Don’t Need Your RMD – Don’t Take It In an effort to blunt the impact of an economic downturn set in motion by the global coronavirus pandemic, Congress passed the Coronavirus Aid, Relief and Economic Security (CARES) Act in March 2020. With most forecasters predicting that the U.S. economy was headed into a recession, lawmakers agreed on legislation that devotes historic government funding to support large and small businesses, individuals, families, hospitals, gig workers and independent contractors. At over $2 trillion, this is the largest rescue package in U.S. history. The major relief provisions talked about include: n

Loan and grant programs for small businesses Expansion of unemployment benefits to include furloughed workers, gig workers, and freelancers Increased unemployment benefits by $600 per week. Directed stimulus payments to families Money for hospitals, health care systems, and providers Loans to corporate America Grants to airlines, air cargo carriers and airline contractors for payroll support Money for local and state governments One provision not discussed as

much is the affect on retirement plans. The plan allows people to take special distributions and loans from tax-advantaged retirement funds up to $100,000 without facing a penalty. The 10% penalty on early withdrawals up to $100,000 from 401(k)s has been waived. Loans can be repaid over the next three years. If you recently turned 70 ½, you were supposed to take your first Required Minimum Distribution on April 1st ... but the CARES Act actually waived that requirement. In fact, everyone facing an RMD in 2020 is off the hook. This is good news, and will save on your taxes because you WOULD have been paying tax on the RMD that was based on your account’s value at the end of 2019 -- when almost everything was worth more. These measures apply to anyone directly affect ted by the disease itself or who faces economic hardship as a result of the pandemic. Area Agency on Aging Providing Essential Services
n these unusual times, it seems everyone is having to find new ways to do things, to take care of necessary business and to keep connected to others. But this is especially true for older adults, people with disabilities, and those with chronic conditions. Likewise, those of us who provide services to these same individuals have to be especially thoughtful in redesigning and reimaging our services. But we want you to know, the Area Agency on Aging has never stopped providing services, many of them essential to maintaining independence at home. We are still here and we will be here throughout the pandemic. We may not be seeing as many of you, but you are our focus every day we come to work. In this article I want to let you know how we are maintaining services while we are all staying safer at home or restricting activities.
We never stopped answering our phones at our Aging & Disability Resource Center, and will continue to be there to Julie McGee help Monday to Friday from 8:00 AM to 4:30 PM. We have been able to work with our community partners to keep all Area Agency on 27 senior centers providing hot “to go” meals to seniors and Aging Director to the homebound. To respond to increased needs for home delivered meals, we doubled the number of people age 60 and over we deliver frozen meals to each week. We were able to arrange for 5,600 fresh food boxes to be distributed in June. We are addressing some of the serious problems of social isolation by staff calling seniors receiving home delivered meals, Medicaid Waiver or caregiver services. Our South Alabama RSVP volunteers are sending cards to residents of nursing homes, and sewing masks for essential workers. We are also piloting the use of robotic pet cats and dogs for those with dementia, where studies have indicated positive benefits. We are learning to go virtual with Teletown halls, Facebook Live, Zoom, small group conference calls, and on-line educational and recreational programming. We obtained Trualta, a new online education and support platform for caregivers, just call us to enroll. We are offering our Chronic Disease Self-Management class in a home study format, shipping manuals and CDs followed by instructor lead small group calls. We are developing a program to help people with services when they leave the hospital. We are working to keep seniors safe and engaged. So while we won’t have our annual Senior Expos this fall or see you at community events, be assured we are open for business. We invite you to contact us if we can be of assistance. Baldwin County Seniors 60 and older can place their hand-crafted items in the shop on consignment. •C lasses – Ceramics classes are held on Tuesdays and Thursdays each week. Yarnspinners—Knitting and crocheting class meets on Wednesdays at 9AM. •N utrition Sites – Six nutrition centers are located throughout the County. Centers provide hot nutritious meals 5 days a week. QMB is a program for seniors and persons with disabilities who are on Medicare and who have limited income. It covers the remainder of everything for which Medicare pays part, such as all hospital and doctor bills, plus the $198 annual deductible. It also pays the monthly Medicare premium that is withheld from most Social Security checks, so your check will increase if you get QMB. YOU DO NOT NEED ANY OTHER MEDICARE SUPPLEMENT (such as Blue Cross C+, AARP, etc.) IF YOU QUALIFY FOR QMB.

INCOME LIMITS FOR QMB $1,084-one person unmarried or married but not residing together $1,457-married couples who reside together(combined income) To determine if your income falls below these limits: first, take the gross monthly amount that you make from working at a job. (The gross amount is the amount before any deductions for taxes, insurance, or anything else.) Subtract $65. Then divide the remainder by 2. This is how much of your earning from a job count toward the QMB limit. Second, add the gross amount of all other monthly income such as rental income, Social Security, other pensions, and veterans benefits. DO NOT include interest and dividends. You must count the gross amount of these checks before anything is deducted or taken out. For example, Medicare takes a Part B premium out of the Social Security checks for most people. This amount must be added back to the check you receive to determine how much counts for QMB. If the amount that counts is less than $1,084 and you are not residing with a spouse or $1,457 combined for a married couple who reside together, then you are eligible for QMB. ________________________________________________________________________________ SLMB and QI are programs for elderly and disabled persons who are on Medicare and who have limited income but do not qualify for QMB. SLMB and QI will pay your Medicare premium for you each month if you qualify. Thus, your Social Security should increase by the amount of the Part B premium each month. Since deductibles and copayments for Part A and Part B are NOT covered by the SLMB/QI program, YOU STILL NEED YOUR MEDICARE SUPPLEMENT OR A MEDICARE ADVANTAGE PLAN. INCOME LIMITS FOR SLMB $1,296-one person unmarried or married but not residing together $1,744-married couples who reside together (combined income) Determine your income the same way as stated above for QMB eligibility. Handling Legal Business During Conducting business as we A Pandemic age is no fun, but in the midst of a pandemic, it is easy for people to simply postpone that important business.
This is not a good idea, particularly since our aging population has proven to be vulnerable to the COVID-19 virus. We can give you some tips to get your business taken care of quickly and painlessly. Your general business needs include: 1. Banking. 2. Estate planning. 3. Taxes. 4. Insurance. 5. L icenses, tags, other Government functions. You may find locations closed or not taking appointments, longer lines and no seating present, and additional roadblocks to smoothly handling transactions, so STEP ONE: Be familiar with your state’s orders regarding safety or health restrictions that are currently in place, for example, can you get out to the bank in person, do you have to wear a mask? Is there seating if there is a wait? STEP TWO: Determine deadlines and mark your calendar. For example, calendar the date to change your health insurance (for most Medicare recipients this is October 15-December 7), and enter a recurring reminder in your calendar.

STEP THREE: List important telephone numbers and be persistent. Call the place of business. Get specific information for accomplishing your task. If you need to renew a CD, can you give instructions over the phone, online, or by mail. STEP FOUR: Schedule the appointment, even if it is virtual. Many businesses permit tasks to by mail or online like renewing your driver’s license or car tag, to avoid overcrowding the DMV. For estate planning, we are scheduling in person or online consultations via video, and can mail documents. Video notarization is available in Alabama during the pandemic. STEP FIVE: Now is the time to take that computer class (online) and become more proficient in using your computer, tablet or phone. This will allow you to conduct a great deal of business on the internet from the safety of your home. STEP SIX: Enlist allies. Have a trusted friend that can assist in conducting business. Contact your attorney and prepare a durable power of attorney to give that person the ability to handle your important business if you cannot out. Background & Info on Baldwin County Memory Screening/ Balance Testing For years the Alzheimer’s Foundation of America (non-profit, NYC group which primarily raises funds in aid of caregivers/ support groups) has supported and run a National Memory Screening Program. In 2015, I saw a map AFA published noting locations for free memory screening across the USA. Noting that Huntsville was the only Alabama screening location, I contacted AFA and started a site in South Baldwin County. “Testing 1, 2, 3” went live in November, 2015, holding screenings since then every year— one in October/November for locals and one in February for Snowbirds. Over 60 volunteers now support our screening efforts. In the fall of 2016, we added Balance Testing which can show how to improve balance, regardless of age. We rely on medical professionals— retired nurses, dental technicians, pharmacists, caregivers, etc.— to conduct memory screenings. Non-medical volunteers are Greeters and Balance Testers. The events run approximately 3 hours each, 20+ events a year (about 60 hours of active screening/testing).
On average, we screen 500+ people, the numbers growing each year. The biggest contribution “Testing 1, 2, 3” makes is to the 75% of those screened who have never had a memory test and do not know whether memory loss is normal or not. All seniors suffer memory loss, particularly short term. To be screened and learn that one’s memory loss is normal, age-related can be very comforting. To those who fail the test, we explain that the test is not a diagnostic tool itself, but can be an indicator of significant memory loss. To those, we give forms to take to personal doctors for further evaluation. Since memory loss can be caused by many factors (other than dementia), a more complete evaluation is important. the foundation that started it all.
Initially named the Mobile Regional Senior Community Center Foundation, Inc. when the Articles of Incorporation were filed in 2008, the foundation was organized as a 501 (C) (3) to gain funding and bring power to the idea that Mobile needed to support seniors in our area with a center and programs designed for senior adults age 55 and up. In 2008, the 20,000 square foot center opened and offers a thermal plunge pool, a fitness room, billiards room, computer classes, and exercise, dance and art classes. A variety of programs are offered to meet the needs of both retired and working senior participants. There is an active roster of 5,000 members for whom the center offers not only activities but has become a place to meet and make friends, and for many, a home away from home. In 2013, the name of the center was changed by the Mobile City Council to the Connie Hudson Mobile Regional
Senior Community Center to honor now County Commissioner Connie Hudson, who spearheaded the effort to fund and build the center. The foundation works as a partner to support and continue to enhance the center by providing resources for furnishings, appliances, computers, art equipment, pool tables and fitness equipment. Managed by a Board of Directors with assistance from an Advisory Board, the foundations hosts fundraisers and accept donations from supporters and friends. Because needs are always constant and changing, contributions are always welcome. The Greatest Generation:Thriving During Covid The Via Health, Fitness and Enrichment Center is a community center for active, older adults located in Midtown, Mobile. The organization serves more than 10,000 seniors and community users each year through health, fitness and enrichment programs and Community Needs Outreach services. Though still closed to the public, the Via Center has re-tooled its programs and services during coronavirus…and our community’s Senior Citizens have been with us every step of the way. Seniors have faced the most serious challenges during the Covid pandemic of any age group. When seniors contract the virus, their symptoms are harsher, and they face a higher death rate. Safer-atHome and Stay-at-Home orders have disrupted their family and community networks and increased social isolation. Everyday activities, such as going to the doctor, grocery store or bank carry serious health and safety implications. We hear and read daily that Seniors are distressed, depressed and the only thing they can do is “hunker down at home.” However, the same Seniors who sacrificed to help America turn the tide in World War II, have done battle with the coronavirus and are winning in many ways. We typically don’t think of Seniors as easily adaptable in this fast-paced world we live in. But, the thousands of Seniors our organization has been interacting with during the 20 weeks of the pan-

demic, have proven themselves to be resilient and strong in many ways. Technology and Digitally Connecting Seniors have learned to access and use technology for social interaction, educational programs and news and information: iPhones, the internet, e-mail communication and social media platforms like Facebook and Instagram. They have learned how to play bingo, assemble puzzles, and take virtual field trips to places like the Smithsonian Museum and the Sistine Chapel in Rome. Health and Wellness Used to in person and hands-on doctor visits, many seniors have embraced Telemedicine. They are now using Zoom and their iPhones to work with their healthcare professionals to ensure they stay healthy. They are using virtual fitness programs to remain physically healthy and mentally sharp. Creating New Social Networks At the Via Center, we have groups that have formed such as our Monday “Lunch Bunch,” who meet under our portico, share a snack, and swap stories of their week. Many seniors now attend on-line church services and participate in family conference calls to check in with loved ones. In Mobile County, they have formed community walking groups (early in the morning to avoid the heat!) to get their fellow older adults out of their houses and combat social isolation. US. Many seniors, who were already vulnerable and largely alone, have been asked to isolate even more.
Grandparents who get so much joy from visiting with grandchildren are no longer allowed to. Socialization with friends at dinners or meal times in facilities have stopped. No-one is allowed in facilities to visit. We ask our grandparents to remain home and only leave when absolutely necessary. Physical distancing is such an important step in helping to control the spread of Covid 19. But one of the effects of social distancing is isolation and loneliness. Our senior loved ones are incredibly vulnerable as many were already experiencing limited contact with others. Removing all contact is being met with a serious secondary side effect from the novel corona virus, which is isolation and loneliness leading to depression. Social isolation and loneliness are not mutually exclusive. One can be socially isolated but have the parameters in place to feel as if they are still connected. This can be done largely in part though technology. Unfortunately less than 34 percent of our seniors use technology to connect. Learning to use FaceTime, Facebook and Instagram can immediately connect seniors with loved ones. Being able to have a conversation in real time, to see a loved ones face, or share a message with a friend helps to bridge the gap
between eventual visits. These are wonderful, free tools that are easily accessed. This also provides a wonderful way to Connect Our Generations. Grandchildren can remain in contact with grandparents and even great grandparents! These platforms are a beautiful easy way to share stories and experiences from years past as well as current. Finding programs, such as the Connecting Our Generations program, makes it easy to learn how to use these platforms to stay connected with one another. Encouraging discussions and contact has been proven to lift spirits, to help families feel more connected, and has a significant impact on depression. We can see a much stronger bond fostered between our generations by finding new ways to connect throughout the pandemic, and after. Connecting our generations is a program created to connect seniors with their loved ones. This is a hands on, user friendly program that teaches Seniors (1) How to stay safe online and (2) How to set up a social media account to stay connected with loved ones. The program is available to be taught in person or through webinar format. ~

Community Leadership Seniors have been leaders during the coronavirus. Seniors are a group that has been protecting themselves and the community by wearing masks and social distancing. They are setting the example for younger groups (age 29-45) who have been slow to embrace wearing masks and altering their behavior for the good of the community. Volunteering We have seen a surge in Seniors volunteering to help others in their community. Volunteering through calling home-bound Seniors, knocking on neighbors’ doors and asking what they can do for them (need groceries… need your dog walked… need your trash taken out), making care packages with food, books, puzzles, and even volunteering for programs like Meals on Wheels. We have an amazing group of Seniors who donates food and put together bags for our monthly drivethrough community food distributions. Pandemic Grandparenting The latest research shows that 40% of grandparents are a key support structure in their family units

(Parenting Magazine). While seniors have had to forgo much of their one-on-one contact with their grandchildren, they have continued to play their important role. Nothing provides comfort to grandkids experiencing anxiety during Covid like grandma’s treats, and our Seniors are doing more baking and cooking than ever. Seniors are also using video calls and good, old fashioned letters to communicate. By encouraging letter writing, Seniors are keeping in touch with grandkids and helping them work on their writing skills and spelling in the absence of classroom instruction. There will always be Seniors who are more vulnerable and need special care. However, the phrase “70 is the new 50” have proven to be true for many of our Seniors in Mobile. Non-Medical Home Care Definitions 24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. COMPANION/HOMEMAKER SERVICES – Refers to services provided by a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The
companions may provide assistance with routine household activities – cooking, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to physician’s offices, grocery and clothing shopping as well as recreational activities. Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing, dressing, toileting, ambulating nor dispense medications for their clients. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or
maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided:
Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

RESOURCES FOR AGING WELL
IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.
PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.
INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...) INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

Until 7 years ago, Alzheimer’s was not diagnosable as the cause of death without an autopsy. Today, the condition can be diagnosed with a brain scan.

DEMENTIA is a general term that describes a group of symptoms such as memory loss and judgement/ language impairment. It is a permanent and progressive disease. It is very common among the elderly. ALZHEIMER’S is the most common form of DEMENTIA. It is progressive, irreversible and leads to death.

The cycle of Alzheimer’s is roughly 12 years, depending on when it is diagnosed. 12 years would be an early diagnosis and most diagnoses come about the 8th year. The cycle is the length of time from diagnosis to death. It is now possible to diagnose Alzheimer’s even earlier than 12 years but a diagnosis would seldom be requested because the symptoms at that point are relatively mild and are similar to normal age related memory loss. Early onset Alzheimer’s, which might occur in a person’s 50’s, generally moves ‘faster’ than later cases.

The odds of a healthy woman at age 65 developing Alzheimer’s is 1 in 6. For a man, it is 1 in 11. The risk of DEMENTIA doubles every 5 years between 65 and 95 years of age. Alzheimer’s in women progresses at a faster rate than in men.

Early detection of Alzheimer’s is thought to be possible by measuring a person’s sense of smell, a reading of the carotid artery & through a

vision test. All are still in the study phase.

Chronic Traumatic Encephalopathy (CTE) is getting a lot of press these days because of the number of football players in the NFL who have suffered from the disease. Over 95% of those who died “early” and had their brain donated to research, suffered from CTE. The symptoms of CTE are almost identical to those of someone with Alzheimer’s and many people with CTE are diagnosed with Alzheimer’s. Both are diseases where Beta Amyloid Plaque builds up in the brain and eventually ‘short circuits’ the wiring of the brain. Today, CTE can only be determined by an autopsy since a brain scan cannot differentiate it from Alzheimer’s. CTE is caused by repetitive hits to the head. They do not have to be severe blows but it is the multiple blows over years that cause the condition. IN SUMMARY Our volunteers stand ready to provide Memory Screenings and Balance Testing at any event held in Baldwin County where seniors will be in attendance. It only takes about 15 minutes of the participant’s time and it is TOTALLY free and confidential.
