout their lifetime. daY CaRe – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed ALFs are prohibited by law to promote these services without a license.

iNdepeNdeNt / RetiReMeNt CoMMuNities – Housing designed specifically for the elderly. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths. Rates are monthly and based upon size/location of apartment. MeMoRY CaRe – These communities are either licensed assisted living facilities that specialize in delivering care to victims of dementia or a unit of a skilled nursing facility. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Staff is usually specially trained. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care. Payor sources usually accepted are private pay or some insurance plans. Respite CaRe – Provides a temporary break for caregivers by allowing a resident to have a short-term stay in a community that can meet their

skilled NuRsiNg CaRe – Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring a high level of need and routine services. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Most facilities provide rehabilitation following a hospital stay, and accept Medicare, Medicaid, and private pay.

It is documented that the caregiver for someone with a disability or illness often has more urgent medical concerns than the patient themselves. While that may seem odd to some people, just ask a caregiver when the last time they had a vacation, a night to themselves, a day to do nothing or have been to a doctor or dentist appointment. The fact is, that as caregivers, we often give all our energy and resources to the one we care for and at the end of the day, there is very little to give ourselves. Support groups, counseling and educational seminars are wonderful resources but it has been my experience that some caregivers find the idea of an outing overwhelming. But, there is hope. Just like trying to change your exercise habits or lose weight, you have to take small consistent steps and not try to run a marathon on the first day.

Know what resources are available to you Become an educated caregiver by taking classes and reading articles Know when to ask for help and do not be afraid or embarrassed to ask for help Take care of yourself by going to the doctor and dentist for regular checkups
The first step is to respect and know yourself. When you think about tools and tips that are available, use what works for you but do not be afraid to try something new. If it works, continue to do it, but if doesn’t work for you, then be confident enough to move to another technique. Remember that you are

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a "Medigap Plan." Medicare Part A HOSPITAL INSURANCE

For most people the monthly cost is: $104.90.
Premium varies by company, State, & other factors.
$147 annual deductible.
These plans generally pay the A & B deductibles and other eligible costs that are not paid by Medicare.
Program has a $1,260 deductible per benefit period.
80/20 after deductible.
It is possible to pay $1,260 several times a year. Plan covers: inpatient hospital care, home health care services, hospice inpatient skilled nursing & more. Providers are paid by Medicare assignment. No out-of-pocket maximum. Limited coverage outside the USA. Many people choose to add a Medigap or Advantage Plan.
Plan covers: health care providers, ambulance, emergency department, outpatient surgical services, & more. Strong preventive care benefits. Many Doctors accept Medicare patients. No out-of-pocket maximum. Many people add a Medigap plan, Part D or an Advantage Plan to fill the "gaps" in the coverage.
When this plan coordinates with Medicare many benefits are covered 100%. These plans coordinate with any hospital or provider that accepts Medicare. Plans are available though insurance companies. People can add a Part D plan to cover prescription drugs. Must have Part A & B to enroll.

Deductable (Phase I) $0 -$320 deductible Inital Coverage (Phase II) co-pay coverage until the total cost of the drugs = $2,960 Coverage Gap (Phase III) The member now pays almost all costs: 65% coinsurance or co-pay for covered generic drugs. 45% coinsurance for covered brand name drugs. You stay here until member has paid $4,700 Catastrophic Coverage (Phase IV) Then for the rest of the calendar year covered drugs are: $2.65 / $6.60 or 5% depending on drug tier. Many Part C plans include Part D drug benefits. Part D can be separate with medigap & PFFS plans. There is a penalty for late enrollnet into Part D. Must have Part A or B to enroll
