We work hard for financial assets built over a lifetime and may not want those assets drained by long-term care costs or medical bills. Proper planning (including Trusts and other estate planning documents) allows you to maximize benefits, like Medicare and VA, to ensure your family home and other assets are not subject to recovery by the government. What happens when I can no longer make medical and financial decisions? While we hope to always be involved in medical and financial decision-making, sometimes that is not possible. If the power to make decisions is ever not your own, ensure you have nominated the right people for this role through Powers of Attorney. When someone has not done so court proceedings determine who will step in to make decisions for you. Understanding how the process works and what your rights and responsibilities are is critical. Do I really need an Estate Plan? Everyone needs proper documentation in place that shares your wishes. How do you want your assets distributed? Are there special considerations like disabili-

ties or beneficiaries who need more support? Can I protect assets and beneficiaries so nothing is wasted, stolen, or used unwisely? You can also determine who makes decisions for you while you are alive and after you pass away. Why should I have Powers of Attorney? These documents allow you to nominate someone to make decisions for you while you are still alive, for both medical and financial situations, making that person legally responsible for acting on your behalf. You can make the decision, not a court proceeding. Can I really avoid Probate? Probate is the court procedure by which your assets are transferred after you die to those you name in your Will, or to the people listed in the inheritance statutes of Colorado. Probate is public and it can take 8 to 12 months or more to resolve everything, even in the simplest of cases. Getting to the grocery store alone can become an obstacle; as well as, having the mobility and endurance to actually prepare nutritious meals on a daily basis. Although some delivery services like Meals on Wheels can be convenient, they are not customized to the individual’s preferences nor do they provide companionship. Boxed meal delivery services still need to be prepared, they’re not highly customized, and can be ex-

pensive. Some home care services provide meal prep, but the care provider probably is not a trained Chef. Chefs for Seniors provide personalized meals prepared in the home by a professional Chef. 3 Facts to Know Regarding Social Security, Medicare and Every day I hear information that has been shared that is not completely Long Term Care accurate. Often times, I am talking to people about their planning goals and they are letting these pieces of information keep them from understanding how it applies to them. Here are three quick items to provide you a better understanding so you can choose how they specifically impact you and your own planning:

Social Security is going broke is often the headline we see. According to the Trustees Report released on April 25, 2019, it is estimated that the two trust funds would run out of money by 2035. That is absolutely true, however when the funds are depleted and if Congress does nothing, they will use taxes currently being collected to pay those obligations. Benefits may need to be reduced from current estimations IF Congress does nothing.

If I work, I will not get my Social Security. This is not entirely true. If you decide to collect social security before your FULL retirement age, for those born between 1943 and 1954, the age is 66, your benefit is reduced by $1.00 for each $2.00 you earn above the earning’s 32 NORTHERN COLORADO - OCTOBER 2019-2020

limit. After full retirement age, the amount you earn does not impact your benefit.

Medicare or Medicaid will cover Long Term Care. This is one of the biggest misconceptions I hear. Medicare provides for skilled care in a nursing home ONLY while you are recuperating following a hospital stay for a related conditions and only for 100 days. Please note that Medicare does not cover long-term care or custodial care. Medicaid is for people with very low assets and limited income. Unfortunately, opportunities to look beyond costs and coverage are sometimes overlooked. There are many other factors to investigate when reviewing plans and considering change. Of most importance is quality of care. The level and quality of care associated with a health plan can play a significant role in the healthcare experience as well as overall health of the member. Find out: what are the quality ratings associated with plans being considered? What does the plan offer that would be valuable to me? Do they simply provide insurance, or do they plan an active role in helping members to obtain or maintain good health standing? Health plans are easy when healthy… what about when sick? What resources are provided to assist in that time of need? Also consider time and communication are the two most crucial elements to success in healthcare yet not all organizations recognize this as many only promote low cost coverage or choice in marketing materials. Consider what role(s), if any, the health plan being considered play in helping the member to reduce

time wasted navigating the system or ensuring proper communication is taking place. Can members only access care in person? Chat? Email? Video? Phone? Text? How hard is it (or easy) for a member to communicate when needed? How soon does one get a response?

Medicare 2020 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

FHA Lifts Ban On Non FHA Approved Condos For Future HECM Loans Door Opens For Spot Approvals For Reverse Mortgage Financing.

As the HECM loan, known by most as the reverse mortgage, gains momentum in our industry, there has always been one insurmountable wall that continued to prevent seniors from enjoying the home of their choice – the Non FHA approved Condo market. Recently, FHA has made a change for the benefit of many: our aging population, many beautiful existing and new condominium projects, builders/developers, Realtors and lenders. Prior to this ruling, many people who wished to take advantage of the FHA HECM (Reverse Mortgage) found that more than 90% of the condos they wished to purchase or refinance

were excluded from the program. Benefits for this change reach well beyond expanding the choices seniors have for the homes they can now choose to spend their retirement years in. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household

duties. These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. TRANSPORTATION – Rides to shopping, appointments and other outings. WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work. ~

Home Health Care Definitions 24 HOUR CARE – Refers to “around the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver. 24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth. HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare


RESOURCES FOR AGING WELL

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendant or home alert system that notifies staff that user is in distress and needs immediate assistance. NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for. PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services

related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

Bridging the Gap from Hospital to For some, hospital discharge Home and Hospice papers can feel like freedom. For others, it can feel like sentencing. What happens when that person is your loved one? Leaving the hospital does not have to mean going through this journey alone. It also doesn’t have to mean relying on a close family member to sacrifice time, resources, and a sense of normalcy to step in as caretaker. There is a third option that not enough people are talking about: affordable, professional inhome care. Patient Advocates There are organizations that can step in when a patient leaves the confines of a hospital bed and returns home for either recovery or for end of life care. Take the example of one Colorado patient who was unaware of his home care options. He hired a caretaker advocate who brought the doctors he needed to his home, guided him to better coverage through Medicaid, and supported him through his journey. The result? He was able to decrease services and live nine more months feeling strong, empowered, and independent.

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be man-

aged elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

Music Can Provide the Right Therapy For centuries, music has played During Palliative a powerful role in overall wellness and has been recognized and Hospice Care for its therapeutic ability.

Music can promote desired changes in a person’s energy level, manage moods, provide distraction during a difficult situation or provide the necessary support to move through the same difficult situation. Music is an inherent part of being human and can be thought of as its own health domain. When a loved one is in palliative or hospice care, they often need comfort or help from something that does not come from medication or other traditional treatment. Music therapy, as described by the American Music Therapy Association, “…uses music to address physical, emotional, cognitive and social needs of individuals of all ages.” Music therapy can be key in helping patients deal with many of the emotional and physical challenges they are facing during a difficult time. Family and loved ones can also benefit from music therapy to help them navigate through grief, stress and overwhelming feelings. In hospice or palliative care, music therapists focus on identifying and expressing emotions, increasing socialization, improving mental and physical capabilities and supporting spiritual needs. Music therapists, who are Board Certified by the Certification Board for Music Therapists, use different techniques depending 92 NORTHERN COLORADO - OCTOBER 2019-2020

on a person’s situation. For instance, lyric analysis promotes mental stimulation and can prompt discussion and reflection on a patient’s own life. Using instruments, such as beating on a drum, can help with rhythmic movement and coordination. Singing along with music encourages breath work and speech articulation. It’s a form of therapy that offers many tactics to help a patient and their specific needs. For family and loved ones, music can be a way to express grief, alleviate stress and even share a memory with the patient by making a connection into their past together. It creates a familiar and comfortable environment for people to explore and share their feelings and thoughts. Music therapy is very much seen as enjoyable and non-threatening so it is often welcomed by patients of different ages and abilities and can be executed individually or within a group. Trying to Understand Hospice! HOSPICE! Just the mention of the word creates fear and anxiety! Not long ago, this author had to wrestle with understanding, not only the definition of HOSPICE, but the ramifications as well! My amazing Mom was diagnosed with terminal cancer. Seemingly overnight, my family and I were challenged to learn as much as we could about this philosophy of care that had invaded our lives! Actually, Hospice does not indicate a death sentence at all, just a change of direction. The aim of Hospice is not curative, but has comfort as its goal. Friend, entering into hospice care is not about dying. Rather, it’s

about living, and living as well as possible, for as long as you have left. In Hospice Care, the Patient’s pain and symptoms are addressed, and emotional and spiritual support is also provided. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

Senior Housing Definitions

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas

for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.

SENIOR HOUSING DEFINITIONS

ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible. INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner. LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care. PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different

needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence. PUBLIC HOUSING (PH) – Affordable apartments for low-income seniors and persons with disabilities. REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients. RESIDENTIAL HOME – Private residential homes that typically have a small number of residents who live together and receive care from live-in caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing

Initial reasons to participate: C an always learn more about this subject. n I n our political climate, so many opportunities for conflict. n L earn how to feel compassion without getting caught up in other’s issues. n H ow to understand people different than yourself. n a ffects residents and learn how to support staff. n L earn more tools in my work and life to help improve my wellbeing. n B etter understand compassion vs empathy. n H ow to ensure and honor staff’s purpose. n T raining for residents, families, and staff is vitally important to make sure everyone is on the same page. n

From the Stanford Medicine Center for Compassion and Altruism Research and Education, they define compassion as the following 4 step process: n A wareness of suffering in others. n F eeling of being emotionally moved by suffering.

M otivation to see the relief of that suffering. n A ction and willingness to help relieve that suffering when possible. n

A take away from a participant: “I signed up for it because I have never really had a conversation about compassion so I was curious how it would tie into my work. My biggest take was the piece about purpose and it got me thinking about what is my purpose at work and how my purpose has changed over time. This training allowed me to think about purpose in a different way and I think it can be really helpful when trying to make connections with co-workers by learning what their purpose is and how it ties into their work.”
