There are many ways a community can collaborate and begin dementia friendly initiatives, the faith community being one of many avenues to approach. Dementia Friendly is a ministry that can be easily embraced by faith communities. Dementia Friendly churches encourage people with dementia and their loved ones, recognizing their positive value to the congregation. A connection with a local church can make all the difference to these people. By showing understanding, offering support, and making small changes, churches can enable someone with dementia to continue attending services or take part in church life in meaningful ways. There are three keys to developing a Dementia Friendly mindset at church, which are 1) building awareness, 2) developing acceptance; and 3) taking actions that allow persons with dementia to participate in church life and provide support to their care partners. Reinert is taking one of these actions steps by planning a memory café and related outings where persons with dementia and their family and friends are welcomed to go and socialize in a welcoming setting such as an ice cream social or coffee shop. Volunteering Provides New Challenges and Opportunities Legacy Leadership Institute for Older Adults If you think you may want to volunteer but are unsure which direction you want to go, consider attending the Area Agency on Aging’s free Legacy Leadership Institute for Older Adults to be held March 11-15, 2019. This one week program held on the campus of the University of South Alabama utilizes faculty and vising speakers to educate attendees on aging issues and impactful volunteering. It is designed to help retired professionals and baby boomers find their niche in the world of volunteering among opportunities at the Area Agency on Aging and in their communities. The South Alabama Retired Senior Volunteer Program (RSVP) has taken root and is growing in Baldwin County. RSVP is one of the largest networks in the nation for people 55 and over. The Area Agency on Aging provides a Transition Coordinator to help individuals plan for a safe and successful move out of institutions such as nursing homes to the community. Gateway to Community Living is an initiative of the Alabama Medicaid Agency that helps Alabamians who are aging or have disabilities live at home in their own communities. It allows eligible people who currently live in an institution to enroll in one of Alabama’s Home and Community-Based Waiver programs so they can still get the services and supports they need at home. For those interested in moving from a facility to the community, a trained Transition Coordinator from the Area Agency on Aging can help them find out if they are eligible and talk with them about available services that would best suit their needs for safe and healthy community living. Medicaid Waiver Programs Provide Home and Community Care The Area Agency on Aging operates the Elderly and Disabled Medicaid Wavier program. It is designed to allow elderly and/or disabled individuals of any age (including children) who would otherwise require care in an institutional setting or nursing home to remain at home by providing case management and supportive services such as personal care, respite care, adult day care, companion services, and meals. A personal choices option is available that allows families to hire their own workers and have more flexible services or save for expensive equipment purchases. The Area Agency on Aging also operate Technology Assisted and Alabama Community Transition Medicaid Waivers. The TA Waiver is for those individuals age 21 and older with complex skilled medical conditions who are ventilator dependent or who have a tracheostomy and covered services include private duty nursing, personal care/attendant services, medical supplies and assisted technology. The Alabama Community Transition Waiver (ACT) provides services to individuals with disabilities or long term illnesses who currently live in a nursing facility and who desire to transition to the home or community setting. Those eligible receive an array of services to help them move back into the community and remain at home. All programs have medical and financial criteria. Long-Term Care Ombudsman The Area Agency on Aging provides the local Ombudsmen for Mobile, Baldwin and Escambia counties. The Ombudsmen are advocates for residents in long-term care facilities - Nursing Homes and Assisted Living Facilities. They work with all parties to protect the health, safety, welfare, and rights of Alabama’s long-term care residents. Anyone can use the Ombudsman, including residents, their friends and family, or employees of long-term care facilities. An Ombudsman’s role is to: rovide information to the public including how to choice a facility, • P and residents’ rights in long term care facilities. Staff are available to make presentations. • E ducate residents, family members, and facility staff, and assist with resident and family councils. • V isit each facility to evaluate conditions and help ensure residents receive quality care to which they are entitled. • Investigate and resolve complaints on behalf of residents. • Act as a mediator between residents, family members and staff. Ms. Reinert says, “Caregivers and people with dementia tend to become isolated in their homes, and withdraw from their usual activities due to the disease progression, the stigma associated with dementia, and embarrassment due to the lack of understanding from the general public, previous social interactions, friends, and family.” She understands this first hand as she cared for several family members who had Alzheimer’s disease. According to Dementia Friendly America, “Dementia friendly faith communities can provide an important spiritual respite. As welcoming, compassionate environments, they offer families with dementia opportunities for meaningful engagement and spiritual connection. Behavior Concerns with Alzheimer’s Disease When a person is living with Alzheimer’s or another related dementia, as the disease progresses the person may experience changes in their behavior, that may cause concern for their family members, and/or their care givers. Understanding that there is actual damage to the brain in many areas is the first step in coping with the behavioral issues. In certain brain diseases, the brain connections are being loss, and the cells are dying.

ehavioral concerns can vary from a lack of inhibitions, (such as cursing, or doing something that is socially unacceptable), to as severe as becoming physically aggressive. Behaviors do not just simply happen, they are usually considered to be a form of communication as well as a reaction to something that is real or ‘perceived’ as real. Trying to decipher the meaning is the real challenge. Numerous triggers can be the cause of the behavioral concern. Triggers can be categorized into five types, other than the brain

deterioration itself. “Fear” is one of the most powerful emotional triggers. Emotional triggers may also include: frustration, sadness, loneliness, angry… “Noise” is the most common environmental trigger as well as: overstimulation, cluttered space, too crowded, too much unstructured time, too hot or too cold. “Pain” is one of the most powerful physical triggers. Other Physical triggers may include but are not limited to urinary tract infections, constipation, dehydration sleep disturbances… “Tasks” can be a task trigger, tasks that can be too confusing; or tasks with too many steps. 95% of the time

In handling behavioral concerns, first one needs to understand that the behavior cannot be modified or changed, but we must change ourselves and our approaches to impact the behavior. One of the first effective and successful interventions is actually difficult for most people, that is to “ACT” and “Not to React” to the behavioral concern. When reacting to behavioral concerns it only makes the situation worse. Interventions are variable, and what works one time may not work the next, and not all interventions will be successful for every person, they need to be individualized for the person. In developing successful intervention for any given person with any given behavioral concern, knowing the who, what, when, where, and how of the behavior is vital. Once those questions are answered, a plan can be developed. Some interventions are routine for all persons. A sampling of interventions may include: n

Observing for visual, or verbal prompts that a behavioral concern is about to happen, and intervene before it happens; e aware of what your body B language is portraying;

“People” cause behavioral issues. People, who do not approach the person in the correct manor, asking too many questions, trying to reason with the person living with Alzheimer’s or another related dementia, trying to use reality orientation to reason or change the persons way of thinking.
Constantly be building trust; Give up ‘Control’, and the need to be right; Utilize retained abilities’ – Apologize, by saying, “I am sorry”; Be Patient!
In summary, each person with dementia is unique and has distinctive changes that occur with the loss of brain functioning. Every single human has the desire to be respected, in control and loved; just because a person is living with a dementia condition, those desires do not change. The person living with dementia cannot change; therefore, those caring for them must change their approach, and expectations. The YELLOW DOT Program

his will help to ensure that you receive the medical attention you need during that first “GOLDEN HOUR” after a crash or other emergency involving your vehicle. To participate in the YELLOW Dot Program, the four items needed are a photo, a completed personal information form, a YELLOW DOT decal, and a YELLOW DOT folder. First, a photograph is taken of the participant. The photo is later taped to the front side of the completed information sheet. The next step is to complete the form which contains the participant’s information; name, address and phone number of person to contact in case of an emergency; medical history; allergies; medications and dosage taken; and names 36 ALABAMA GULF COAST – FALL 2018-2019

H•H OVER THE LAST FEW YEARS, STUDIES HAVE ESTABLISHED a relationship between untreated hearing loss and dementia in older adults. New medical research* has found that hearing loss actually contributes to dementia and mental decline. It’s estimated that as much as 36 percent of dementia risk can be attributed to hearing loss, and hearing loss is linked to 30-40 percent greater risk of cognitive decline in people with hearing loss versus those without. Furthermore, in 2011, another study on dementia found that “the worse the initial hearing loss was, the more likely the person was to develop dementia. Compared with people of normal hearing, those with moderate hearing loss had triple the risk.” The good news? Hearing aids can help! Researchers at Columbia University Medical Center found a direct correlation between improved cognitive function and hearing aid use in older adults with hearing loss. “Our study suggests that using a hearing aid may offer a simple, yet important, way to prevent or slow the development of dementia by keeping adults with hearing loss engaged in conversation and communication,” suggests Dr. Anil K. Lalwani, M.D., professor at CUMC. The survey confirms what hearing healthcare providers have known for decades — that correcting hearing loss with hearing aids provides benefits in addition to improved hearing, such as improved quality of life, improved relationships at home and work, feelings of safety and independence, and improved mental health. Medicare 2019 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

In this corner – HOPE! Sometimes life can throw a pretty mean sucker punch, which is exactly how Gary Ellis, a successful, local businessman, felt when he was told he had Parkinson’s Disease. He was stunned and afraid, but immediately set out to find a way to fight this incurable disease. After seeing a feature story on 60 Minutes about Rock Steady Boxing, Gary was inspired. He learned that Rock Steady Boxing, a non-contact, boxing-inspired program, can delay, reduce and even reverse the symptoms of Parkinson’s Disease. You see, Parkinson’s Disease causes a loss in many of the same elements that boxers condition to improve, such as optimal agility, speed, muscular endurance, accuracy, balance, hand-eye coordination, footwork and overall strength. At Rock Steady Boxing, the opponent is Parkinson’s Disease. Gary knew this was something he wanted to do. Unfortunately, the nearest class was in Birmingham, AL, but that didn’t stop Gary. He searched to find just the right person to undergo training and facilitate the Rock Steady Boxing program in this area. Chris Wheeles, owner of Wheeles Karate Academy, would prove to be that person. Committed to helping fight this disease, Chris received intense training and was ready to make a real difference in our community. Classes began in Gulf Shores in August 2017. Today, you can find an average of 20-25 boxers in each class fighting their way to a healthier and happier life. For Gary, Rock Steady Boxing is nothing short of a miracle. Those once referred to as “patients” are now call “boxers” – proudly fighting the same fight. With the help of Rock Steady Boxing, it’s Parkinson’s Disease that’s taking the hit!

24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. COMPANION/HOMEMAKER SERVICES – Refers to services provided by a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine household activities – cooking, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to
physician’s offices, grocery and clothing shopping as well as recreational activities. Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing, dressing, toileting, ambulating nor dispense medications for their clients. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you. CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above.
Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth. HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.
NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor

functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled
nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

Hospice Care Definitions
CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team. 24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. Hospice Care Definitions continued from page 92 PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the
hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.
INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...) INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

magine shopping at a store with non-skid floors, shelving that is neither too high or low, easy-to-read signage, and emergency call buttons located throughout. Imagine shopping carts with built-in seats and attached magnifiers or even stores with chairs set around the perimeter for quick breaks. Imagine going to an event where transportation to and from is accessible and free or low-cost, parking is reasonably close, and seating is plentiful. Imagine sidewalks and parking lots surrounding the event are free of snow, ice, wet leaves, puddles, broken cement, and clutter. Imagine the timing and location of the event are suitable to anyone attending. And who wouldn’t want alternatives to long lines? These are examples of age-inclusive thinking. Age-inclusive thinking means we consider the shared needs of everyone because we all have different needs as we transition between the many stages of life. It means leaders, planners, business owners, and elected officials care about the wants and perspectives of older adults, as much as 98 ALABAMA GULF COAST – FALL 2018-2019
they care about people of other ages. Understanding what age-inclusive thinking means and then

Older adults are living longer and want to be engaged in the community. Without the physical and organizational structures necessary to help them age well in their homes and communities, the quality of life—for people of all ages— will be compromised. That’s why older adults (and their caregivers) play an important role in ensuring communities apply age-inclusive thinking. Only they know what it’s like to go about their daily routines and can speak to what is needed for their community to best support them as they age. Wouldn’t it be nice to no longer have to imagine the examples of age-inclusive thinking listed above and instead experience them because they’re automatically happening in the community? It begins with a conversation; with older adults speaking up and letting their wants and perspectives be known and heard. What ideas do you have for ensuring the places and happenings in your community are accessible and inclusive to people of all abilities and mobility? No matter how big or small of an idea you might have, I encourage you to reach out to your community’s planners, elected officials, business and organization leaders, aging service providers, and media and let your voices resound. ~
