Centura Center for Rehab

REHAbiLiTATiON HOSPiTALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Consider Dental Plans for Savings Dental insurance has a low annual limit on benefits-- typically $1200. AARP’s basic plan costs $478 per year, which gives you a maximum of $1000 for dental work. Or they have a plan with a $1350 maximum for $664 per year. This is typical of most stand alone dental insurance coverage. Though you usually have twice a year cleanings and exams that do not count against the maximum, are they really “free” when you are paying those types of premiums?

Medicare 2017 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Long Term Care Planning “A little bit of planning goes and Assistance a long way.” We’ve heard that quote our entire lives, and in the area of Long Term Care, a little bit of planning…. Even in a crisis goes a long way. It is estimated that over 40% of people over 65 will need some type of Long Term Care (LTC). Terms like Medicare, Medicaid, long term care insurance, trusts, life estates, transfers, gifts, annuities, and other strategies come to mind but having a qualified professional who can navigate a pathway to assistance based on your family’s situation is critical. It’s confusing, and to make matters worse, most of us don’t address long term care issues until it is fully upon us either as a caregiver for a parent, or a caregiver of a spouse. Colorado has many resources for help. One such resource for financial help is Health First Colorado. This is Colorado’s Medicaid Program. Most Colorado elderly feel that they don’t qualify for Medicaid because they have too much income or too many assets. In reality, many could qualify if they would engage a qualified Medicaid Planner, holding a Certified Medicaid Planner (CMP) designation. The role of a CMP is

to help an applicant identify problems in obtaining eligibility and solve them. The goal of the CMP is to build certainty around how assistance with paying for LTC. This valuable planning can alleviate the stress of financial uncertainty. The rules around Medicaid eligibility are readily available on line. However, it is the proper application of the guidelines that is difficult causing most to think they don’t qualify and therefore, overlook this valuable resource to assist in the costs of LTC. Planning will identify a legitimate means of preserving valuable or sentimental assets and income. Here in Colorado, to the surprise of many, Medicaid guidelines afford numerous opportunities to protect income and assets. Consulting with a CMP who specializes in LTC will help you and your family develop a successful pathway to LTC financial assistance.

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

We have prevented helplessness through the Lifeline medical alarm program. Our other programs have fed hungry elders, transported folks to medical appointments, grocery stores and in some cases, life-saving dialysis treatments. We have provided light housekeeping, Medicare counseling, volunteer opportunities, grandparent mentoring to children, and even mowed lawns. These are all necessary services that enable our SRDA family to age safely and comfortably in their homes. While these programs are bright spots in the lives that we impact, SRDA’s Active Adult Center is truly the “Silver Lining.” When Joseph H. Edwards generously gifted to the Pueblo Senior Community the senior center, his vision was to have a place where seniors could eat, meet, get off their seat and participate. Through the years, aging has evolved and for some, the 70’s are the new 60’s, 60’s the new 50’s, 50’s the new 40’s and what is the answer to the change? Healthier lifestyles that include activity is certainly one part of the equation. The SRDA’s Active Adult Cen-

ter located on the 2nd floor of the Joseph H. Edward’s Active Adult Center is buzzing with activity. We are doing our part in preventing chronic conditions through our comprehensive healthy living classes such as Seniors in Motion, Zumba, Tai Chi and Line Dancing. There is a fine array of treadmill, stair stepper and bicycle equipment available for cardio fitness. Art and computer classes help to round out the active lifestyle by exercising the brain through creativity and critical thinking.

Non Medical Home Care Definitions 24 HOUR CARE – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent

with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. MINIMUM HOURS PER VISIT – This refers to the minimum number of hours that an agency provides service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

Dignity is Found in the My father has always been a very Little Things capable man. He worked hard his whole life as a roofer, put three kids through university and believed that with hard work and focus, anyone could succeed. He is understandably, a proud man.

About 18 months ago, my siblings and I noticed that our dad was having difficulty performing even the simplest of daily tasks without someone available to assist him. We raised the hard topic of securing a company to provide in home care and through some very hard discussions with our dad, he was able to be vulnerable enough to share some of his feelings and fears. We kept what he shared in mind when we looked for and identified a provider company and we meet regularly with is caregivers to be sure they remember and are remaining sensitive to them as well. First, no matter how patient and graceful people are when helping him, my dad shared that he remains extremely embarrassed that he now needs help with simple things like putting on his socks or having a caregiver standing nearby when going to the bathroom. He shared that no matter how much patience and grace is given to him, he is always embarrassed and frustrated that he can no longer do these things on his own. He is frustrated that those things that he once could once do quickly and easily (brushing his teeth, lying down in bed, getting in/out of the car, etc…) now takes so much longer!

My dad went on to share that, because of this constant embarrassment and frustration, sometimes when he asks for things it comes out with a little more edge than he means and is a reflection on his own feelings and not anything his caregivers are or are not doing. Second, he shared how important it was that, even though some tasks would be quicker and easier for caregivers to do themselves, he still wants to do them. To be more direct, my dad shared that he needed to be able to do things like folding his own clothes or even making a simple lunch because, in some small way, he was reminded that he still had, on some level, the ability to take care of himself. That small, little feeling is incredibly important to him because it gives him dignity. So as you head into the New Year with fresh anticipation and perspective, we share these two simple thoughts with you. Little acts give dignity and sometimes the edge you experience with a loved one is just because they are embarrassed and a little frustrated that they need you so much. ~ Editor’s Note: This article was submitted by Todd Slechta. Home Health Care Definitions 24 HOUR CARE – Refers to “around the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver. CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance. NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for. PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.

THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the thera-peutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

Services Provided:

VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs

HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

Donnie said he joined the Navy because he could; he joined the Navy because he had to. “I was going to be drafted so I enlisted,” he said, shrugging off the realness. He was a young man employed by the Santa Fe Rail Road, working Saturday nights as a DJ. Donnie suddenly found himself in Adak, Ala. The Navy asked him what his interests were and he ended up running the Armed Forces radio station for the year he was there. After that Donnie was on a constellation ship for three years, patrolling the boarders in Vietnam. “I loved it; I was in charge,” he beamed. I asked him if he liked being in charge and he answered emphatically, “You bet I like being in charge! I don’t like having to answer to nobody,” stressing the nobody. After, he settled back into life in La Junta, Col, returning to the railroad. He worked in the yard as a master in charge of the switchman. After 41 years, he retired. I asked what led to the decision to be on hospice? “Donnie said no more hospitals,” his wife Barb said, referring to what she calls ‘The Episode,’ leading to a sequence of events. Barb described a blissful night to me, celebrating Donnie’s 71st birthday at his favorite restaurant; or so it began, after, Donnie

passed out in the car. He ended up in the ER that night; ICU; and eventually, extended care. Donald Hjelmstad was diagnosed with Chronic Obstruction Pulmonary Disease (COPD) and Congestive Heart Failure (CHF), originally believing, a year or two prior, that it was sleep apnea. “It was in the extended care facility that one of the nurses told us about palliative care,” said Barb. Palliative Care is for patients who are chronically ill. Nurses provide home visits, acting as a liaison between the patient and his physician, allowing him to stay safely in his home. Hospice is for those who have a terminal illness. Donnie started on palliative care and as his disease has progressed has been moved over to hospice care. Barb said knowing the doctors, nurses, the chaplain, has helped to make it an easy transition from palliative to hospice care. Donnie said the thing he misses the most is his freedom. I asked him if he could do anything he wanted to do right now, what would it be? “I’d go to Cripple Creek,” he said without hesitation. ~

Improving Quality of Life – With Hospice & Palliative Care continued phases of life. Hospice care focuses on all aspects of a patient’s life and well-being. Physical, social, emotional and spiritual services are provided in developing a care plan tailored to the patient’s individual needs. Emotional and spiritual support is also offered to the patient’s loved ones as well, including grief counseling. Palliative care is an approach that focuses on the relief of pain, symptoms and the emotional stress brought on by serious illness. Research published in the Journal of Pain and Symptom Management found that terminally-ill patients who received Hospice care lived on average 29 days longer than those who did not opt for hospice near the end of life. Seeking Hospice or Palliative services earlier, rather than later, allows patients and their families to take advantage of their options in seeking help. the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end

of life or other crises that requires a constant presence by a nurse or care team. 24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays.

AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting. INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. That Tough Conversation “Dad, I don’t think it is safe for you to drive anymore.” “Mom, have you considered Assisted Living? You are not eating very well and I noticed you did not take your medications again last night that I left out for you.” “Mom and Dad, the house is falling apart around you. Have you considered bringing in some help with yard work, repairs, and maybe housekeeping?” As an adult child who is helping to care for a parent, you might be facing a tough conversation with a loved one. There are ways to make those discussions easier and most of all, respectful, to those that have loved and cared for you over the years. Starting the conversation can be as easy as “Dad I am concerned about (fill in the blank)” You may find that they have been sharing some of these same concerns. First, be respectful of the aging process. Physical and cognitive changes can be scary, and sometimes hard to acknowledge and accept. Educate yourself on options available, from help at home with household chores, additional med-

ical care or assisted living options. Understand that you are possibly asking a loved one give up a portion of their life, whether it is driving, making meals, or even living in their home. You are possibly proposing that they close a chapter of their lives. Secondly, don’t ignore things such as not eating, failing to take medications or a change in how the home is kept. Do try to open up a conversation on what you are observing and share information you learned from educating yourself on options which could help them. Get help or guidance from professionals if appropriate. Lastly, understand that the choice, whatever it might be, is ultimately theirs to make. Asking a parent to accept help with cleaning, with cooking, or even help with bathing can be a big deal to them. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing pa-

tients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
