Helen Resides at Hillcrest Rehabilitation Center, which is considered a “SNF” What exactly is a SNF? As recent as ten years ago, when people referenced SNFs, they were generally talking about a traditional long term care nursing home. In recent years, the SNF model has evolved to include specialized short term rehabilitation centers, geared towards helping people return home after a serious illness, injury or operation. These centers are staffed with a team of licensed therapists, compassionate nurses and doctors. Admission eligibility depends on a few factors. First, are you in need of long term care or short term rehabilitation services? If you are in need of short term rehabilitation and you have Medicare or private insurance, both will help pay for these services. Prior to entering a SNF, Medicare beneficiaries must have been hospitalized for a minimum of three consecutive nights. Medicare covers the first 20 SNF days paid in full. Days 21-100, the Medicare beneficiary either pays a daily co-insurance rate or may have opted for a secondary insurance that picks up these co-payments. Medicaid traditionally pays for long term care services. What should you do if you’re discharged home but don’t feel strong

enough to be home? Medicare allows a 30-day window for people who return home to be admitted into a SNF, as long as the patient was in the hospital for at least three consecutive midnights. There is no need to readmit to the hospital, as you can be directly admitted to a SNF. There are multiple skilled nursing facilities in your area, how should you choose where to go? Visit Seniorsbluebook. com for a complete list of providers as well as www.Medicare.gov/nursinghomecompare offers a reliable rating system based on staffing, quality measures and health inspection outcomes. Understanding the 5-star rating system is crucial to ensuring that you receive exceptional clinical care from a reputable facility. New SNFs will not obtain their 5-star rating until after one year of operation. As with rated facilities, it is incredibly beneficial to schedule a tour for yourself or a loved one to gain understanding about your SNF of choice. While on a tour of the SNF be sure to observe the staff friendliness, building cleanliness, amenities and overall feel of the facility. It’s our privilege to guide you through this process and be your personal resource on the path to optimal wellness. ~

Check into the next step soon. Get it set up before you absolutely have to. Because, if you plan early, you get to make the choices. Do you want someone else choosing for you?

What is Care Management? Geriatric Care Managers are professional resource specialists and advocates for older adults and their families. They can be social workers, nurses, gerontologists, psychologists or other health and human services specialists. They are dedicated to assisting elders and their loved ones navigate the health care system. They assist with current day to day care issues, crisis management, and future care planning. They are skilled at securing appropriate living arrangements and coordinating services for complicated medical/ psychiatric needs. Geriatric Care Managers are professional problem solvers and can be your guide to the complicated and sometimes confusing array of services that are available in our community.

n

n

n

Benefits for older adults or people with chronic illness and their families: n

n

C are Managers draw on their training and experience in the senior care industry to provide professional care planning and high quality, cost effective services. C are Managers conduct a comprehensive needs assessment addressing medical, social, emotional, legal, financial, and housing needs. Options for care are based on the individual situation and client/family needs.

n

Care Managers assist in navigating a variety of services as the client’s condition changes. They provide continuity and coordination with doctors, home health care agencies and other providers. This includes monitoring care and advocating on the senior’s behalf as often as necessary to ensure quality care. Care Managers are a fee-for-service entity. They do not receive payment from Medicare or Medicaid and they do not take referral fees from third party providers, insurance companies or equipment manufacturers. However, some Long Term Care Insurance policies now recognize the importance of these services and cover the cost. Care Managers can be the ‘eyes and ears’ for out-of-town family members; not only to assist with in-home services, but, also to objectively assess care in a facility and function as a liaison with far away family. Care Managers facilitate family communication and aid in the decision-making process.

When to call a Geriatric Care Manager: n When you are overwhelmed with options or don’t know where to start researching options. n When you are out of town and unable to attend to the needs of your loved one.

Estate Planning: Not Just A well-executed estate plan is for Boomers! critical for everyone. A will or trust, and powers of attorney are the best way to ensure your wishes are honored after your death, and during any period of incapacity.

In past columns we have discussed the importance of estate planning for seniors. Here, we’ll look at these issues for the younger generation. Planning for these events seems natural as we age and families grow. It is especially important for younger adults and for unmarried couples to develop or update a plan that accurately reflects their wishes. People without a plan in place face the very-real possibilities of passing their assets to someone for whom they never intended to provide, leaving someone out altogether, or having someone they do not trust (or even know!) make decisions on their behalf. Once we turn 18, we have the right to make our own financial and medical decisions. While we might still rely on our parents’ advice and direction, the law does not give parents the power to make decisions for their adult child, even if the child is incapacitated. In a situation such as this, a parent would likely have to

12 GREATER TWIN CITIES AREA - PROTOTYPE

go to court to be appointed as their child’s guardian. Executing powers of attorney documents can prevent this. Powers of attorney designate someone (an “agent”) to act on your behalf should you become unable to do so. Anyone 18 years of age or older can, and should execute medical and financial powers of attorney, to avoid unnecessary delay, expense, and to ensure the decisions made on their behalf come from a trusted person. Unmarried couples face similar risks. Without a document designating an agent, an unmarried partner is not treated the same as a spouse. In fact, the partner may be left out of any medical decision-making, entirely. Further, if an unmarried couple does not designate each other as beneficiaries of their assets, the property passes according to law at their death, rather than to each other. So, a loved one could be left out of their partner’s estate altogether. Therefore, planning for these events is crucial.~ Editor’s Note: This article was submitted by Richard E. Romeo with Vincent, Romeo & Rodriguez, an established law firm.

your life beneficiary designations? Are your beneficiaries up to date? Your phone, your wardrobe, your contact information. These three things have something in common! Can you guess what it is? If you haven’t reviewed your beneficiary designations recently, ask yourself these questions:

H as your marital status changed? H ave you had a child? n H ave you changed jobs recently? n n

If you answered “yes” to any of these, you probably should do a quick beneficiary designation review. Why is it important? G rief is already hard enough – The people you care about will already be dealing with a lot if something should happen to you. By keeping everything up to date, you can help make the whole process a little easier for them. n S econdary = just as important – If something happens to your n

primary beneficiary, your secondary beneficiary is next. Make sure to keep your contingent beneficiaries up to date too. n What will? – Certain things, like life insurance policies, bank, and retirement accounts take precedence over your will. If the identified beneficiary of those policies and accounts don’t match your will, then your assets could go to someone you don’t want them to.

Medicare 2018 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Medicare Part B Medical Insurance

Supplemental Policy Plans A - N Medigap

Generally no cost (If you paid Medicare taxes while working)

Most people pay monthly $134.00 (Premium could be higher based on income)

Premium varies by company, age, location & other factors.

$1,316 deductible (per benefit period) It is possible to pay the $1,316 deductible and other daily charges for multiple stays & extended care. Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more. Providers are paid by Medicare assignment. No coverage outside USA. No out-of-pocket maximum. Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

$183 annual deductible. 80/20 after deductible. Plan covers: health care providers, ambulance, emergency care, outpatient surgical, preventive services and other Medicare approved benefits. Most doctors accept Medicare patients. No coverage outside the USA. No out-of-pocket maximum. Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

These plans generally pay the Part A & B deductibles, the 20% not covered by Part B & other Medicare eligible benefits. When this coordinates with Medicare, many benefits are 100% covered Plans coordinate with any doctor or hospital that accepts Medicare Can provide benefits outside USA Plans are only available through insurance companies. Need to add a Part D plan to cover prescription drugs & avoid penalties. Must have Part A & B to enroll.

No matter how one comes to the decision to move, the factors and issues related to each older adult’s transition is unique. But at some point, inevitably, moving involves a feeling of being overwhelmed. There are a number of aspects that make an older adult’s transition difficult which include; sifting through a home of many years, deciding what to do with possessions that are not going to the new

residence (e.g. estate sale, consignment, charity, kids, online selling options, etc.), some health challenges that did not exist in the early years making the physical aspect daunting, plus the emotional aspect of leaving “the old neighborhood”, lifelong memories and the comfort of familiar turf. Enter a fairly new but growing industry, senior move management. Due to the rising numbers of older adults and the increase of senior housing options, senior move management is an emergent field.

The Making of a Move All moves share some common elements including why move, when to move, where to move and how to move. The answers depend on individual circumstances and one’s age. We’ll consider moves from the viewpoint of a senior. Why and when often can be addressed together. The motivation for a senior to move can be initiated by needs related to physical health, social activity, memory support. Some people anticipate their needs and tour senior communities and get on waiting lists. Others wait to make a decision until they are forced to, which is usually the result of a health crisis and they are told by a doctor that they can no longer live in their home. Where to move includes decisions

that must be made about the type of living situation one desires. Options include everything from a condo, to independent senior living communities, assisted living, and memory care. Those who plan in advance will have more options regarding location, amenities, etc. Those who move in crisis mode generally have fewer options as they must settle for what is available at the moment. How to move also has options: you can pack and move yourself, hire a moving company to move your things, or hire a move management company to plan your move, pack and move your things, and unpack everything for you. Whether or not you have planned your move or are moving in crisis mode, hiring a move management expert will greatly reduce the stress of the situation and get you settled quickly.

What Is A Reverse Mortgage – Ask a Certified Reverse Mortgage Also Known As Professional not your neighbor, A HECM? doctor, car mechanic or bartender. The purpose of this article is not to send glowing endorsements for the HECM program, good, bad or indifferent. Tom Selleck and Henry Winker have done that. It is not to tell you how they would work or not work for you. It’s not to give you technical understanding. It is simply written for you to ask yourself do I really know what a HECM is, how they actually work and would it benefit me. Since 1989 HUD/FHA has endorsed over 1,000,000 HECM’s. There are wide spread misconceptions, untruths, surrounding HECMs. They are bad, they are scams, they are only for poor people, the bank will own the house. HECM’s are home equity loans with options not available on traditional home equity loans or HELOC’s as they are called. General statements from critics have deafening effects to the HECM program. When misinformed people

make general assumptions or statements about the HECM program, at times, that has a stronger influence on people than the professionals who could actually show you how the program really works. Maybe I should ask my physician which car would be best suited for me or better yet ask my auto mechanic their opinion as to why my shoulder hurts? The point is, ask professionals in their field of expertise to explain HECM’s and HECM options, if this is a path that you are wishing to explore. They have been around for almost 30 years helping seniors in their retirement. Ask an expert. That way you are making informed decisions concerning your retirement dollars. ~ Editor’s Note: This article was written by John Leer, Reverse Mortgage Banking Officer, Certified Reverse Mortgage Professional

COMMUNITY RESOURCES AND SERVICES

A New Lease on Life When Mike Cullen began having trouble playing just three or four holes of golf in 2013, he knew something was wrong. For 34 years, he enjoyed putting in long hours as a beloved teacher, principal and superintendent at local South Dakota schools while he and his wife, Betty, raised their two boys. His love of working with students meant he took on many additional roles over the years too: athletic director, football game announcer, substitute bus driver and even temporary cheerleading coach. After a series of doctor’s visits and tests, Mike learned that kidney disease was to blame for his poor golfing and the host of symptoms he was experiencing. He began dialysis sessions three times a week and a course of prescribed medications, but knew a transplant was the solution he really needed to feel better. Mike decided to retire in July 2015 and hoped that his name would soon come up on the long kidney donor list. Then something extraordinary arrived in the mail — a box from a former student filled with kidney beans and a message that she wanted to donate her kidney to thank him for giving her “a new lease on life” years before. This former student, nurse Elizabeth Blankenfeld, learned about Mike’s need for a transplant via a local TV story and decided to reach out.

The following month, Mike gratefully received the kidney transplant and gradually started to feel more like his old self. He and his wife moved to Minnesota to be closer to their grandson, and Mike began morning swims to build his strength. But he wanted a meaningful way to fill his free time now that his career had ended. “I wanted to do something to make a difference, and my son mentioned that the new LSS Neighbor to Neighbor service needed volunteers to support older adults,” he said. The service, launched in September 2016, offers older adults a little extra support at home and in the community. Mike dove into the opportunity. He completed training and began supporting Bill Wherland, a Minneapolis resident. “I was surprised how open he was,” Mike said. “You get close and become friends almost immediately.” Bill signed up for the Neighbor to Neighbor service following a stroke, and Mike visits him three to four hours each week. Mike drives him to speech therapy appointments aimed at improving his pronunciation. They catch up over lunch, collect groceries and walk outdoors to improve Bill’s balance. The visits also give Bill’s wife a much-needed break from caregiving. It is a time she can run her own errands, visit with friends and family, or just catch-up on housework.

SENIOR HOUSING DEFINITIONS

Senior Housing Definitions

(Sec 236) Property owners in this program make mortgage payments that are based on a 1% mortgage interest rate. HUD then provides a subsidy to their lender to cover the difference between 1% and the market interest rate on the property’s loan. Participating households are required to pay rent equal to the greater of 30% of their adjusted income (not to exceed the market rent) or the basic rent amount set by HUD for that particular property. Any amount paid by the household that is more than basic rent is considered excess rent, which the owner pays back to HUD in repayment of the subsidy.

LiveLife Plan Ahead

lessons

At some point, we all need help. Pete, 67, has this advice for people who, like him, are experiencing health issues:

Check into the next step soon. Get it set up before you absolutely have to. Because, if you plan early, you get to make the choices. Do you want someone else choosing for you?

Location was key in selecting Crest View Senior Community at Blaine. During Pete’s six-week hospital stay last summer, he asked his son to make arrangements for assisted living at Crest View, the nonprofit, faith-based campus that had opened a few months earlier. Pete had watched Crest View rise from the ground just down the street from his apartment. Another plus was his son lived a few miles away, along with Pete’s adorable granddaughters, ages 3 and 8. See the rest of Pete’s story at CrestViewCares.org.

Talking about the granddaughters makes his eyes sparkle. They’re dancers. “Point a camera at them and they go into a dance pose,” Pete said. He attends their dance recitals and enjoys frequent visits. Pete didn’t want to look anywhere else. Crest View Senior Community at Blaine was brand new, yet owned and managed by a nonprofit with 65 years of experience, and in his neighborhood close to family, How did he decide to move into assisted living? He saw the writing on the wall. How many times can you call 911 after a fall in your apartment before someone else makes decisions for you? Pete wondered, and worried.

Meet a few of the guys at The Boulevard, Crest View Senior Community’s independent senior housing on Reservoir Boulevard in Columbia Heights. Art, moved in 14 years ago, after retiring from the University of MN. He remembers seeing Lloyd, who lived nearby, walking his dog. Lloyd, who worked as a technician in the garage that launched Medtronic, met Shirley Barnes, CEO, when she came to his rummage sale about 22 years ago. Lou, a member of a restaurant family known for fabulous Northeast Minneapolis food, used to visit his mother on the campus.

Bill, Art’s brother, regales the guys with stories of his days bartending in a downtown Minneapolis hotel. It’s fun to look back, but their days are full, too. A weekly happy hour, card games, bingo, sing-alongs, rides to the grocery store or a scheduled outing—these are the kinds of simple but essential choices that keep them connected to a community.

“ says Lou.

Having something to do is key,

Living on a campus multiples your choices of things to do with your neighbors. Different buildings on the campus, all connected, blend their activity calendars.
