Care at the E.A. Roberts Alzheimer’s Center Helping families cope with the varying degrees of dementia

L

ike many couples, John T. and Barbara Knight worked hard, reared their son and looked forward to their fun-filled golden years. That all changed with a diagnosis of dementia. Now Barbara spends her days caring for a husband who can no longer complete a sentence. She is his whole world. “Until you’re in this situation, you can’t understand the pressures of caregiving and the toll that it takes on a family,” said Laura Harrington, R.N., manager of the E. A. Roberts Alzheimer’s Center at Mobile Infirmary in Mobile. “Of course, you would never know what a family endures from looking at Barbara. She never complains. ACCESS ALABAMA, the Aging and Disability Resource Center Your local Aging and Disability Resource Center (ADRC), housed inside the Area Agency on Aging, provides easy access to programs and services for seniors and people of all ages with a disability. New South Alabama RSVP opens office in Daphne for Local Volunteers The City of Daphne has partnered with the Area Agency on Aging of the South Alabama Regional Planning Commission to house the new South Alabama Retired Senior Volunteer Program (RSVP). The office is located on the first floor of City Hall at 1705 Main Street in Daphne. RSVP is enrolling both volunteers, and organizations that need volunteers throughout Baldwin County. The South Alabama RSVP serves both Baldwin and Escambia counties and gives adults age 55 and over meaningful and rewarding volunteer experiences in their local communities. RSVP is one of the largest volunteer networks in the nation. Volunteer Guardian Program The Mobile County Volunteer Guardian Program is available to adults of all ages. This rewarding program is a partnership between the Probate Court of Mobile County and the Area Agency on Aging. Motivated, compassionate volunteers will be appointed by the Mobile County Probate Court to make informed decisions about the health and well-being of individuals who can’t make those decisions for themselves and have no one else willing or able to serve as their guardian. Legacy Leadership Institute If you think you may want to volunteer but are unsure which direction you want to go, consider attending the Area Agency on Aging’s free Legacy Leadership Institute for Older Adults to be held March 27-31, 2017. Taking Charge of Your Health The Area Agency on Aging is offering free classes to help individuals take charge of their health, stay healthier and remain independent. Living Well Alabama, a program for chronic disease self–management, is designed to help those with ongoing health conditions live healthier lives. Long-term conditions may cause fatigue, a loss of physical strength and endurance and requires new strategies to delay disability, improve function and help people manage their own health on a day-to-day basis. The Area Agency on Aging also offers A Matter of Balance, a program to help reduce the risk of falls by teaching fall prevention strategies. Falls are a leading cause of injury and death among adults age 65 and older, but falls are not a normal part of aging and most are preventable. After taking A Matter of Balance, 98% of participants reported greater confidence in fall management. Caregiver Services The Alabama CARES caregiver program offers support to caregivers through information, assistance and education including Caregiver Colleges, support groups, counseling, and an Alzheimer’s-dementia helpline. Limited respite care and supplemental services may be available for those who qualify. Respite for caregivers provides temporary relief from caregiving by providing sitters or companions, personal care, homemaker services, adult day care and other services requiring a skilled helper in the home. Limited supplemental services may include services such as emergency personal alert systems. The Alzheimer’s & Dementia Helpline of South Alabama is dedicated to helping families, caregivers and professionals obtain local resources and information on Alzheimer’s disease and other dementias. Monthly welcome to Medicare meetings have the answers you need When seniors in Baldwin, Escambia and Mobile Counties have questions about Medicare, the Area Agency on Aging is there with answers. Assistance is available by visiting www.Medicare.gov, or by calling 1-800-MEDICARE, but many times a personal touch is needed. That is where SHIP, or the State Health Insurance Program, comes in. SHIP offers free, unbiased information as your health care advocate. SHIP counselors work for you and will never try to sell you insurance. In fact, they not only give you advice on selecting the best plan for you, they’ll also see if you’re eligible for Medicare savings programs to help with premiums and copayments. The Supplemental Nutrition Assistance Program (SNAP) In Alabama, 70,250 seniors are using SNAP to buy healthy food. Are you? Alabama has made a special effort to help seniors age 60 and over have healthy diets through its improved SNAP food assistance program that provides money to buy groceries tax free. For seniors who are retired or unemployed with no earned income, there is no office visit necessary, benefits under the Elderly Simplified Application Program are awarded for 3 years unless maximum income is exceeded, no resources or savings are counted, income is self-declared, and a standard medical deduction is available to increase benefits. It is estimated that over 70% of seniors who qualify never apply, leaving an average of $850 in benefits on the table. If your income is not over $1276 for one person OR if you have high medical expenses, call the Area Agency on Aging to see if you qualify at 251-706-4680 or 1-800-243-5463.

Senior Centers and Nutrition Services Senior centers are local gathering spots where older adults have access to many aging services, including hot meals at centers with SAIL programs. The centers offer fun activities, health and wellness education, arts and crafts, transportation, volunteer opportunities, education and local trips. South Alabama has 27 SAIL Senior Centers in Baldwin, Escambia and Mobile Counties which provide seniors with nutritious meals and opportunities for activities. In some areas, a meal delivery program is available for homebound seniors who qualify. These meals are available free to persons age 60 and older, their spouses, and persons with a disability residing with an eligible participant. Ombudsmen are advocates for residents in nursing homes and assisted living facilities, and the Area Agency on Aging provides the local Ombudsmen for Mobile, Baldwin and Escambia Counties. They work with all parties to protect the health, safety, welfare and rights of Alabama’s long-term care residents. Anyone can use the Ombudsman, including residents, their friends and family, or employees of long-term care facilities. Senior Employment The Senior Community Service Employment Program, also called the Senior Aides program, is a community service and work-based training program for older workers that provides useful community services and fosters individual economic self-sufficiency through training and placement into unsubsidized jobs. While in the program, participants are paid minimum wage for up to 20 hours a week. Gateway to Community Living The Area Agency on Aging is now providing a Transition Coordinator to help individuals plan for a safe and successful move out of institutions such as nursing homes to the community. Gateway to Community Living is an initiative of the Alabama Medicaid Agency that helps Alabamians who are aging or have disabilities live at home in their own communities. It allows eligible people who currently live in an institution to enroll in one of Alabama’s seven Home and Community-Based Waiver programs so they can still get the services and supports they need at home. For those interested in moving from a facility to the community, a trained Transition Coordinator from the Area Agency on Aging can help them find out if they are eligible and talk with them about available services that would best suit their needs for safe and healthy community living. Gulf Coast Masters Games Held May through July The Area Agency on Aging sponsors the Gulf Coast Masters Games for Baldwin, Mobile and Escambia counties. The Masters Games of Alabama were organized in 1989 to involve senior adults in recreational activity and fitness. Any resident of the state of Alabama age 50 and older is eligible to participate in the games. The winners for the local events may compete in statewide competitions. The local games are held every summer at various locations in Mobile, Spanish Fort, Chickasaw, Orange Beach and Robertsdale and include basketball free throw, billiards, Frisbee throw, Nerfball throw, softball throw, horseshoes, rook, shuffleboard, table tennis, checkers, dominos, doubles dominoes, and bowling. Opportunities to compete at the state level in swimming, golf and three-on-three basketball are also available. Local champions will compete in the statewide games in October in north Alabama, and a chartered bus will be available. South Alabama senior athletes bring back many state medals annually. The Area Agency on Aging operates a Medicaid Waiver program for the elderly and/or the disabled of any age. The program allows these individuals, who would otherwise require care in an institutional setting or nursing home, to remain at home through case management and supportive services. Those services might include personal care, respite care, skilled respite, adult day care, companion services and home-delivered meals. A similar Medicaid Waiver program is offered for persons living with HIV. Both programs have a “Personal Choices” option which allows families to hire their own workers and have more flexible services.

Medicaid Waiver Programs provide Home and Community Care

The Area Agency on Aging also operates the Technology Assisted and Alabama Community Transition Medicaid Waivers program. The TA Waiver is for individuals age 21 and older with complex, skilled medical conditions who are ventilator-dependent or who have a tracheostomy. Covered services include private duty nursing, personal care/attendant services, medical supplies and assisted technology. The new Alabama Community Transition Waiver (ACT) provides services to individuals with disabilities or long-term illnesses who currently live in a nursing facility and who desire to transition to the home or community setting. Eligible recipients receive an array of services to help them move back into the community and remain at home. Each of the programs has medical and financial criteria which must be met to qualify. SenioRx & Charitable Pharmacy offer Local Prescription Assistance SenioRx is a prescription assistance program of the Area Agency on Aging that helps persons of any age with chronic health conditions obtain free or low cost prescription medications who have no or inadequate health insurance coverage, or find themselves in the Medicare Part D prescription coverage gap. Area Agency on Aging staff, and their partners at the Baldwin and Escambia County Councils on Aging, help people apply to national pharmaceutical companies for free prescription medications. hen family caregivers become overwhelmed with navigating the complexities of the healthcare system and the daily challenges of caregiving, a geriatric care manager may provide a lifeline. Geriatric care managers (GCM), or aging life care professionals, are different from other healthcare providers in that they may not provide handson care. Medicare 2017 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

ccording to a 2015 AARP survey, 34.2 million adults in the U.S. said they had been a caregiver to an adult loved one or family member age 50 or older in the prior 12 months. An additional 6.5 million were caring for both an older adult and a child. When caregiving becomes too challenging and outside help is needed, knowing where to start can be overwhelming. Now science tells us we’re happier and healthier even when those friends are furry or feathered. “Any pet parent will tell you there are obvious, immediate joys that come with owning a pet. They entertain us, and they offer companionship and affection,” said Michael Graham, executive director of The Haven, “But now we’re learning that they’re good for our physical and mental health.” According to the Centers for Disease Control, pets can decrease blood pressure, cholesterol levels, triglyceride levels and feelings of loneliness. The American Heart Association has linked the ownership of pets, especially dogs, with greater longevity and a reduced risk for heart disease. One study even found that pets help us get rid of bad habits. More than a quarter of pet-owning smokers tried to quit smoking once they learned about the negative health effects of secondhand smoke on pets, according to the journal Tobacco Control. Studies are now suggesting that these health benefits may be especially pronounced in older pet owners. A study published in the Journal of Personality and Social Psychology found pet owners age 65-plus make 30 percent fewer visits to their doctors than those without pets. Pet owners averaged 8.42 visits to the doctor during the year, versus 9.49 visits for people without pets. Dog owners were the lowest with an average of 8.20 visits. Pets have even been found to decrease the symptoms of aggression and “sundowning,” or behavioral changes later in the day, in Alzheimer’s patients, according to a study published in the Journal of Psychosocial Nursing. Graham said, “At home, our pets are a source of affection and positive, nonverbal communication, but they also increase opportunities outside the home for exercise, outdoor activities and socialization.” For pet owners looking to connect with others, Graham suggested online communities. CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. HOSPICE CARE - DEFINITIONS

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community,

an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have

shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

n 1913, Dr. Lloyd Noland came to Jefferson County, Alabama at the invitation of the Tennessee Coal, Iron and Land Company (TCI) to address the public health needs of workers who suffered from smallpox, typhoid, and other epidemic diseases. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants. INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...) INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive

outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner. LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care.

Senior Housing Definitions continued from page 125 Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients. RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from live-in caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances. RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a per-

son for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/location of apartment. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision. SHORT-TERM CARE – Acute care provided on a short term basis. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders. SKILLED NURSING FACILITY – Facilities provide 24/7 care for Continued on next page

Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. ~
