Adult day programs are for elders who need a place to stay during daytime hours because they have difficulty functioning alone (i.e. dementia or physical limitations). Among other things, a DMM can assist with bill paying, organization of bank, investment statements and tax documents, negotiating with creditors, budget assistance, understanding and reconciling medical insurance claims, Medicaid applications and Medicare choices. A DMM can identify pertinent issues and work with and make referrals to other professionals such as geriatric care manager, lawyer, financial advisor or tax counselor. EMERGENCY RESPONSE SYSTEMS A Personal Emergency Response System (PERS) is an electronic device designed to let you summon help in an emergency. A PERS has three components: a small radio transmitter (a help button carried or worn by the user); a console connected to the user’s telephone; and an emergency response center that monitors calls. When emergency help (medical, fire, or police) is needed, the PERS user presses the transmitter’s help button. It sends a radio signal to the console. The console automatically dials an emergency response center where the caller is identified. The center will try to determine the nature of the emergency. If the center cannot contact you or determine whether an emergency exists, it will alert emergency service providers to go to your home.

Hospice Care—The Myths. The Reality. Every culture, every community has its own set of myths and fears about terminal illness and the dying process. One of the challenges for hospice caregivers is to help dispel these myths and reassure patients and families about the realities of effective end of life care. Consider these mistaken notions about hospice care. The realities offer hope in place of sadness.

Myth #1: Hospice is a place. Reality: Hospice isn’t a place. It’s the treatment of physical,

social and emotional pain and symptoms near the end of life. Recent studies show that 90 percent of patients faced with a lifethreatening illness prefer to remain in their homes, in comfortable and familiar surroundings, surrounded by family and friends. Whether they live in their family home, an adult living community or a long term care facility, they want to age— and die—in place. Hospice respects that wish. Myth #2: Hospice means giving up. Reality: When medical treatments cannot cure a disease, the

Myth #3: Hospice is for cancer patients.

Myth #4: Hospice is expensive. Reality: Families don’t have to spend themselves into poverty to

qualify for hospice coverage. The vast majority of hospice patients are elderly and, therefore, Medicare beneficiaries. The Medicare Hospice Benefit is all-inclusive—covering 100 percent of the cost of hospice care. There are no deductibles; there are no out-of-pocket expenses to patients or their loved ones. In most states Medicaid also provides hospice coverage. Most private insurance plans also include a hospice benefit.

SKILLED CARE DEFINITIONS DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

SKILLED CARE DEFINITIONS (continued from page 73) WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

NON-SKILLED CARE DEFINITIONS 24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver. 24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you. ACCREDITED – Accrediting agencies are professional groups that establish and monitor voluntary, industry wide standards to ensure that patients receive quality home health care services. The Commission on Accreditation for Home Care, Inc. (CACH) and Community Health Accreditation Program (CHAP) are two of the accrediting agencies. ALZHEIMER’S DEMENTIA CARE – This refers to personnel of the agency that are specifically trained to provide care to those that are afflicted with Alzheimer’s or other dementia related problems. BONDED – Means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. FREE ASSESSMENT – Agencies that do not charge for the initial assessment of the client which is usually facilitated by a nurse or other professional capable of providing a detailed care plan. GO/JACC – In 2009 the Department of Health and Senior Services consolidated three Medicaid supported home and community based service programs into a single program known as Global Options (GO). Do you believe that the age you will become is inferior to the age you are? Do you see getting older as inferior to being younger? Do you avoid telling your age or make fun of your age? Do you fear getting older? If so, I am sorry to say that you are an ageist. Statements like “you look great for your age” or “you’re so young at heart” are ageist statements, not compliments. It reinforces the idea that young is good and old is bad. Aegism is prevalent in our society and we are all at risk to buying into ageist stereotypes. Our culture devalues growing older. We think nothing of products that are sold as having “anti-aging” benefits. We accept this as normal. The tragedy of ageist beliefs is that ageism is a prejudice against all of us and has serious repercussions. Ageist beliefs significantly strip us of our confidence to be and do as we age and makes the very old insignificant. Chronological age is an increasingly unreliable benchmark of pretty much anything about a person. The sooner we stop dreading growing older, the better equipped we will be to benefit from countless ways in which we can enrich ourselves. Instead of being fearful of getting older, we should change our thinking and all get into “training for longevity”. What we really want is to live

life in a way that ensures us the best overall health for the long haul. Be conscious of aegist views and don’t allow them to hold you back. The senior living industry is not immune from aegist thinking. Often when you visit a community, you’ll be asked your age before you are asked what your needs and wants are. Aegist compliments such as you are doing so well for your ages are used all the time. Accomplishments are labeled “cute” and “sweet”.

Start bringing an awareness regarding the prejudice of aegism. Promote a dialogue that questions aegist views. Remember, we all want to get older every year. This is a good thing. Located on 14 wooded acres in suburban North Haledon, the Holland Christian Home is a Continuing Care Retirement Community licensed for 118 residential beds and 52 skilled nursing beds. With a staff of approximately 150, the Home offers professional care and modern amenities in a warm, homelike environment. Applicants must be at least 75 years of age and in relatively good health. At the Holland Christian Home we provide a tiered approach to healthcare. SENIOR HOUSING DEFINITIONS (continued from page 97) ASSISTED LIVING COMMUNITIES – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. Amenities vary for each community. CCRC – Continuing Care Retirement Communities offer all levels of care on one campus (independent living, assisted living and skilled nursing). An endowment fee may be required that entitles residents to use assisted or skilled services when needed throughout their lifetime. Each community offers varied buy-in and payment plan options. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc.). INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctors orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner. Take a moment to observe the family members of prospective patients and residents who are touring your care centers. What are they looking for? As the child of a parent who presently requires long term care, I know that I’m happiest when my dad is safe and happy. Sound familiar? Our caregivers ultimately steer our reputation – for better or worse. They may not appear on a balance sheet, but they are our most valu-

able assets. Remember how important they are, remind them of their value, and take excellent care of them. That’s the secret. This month I received the American Health Care Association’s Joe Warner Award for Patient Advocacy. I

am honored. But I know that the real credit behind this award belongs to everyone with whom I work, day in and day out. I applaud them for making our patients and families truly happy. SENIOR HOUSING DEFINITIONS (continued from page 109) TYPE 1 CARE – Residents may need significant assistance with 2 ADL’s and minimal assistance on more than 2 ADL’s daily. The resident must also be able to evacuate a community under their own power and have stable health. Residents receive 24/7 monitoring and may have an independent home health agency help with services. TYPE 2 CARE – Residents receive full assistance with ADL’s and are able to permit aging in place. Residents may require the assist of one person for transfers and evacuations. Residents receive 24/7 monitoring and may have an independent home health agency help with services. PRICE ALL INCLUSIVE – Price listed includes any and all amenities a community has to offer. RESPITE CARE – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITIES – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly and typically based upon size/ location of apartment. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders. SKILLED NURSING CARE FACILITIES – Providing 24/7 care for residents and offering full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable TV and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources. 130 • Please mention the Seniors Blue Book when you call our advertisers.


As a Method to Become Eligible For Medicaid Medicaid is a program under which, needy individuals can obtain financial assistance with their health care costs. Medicaid Managed Care covers in home care, adult day care, assisted living, medical insurance and prescription coverage. However there is an income cap for eligibility. Until December 1, 2014, individuals were completely excluded from this program if their monthly income exceeded the posted limit (currently $2,199 including social security and pension). This created pressure on needy patients who might be able to get by on Managed Care to seek care at nursing homes, since the income limit is thousands of dollars higher. This did not make economic sense, since in home care and/or assisted living would cost the government far less money. As a result, beginning December 1, 2014 Qualified Income Trusts (QIT’s), were adopted by Medicaid in New Jersey. Basically anyone with income above the limit can now establish a QIT into which the excess income is deposited (and subsequently used to reimburse Medicaid) and thereby become eligible for all Medicaid programs, including nursing home coverage. The income placed in the Trust will not be considered for financial eligibility determination. The income deposited each month into the Trust must then be paid out each

month for the Medicaid beneficiary’s specified post-eligibility expenses. Due to the nature of the Trust, generally almost all income deposited into the Trust monthly will be depleted at the end of each month. An initial amount up to $20 can be deposited to open the Trust, if needed. After that, only income belonging to the Trust beneficiary can be deposited into the Trust. Thousands of people will now be eligible for these valuable government programs who were previously denied. Please make sure that you discuss this program with your elder law attorney or consultant.
