You will find hundreds of different services and organizations that work with seniors and their families. This includes everything from networking to professional services, such as Adult Day, Care Management, Elder Law, Social Security, Senior Centers, etc… 2. Health at Home – Home Health & Personal Care Agencies This section offers four different comparison spreadsheet/grids; one for Homemaker Companion (nonmedical care and personal assistance at home) and two for (Medical Care) Private Pay Home Health Agencies and Medicare Home Health Agencies. First familiarize yourself with the different services that are offered by each agency. You will find definitions about each service in this section. IN FLORIDA ELDER SELF-NEGLECT ACCOUNTS for the majority of cases reported to Adult Protective Services. From a legal standpoint, the issues involved in these scenarios are extensive and complicated. All citizens are mandatory reporters of any suspected elder abuse – self-inflicted or otherwise. Mandatory reporting means that anyone who knows, or has reasonable cause to suspect that a vulnerable adult has been or is being abused, neglected, or exploited shall immediately report such knowledge or suspicion to the Abuse Hotline toll-free at 800-96ABUSE (800-962-2873). All of these issues can lead to conflict that can last for years or result in litigation that costs the family dearly. Critical decisions may be put on hold during the time of conflict. National Cremation and Burial Society Can Answer All Your Questions NATIONAL CREMATION AND BURIAL SOCIETY, 2990 Bee Ridge Road in Sarasota and National Cremation Society, 2525B Tamiami Trail in Port Charlotte have been serving friends, neighbors and families in the surrounding communities for over 40 years. Offering both arrangements at the time of death and arrangements in advance of death, the staffs at both locations are highly trained and compassionate professionals available 24 hours a day. Cremation without viewings or services, known as direct cremation, is the specialty at the office in Port Charlotte, which offers direct cremation packages at affordable prices and a variety of memorialization products, such as urns and jewelry. Full service cremations, which include a visitation and a rental casket, and cremation with memorial service, which may include the urn, is available at the funeral home in Sarasota, which also offers full burial packages, a chapel for services, a variety of urns, caskets, and memorial products and menu choices for your after- service gathering. For the men and women who have served our great country, a veteran’s package is available in Sarasota, with value-added extras like a flag case and veteran memorial book and folders and includes a service at Sarasota National Cemetery. Our Compassion Helpline is offered at both locations to all of our

families. Both locations also offer a nationwide chain of sister funeral homes should death occur while traveling or on vacation. The NICHE Program at Venice Regional Bayfront Health 80 PERCENT OF PATIENTS AT OUR HOSPITAL THIS YEAR were 65 or older? That’s pretty remarkable, considering that the average age of hospital patients in the state of Florida is 54. The acronym NICHE stands for Nurses Improving Care for Healthsystem Elders. The goal of NICHE is to ensure that hospitals throughout the country operate in a seniorfriendly manner. They set standards for nurse training and the hospital’s environment and procedures to assure that the needs of geriatric patients are being served. Venice Regional meets all the standards. For most hospitals that have earned a NICHE designation, it is almost always for a single department within their hospital. In our case, the entire hospital is NICHE-designated.” To obtain a NICHE designation. Every policy and procedure has to be examined. For example, we can’t have our floors too shiny. An elderly patient sees a shiny floor and thinks it’s slippery. That can cause a fall because the floor isn’t slippery, and light reflected off of a shiny floor can make it difficult for some elderly patients to see where they’re going. Beds need to be equip-

ped with weight-change alarms, to alert the nursing staff if a weak or disoriented patient who shouldn’t be out of bed suddenly decides to try. Video monitors may be needed to keep track of patient movements, to avoid their getting into trouble or rescue them if they do. But being senior-friendly goes way beyond a hospital’s facilities. Venice Regional Bayfront Health has 20 nurses who have been specially trained to be Geriatric Resource Nurses, helping other staff deal with the proper care of elders. Elders may have age-related changes in their skin, nutritional needs, sleep disorders, depression and the biggest of them all fall prevention. “In addition to bed alarms and video monitors, many other steps must be taken to prevent falls.” Nurses must make frequent rounds with patients if they have had surgery, have an IV, are using a cane or walker, are not alert or have a history of falling. They are assessed twice a day. It’s not easy being a senior, and it’s not a piece of cake to take care of a senior either. So it’s nice to know that if you have to go to a hospital, it has a NICHE for you.

Medicare 2014 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a "Medigap Plan." Medicare Part A HOSPITAL INSURANCE

For most people the monthly cost is: $104.90.

Premium varies by company, State, & other factors.

Program has a $1,216 deductible per benefit period.

These plans generally pay the A & B deductibles and other eligible costs that are not paid by Medicare.

It is possible to pay $1,216 several times a year. Plan covers: inpatient hospital care, home health care services, hospice inpatient skilled nursing & more. Providers are paid by Medicare assignment. No out-of-pocket maximum. Limited coverage outside the USA. Many people choose to add a Medigap or Advantage Plan.

80/20 after deductible. Plan covers: health care providers, ambulance, emergency department, outpatient surgical services, & more. Strong preventive care benefits. PRIVATE PAY HOME HEALTH DEFINITIONS ACCREDITATION: The process of review that healthcare organizations participate in to demonstrate the ability to meet predetermined criteria and standards of accreditation established by a professional accrediting agency. Accreditation represents agencies as credible and reputable organizations dedicated to ongoing and continuous compliance with the highest standard of quality. ACHC collaborates with industry experts to create standards to ensure that quality is maintained throughout all aspects of the organization. COMPANION/HOMEMAKER SERVICES Refers to services provided by a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine household activities such as cooking, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to physician’s offices, grocery and clothing shopping as well as recreational activities. Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing, dressing, toileting, ambulating nor dispense medications for their clients. PRIVATE PAY HOME HEALTH AGENCY: An agency that provides services to patients in private homes, assisted living facilities, and adult family care homes. Some of the services include nursing care; physical, occupational, respiratory, and speech therapy; home health aides; homemaker and companions; and medical equipment and supplies. Along with services in the home, an agency can also provide staffing services in nursing homes and hospitals. Home health agencies are licensed and surveyed by the State of Florida. Their services are paid for by a variety of health insurance and Long Term Care Insurance Policies or on a fee for service basis. NURSE REGISTRY: An company that acts as employment agency between an individual patient and nurses, nursing assistants, home health aides, companions and homemakers for services in the patient’s home. Nurse registries also provide temporary staff for nursing homes, hospitals, and other businesses. Each individual health care worker is an independent contractor and is not a direct employee of the nurse registry. Nurse registries are licensed and surveyed by the State of Florida. Their services are paid for by a variety of health insurance and Long Term Care Insurance Policies or on a fee for service basis. MEDICARE HOME HEALTH AGENCY: If you have Medicare, you can use your home health benefits if you meet all the following conditions: You must be under the care of a doctor, and you must be getting services under a plan of care established and reviewed regularly by a doctor.. You must need, and a doctor must certify that you need, one or more of the following: Intermittent skilled nursing care, Physical therapy, Speechlanguage pathology services, Continued occupational therapy. The home health agency caring for you must be approved by Medicare (Medicare-certified). You must be homebound, and a doctor must certify that you’re homebound. A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services. HOME HEALTH AIDES/ASSISTANCE ADL’S: Refers to persons specifically trained to help individuals with their personal care needs and activities of daily living (ADL’s). Activities of Daily Living are activities that reflect the client’s ability to perform tasks that are essential for self-care such as bathing, feeding oneself, dressing, toileting, transferring from a bed to a chair, etc. Other tasks may be assigned to aides per individual agency and family negotiations/contract. Home Health Aides are available for just a few hours per week up to 24-hour care on a temporary or long term basis. SOCIAL WORKER: A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventative, developmental, or remedial in nature. MED-ALERT SYSTEMS: Personal medical alert products activated by pushing a button in case of emergency. There are several different manufacturers. MEDICATION ASSISTANCE: This means that an agency employee may assist the client with medication reminders and assistance. Only a licensed RN or LPN can administer medications in the home or Assisted Living setting depending on their licensing. SKILLED NURSING: Refers to services performed only by a licensed Registered Nurse or Licensed Practical Nurse that is registered with the State Board of Nursing in their respective State. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician. PHYSICAL THERAPY (PT): Refers to assessing and treating large motor function, utilizing rehabilitative techniques of exercise, gait training, prosthetics, and heat to restore the client to his/ her highest functional level of strength, range of motion and mobility. OCCUPATIONAL THERAPY (OT): Assesses and treats small motor function. Utilizes rehabilitative techniques of exercise, splinting and assistive devices to increase the client’s ability to carry out normal day to day activity such as feeding, dressing, grooming, and performing household tasks. Guides the client through specialized regimes and encourages sensory and muscle return. Exercises and activities are used to increase upper body extremity function. SPEECH THERAPY (ST): Assesses and treats speech and swallowing disorders. Provides individually designed treatment programs to maximize communicative effectiveness for the client, such as stroke, head injury, laryngectomy, and voice disorders, as well as cognitive deficits. TRANSPORTATION: Refers to assistance both locally and nationwide. It is very individualized by each Agency. Contact individual Agencies for details. BONDED: Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. INSURED: This refers to what happens if someone gets hurt on the job. If the company Isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. WORKMEN’S COMP: Insurance that covers medical costs, rehabilitation costs and lost wages for employees injured at work.

Senior Housing - Definitions

SENIOR HOUSING DEFINITIONS ACTIVITIES OF DAILY LIVING (ADLs): Define the personal care services available in assisted living and include bathing, dressing, grooming, bathroom assistance, eating and ambulation. Medication Administration and transferring are not ADLs but are factors in determining a person’s level of functioning.

ADULT DAY STAY PROGRAMS: There are two general types of adult day care programs. One is based on a medical model and the other on a social model. The medical model provides comprehensive medical, therapeutic, and rehabilitation day treatment. The social model offers supervised activities, peer support, companionship, and recreation. Both models assist older adults and those with chronic conditions to remain as independent as possible, for as long as possible.

ANYTIME DINING: Many CCRC’s and Independent Living communities offer anytime dining which gives you the freedom to enjoy your meals at times that are convenient for you. Entitlement, in this situation means that if one meets the eligibility requirements, then they are entitled to receive benefits. With waivers, one might meet the eligibility requirements however the waivers may only be available to a certain number of individuals and therefore the person may be put on a waiting list to receive benefits. They are not immediately entitled to those benefits.

SHORT TERM/RESPITE CARE: Circumstances require a caregiver to seek temporary senior housing for your loved one. Knowing they can move seamlessly into an emergency care environment that supports the wellness needs of seniors allows peace of mind when you are called away for business or vacation, or if you just need a short break from caregiving SKILLED NNURSING FACILITY (SNF) This level of care is typically found in a nursing center or a transitional care unit in a hospital. Daily rates typically include a furnished bedroom & a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans & private sources.

WILLIAM CHUPKA AND WIFE DEBORAH OF 32 YEARS HAVE much for which they are thankful. William has enjoyed a successful business career for 41 years, and now is able to enjoy his retirement in beautiful Placida Florida, where they can enjoy going to the beach, collecting shells, creating shell art, cooking together, and dining at all the wonderful restaurants this area has to offer. “Especially the Pink which is owned by the Gasparilla Inn and on Boca Grande, William Chimed in.” They have a beautiful daughter they adopted from the Philippines, and truly enjoy doing everything together as a couple. When Bill’s sister passed she resided in Placida: accordingly they decided to come down and make her house their new Florida home. Recently, Deborah experienced a health scare that required immediate attention. They were directed to the Englewood Community Hospital Emergency Room. “From the moment we walked in the receptionist was attentive, asked if there were any chest pains, and said she would be right with us, and she was. Deborah unfortunately has had a myriad of healthcare issues over the years and unfortunately they have experienced many Emergency Rooms in their time. None of which can come close to the wonderful experience they shared

at Englewood Community Hospital. In the meantime the attending physician Dr. Karen Roe wasted no time and introduced herself, and said she would be over-seeing all of the procedures. Taken immediately in the back the Nurse Physician Assistant came in, assessed Deborah’s condition, and ruled out the condition we feared most, a stroke. Of course, blood was taken, a brain scan, and a lung scan as well. I asked how long it would take for the results to come back and she said about an hour. I don’t think it took a moment longer. This is generally not been our experience in many other ER’s we have been to in other areas, Bill explained. Fortunately, everything came back normal to our great relief. From the wonderful CNA’s that drew blood, did the EKG, took her down for the scans, to the head doc in charge the entire staff was absolutely wonderful!” “The whole visit was 2 hours from start to finish, and the test results were returned in one hour. After experiencing a health scare like so many others, The Chupka’s are forever grateful for the service they received at Englewood Community Hospital. It’s a small hospital with a huge heart! The Chupka’s say they can relax and enjoy their Florida Lifestyle knowing that top notch medical care is just up the road from their home.

Accelerating Solutions for Positive Aging THE INSTITUTE FOR THE AGES, a 501(c)3, was formed in 2011 and is based in Sarasota County, the oldest large county in the US. As you probably already know, Sarasota County is home to the highest concentration (32.5%) of people over the age of 65, more than double the percentage of older adults in the United States. This difference is even more profound in the number of individuals 85 years of age and older – 5% in Sarasota County compared with approximately 2% nationwide. We are an education and research center focused on accelerating solutions to enhance the quality of life for older adults. Our purpose is two-fold – to improve lives by influencing innovations for positive aging and to drive economic growth by facilitating research and development. Our mission is to transform aging through the authentic voice of the older adult. We have a fast-growing 50+ Community Research Network – residents who have volunteered to participate in research to support our mission and have their voices heard. Your experience and wisdom helps researchers, product and service developers, and policy makers obtain the necessary insights and data to accelerate solutions in such areas as: health and wellness, communication and information, respect and social inclusion, transportation and mobili-

ty, aging in (the right) place and meaningful involvement. Your insights and the work being done by the Institute for the Ages has the potential to change the conversation about aging and influence positive future outcomes for people of all

A Snapshot of the Florida Medicaid Managed Medical Assistance Program STATEWIDE MEDICAID MANAGED CARE (SMMC) and Managed Medical Assistance Program (MMA). The Florida Medicaid program is implementing a new system through which Medicaid enrollees will receive services. This program is called the Statewide Medicaid Managed Care Managed Medical Assistance program. The Managed Medical Assistance program is comprised of several types of managed care plans: Health Maintenance Organizations Provider Service Networks Children’s Medical Services Network Most Medicaid recipients must enroll in the MMA program. Who is NOT required to participate? The following individuals are NOT required to enroll, although they may enroll if they choose to: - Medicaid recipients who have other creditable health care coverage, excluding Medicare - Persons eligible for refugee assistance - Medicaid recipients who are residents of a developmental disability center - Medicaid recipients enrolled in the developmental disabilities home and community based services waiver or Medicaid recipients waiting for waiver services What region am I in? Sarasota and Charlotte Counties are Region 8 and Manatee County is

Region 6. What do I have to do to choose a Managed Medical Assistance plan? Choice counselors are available to assist recipients in selecting a plan that best meets their needs. This assistance will be provided by phone. In-person visits are also available by request for recipients with special needs. Recipients can also enroll online at: flmedicaidmanagedcare.com. What mma standard non-specialty plans are available in Region 8? - Intregal, Prestige, Sunshine and Staywell What are MMA Specialty plans? Specialty plans will serve populations with a distinct diagnosis or chronic condition. These plans are tailored to meet the specific needs of the specialty population. Can I change my MMA plan once I make a selection? Recipients are encouraged to work with a choice counselor to choose the managed care plan that best meets their needs. What Medicaid covered services are provided under the Managed Medical Assistance program? Advanced registered nurse practitioner services, Laboratory and imaging services, Ambulatory surgical treatment center services, Medical supply, equipment, prostheses and

What benefits not otherwise covered by Medicaid are available from MMA plans? Dental, Vision, Art Therapy, Equine Therapy, Influenza vaccine, Medically related food and lodging, Nutritional Consulting, Outpatient hospital services, over the counter medication and supplies, pet therapy, physician home visits, post discharge meals, Primary care visits, Shingles vaccine, Waived co-payments Additional Qualified Providers Under the MMA Program Include: Anesthesiologists Cardiologists, Cardiovascular Surgeons Chiropractors, Dermatologists Endocrinologists, Gastroenterologists, General Dentists, General Surgeons, Infectious Diseases, Nephrologists, Neurologists,

Neurosurgeons, Obstetrics/Gynecologists, Oncologists, Ophthalmologists, Optometrists, Oral Surgeons Orthodontists, Orthopedic Surgeons, Otolaryngologists, Pathologists, Podiatrists, Pedodontists, Pulmonologists, Rheumatologists, Therapists, Urologists, Pharmacies, Board Certified or Board Eligible Psychiatrists, Licensed Practitioners of the Healing Arts, Inpatient Substance Abuse Detox-ification Units, Fully Accredited Psychiatric Community Hospitals or Crisis Stabilization Units. Will my services be interrupted? To ensure the movement to the new program is a smooth one, the Agency has in place several core program requirements that will allow for seamless continuity of care during the transition period: Health care providers should not cancel appointments with current patients. Health plans must honor any ongoing treatment that was authorized prior to the recipient’s enrollment into the plan for up to 60 days after MMA starts in each region. Providers will be paid. Providers should continue providing any services that were previously authorized, regardless of whether the provider is participating in the plan’s network. Plans must pay for previously authorized services for up to 60 days after MMA starts in each region, and must pay providers at the rate previously received for up to 30 days.
