Letter from the Publisher Dear Valued Readers, Welcome to the 25th edition of the Seniors Blue Book. With each passing edition the Seniors Blue Book continues to grow in size, accessibility, and information. I want to offer you my heart felt thanks for continuing to turn to and trust the Seniors Blue Book as your primary go to source for anything and everything that is related to seniors, caregivers and their well being. It is our mission to provide you with all possible options and opportunities that are available in an easy to read format. The guide is broken down into three main sections; Community Resources and Services, Home Health Care & Non-Medical Home Care Agencies and Senior Housing. We include definitions of these services and indicate which companies offer these in the appropriate Home Health and Housing Grids. We continue to offer the Solutions for Seniors resource help line at 973-560-0064 for those that need help navigating the information within our publication and website and to offer FREE assistance to those that wish to ask for our information assistance. SeniorsBlueBook.com is our dynamic website that will not only offer all of the information listed in the Seniors Blue Book, but empower the viewer to find local events and activities and post their own events and activities. The community calendars will be interactive and I am expecting avid participation from both seniors and their providers. SeniorsBlueBook.com will also be offering senior professionals their own senior calendar for networking opportunities, promotion of their own events and job opportunities. The community calendars are only a smidgen of the new information that will be available on SeniorsBlueBook.com. Please log on and check it out for yourself. I promise that you will bookmark SeniorsBlueBook.com and come back again and again. Thank you for your continued support and give me a call or an e-mail if you have any suggestions or comments that will further enhance the Seniors Blue Book.

First you should familiarize yourself with the many options available and determine which one is best for your needs. You will find definitions on each type of housing within the housing section. Then turn to our easy to read comparison grids. The grids are divided into 4 categories, depending on the type of community: Independent Living, Assisted Living, Alzheimer’s Assisted Living, and Skilled Nursing. The buildings are listed by county so you can zero in on the specific areas you are looking for.

DAILY MONEY MANAGEMENT A Daily Money Manager (DMM) provides personal assistance to seniors who have difficulty managing their financial paperwork. Among other things, a DMM can assist with bill paying, organization of bank, investment statements and tax documents, negotiating with creditors, budget assistance, understanding and reconciling medical insurance claims, Medicaid applications and Medicare choices. A DMM can identify pertinent issues and work with and make referrals to other professionals such as geriatric care manager, lawyer, financial advisor or tax counselor. Many DMMs are Certified Senior Advisors (CSAs) as well as notaries and can help seniors maintain independence by avoiding errors which might raise concern of loved ones.

EMERGENCY RESPONSE SYSTEMS A Personal Emergency Response System (PERS) is an electronic device designed to let you summon help in an emergency. A PERS has three components: a small radio transmitter (a help button carried or worn by the user); a console connected to the user’s telephone; and an emergency response center that monitors calls. When emergency help (medical, fire, or police) is needed, the PERS user presses the transmitter’s help button. It sends a radio signal to the console. The console automatically dials an emergency response center where the caller is identified. The center will try to determine the nature of the emergency. If the center cannot contact you or determine whether an emergency exists, it will alert emergency service providers to go to your home.

Hospice Care—The Myths. The Reality. Every culture, every community has its own set of myths and fears about terminal illness and the dying process. One of the challenges for hospice caregivers is to help dispel these myths and reassure patients and families about the realities of effective end of life care. Consider these mistaken notions about hospice care. The realities offer hope in place of sadness.
Myth #1: Hospice is a place. Reality: Hospice isn’t a place. It’s the treatment of physical,
social and emotional pain and symptoms near the end of life. Recent studies show that 90 percent of patients faced with a lifethreatening illness prefer to remain in their homes, in comfortable and familiar surroundings, surrounded by family and friends. Whether they live in their family home, an adult living community or a long term care facility, they want to age— and die—in place. Hospice respects that wish. Myth #2: Hospice means giving up. Reality: When medical treatments cannot cure a disease, the
VITAS interdisciplinary team of hospice professionals can do a great deal to control pain, reduce anxiety and provide medical, spiritual and emotional comfort to patients and their families so they can make the most of their time together.
VITAS brings hospice home in New Jersey. 800.723.3233 • VITAS.com

Myth #3: Hospice is for cancer patients.
Myth #4: Hospice is expensive. Reality: Families don’t have to spend themselves into poverty to
qualify for hospice coverage. The vast majority of hospice patients are elderly and, therefore, Medicare beneficiaries. The Medicare Hospice Benefit is all-inclusive—covering 100 percent of the cost of hospice care. There are no deductibles; there are no out-of-pocket expenses to patients or their loved ones. In most states Medicaid also provides hospice coverage. Most private insurance plans also include a hospice benefit.

Reality: Hospice provides care for adult and pediatric patients with a wide range of life-limiting illnesses, including but not limited to cancer, heart disease, stroke, lung disease, liver disease, kidney disease, multiple sclerosis, ALS, Alzheimer’s, Parkinson’s and AIDS.

A Senior Move Manager is a professional who specializes in assisting older adults and their families with the emotional and physical aspects of relocation and/or “aging in place”. Health at Home – Non-Skilled Care Agencies
SKILLED CARE DEFINITIONS (continued from page 72) Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.
NON-SKILLED CARE DEFINITIONS 24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver. 24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you. ACCREDITED – Accrediting agencies are professional groups that establish and monitor voluntary, industry wide standards to ensure that patients receive quality home health care services. The Commission on Accreditation (continued on next page) 86 • Please mention the Seniors Blue Book when you call our advertisers.
for Home Care, Inc. (CACH) and Community Health Accreditation Program (CHAP) are two of the accrediting agencies. ALZHEIMER’S DEMENTIA CARE – This refers to personnel of the agency that are specifically trained to provide care to those that are afflicted with Alzheimer’s or other dementia related problems. BONDED – Means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. FREE ASSESSMENT – Agencies that do not charge for the initial assessment of the client which is usually facilitated by a nurse or other professional capable of providing a detailed care plan. GO/JACC – In 2009 the Department of Health and Senior Services consolidated three Medicaid supported home and community based service programs into a single program known as Global Options (GO). GO is an income restricted program. JACC (Jersey Assistance for Community Caregiving) is a program similar to GO but for individuals with slightly higher income and not eligible for GO. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companionship and other household duties. These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. TRANSPORTATION – Rides to shopping, appointments and other outings. WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work. = Web Link on www.SeniorsBlueBook.com • Fall - Winter 2016 • 87

Lord your God is giving you.
As parents get older the commandment to Honor Your Mother and Your Father is put to the test. Adult children are often asked to step in and be helpful. How does an adult child help while maintaining the dignity and independence of the parent’s who put their heart and souls into raising them? Have you heard the term “helicopter parent”? That is a parent who “hovers” over their children overseeing everything despite the child’s ability to manage themselves and worse prevents the child from making mistakes and learning from them. In the senior living world, adult children are at risk of becoming “helicopter” children. Those are the children who are over involved in their parent’s lives and overprotect them. They “hover” by getting involved in areas that their parents can manage on their own and take over their parent’s life. Moreover, they become so frightened about their parent’s well being that they don’t allow them try out different ways of managing. Now the act of “helicoptering” comes from a place of wanting to be there for someone you love, our parent, mingled with our personalities. Adult child who have a “take charge” personality are especially
at risk of becoming a “helicopter child”. The “helicopter child” demoralizes and demeans the older person without intending to do so. I have heard residents at the senior community where I work and their children say that the roles are reversed as parents’ age. I take issue with that. While our parents may need a hand, they never become our children. Honoring your parent means never forgetting that they raised you, not the other way around. The balance between autonomy and support is a delicate one in any relationship. The helicopter relationship is not healthy and has these two concepts, support and autonomy out of balance. Now you have heard it takes two to tango. If you are the parent reading this, I would like to suggest two questions that you ask yourself before bringing your needs to the attention of your children. The first question that should be asked is what specific support do I need? The more specific you are with your children the easier it will be for your children to respond with that assistance and not take over. The second question is my children’s response respectful of my autonomy? Now if you happen to be an adult child reading this I will (continued on page 98)

Are You Honoring or “Hovering Over” Your Aging Mother and Father? (continued from page 95) suggest that in all situations you ask your parents what they specifically need. If your parent is unable to answer, then work to be specific yourself and not simply take over. Always ask yourself how you can be helpful while still respecting or “honoring” my parent’s autonomy. Parent-child relationships are such a central and important part of our lives forever. Like all relationships, it takes work and requires reflection and balance. Each time things change in either a parent’s or a child’s we are at risk for getting out of balance. Keep communication open, pick your battles, and set realistic expectations. Most important is
remembering to honor your parents as they age by offering support without infringing on their dignity. Patient Advocacy Is What If you meet anyone in our Drives Van Dyk profession who doesn’t consider themselves a patient advocate, Health Care then you just met someone who needs to consider a career change.
The simple truth is that we are all capable patient advocates. As owners and operators of care centers, we find unique ways to demonstrate our commitment to every patient. But there is no better example of that commitment than our frontline caregivers. Whether it’s a licensed nurse or a loving family member or friend, caregivers are the true heroes of our industry. The most significant contribution we can make to patient advocacy is taking care of our caregivers.
I remember my 4-year-old son declaring, “When Mommy is happy, everyone is happy.” Out of the mouths of babes! I have followed that wisdom throughout my career, firmly believing that when our staff is happy, our patients and families are happy. Nobody does it better than Disneyland, “The Happiest Place on Earth.” Have you wondered what led to Disney’s unparalleled success? A simple understanding that parents are happiest when their children are happy and safe. (continued on page 132 )

Qualified Income Trusts (Miller Trust)... As a Method to Become Eligible For Medicaid Medicaid is a program under which, needy individuals can obtain financial assistance with their health care costs. Medicaid Managed Care covers in home care, adult day care, assisted living, medical insurance and prescription coverage. However there is an income cap for eligibility. Until December 1, 2014, individuals were completely excluded from this program if their monthly income exceeded the posted limit (currently $2,199 including social security and pension). This created pressure on needy patients who might be able to get by on Managed Care to seek care at nursing homes, since the income limit is thousands of dollars higher. This did not make economic sense, since in home care and/or assisted living would cost the government far less money. As a result, beginning December 1, 2014 Qualified Income Trusts (QIT’s), were adopted by Medicaid in New Jersey. Basically anyone with income above the limit can now establish a QIT into which the excess income is deposited (and subsequently used to reimburse Medicaid) and thereby become eligible for all Medicaid programs, including nursing home coverage. The income placed in the Trust will not be considered for financial eligibility determination. The income deposited each month into the Trust must then be paid out each
month for the Medicaid beneficiary’s specified post-eligibility expenses. Due to the nature of the Trust, generally almost all income deposited into the Trust monthly will be depleted at the end of each month. An initial amount up to $20 can be deposited to open the Trust, if needed. After that, only income belonging to the Trust beneficiary can be deposited into the Trust. Thousands of people will now be eligible for these valuable government programs who were previously denied. Please make sure that you discuss this program with your elder law attorney or consultant. If you have any questions, don’t hesitate to call me, Michael Manna, at my office (201/447-2800) to review the process at no charge. Editor’s Note: This article was submitted by Michael A. Manna. He is a 1970 Magna Cum Laude graduate of Boston College (graduating 7th in his class) and a 1973 Cum Laude Graduate of Boston College Law School. After working in the tax department of the CPA firm of KPMG, Mr. Manna entered the private practice of law in 1975. Mr. Manna is the senior partner in his firm and is admitted to practice in New Jersey, New York and Massachusetts and is a member of the bar of the Supreme Court of the United States. He is also a member of the National Academy of Elder Law Attorneys and the Elder Law Committees for both the Bergen County Bar Association and the New Jersey State Bar Association. Patient Advocacy

So how does Disney create that experience over and over again? They focus on the Selection, Training and Caring of its employees. By recruiting the right people, giving them the best tools to succeed and reminding them that the company cares about them, Disney consistently creates a safe and happy experience for people of all ages. If we are serious about our role as patient advocates, then we need to adopt this philosophy. It starts with our staff. We know what it takes to create a quality experience. Only the finest caregivers with all the right training and care from us will achieve this goal. Take a moment to observe the family members of prospective patients and residents who are touring your care centers. What are they looking for? As the child of a parent who presently requires long term care, I know that I’m happiest when my dad is safe and happy.
Sound familiar? Our caregivers ultimately steer our reputation – for better or worse. They may not appear on a balance sheet, but they are our most valuable assets. Remember how important they are, remind them of their value, and take excellent care of them. That’s the secret. This month I received the American Health Care Association’s Joe Warner Award for Patient Advocacy. I am honored. But I know that the real credit behind this award belongs to everyone with whom I work, day in and day out. I applaud them for making our patients and families truly happy. And I dedicate this award to them. Editors Note: Robert Van Dyk is the president and CEO of Van Dyk Health Care and past chairman of the American Health Care Association.
