Stress, you have it, I have it, we all have it and it doesn’t look the same for everyone. Stress can look like emotional distress that can be seen by others: the agitation, anger, or tremors. Or maybe your stress is more subtle, maybe you don’t even think of it as stress, such as you can no longer handle certain types of foods, whether it’s nightshades, gluten, corn, or wheat, those are all signs of stress on the body. More dramatic cases of stress can lead to diseases; heart disease, blood sugar disorders from lack of a proper diet, due to stress not eating properly, or other life threatening conditions. So what can be done for stress? There are options: meditation, relaxation, a vacation, exercise or more direct therapies such as Emotional Freedom Technique (aka Tapping), journaling, and biofeedback. What we see too much of when dealing with stress is resorting to drugs. Some drugs lead to addictions and side effects, and more problems rather than solutions.

Most drugs only mask the real problem. So why does it seem younger people are more active, vital, and interested in life? What happens to us? As we go through life we compound stress, one stressor added to another. Advantage Plans Advantage Plans allow a private insurance company to take on the risk and administer the health benefits of a Medicare participant instead of the government. The insurance company receives a reimbursement from the government which funds the Medicare Advantage plan. Many Medicare Advantage plans offer prescription drug coverage. Advantage plans generally carry a low monthly premium, even $0. Advantage plans carry small, predictable co-pays such as $5-$35 for a doctor’s visit. Advantage plans also offer additional coverage that original Medicare and supplements may not. These may include a gym membership, limited dental and vision coverage, and podiatry services. (Part C Advantage Plans & Part D) Medicare Advantage plans are governed by Medicare. They are only offered through insurance companies that have an annual contract with Medicare. They are NOT the same as Medigap policies. They must cover all Medicare approved services. Most Advantage plans include Part D prescription drug coverage. Deductible (Phase I) $0 - $400 deductible Initial Coverage (Phase II) co-pay coverage until the total cost of the drugs = $3,750

Plans have an annual out-of pocket maximum.

Coverage Gap (Phase III) The member now pays almost all costs: 51% coinsurance or co-pay for covered generic drugs. 40% coinsurance for covered brand name drugs. You stay here until member has paid $4,950

Plans may have additional services and discounts, including dental, vision, gym membership & more.

Catastrophic Coverage (Phase IV) For the remaining calendar year, covered drugs are: $3.30 / $8.25 or 5% depending on drug tier.

There is an Annual Election Period (AEP) which runs from October 15 - December 7 of each year. This is time to make changes in company or coverage.

Part D can be separate with medigap & PFFS plans. There is a penalty for late enrollment into Part D.

Encouraging Independence Essential Seniors who can do for themselves to Senior’s Quality are typically more content with of Life life than those who depend fully on others.

In-home senior care services provide compassionate care and encourage seniors to stay as active as possible and do as much for themselves as they are able. A truly interactive approach to caregiving can offer seniors companionship as well as assistance with those activities of daily living that can be challenging – for example; getting in and out of bed or standing from a chair. This approach encourages seniors to stay physically active. And the Agency for Healthcare Research and Quality says that physical activity preserves and improves seniors’ ability to function well and maintain independence, even when they have health problems. In addition to encouraging seniors to stay involved in household tasks like housecleaning, laundry and meal preparation, in-home care provides opportunities to get regular physical exercise, such as walking, and mental exercise, such as playing games, working on puzzles and staying engaged in hobbies. Physical exercise contributes to seniors’ independence in a variety

of ways. It increases metabolism to control weight and helps build muscle mass, which further enhances the ability to burn calories and lose weight—which in turn makes it easier to stay physically active. Physical activity also positively impacts control of health conditions such as high blood pressure and diabetes. It improves immune function, which reduces the risk of acquiring infections. It also promotes heart health, increases bone density, and enhances mobility, flexibility and balance—which all contribute to lowering seniors’ risk of falls. Additionally, improved physical health contributes to improved self-esteem, self-confidence, mental health and ability to sleep. Activity, physical and mental, boosts brain function, preventing memory loss, slowing cognitive decline and reducing the risk of dementia. To sum it all up, encouraging your senior loved one to live as independently as possible—and providing him or her the assistance needed for safe, in-home living— boosts quality of life. ~

spouses could conserve retirement dollars by receiving a pension benefit for health related expenses.

Those that served the country deserve the benefits they’ve earned to remain independent in their homes. The Aid and Attendance VA pension provides money to pay for assistance with everyday activities of daily living (ADL’s). The pension is paid monthly to the Veteran or Widow to use at their discretion for the rest of their life. In five years this pension may provide up to $127,200, tax free, to help Veterans and their Widows pay medical expenses. The basic requirements for the standard pension are:

Individuals who exceed VA regulation asset limits may need the services of a VA Accredited Estate

Attorney to receive the pension, while most veterans and widows will qualify without any attorney assistance. You do not need to be impoverished as this pension is not a welfare program. The application is filed by Veteran Service Officers (VSO’s) who are versed in the application process and whom also interact with the VA as your advocate. Financial Companies that specialize in assisting Veterans and Spouses with Home Care expenses can cover the in-home care costs prior to the pension starting, and will work with the Veteran to keep them enrolled for the duration of their lives. This private financial assistance program allows for care to begin within days of the application being fully submitted and before the VA determines eligibility, which can take up to 12 months. These financial services carry no risks, costs or obligations to Veterans or Widows. This financial program has been reviewed by the VA and has been operating nationally for years. Typically lasting 21 days or less, short-term rehab can be extremely beneficial following a surgery or hospitalization after an injury, stroke or major medical issue. One big benefit of short-term rehab is that it can target a variety of needs. Many programs offer physical, occupational, speech, respiratory and cardiac rehabilitation services. By offering a comprehensive array of therapies and rehabilitative services, short-term rehabs can address each patient’s unique needs. Another benefit of short-term rehab facilities is a more comfortable environment. Hospitals can often feel clinical, but the right rehab facility is clean and modern with a warm, comforting ambiance. Many rehab centers also offer amenities such as rehabilitation gyms, concierge services, private rooms and bathrooms, and cable TV and WiFi access. Staff is key to the rehabilitation experience, too. It’s essential they

are qualified and experienced, but it’s also important to be attentive, caring and empathetic. Employing a sufficient number of staff is critical as well, as health and recovery issues can arise when patients’ needs aren’t met promptly. Short-term rehab programs are often located within skilled nursing facilities, meaning that if a short-term rehab patient’s health takes a turn, they are able to easily move that patient into skilled nursing for more intensive care until a strong recovery is made. This puts less stress on the patient during a vulnerable time, as transitioning between facilities can be difficult and disorienting.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain,

and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a 94 GREATER ORLANDO AREA – WINTER-SUMMER 2017

plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RESOURCES FOR AGING WELL

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility.

AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting. INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropri-

ate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
