The Denver Regional Council of Governments is the Area Agency on Aging for the eight-county metropolitan area. It is federally designated to provide services and information under the Older Americans Act. It plans, funds and provides services for older adults and individuals 18 and older with disabilities.

Aging and Disability Resources Options Counseling

Talk with a trained professional in the field of aging and disability resources. The options counselor will help you create a plan for meeting your basic needs and support decision-making processes in long-term care planning. Call 303-480-6700.

Network of Care is a comprehensive online community service directory for:

• individuals with intellectual and developmental disabilities

Ombudsmen

If you live in an assisted living residence or nursing home, or are a participant in the Program of All-Inclusive Care for the Elderly, you might need someone who can help you understand your rights. An Ombudsman advocates for the rights of PACE participants and people who live in assisted living residences and nursing homes.

An Ombudsman works with you to make sure you receive the care you want and need. They help residents of long-term care residences and Program for All-Inclusive Care for the Elderly participants maintain their quality of life and receive quality care. An Ombudsman can help you resolve issues related to your care, health, safety and rights. For more information call 303-480-6734.

Case Management

If you’re 60 or older, an Area Agency on Aging case manager may be able to provide one-on-one help. Staff will conduct assessments regarding your service needs, help you create a care plan and assist you in getting services in place so you can live safely and independently. Call 303-480-6700.

State Health Insurance Assistance Program (SHIP)

Do you currently have Medicare, or will you soon be eligible for Medicare? SHIP counselors provide unbiased information, Medicare counseling, information about your benefits and eligibility for cost-saving programs, and enrollment assistance at no cost. In addition, they are available to provide presentations on enrollment and fraud prevention to community groups. Call 303-480-6835 or visit drcog.org/ship.

Veteran Directed Care

Veteran Directed Care provides case management services for eligible veterans of any age. The Department of Veterans Affairs determines veteran eligibility to participate in the Veteran Directed Care program and gives veterans a budget to self-direct their care and pay trusted caregivers to help them continue living at home. Call 720-278-2351.

Choice Services Program

The Choice Services Program provides up to two hours a week of home care and can offer select types of free transportation assistance to qualified individuals 60 and older living in the Area Agency on Aging’s metro Denver service area. Call 303-480-6700.

Older Adult Refugees and Friends

The Older Adult Refugees and Friends Program reduces isolation and increases community connections for older adults from refugee and immigrant communities. The program’s services help older adults discover resources, make social connections, meet friends, get navigation support, and learn ways to stay healthy, well and independent. Multiple language services are available. For more information call 303-480-6700.

Community Options/Medicaid Transitions

Medicaid members 18 and older living in long-term care settings like nursing homes can receive one-on-one options counseling to learn about returning to community-based living with Medicaid supports and services to thrive and stay independent. Through options counseling, members may choose to enroll in Medicaid Transition Services which provides additional resources including life skills training, home-delivered meals, and peer mentorship. All Area Agency on Aging services can be provided in multiple languages.

ADULT DAY PROGRAMS

Adult day programs are for elders who need a place to stay during daytime hours because they have difficulty functioning alone (i.e. dementia or physical limitations). The programs offer purpose and stimulation. Most facilities place participants into groups with specific limitations.

Protecting Your Legacy

We work hard over a lifetime building a financial legacy.

Your legacy may entail property, business endeavors, investments, retirement accounts, collectibles, and jewelry, just to name a few. The details look different for everyone, yet we all have a need to plan for what happens with everything we’ve worked hard for when we pass it on to our spouse, children, family members, friends, a charity. Unfortunately, if we don’t have a plan the state makes one for us.

Two big concerns may be providing for a loved one with a disability or special need and how to ensure any gift you hand down is used in the manner you intend.

First, when you have loved ones with disabilities a lump sum from an inheritance could impact their ability to receive care. If they are accessing disability benefits (typically based on financial resources) inheriting a large amount of money can upset the balance of their accounts and kick them off existing benefit plans. Having a Special Needs Trust (SNT) as part of your comprehensive estate plan can protect any inheritance received by individuals with disabilities, allowing them to access funds for things the government wouldn’t otherwise pay for through public benefits. A SNT puts the trustee of your choice in the role of gatekeeper, allowing the individual

named to ask for financial support and be provided for without having an inheritance dwindle unnecessarily. Choosing a compassionate, financially wise person for this role is crucial.

A second way to protect your beneficiaries and ensure your financial legacy doesn’t end up part

of a divorce settlement, used for an addiction, or spent when creditors come calling, is creating a heritage trust (a specific type of Dynasty Trust). Setting up heritage trusts for your beneficiaries can preserve your financial legacy for multiple generations; protecting what you give your children (or grandchildren) from their creditors, lawsuits, divorce, and in some cases even estate taxes. ~

Is a 5-Star Rated Medicare Health Plan Right For You?

Some Medicare health plans have more star power than others.

The Centers for Medicare & Medicaid Services (CMS) rates Medicare health plans on a 5-star quality rating to help people make better informed decisions on their health care. Plans are rated on a 1 to 5 scale, a plan with 5 stars is considered excellent while a 1-star plan reflects a poor performance.

Every year, CMS rates Medicare Advantage Prescription Drug plans based on major categories that include preventive care, chronic care, prescription drug services, customer service, and member satisfaction.

CMS designed the rating system to help people compare Medicare health and drug plans based on quality and performance. This measurement tool can help identify the best-performing Medicare health plans available to you.

A plan with high ratings has demonstrated that it provides a high level of customer service and effective care for managing diabetes, heart disease and preventive care.

Quality ratings are one important way to evaluate a Medicare health plan. Affordability, coverage, conve-

nience, and seamless care are other key factors to consider. The value of coordinated care and coverage together puts you at the center of care to improve and simplify the care experience.

In Colorado, there are only a few select Medicare health plans that have the highest possible rating of 5 stars and offer affordability, convenience, and seamless care.

If there is a 5-star-rated Medicare health plan offered in your area, you may take advantage of a special enrollment period to join that plan. You can use this special enrollment period only once between December 8 and November 30 of the following year.

When choosing a Medicare health plan, the star quality ratings provide additional info to help choose the right Medicare health plan for you. ~

Medicare 2023 “In A Nutshell” (Parts A, B & Medigap Plans)

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Medicare Part A Hospital Insurance

Generally no cost (If you paid Medicare taxes while working)

$1,600 deductible (per benefit period)

It is possible to pay the $1600 deductible and other daily charges for multiple stays & extended care.

Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.

Providers are paid by Medicare assignment.

No coverage outside USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Medicare Part B Medical Insurance

Most people pay monthly $164.90 (Premium could be higher based on income)

Most doctors accept Medicare.

Medicare Advantage plans are governed by Medicare. They are only offered through insurance companies that have an annual contract with Medicare. They are NOT the same as Medigap policies. They must cover all Medicare approved services. Most Advantage plans include Part D prescription drug coverage.

Advantage Plans Part C

INSTEAD of a Medigap Plan (A-N)

Premium varies by company and coverage. (Same premium for all ages).

Part B premium $164.90 is still paid (Premium can be higher depending on income)

There are many plan types available: PPO, HMO, HSA, PFFS, etc.

Plans have co-pay and / or coinsurance for covered benefits & services.

Preventive care is covered 100%.

Plans have an annual out-of pocket maximum.

Plans may have additional services and discounts, including dental, vision, gym membership & more.

There is an Annual Election Period (AEP) which runs from October 15–December 7 of each year. This is the time to make changes in company or coverage.

Medicare Part D Drug Program

Premium varies by company and coverage. (Same premium for all ages).

Plans are generally a variation of the following: Deductible (Phase I)

$0–$505 deductible

Initial Coverage (Phase II) co-pay coverage until the total cost of the drugs = $4,660

Coverage Gap / Doughnut Hole (Phase III)

The member now pays much more for covered drugs 25% generic / 25% brand. There is a drug manufacture discount on brand name drugs that can count toward the “True out-of-pocket” (TrOOP) You remain here until the $7,400 (TrOOP) has been met

Catastrophic Coverage (Phase IV)

For the remaining calendar year, covered drugs are: $4.15 / $10.35 or 5% depending on drug tier As an upgrade, Insulin costs will decrese in 2023

Part D can be separate with Medigap & PFFS plans. There is a penalty for late enrollment into Part D.

Annual Election Period is Oct 15–Dec 7 each year

Must have Part A or B to enroll.

Stretching is Important for Seniors

Our bodies, like any machine, undergo many changes as we age, which can make staying active and mobile all the more challenging.

In addition to muscle and movement maintenance, stretching exercises can help delay the onset of diseases, such as diabetes and arthritis – it can even improve mental health.

No matter your level of activity, stretching exercises yield many health benefits for seniors ranging from injury prevention to mobility maintenance.

When To Use Stretching Exercises

For active seniors who routinely exercise, the best time to perform these stretches is at the end of the activity while your muscles are still warm and loose. For less mobile seniors, stretching can be done at virtually any time of the day. As with active seniors, make sure to walk around for a few minutes before stretching to ensure your muscles are loose and warm.

3 Ways to Enhance Your Walking Routine

Walking is one of the simplest and most effective ways for older adults to stay active and strengthen their muscles.

It is a low-impact exercise that can improve cardiovascular health, increase strength and flexibility, and reduce the risk of chronic diseases.

1. Start Slowly

We are all on our own paths when it comes to fitness, and if you are just getting into a walking routine, it is essential to start slowly and gradually increase the intensity of your walks. Walking for a few minutes every day can help you build endurance, strengthen your muscles, and improve your cardiovascular health. Once you feel comfortable walking for a certain period of time, you can increase the duration and intensity of your walks gradually. Start by adding a few minutes to your daily walk or increasing your walking speed.

2. Utilize a Fitness Tracker

Using a fitness tracker can be an excellent and fun way to track your progress and stay motivated. These devices can help you monitor the number of steps you take, the distance you cover, and the calories

you burn during your walks.

You can also set daily step goals and work to achieve them. In addition, most fitness trackers come with features that allow you to compete with friends and family members, which can also be a great tool for motivation.

3. Join or Create a Walking Group

Joining a walking group can be an excellent way to stay motivated and make new friends. Walking groups provide a supportive environment for individuals to socialize, share experiences, and stay accountable. You can look for local walking groups in your area or even connect with residents in your community who are looking to get more active.

People expect “home” to provide independence, accessibility, safety and peace of mind –regardless of disabilities, limitations or health challenges.

Many are choosing to stay in their homes for as long as possible to avoid moving to senior-care communities.

This movement isn’t restricted to people who are aging. It applies, as well, to patients with sudden health changes – due to accidents, stroke, spinal cord injury – as well as to those with degenerative conditions such as arthritis, amyotrophic laterals sclerosis (ALS), dementia, muscular dystrophy, multiple sclerosis or Parkinson’s Disease.

If you share the desire to remain at home, will the house where you are living sustain your independence for years to come? As your mobility diminishes, what home modifications and changes would be essential to ensure accessibility and safety?

Great design should be part of every home, and two schools of thought have emerged. Universal Design dramatically improves the use by everyone, children, people with disabilities, the aging population, and everyone in between. However, Accessible Design is amazingly personal, and can create a personal environment specific for your needs. Which one is better and how are universal design and accessible design different?

Differences Between Universal Design and Accessible Design

In your home, it comes down to the degree of personalization. By nature, Universal Design is improving home for use by everyone vs. your individual use. You may choose to use some of both to make your home special.Many of these concepts will help your home be easier to use. But Accessible Design goes farther, and can really future proof your home. These can also be called accessible home modifications.

Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other

24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

ALZHEIMER’S/DEMENTIA CARE –

Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION –

Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button.

MEDICATION REMINDER –

Assistance with reminding an individual about medications but not dispensing medications.

TRANSPORTATION – Rides to shopping, appointments and other outings.

WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work. ~

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to

health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) –

Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s

veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE –Assistance with Activities of

Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.

SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.

Respiratory Therapy – Refers to services for care of patients with

It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care

administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Palliative Care

Your palliative care nurse works with your physicians and specialists. Hospice Care

Hospice care isn’t just for cancer patients or seniors. It provides emotional, spiritual, and physical comfort to anyone with a life-limiting illness. Wherever a patient calls home, hospice care comes to you. Hospice caregivers also deliver medications, medical equipment, and supplies.

Additional Services

When you think of healthcare, you rarely consider the extra help you

may need to get through your day. Volunteer, chaplain, and bereavement services go above and beyond normal service offerings. In finding the care provider that’s right for you, be sure to ask about all of their services and compare multiple providers.

Covered Expenses

Medicare or Medicaid cover almost all palliative and hospice expenses, while private insurance picks up the rest. If your condition improves, you can transfer from hospice to palliative care.

Selecting healthcare services can feel daunting. Remember, you are more than a patient. You are a person. You deserve to be treated with dignity and respect at every stage in your life. N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed

SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs

CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care:

RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is respon-

sible for financial arrangements with the Long-term care facility.

GIP – General In-Patient Care –Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

We’ve all heard this statement for years. The first time I heard it the reference was raising children.

As the father of 12 children, I had to pause and think about that for a minute. While I didn’t really like the idea of a “village” raising my children, upon reflection, I came to an obvious conclusion that it took not just my wife and I to raise our “tribe” but also grandparents, aunts and uncles, cousins, a church, teachers, friends, doctors, lawyers, siblings, etc. That just might be considered a “village” alright!

Now that the children are “mostly” grown, I have been given the opportunity to experience the next phase of my life: caring for aging parents. Just as I didn’t realize how much help would be needed to raise my children, it previously never entered my mind that I would need so much help caring for my mother and father as they aged.

Mom has dementia at age 92 and Dad, also 92, fell and broke his hip in their living room. Off to the hospital for Dad. My brother who lived in another state had to drive home to care for Mom (she couldn’t

function alone). My sister had to go to the hospital for Dad several times a week although she lived 2 hours away. Then we had to find an assisted living arrangement for the both of them. Doctors, nurses, caregivers, family, church, neighbors and my siblings all got involved in the process of caring for my parents. IT TOOK A VILLAGE!!!

Fortunately, we knew about senior placement companies. Not the kind you find online that just sends out emails. I’m talking about local companies; the kind that gives you “hands on” personal service to find the right fit for your loved one. They help you unravel the complex issues of health care, geography and finances to give your loved one the best quality of life possible in their “golden” years. These companies are a free service! Be sure you include them in your “village” caring for your loved one. ~

Cindy was unable to work because of her daily visits to her father. She made sure that he would eat and was safe. That experience took a toll on Cindy, her husband, and two young children.

They found Jaxpointe at Flower Ct. and moved Tommie two and a half years ago. It is the perfect environment for Tommie to call home for the rest of his life. Parkinson’s disease is a movement disorder that impairs balance and coordination, slows movement, and leads to tremors and stiffness of the limbs.

Equally as important are less visible symptoms such as a weak voice, loss of facial expression, depression, pain, fatigue, anxiety and slowed thinking. More symptoms can occur as time goes on, and existing symptoms usually worsen. Parkinson’s is a chronic and progressive disease, and these symptoms vary from person to person.

There is currently no cure for the disease, but there are many treatments available to improve quality of life including: medication, surgical interventions, speech therapy and regular exercise. Specifically, exercise is understood to be critical in easing the symptoms and progression.

Parkinson’s is caused by the loss of dopamine producing cells in specific parts of the brain and affects more than one million people in the U.S. with approximately 25,000 of these individuals living in Colorado. While the average age of a Parkinson’s diagnosis is 65, it may occur in individuals as young as 30.

n Parkinson’s disease is second only to Alzheimer’s as a degenerative neurological disorder. In the United States, Parkinson’s disease affects more people than the total of those living with multiple sclerosis, muscular dystrophy, and Lou Gehrigs’ disease (ALS).

n Medications can help control to the symptoms of Parkinson’s disease, but there is currently no cure.

RETIREMENT COMMUNITY –

Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces.

SECURED UNIT – Dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SHORT-TERM CARE – Acute care provided on a short term basis.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which

are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

SKILLED NURSING FACILITY –

Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. ~
