Adult day programs are for elders who need a place to stay during daytime hours because they have difficulty functioning alone (i.e. dementia or physical limitations). DAILY MONEY MANAGEMENT A Daily Money Manager (DMM) provides personal assistance to seniors who have difficulty managing their financial paperwork. Among other things, a DMM can assist with bill paying, organization of bank, investment statements and tax documents, negotiating with creditors, budget assistance, understanding and reconciling medical insurance claims, Medicaid applications and Medicare choices. A DMM can identify pertinent issues and work with and make referrals to other professionals such as geriatric care manager, lawyer, financial advisor or tax counselor. Many DMMs are Certified Senior Advisors (CSAs) as well as notaries and can help seniors maintain independence by avoiding errors which might raise concern of loved ones.

Center is transforming its approach to cancer care, with construction of a high-tech facility developed to improve outcomes and quality of life for patients living with cancer.
The new Cancer Treatment and Wellness Center began its phased opening in 2014, providing personalized and seamless screening, imaging, diagnosis, and treatment services in one convenient locations. A multidisciplinary team of medical and surgical specialists focuses on breast, colon, gynecologic, thyroid, kidney, lung, prostate, brain, pancreatic, and gastrointestinal cancers. “We have pulled together technology and experts to help ensure the best individualized care for each and every patient,” said Dr. Steven Brower, newly appointed medical director of the Cancer Treatment and Wellness Center. “Our arsenal to beat cancer comprises precise, mini-

EMERGENCY RESPONSE SYSTEMS A Personal Emergency Response System (PERS) is an electronic device designed to let you summon help in an emergency. A PERS has three components: a small radio transmitter (a help button carried or worn by the user); a console connected to the user’s telephone; and an emergency response center that monitors calls. When emergency help (medical, fire, or police) is needed, the PERS user presses the transmitter’s help button. It sends a radio signal to the console. The console automatically dials an emergency response center where the caller is identified. The center will try to determine the nature of the emergency. If the center cannot contact you or determine whether an emergency exists, it will alert emergency service providers to go to your home.

HOSPICES Hospices provide individuals facing terminal illness with clinical and psycho/social care at home or in a healthcare facility. The mission of hospice is to assist patients and allow them to live the last stage of their lives with dignity; and to supplement routine clinical care with expertise in pain control, symptom management, grief and loss counseling, as well as support services. Hospice Care—The Myths. The Reality. Every culture, every community has its own set of myths and fears about terminal illness and the dying process. One of the challenges for hospice caregivers is to help dispel these myths and reassure patients and families about the realities of effective end of life care. Consider these mistaken notions about hospice care. The realities offer hope in place of sadness.
Myth #1: Hospice is a place. Reality: Hospice isn’t a place. It’s the treatment of physical,
social and emotional pain and symptoms near the end of life. Recent studies show that 90 percent of patients faced with a lifethreatening illness prefer to remain in their homes, in comfortable and familiar surroundings, surrounded by family and friends. Whether they live in their family home, an adult living community or a long term care facility, they want to age— and die—in place. Hospice respects that wish. Myth #2: Hospice means giving up. Reality: When medical treatments cannot cure a disease, the

Myth #3: Hospice is for cancer patients.
Myth #4: Hospice is expensive. Reality: Families don’t have to spend themselves into poverty to
qualify for hospice coverage. The vast majority of hospice patients are elderly and, therefore, Medicare beneficiaries. The Medicare Hospice Benefit is all-inclusive—covering 100 percent of the cost of hospice care. There are no deductibles; there are no out-of-pocket expenses to patients or their loved ones. In most states Medicaid also provides hospice coverage. Most private insurance plans also include a hospice benefit.

Nursing home costs are now averaging 12,000 per month. Many families make no advanced preparation for this and these costs are driving many families into poverty. Every aging American should take the following three steps: First, make sure you have the proper will, financial power of attorney and living will and your children know where to find them. My advice is don’t cut corners, hire an elder law attorney. Only a few lawyers are true elder law attorneys. They should be a member of the National Academy of Elder Law Attorneys. There are only 4,000 members in the United States. If he is not a member, look elsewhere. Second, make sure your children are thoroughly familiar with all of your assets, liabilities and medical experts. Finally, discuss your wishes should you become disabled. Do you want to stay at home with in home care
takers, or take up residence in an assisted living facility or nursing home if appropriate. Consider hiring a geriatric care manager who can assess your needs and coordinate your long term care. Make sure you understand the different levels of care you may need, and which alternatives will fulfill that need. Also, consider whether you can afford these alternatives. Consider entering into an asset protection plan designed by a well qualified elder law attorney, so that your assets can be preserved and government programs can be tapped to pay some or all of these expenses. Knowledge is power. Advanced planning can literally save you tens of thousands of dollars. So don/t let the approaching storm catch you by surprise. Editor’s Note: Michael A. SKILLED CARE DEFINITIONS DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. SKILLED CARE DEFINITIONS (continued from page 68) Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.
NON-SKILLED CARE DEFINITIONS 24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver. 24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you. ACCREDITED – Accrediting agencies are professional groups that establish and monitor voluntary, industry wide standards to ensure that patients receive quality home health care services. The Commission on Accreditation for Home Care, Inc. (CACH) and Community Health Accreditation Program (CHAP) are two of the accrediting agencies. ALZHEIMER’S DEMENTIA CARE – This refers to personnel of the agency that are specifically trained to provide care to those that are afflicted with Alzheimer’s or other dementia related problems. BONDED – Means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. FREE ASSESSMENT – Agencies that do not charge for the initial assessment of the client which is usually facilitated by a nurse or other professional capable of providing a detailed care plan. GO/JACC – In 2009 the Department of Health and Senior Services consolidated three Medicaid supported home and community based service programs into a single program known as Global Options (GO). GO is an income restricted program. JACC (Jersey Assistance for Community Caregiving) is a program similar to GO but for individuals with slightly higher income and not eligible for GO. How to have confidence when moving to a senior community During my twelve years of working in my current community, first as the Director of Sales and then as the Executive Director, I have watched more than 600 people move in. Throughout my entire career I have helped countless people decide on the best environment in which they should live and then assisted them with the process of adjustment. With the many new residents I have been meeting, I thought I would share with you some of my observations of the decision process to move to a community and the process of making a successful adjustment to community life. Like most significant changes in a person’s life it is impossible to 100% sure you are making the best decision the day you move in. You can be 100% sure that you needed to make a change from your previous living arrangements, but it is less likely that you will be certain of the exact road to take. Like any significant life change, at some point you make the best decision you can and then take a leap of faith in your judgment to make good choices for yourself. I will say with certainly that the majority of people that move in, I would say more than 95% feel that they made the right decision and are quite content.
Most people question their decision during the first couple of months. Did I make the right choice? While everyone is different, most feel a bit overwhelmed with the initial process of setting up their new home. In addition to setting up a new home, people are establishing the” next chapter” in their lives. Who will they spend time with and how will they spend their time? Some approach this with optimism and a sense of adventure while others struggle with this process. Most feel however that they find it easier to establish their “next chapter” in the company of others as opposed to alone in an empty house. Here are some suggestions that residents have told me were helpful: Don’t put pressure on yourself to get everything done right away and accept the help of others if possible. Remember you helped others during your life; it is now time to perhaps make your life a bit easier by accepting the help of others. Expect a range of emotions and give yourself time sort them out. Simply said, you cannot rush the process of adjusting emotionally. Initial feelings of uncertainty are not an indication that you will not eventually be happy with your choice. Remember this is common and keep in mind that people get through this and find contentment. (continued on page 85)

These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. TRANSPORTATION – Rides to shopping, appointments and other outings. Reach out to those who have navigated these waters, your fellow residents. I also would like to encourage those residents who have been made the adjustment to reach out to our newer residents and help them through this initial time. After seeing close to 1000 people get through this process, I have complete confidence that you can too!

Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly and typically based upon size/ location of apartment. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders. SKILLED NURSING CARE FACILITIES – Providing 24/7 care for residents and offering full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable TV and all personal/nursing care. This month I received the American Health Care Association’s Joe Warner Award for Patient Advocacy. I am honored. But I know that the real credit behind this award belongs to everyone
