Agency, the Eastern Nebraska Office on Aging is designed to help older Nebraskans live with independence and dignity in their own homes for as long as possible.

Located at 4780 S. 131st St. in Omaha, ENOA offers a network of programs and services for older men and women and their families in Douglas, Sarpy, Dodge, Cass, and Washington counties. As one of the nation’s 622 Area Agencies on Aging, ENOA offers care management, Medicaid Waiver, Meals on Wheels, caregiver support, a variety of in-home services including bath aides, homemakers, and emergency response systems; rural transportation, senior centers, volunteer programs, information and assistance, a longterm care ombudsman, The Aging and Disability Resource Center, and the New Horizons newspaper. The agency is committed to empowering older men and women by giving them a variety of choices and greater control over the pro-

grams and services they receive, the environment in which they choose to receive them, and the delivery manner. To qualify for ENOA’s are management services, men and women must be age 60 or older, live in the agency’s five-county service area, and need assistance with three or more daily activities of living for 90 days or longer. The caring, professional support ENOA offers also helps relieve the caregiving burden for family members. To receive caregiver support services, a person must be age 18 or older and assisting someone age 60 or older who needs help with at least two daily activities of living or who has received a diagnosis of Alzheimer’s disease or a related disorder. In addition to the Omaha location, ENOA has offices in Fremont and Blair. How Do I Know That It’s Time? During the summer of 1992, I took the Certified Nursing Assistant class. Little did I know, that course and the subsequent work at senior communities, would not only help pay college expenses; but more importantly, pave the way for my lifelong advocacy of our aging seniors. For twenty-three amazing years, I have visited with thousands of seniors and their families. I have listened to stories of survival, strength, disappointment, sorrow, and love… each one unique. After hearing so many life stories, sharing tears and laughter together, I have resolved to work diligently to help seniors discover the best options for their personal situation. I want to ensure that the next chapter in life is just a continuation of their life’s adventure, not a stand-alone chapter of heart break and disaster. “How do I know that it’s time?” This question is commonly asked by seniors and their families. Time could be referring to a number of things since each story is so unique. One family may be thinking about moving mom from her lifelong home to an assisted living community and another may be worried about end of life care. A spouse may be concerned about hiring a caregiver to help with his wife’s care or finding

a community where each of them could get support. Each decision is real, emotional and difficult. When a family asks this question, they are acknowledging that whether it is a safety issue, caregiver exhaustion, or senior isolation, the current plan is not working and they need outside

help. In fact, I tell people most likely the time was several months ago. It is so difficult to put emotion aside and make a life changing decision for our loved ones. There are great advocates out there to help you each step of the way. Please don’t wait for just one more fall to decide “it’s time”. Medicare 2019 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Plans may have additional services and discounts, including dental, vision, gym membership & more.

Catastrophic Coverage (Phase IV) For the remaining calendar year, covered drugs are: $3.40 / $8.50 or 5% depending on drug tier

There is an Annual Election Period (AEP) which runs from October 15 - December 7 of each year. This is time to make changes in company or coverage.

Part D can be separate with Medigap & PFFS plans. There is a penalty for late enrollment into Part D.

“Will My Insurance That’s the question that is looming Cover That? in every patient’s mind before a medical procedure, after a newly diagnosed illness, a hospitalization or worse yet a long rehabilitation stay. Let’s be honest. Healthcare expenses can add up in a hurry making it critically important that people understand (to the best of their ability) what benefits their health insurance plans provide. Unfortunately, that is easier said than done, right? Group plans are ever-evolving with deductibles creeping higher and the introduction of Health Savings Accounts and Health Reimbursement Accounts to help members fund increased out of pockets costs. Marketplace and short term medical plans change each year making it challenging for members to keep things straight. Medicare is riddled with complexities and often referred to as “Alphabet Soup”; (I’ve made a fun jingle about it!) including BIG differences between Medicare supplements and Advantage plans. So then, what is the answer? Members have many resources when it comes to exploring their plan benefits. Plan materials and policies are sent out to members either by mail or electronically that contain volumes of information. Insurance carriers provide customer service phone numbers that can help address

questions during regular business hours. In most cases, members can register for online accounts to view current benefits, claims history, print new ID cards, premium information and much more. Providers can submit pre-authorization requests to help members plan for future services. Did I mention agents? Reputable agents will help their clients navigate their benefit questions. Local senior service programs such as ENOA and Aging Partners can help provide information and direction. Medicare specifically publishes a new handbook each year titled “Medicare & You” and makes it available by print or online at www.medicare. gov. In addition, Medicare recently developed an app for smart phones that helps determine if an upcoming service is covered. Ask questions, advocate for yourself and family members and be sure to understand your plan benefits BEFORE you enroll. Cardiac care is a program designed to help a person with heart disease return to health. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Home Health Care Definitions continued are a few of the ways personal care can be paid for. PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities.

Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Hospice Care Definitions continued The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and

objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious

illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. When the Crisis Hits... “When the Crisis Hits”, families may have very little time to decide on the best care facility for a loved one. Trying to find a Skilled Nursing Care center as the clock races can make you feel overwhelmed or even paralyzed. Fortunately, there are always care options available, even when time isn’t. We recommend breaking down your decision into three steps: Step 1: Research Ask family and friends to recommend caregivers or centers. Find and compare Skilled Nursing Care Centers online with Medicare’s “Nursing Care Compare,” a rating site for Medicare- and Medicaid-certified nursing homes, professionally called Skilled Nursing Care centers. Medicare has developed a fivestar quality rating system based on health inspection results, care center staff data, quality measures, and fire safety inspection results. Though there is plentiful information you can find, relying on ratings alone is not sufficient enough to make a decision. Be wary of for-profit placement services, you want your decision to be based on careful consideration, not their commission. Step 2: Consider Based on your research, choose about three centers to visit. Call each to schedule a tour. Communi-

cate the immediacy of your situation and ask to speak with an admissions coordinator. The admissions coordinator can tell you more about the center and availability. Only visit centers that you know have available accommodations.

Step 3: Select After visiting and doing your research, select the center that best meets your care needs and consider giving yourself one night to sleep on your decision. Once you have made your care choice, learn more about what to expect during the transition. As you go through the decision process, remember your loved one will need you as much now as ever. You’ll remain an integral part of your loved one’s care and decisions.
