InnovAge Helps Tanya On her morning ride on an InnovAge Stay Independent bus to her day center, 63-year-old Tanya looks forward to the day’s activities, listening to good music, and laughing with her friends.

Tanya goes to her local InnovAge center in Northern Colorado, as a participant in the Program of All-inclusive Care for the Elderly (PACE), an innovative Medicare program for older adults. By providing coordinated healthcare, in-home services and day programs, PACE helps individuals remain living in their own homes and communities. For Tanya, Northern Colorado is home and that is why PACE is a great fit for her – it allows her to get the healthcare and support she deserves, in a community that she knows and loves. When Tanya visits the center, she looks forward to the day’s activities, laughing with her friends, and listening to good music. Tanya is a huge fan of country singer George Strait (and all live music), so her favorite days are when community musicians 8

come to the center and play for participants. When asked what she most values about PACE, Tanya says that InnovAge transportation to and from the center has been a huge time saver over the city bus. Plus, she adores her doctor and appreciates having easy access to medical care. The PACE program operates on an interdisciplinary team model concept. That means care team members such as a dietitian, bus driver, physician, physical therapist and social worker all work together to coordinate each participant’s care. Tanya’s care team helps her to stay healthy so that she is able to enjoy the center three days a week. These days, she has enough zip to serve as vice president of the center’s participant council, root for the Colorado Rockies baseball team and spoil her mini Dachshund, Jake. See ad on page 41.

Our Purpose: We are a free advocacy service mandated by the Older Americans Act to help all nursing home and assisted living residents maintain their quality of life by ensuring that their rights are protected. Who We Help: Residents, families, and friends of longterm-care residents are encouraged to contact the Ombudsman. We work with facility staff, hospice, adult protection, and others to get the resident what they need. What We Do: Within facilities, we help with things as simple as hot coffee or negotiating shower times. Feeling uncomfortable for any reason, your Ombudsman will listen and keep what you say confidential until you give us permission to act. We also investigate serious violations of resident’s rights. Substandard care, verbal or physical abuse, privacy, and the right to selfdetermination are examples. If you are considering long-termcare for a loved one, we can provide information and suggestions that will make choosing a long-term-care

Don’t Believe You Need Estate planning can mean An Estate Plan? many things to many people. For some, it may simply mean the creation of wills or trusts. For others, it may involve the creation of a special needs trust to provide a safety net for a child with developmental disabilities. And many people are concerned over the prospect of paying for long-term care and the fear they could lose their home and their life savings. An estate plan can address all of these issues and more. Having a will should be a necessity. If you do not have one, then your assets may pass under the laws of intestacy, which may not be how you wanted the assets distributed. By creating a will you can identify who receives your assets; when and how much they receive; and who will serve as your executor or trustee. It can ensure the orderly transfer of your estate as you want it, rather than as mandated by the State, and can help to minimize potential disputes among your family members. Medical durable powers of attorney and advance healthcare directives are valuable parts of any estate plan. Not only do they provide peace

18 NORTHERN COLORADO - OCTOBER 2016-2017

of mind for you, but they can also reduce the stress that your spouse or children would encounter not knowing your intentions concerning medical care and treatment. An estate plan should also include a financial durable power of attorney in which you have identified who can make decisions for you should you become incapacitated. Absent a financial power of attorney, it may be necessary to obtain a court-ordered conservatorship, which can be quite costly. While predicting the future is inherently risky, what is far more risky is to assume that things will be okay and will work out. Veterans Program Anyone? Questions to Ask Any Non-Medical Home Care Agency Does your agency utilize an existing VA benefit called ‘Aid & Attendance’? Some agencies offer help to Veterans but it’s mostly just gathering paperwork necessary for the A&A... and that’s it! Care typically doesn’t start until the VA pays the agency, which can take months with those agencies unfamiliar with the process. If you call an agency and they don’t know anything about the A&A benefit OR they do offer it and/or it takes longer than 2-4 weeks to START care… move on! Instead, try and find an agency that understands the complexity of the A&A, can start care within 2-4 weeks and has no hidden costs to get started. Do you have experienced VA Accredited Agents within your office to help with this process? Some agencies will outsource this critical step to local Attorneys or Financial Planners that may have accreditation, but the A&A benefit is just one of many products they offer. Find an agency that has VA Accredited Agents on their team that are dedicated and focused only on capturing the A&A benefit and offer no other products or services.

Are there any upfront costs to start the process? Again, some agencies work through an Attorney or a Financial Planner that technically can help you with this process; however, they have creative ways of collecting payment for ‘other services’ so they can get paid up to an estimated $500 or more. Instead, try and find a Veteran-dedicated agency that offers this at NO COST OR RISK for those who apply. There is no reason for you to pay out-of-pocket for these services. How long will it take before I can start care? Most agencies don’t start care immediately simply because they don’t have the funds available to absorb the cost of labor for weeks or even months waiting on the VA to begin payment. So, they will process the paperwork and make you wait until the VA releases the funds. Try and find an agency that starts care within 2-4 weeks with no costs upfront and that offers a complete Veterans Program. The fact that consumers can now purchase these items without a medical recommendation has helped boost the number of seniors consuming cannabis because they no longer have to navigate the medical card process. At any retail cannabis dispensary, a customer only needs a state issued, picture ID that states they are over the age of 21. The changing attitude toward cannabis is also helping to boost the number of senior cannabis consumers. Catastrophic Coverage (Phase IV) For the remaining calendar year, covered drugs are: $2.95 / $7.40 or 5% depending on drug tier.

There is an Annual Election Period (AEP) which runs from October 15 - December 7 of each year. This is time to make changes in company or coverage.

Part D can be separate with medigap & PFFS plans. There is a penalty for late enrollment into Part D.

Preventive care is covered 100%.

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. It can be overwhelming. You are not alone. There are more than 60 million family caregivers in the US today. While balancing your responsibilities is no easy task, it is important to remember you are doing an important and wonderful thing. With increasing life expectancies and advancing medical treatments, the number of family caregivers in the US is also increasing every year. It’s important to support our caregivers as they give so much of themselves to others. If your family member is in need of care, what can you do to prepare for your future as the family caregiver? Educate yourself. Knowing symptoms and the progression of the illness can help you to feel less anxiety about the future. This will help in being prepared with questions at doctor appointments. nF ind support. There are many local and online support groups for family caregivers. Find one that can be an outlet for you. nA sk for help. It’s ok to know your limits and ask for help. Family members, friends and professional caregivers can help to lighten the load. You just have to ask for help. n

of your loved one. In addition to scheduling respite care for your family member, you can take these steps to ensure you stay at the top of your game. Keep a journal as a way to document and give release to your emotions. nD e-stress when needed. Try yoga, meditation, or other activities that help you to relieve stress. nT urn to a support group to talk through your feelings. nK eep a balance of activities in your life. Keep seeing friends, reading books or visiting places you enjoy. Do things that make you happy. nE at right and exercise regularly. This will keep you healthy and energized. Exercise is also a great way to relieve stress. nG et enough sleep so you can stay productive, level headed and keep your energy level up. n

Being the family caregiver isn’t easy. It’s a job that requires constant dedication and it is often thankless. Your loved one needs you and you can help them find peace during this difficult time. Take care of yourself so you can take care of them. Why Fall Prevention and Home Care More seniors are choosing to remain independent in their own homes and age in place. When faced with healthcare challenges, many are opting for Home Care and the flexibility it provides. Supports safe, independent living. Home Care can help reduce the risk for falls. Transferring from a chair, bed or wheelchair can be a particularly dangerous moment. Caregivers can aid with this supervision and support to help promote safety while encouraging independence. Home Care can assist with the client’s personal hygiene such as bathing, grooming, dressing and other personal tasks. While all falls cannot be prevented, in the event a fall occurs, knowing the client’s caregiver is nearby, provides security and offers prompt attention when the client needs it most. Supports physical activity. When a near-fall or fall occurs, the senior adult can develop a fear. They may feel safer just sitting in their favorite lounge chair. Their fear promotes inactivity. Inactivity can lead to loss of muscle tone and balance. Many times it can also lead to depression. Caregivers encourage activity. They can transport clients to therapy, to indoor pool programs, to exercise classes, to Yoga, to their favorite Senior Activity Center, or to their local Senior Sneakers program at the

YMCA. This encourages friendships and social interaction as well as offers a level of confidence by having a caregiver close at hand. Caregivers also encourage home exercise programs or other activities prescribed by their healthcare provider. Supports safety. The CDC confirms each year that more senior adults fall at home than anywhere else. By conducting a Falls Risk Assessment as well as a Home Safety Assessment Checklist, ongoing education, and support, Home Care can help reduce the risk for falls in the home environment. Support medication safety. Caregivers can take the client to the physician, to the pharmacy to pick up prescriptions, and remind the client to take their medications at the time and in the manner prescribed by their healthcare provider. These are just a few reasons why Home Care might be a good option for those that want to Age in Place with Dignity and Grace™. Non Medical Home Care Definitions 24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. TRANSPORTATION – Rides to shopping, appointments and other outings. WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work.

Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance. Choosing The Right Home Let’s start by reviewing the Care Agency differences between agencies. Class A Agency (Medical and Non- Medical) – These agencies can provide Medical and Non-Medical services, as required. They can employ Registered Nurses (RN), Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and Caregivers. They usually accept different insurances, Private Pay, and Veteran’s Benefits. They can provide services that extend beyond what Medicare and Medicaid typically offer. These Agencies can start services in a non-medical capacity and move up to providing skilled medical services as the client’s needs increase. Class B Agency (Non-Medical) – These agencies can only provide non-medical services. They employ Caregivers which cannot act as medical personnel even if the Caregiver has a medical background or licensure. They can only provide nonmedical services such as household chores, companionship, transportation and medication reminders. Medicare/Medicaid Agencies – These agencies can provide short term help after you have been hospitalized or return home from a rehab facility. They do not provide any extended hours of care and only provide skilled services such as RNs, Physical Therapists (PT), Occupational Therapists (OT) and Speech Therapists (ST), depending

on your individual situation. They do not provide any services that a family member or a Caregiver could perform. Most visits are one hour or less. Here are some simple questions to ask yourself and your family. n Do I need short or long-term care? n Will I have to change agencies or consider a facility as my needs increase? n What are my goals for having home-care in place? n Will I be able to stay at home till the end of my life? n Does my family support my decision to stay at home and will they be able to help if needed? n Can your home-care provide services in a facility or work with Hospice? This article should help you navigate between the different types of agencies in your community to help you choose the right home-care solution for your specific needs. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

Navigating the Landscape of The palliative care industry Palliative Care emerged as a way to provide comfort and support for those navigating long-term illness and health care options. Palliative care is comprised of various pain and symptom management services, along with counseling and support for patients and their families.

What is “symptom management?” Symptom management refers to various methods used to alleviate the pain and discomfort surrounding serious illness. Some challenges that palliative care addresses include shortness of breath, anxiety, depression, lack of appetite and nausea.

How does counseling help? Many families are faced with the struggle of caring for a loved one suffering from serious illness but are not equipped to handle the situation alone. Palliative counseling helps families identify their goals and determine the best method of care based on each unique situation. Once the method(s) of care have been chosen, palliative care professionals continue to offer guidance in the form of coping mechanisms, community resources, financial concerns and more. Navigating the Landscape of Palliative Care continued from page 92 Palliative v. Hospice Oftentimes, the term “palliative” is confused with “hospice,” but the two methods of care differ greatly. Whereas hospice care is reserved for patients with a prognosis of six months or fewer, palliative care is available for patients of all ages and during all stages of serious illnesses, including but not limited to cancer, stroke, diabetes, Alzheimer’s, etc. Palliative v. Primary Physician Palliative care does not work against primary medical care but in collaboration with such care. While doctors focus mostly on general health and treating the disease, palliative care professionals have the specialized skills to provide daily comfort and additional guidance.

Where is Palliative Care available? Pathways in Fort Collins is a nonprofit hospice, palliative care and grief management organization that has served Larimer and Weld Counties for nearly 40 years. Our palliative care team consists of a physician, advanced practice nurse or nurse practitioner and medical social worker, all with expertise in pain control, other symptom management, and psychosocial intervention. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. care guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting. INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.

IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

SENIOR HOUISING DEFINITIONS

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee

the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care.

Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances. RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to

provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/ location of apartment. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision. SHORT-TERM CARE – Acute care provided on a short term basis. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

Be Proactive About Senior Care – In the perfect world, we all want The Potential to age in our own home. It is generally the setting where we are Cost of Waiting most comfortable. However, life often has different plans for us.

As we age, our daily activities can get more challenging. Some warning signs are; it is unsafe to be at home alone, the amount of home care needed gets too expensive, family/neighbors/friends can no longer help enough, or loneliness or boredom is starting to set in. Senior Living such as Independent or Assisted Living can often help make life easier, more fulfilling, and provide any care that is needed. By being proactive about moving into Senior Living, we often find that there are several advantages of moving sooner than later. We get a lot more out of the community or home by moving in when we are more active. We make new friends easier, we get involved in the activities, enjoy the food, go on outings, and rekindle old friendships. Plus, time with family becomes Quality Time instead of loved ones simply helping with chores, transportation, etc. We often see the downsides of waiting too long. For example, something happens as we wait to make the move; a fall, a broken hip, an accident due to dementia. The unfortunate thing is that 124 NORTHERN COLORADO - SEPTEMBER 2016-2017

when something like that occurs, Senior Living may not be an option anymore. We miss the window of opportunity and then only far more expensive options are available (i.e. a Nursing Home). A proactive search can save a tremendous amount of money, and also affect quality of life. Waiting for “an event to happen” that forces a move can be very stressful to ourselves and our loved ones. By moving into Senior Living earlier, the safer environment can often help avoid more intensive settings later, and quality of life is often enhanced, not diminished. We usually find a great place while we have plenty of time to look, and are not under stress because of a short time line. We have a better chance of finding the perfect place when we look earlier. Living at home can be a challenge as we age. A common phrase we hear is “I wish I would have done this sooner”. Editor’s Note: This article was submitted by Ed Wagner.
