They help residents maintain their quality of life and receive quality care in licensed long-term care communities. 3 Tips To Make The Transition To Moving a loved one into residential Senior Living Easier care is a big life change. Here are three tips to help make the transition easier for everyone involved.

Communication. Ask how they will keep you updated on your loved one’s care. One of the hardest things about this time is not knowing how your loved one is doing. Questions like whether the doctor rounded, or if mom got her haircut. A general Facebook group may give you the generic story, but ask for regular, specific communication about your loved one. Expectations. Ask the community to spell out specifically

what they do and what they do not do. Not all residential care provides insulin shots or physical therapy, for example. Relationships. You’re hiring a team to help care for your loved one. Ask them what you can do to help build relationships with the care team. As family, think of yourself as the ‘CEO of the care team.’ ~

Trust Planning Completes Your Wills and powers of attorney make Estate Portfolio up a simple estate plan. When your needs and wishes require more detailed instructions, adding a trust to your plan allows you to specifically direct where your assets go and how those assets can be used when you pass away. A properly funded trust keeps your wishes private, not a part of public court proceedings, and is more difficult to contest than a will only. There are many different types of trusts, each with specific purposes. Any trust can be tailored to meet your specific needs and wishes for your loved ones. A Revocable Living Trust can be modified or revoked by the creator. Trust assets go directly to beneficiaries, not through your will. One key benefit is planning for estate tax concerns and establishing protections to keep assets in your family. An Irrevocable Trust cannot be changed or modified and is managed by a trustee, who is responsible for carrying out the wishes of the creator of the trust and managing assets in the best interest of beneficiaries. These are frequently created as part of a plan for helping a person pay for their long-term care, especially in a nursing home. Heritage or Dynasty Trusts allow for assets to remain within your family, passing from one generation

to the next. This type of trust can keep assets within families even if your children or grandchildren divorce and allow for protection from creditors. A Special Needs Trust can be a valuable tool when a loved one has a disability or needs finances protected from receiving an inheritance all at once, allowing for continued access to public or government benefits and services. Charitable Trusts leave a legacy to a charity based on your interests or religious beliefs. Assets can be given before or after other beneficiaries, in a fixed amount or a percentage of all assets. When it comes to choosing the best Medicare plans for yourself or a loved one HMOs (Health Maintenance Organization) are often a viable choice due to affordability, predictability, and financial safety. All HMOs operate based on the concept of managed care… but not all have integrated care. It is important to be aware of the difference. Let’s start with managed care. This is when health insurance organizations contract with select providers, pharmacies, and hospitals in their service area to create a network that allows for better control of costs. As a result of this arrangement, members agree to seek non-emergency/non-urgent care with network providers in exchange for lower costs of care. The primary advantage of the managed care aspect associated with HMOs is the focus on the cost of care which benefits the member and the health plan. Integrated care, however, is when the health plan and providers work together, in close coordination and collaboration, they have a shared focus on the patient experience, quality of care in and cost of care. The additional focus on patient

experience and quality of care is often associated with ease of access or robust IT tools resulting in easier communication across the care continuum. STAR ratings are solid indicators regarding performance for quality of care for health plans. When it comes to IT, you’ll want to ask what electronic communications options exist with the health plan you are considering. If available, they may allow for better communication to patients and providers, as well as between providers.

There is so much more to learn about with respect to integrated care… be sure to ask your potential health plan what tools or resources are available that promote integrated care. Some health plans with impress and overwhelm you… and some may not be able to give you a straight answer. Think of it this way: all integrated care HMOs have managed care, but not all managed care HMOs have integrated care. What Has COVID-19 Taught Me? In the beginning, it felt as if new guidance, protocols, emergency health orders, and ideas about how to manage this disease were changing every 15 minutes. The pace was unbelievable. During that time, I told myself – don’t look back. The decisions that were made from a national, state, local, and at our facilities was made with the best information at the time. Our medical director told us this could go on for 2 years. I hoped she was wrong. I don’t think she is. I told myself – I’m going to have to buckle in for the long haul. So, I take each day as it comes and don’t worry too much about next week or next month. 2021 is not even on my radar. Grace. I say this word to myself many times throughout each day. As this pandemic affects EVERYONE, we need to have grace with each other. I know all health care providers are better at infection control. I know regular citizens are better at infection control. I know that most everyone wants to protect their family members, their neighbors, and their com-

munity. I think all of us have had the opportunity to evaluate our priorities – what makes us happy and where do we want to focus our energy. The Long Term Care profession is becoming more focused, together. Efforts through the Institute for Healthcare Improvement (IHI) and the ECHO training program will help to ensure we have the best information, from experts, and the educational model to teach all staff. The national focus on Long Term Care is a positive. We have been the forgotten part of health care for way too long. I look forward to what the next 1-3 years bring for the profession I have dedicated 37 years to. The pandemic has been hard. It has taught us knew ways of caring for each other. It has brought Long Term Care, public health, CMS, CDC, HHS, and others together. Our new learning and experiences must make us better going forward. Otherwise, what was it all for? ~

These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

What Can Home Health Care Home Health Care (HHC) comes in Do For Me? a variety of different services and agencies such as Nursing services, Physical, Occupational and speech therapies it may also include social workers and hospice services. These services are often needed when discharging from a hospital or rehab or in the home when there is a decline in condition. These types of short term services help to ensure you are progressing in a positive direction. Other services and agencies may be in place along with HHC in a supportive manner. You may need a full time Certified Nursing Assistant caregiver or a Registered Nurse to help you while retaining services through another agency. This is allowed when you are paying private or through a long-term care insurance or trust, however, two Medicare agencies getting paid from Medicare cannot have services overlap. Medicare agencies often cover medication fills, injections, wound management and therapies. After a period, they may not be able to cover some of these things. With HHC you have the option of privately paying for an agency to pick up where the Medicare agency may have left off. A common misconception is that Medicare will cover the cost of a Caregiver in your home who would do housekeeping, go shopping, cook meals or sit with you for extended

periods of time. Medicare does not cover the cost to these performed tasks. When private pay is involved, two agencies can often be found in one home at the same time. Over the years I have been asked, “If hospice is already in place and I need help all the time what happens?” The hospice agency cannot be with you twenty-four hours a day or for long periods of time. If you wish for your loved one to remain in the home, it is recommended that you call another agency to help you fill the hours needed and support the hospice agency already in place. It is an easy decision to keep you and your family well cared for while honoring their wishes. The time and labor caring for them following that decision is the challenge. With so many agencies in our area providing different levels of care and supportive services it is becoming easier to keep our loved ones in place longer while honoring their wishes. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. How Can You Know You Are Partnering With High-Quality Guidelines on the Hospice Compare website and how it can help Hospice Providers? Three years ago the Centers for Medicare and Medicaid Services (CMS) launched a useful tool, on the www.medicare.gov/hospicecompare website, that can help anyone when choosing a hospice provider. The Hospice Compare website allows you to easily search for hospices by name or location and compare publicly reported data on hospice providers. The data is divided into five categories, including General Information/Level of Care, Conditions, Location of Care, Family Experience of Care and Quality of Care. The first category, General Information/Level of Care, defines the four levels of care covered in the Medicare Hospice Benefit. Offering all levels of care, including Respite and Continuous care, are important in identifying how hospices can appropriately care for patients. Data for the second and third categories — Conditions and Locations of Care — are extracted from Medicare claims to show general characteristics of hospice providers. You can also see the breakdown of where a hospice agency provides most care, such as at home, in an assisted living facility, nursing facility, or inpatient hospice facility. Finally, the last two categories are most important to consider. These show data on quality measures that

hospice agencies are now required to submit. The Family Experience of Care data is a subjective measure of the experience of a patient’s primary caregiver. It comes from the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey. The data shows what percentage of caregivers would rate their hospice a 9 or 10 (on a scale of 1-10) and what percentage would “definitely” recommend the hospice agency to friends and family. The last category of data is Quality of Care. It shows the percentage of patients who received at least one visit from a registered nurse, a physician, a nurse practitioner, or a physician assistant in the last three days of life. Also under the Quality of Care tab, you will find scoring on how well hospices are doing when patients are first admitted. You should expect a good hospice partner to receive high scores on this Quality of Care data. It is essentially a straight-forward measure of whether hospice agencies are completing essential care processes or not. Alzheimer’s disease can be one of the most emotional and devastating diagnoses that effects an individual and their family. As a progressive brain disorder, the disease depletes memory and thinking skills, making even daily life tasks difficult. Estimates vary, but experts suggest that more than 5 million Americans may have some form of Alzheimer’s. It is currently ranked as the sixth leading cause of death in the United States. Although extensive global research continues to explore the causes and treatments, unfortunately no cure currently exists. Alzheimer’s disease damages the hippocampus, the part of the brain that controls the formation of memories. Vital nerve cell connections to the brain also begin to deplete which prevent the brain from sending messages to the body’s muscles and organs. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All

hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
