Centura Center for Rehab
REHAbiLiTATiON HOSPiTALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.
Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Adult Protective Services Adult Protective Services (APS) receives reports of abuse, neglect, and exploitation of at-risk adults, conducts an investigation into the allegations, and implements services to reduce and prevent further mistreatment. Colorado operates as a stated supervised, county administered system. County APS programs provide for the safety and protection of atrisk adults who are or are suspected to be, victims of mistreatment. At-risk adults are defined as persons age 18 or older who are susceptible to mistreatment or self-neglect because the adult is unable to perform or obtain services necessary for his or her health, safety, or welfare or because they lack sufficient understanding or to make or communicate responsible decisions concerning their person or affairs. Certain professionals are required by law (mandatory reporters) to report physical abuse, sexual abuse, caretaker neglect and exploitation
of at risk elders and at-risk adults with intellectual and developmental disabilities. An at-risk elder is any person 70 years of age or older. An at-risk adult with IDD is a person who is eighteen years of age or older who has an onset of an intellectual and developmental disability before the person is 22 years old. Mandatory reporters must make a report to local law enforcement within 24 hours of witnessing or becoming aware of the mistreatment. Failure to make a report or if you make a false report it is a class 3 misdemeanor. If charged and convicted you could receive a fine of up to $750, or six months in jail or both. An at-risk adult has the right to refuse services and APS assistance when they appear capable of understanding the consequences of doing so, he or she cannot be forced to accept any services. ~

No One Plans to Fail No one wakes up in the morning with a deliberate plan to fail, but I do believe many of us plan for failure by simply procrastinating because while we all know we will die it all seems so far off. Unfortunately, too often the unexpected arises, and families are left scrambling to figure out what to do for those they love. When my mother’s health became such that my father had to place her in long-term care I remember telling him how much the planning to care for her might cost. He told then he would gladly pay more than that for the problem to just go away. My parents had basic estate planning in place, but what they did not have was a plan for life and a plan for death. What is a plan for life? Well, it involves basic estate planning such as a will and powers of attorney, but it also includes contingency planning for the unexpected. For example, what is the plan if one spouse needs long-term care and the other does not? What happens if both require it? What happens if one spouse is in long-term care and the stay at home

spouse dies. What happens if in a situation such as the ones I’ve named the families are blended; who makes what decisions for whom? A plan for death is easier, but often neglected. We found out at my mother’s death that we did not have the first idea how to have my mom interred in the local VA cemetery, and we had to wade through a lot of misinformation until we got the right information. What to do then? Plan for the unexpected. Consult with the necessary professionals today so that the unexpected does not catch you unawares. The decisions you make today will affect your family tomorrow; help them to enact your wishes by giving them the tools they need to implement your plans for both life and death. Do You Know What To Do When It is surprising that even though the Someone Dies? one thing so certain in life’s cycle is death, many people don’t know what to do when that inevitable event occurs. Following is a short guide of what to do:
At the time of death: n C all 911, your local Coroner’s Office or your hospice professional if your loved one is enrolled with hospice. n C all family members or friends and ask for help in notifying others. n L ook through the decedent’s personal papers for burial information (i.e. funeral plan or written instructions). n C ontact a funeral home to arrange the pick up of the decedent. n C heck to see about pets. Day One or Two: n
M eet with the funeral home for further arrangements such as services, location, military involvement, club or organization

involvement, the obituary, burial plots and headstones. n O rder death certificates from the funeral home. It makes sense when you consider these facts: 1 in 10 Americans experience some degree of hearing loss. n Hearing loss can affect people of all ages — varying from mild to profound. n By the age of 65, 1 out of 3 people has a hearing loss. n Hearing loss may be mistaken for aloofness, confusion, personality changes or dementia. n Hearing loss is the 3rd most common physical condition after arthritis and heart disease. n Tinnitus (or ringing in the ears) can accompany hearing loss and may be as debilitating as hearing loss. n Because we hear with our brains, untreated hearing loss can lead to cognitive difficulties and increase the risk of developing dementia and Alzheimer’s. n
It is commonly accepted that brain exercises such as puzzles and Sudoku can help to keep our brain sharp. Now, the research from “Dementia Prevention, Intervention, and Care,” an article in the Lancet Commissions, July 20, 2017, 30 SOUTHERN COLORADO – SPRING 2019-2020
demonstrates that better hearing can help our brains stay healthier and sharper as well. Although most people experience some hearing loss as they get older, they tend to wait approximately 10 years before getting help. We can see now that 10 years is too long to wait. During those years, communication becomes more difficult resulting in an increased isolation from conversations and interaction with loved ones. The findings emphasize just how important being proactive is in addressing any hearing loss we may have. One reason people wait is that hearing loss tends to progress slowly and is often hard to recognize until it becomes extreme. A simple complimentary hearing exam is essential in promoting healthy hearing. It can help detect hearing loss early enough so your Audiologist is able to treat it successfully, preventing or reducing the likelihood of physical, mental and social health complications from affecting your quality of life. How to Preserve Assets When Faced With Long Term Care Costs Meet Charlie and Fran. They have three children, 9 grandchildren and 5 great grandchildren… so far!
They have hopes and dreams for their future generations and have worked hard to ensure their legacy is passed on. As they age, they realize that if either one of them needs Long Term Care, that their property and other assets could be at risk. They need accurate information they can trust so that if Long Term Care needs come on suddenly they are prepared and able to protect their assets. It is estimated that over 70% of people over 65 will need some type of Long Term Care(LTC) according to the US Department of Health & Human Services. Colorado has many resources for financial help like Health First Colorado, Colorado’s Medicaid Program and Veterans Aid and Attendance Pension that will help pay for in home care, assisted living and nursing facility care. Most Colorado elderly who need help paying for LTC feel that they will not qualify for these programs because they have too much income or too many assets. In reality, many could qualify if they had the information from a knowledgeable professional, holding a VA Accreditation or a Certified Medicaid Planner (CMP) designation. The problem is, that often plans for one program will conflict with good planning for the other. With uncertain care costs it is 36 SOUTHERN COLORADO – SPRING 2019-2020
valuable to have one person or firm who understands multiple programs for assistance to ensure that in planning for one solution, an applicant isn’t disqualifying themselves for future aid from other programs. Aid and Attendance is available only to veterans who have served during wartime. It has a cap to benefits and if costs of care exceed the VA maximum benefit, then other programs will offer a better solution. The rules of eligibility are readily available online. However, incomplete or inaccurate advice often causes people to think they must “spend down” to qualify for these programs. This simply is not true and can lead to an unnecessary liquidation of the assets people have worked a lifetime to obtain. There are legitimate means of preserving valuable or sentimental assets and income. Here in Colorado, to the surprise of many, following guidelines carefully afford numerous opportunities to protect income and assets. Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/ or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day. A Labyrinth of Long Term Need information for an aging Care Choices relative or yourself but don’t know where to start? The overall goal of the ADRC (Aging & Disability Resources for Colorado) program is to empower individuals to effectively navigate their health and other long-term support options through free, unbiased assistance.
Sometimes referred to as “one stop shops” or “no wrong door” systems, ADRCs address many of the frustrations consumers and their families experience when trying to find needed information, services, and supports. Through integration or coordination of existing aging and disability service systems, ADRC programs raise visibility about the full range of options that are available, provide objective information, advice, counseling and assistance, empowering people to make informed decisions about their long term supports, and help people more easily access public and private long term supports and services programs. The ADRC program is available to people age 60 and over and age 18 – 59 with a disability that have long-term care needs. INFORMATION & ASSISTANCE ADRC Resource Specialists are very knowledgeable about available programs and services to help meet your needs. Colorado Gerontological Society Offers Programs Medicare Mondays: These formal presentations from October to December provide attendees with information on Medicare changes, rules and premiums, along with a description of the drug and advantage players. Non Medical Home Care Definitions 24 HOUR CARE – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.
INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

WOUND CARE
LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance. NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

Because the type of care provided to the home health and hospice patients are similar, there is not much difference in the staff’s focus. However, caring partnerships acknowledge the difficulty of preparing for transition from rehab, curative care to comfort, end of life care while ensuring the transition is as smooth as possible. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.
TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~
HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides

Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. Taking Care of Grandpa Growing up in Southern Colorado, every summer my grandparents would pick up me and my siblings for a trip to their cabin in Creede. My grandfather is a World War II veteran whose birthday just happens to fall on July 6, so we’d celebrate the country he loves and his birthday all in the same week. This year, my grandfather turned 97 years old. He says his years at the cabin are over, although he’d like to make one more trip and catch one more trout. Recently while at work, I received a text message from a neighbor, saying my grandfather had been taken to the emergency room in an ambulance. He had felt dizzy for many days, but that day was worse than ever and as my aunt went over to visit, she decided to call 911. After a day full of tests and no new diagnosis, my grandfather was sent home with a follow-up appointment with his cardiologist. If you knew my grandfather, you’d know that his one goal for the last years of life is to stay at home. That’s probably why he chose his oldest granddaughter, who works in hospice, to be his medical durable power of attorney. As grandpa’s MDPOA, my goals are the same as his - to keep him in the home he loves as long as it is safe for him to be there. While my grandfather, even at 97, is not sick enough to qualify for hospice care, he benefits from our palliative care team, which visits him

at home. Being a disabled veteran, my grandfather qualifies for Veterans Administration benefits that include payment for palliative care services. Each week, his nurse visits him and checks his vitals, fills his medication planner and answers any health questions he has. The palliative care program has truly helped keep my grandfather independent. At some point, my grandfather will be within the last six months of his life and he can be enrolled in hospice care. At that time, he will have an even larger team of clinicians coming into his home and meeting his needs. I am hoping that my grandfather gets his wish and gets to pass away in the home he loves surrounded by his large family who loves him. If he has to be moved to a facility, I am comfortable knowing the same great hospice care he receives at home can follow him to the facility. Knowing what I do about the end of life, I am very aware that my grandfather is close to needing hospice services. I am familiar with the exceptional level of care he will receive and how much easier things will be for my 95-year-old grandmother, who is his primary caregiver. More than that, I know he will be treated with the dignity at the end of his life which is so important to him. After seeing firsthand how crucial hospice and palliative care services can be for a person, I encourage everyone to become more familiar with the hospice Medicare benefit.

I also saw an elderly veteran whose options, health, and control are slipping away from him. While there is uncertainty in the end, I am so thankful for the care he is guaranteed through the hospice Medicare benefit. I will always want the best end-of-life care for my grandpa, and you should settle for nothing less for yours. ~
HOSPICE CARE
Hospice care is available to anyone who has Medicare part A with zero deductibles or out of pocket expenses, meaning it can benefit everybody, regardless of the ability to pay. While a trip to the cabin did not happen this July, grandpa’s birthday was celebrated on my parents’ back porch with our whole family along to take part in the celebration. As I watched my grandfather celebrate the milestone of his 97th birthday, I saw a boy heading off to war and coming back disabled. I saw a Southeastern Colorado farmer who worked so hard his entire life. I saw a grandpa who taught his grandchildren to fish and so much more.

The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.
AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious
illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Mom’s Big Decision (An I wouldn’t say my mother was illustrative story) ever a social butterfly, but since Dad’s passing the home health nurse and physical therapist had become the week’s highlights.
We just could never get there often enough! Then the pneumonia and fall in the bathroom last winter scared us all. My brother said there was no way Mom could afford a senior home because a coworker told him what it cost for his dad. I did an internet search and made a few calls but there were so many types, terms, and care levels that I gave up. My brother and I both have full time jobs! A few weeks later I had coffee with a friend whose elderly neighbor just moved into a retirement community. They had an advisor who helped the family discover the right place for free. My call to the advisor gave me incredible hope! He came to Mom’s home and asked about her health, where we lived, Mom’s lifestyle before last winter, her finances, and just listened to our story. Right then, the advisor described some communities that seemed exactly what Mom needed. I was totally surprised how Mom responded – it was like she was catching the vision of what it would be like to share everyday life with

others again! We toured four assisted living communities, met staff, observed resident activities, viewed units, and sometimes had lunch. We returned for a second and even one third tour. Our advisor was with us on as many tours as we wanted, and he helped us compare our feelings, observations, and costs. Well, it has now been four months since Mom made the big decision. I am convinced this went so well because we just patiently gave her more information, showed her what assisted living looked like, and considered the little things that made a huge difference. The same home health nurse and physical therapist are still seeing her. Now we visit Mom and hear stories about who moved in, the lady’s son who builds rockets, and the teenager who plays violin and is coming this afternoon. RESOURCES FOR AGING WELL
ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.
DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.
AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.
INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)
ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.
INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.
ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.
DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

Senior Housing Definitions
INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Helping Individuals With The challenges faced by individuals Parkinson’s with Parkinson’s disease (PD), even within their own homes, can be Disease many and varied. PD is a disease that develops and worsens slowly over time. Symptoms range widely from motor symptoms (like tremors, coordination, balance, walking deficits, lack of facial expression, decreased vocal volume and inflection) to non-motor symptoms (like depression, anxiety, constipation, lethargy). Symptoms can vary greatly from person to person, depending on how long they have had the disease, what sorts of treatments they employ, as well as other disease processes and other factors which may impact them. Treatments which can be used to successfully manage the symptoms and may assist with slowing the disease progression include medication management, evidence-based, PD-specific exercise programs and possibly surgery. Skilled home care agencies working with these individuals are in the unique position of seeing exactly what challenges patients with PD and their care partners face in their homes. Symptoms can fluctuate, sometimes in an unpredictable manner. Skilled home health staff can help to improve safety and function within the home, as well as help with the transition back to the community.
Within the home, individuals with Parkinson’s disease may struggle with slow, small movements which can affect their ability to dress, bathe, move in bed and prepare food efficiently and safely. Deficits in walking caused by PD may make it difficult to negotiate changes in flooring, small spaces and doorways, and may also increase an individual’s risk for falls. Individuals with PD and their care partners may require community resources and/or may need to find additional caregiver help in the home and information regarding finances and planning for the future. Lack of facial expression, deficits in speech and decreased breath support may impact communication and individuals may have cognitive deficits. Correct and timely medication regimens are also integral to safe and efficient management of symptoms. Home health clinicians with specialized training coordinate closely with all team members to target the deficits associated with Parkinson’s disease. RNs, PTs, OTs, STs, MSWs and CNAs enable their patients with PD to meet their goals and maximize their potential. ~ Editor’s Note: This article was submitted by Susie Lefebvre, PT. Years ago Charles Kuralt had a news segment called “On the Road,” which aired on CBS Evening News. Charles would stop in a city, grab a phone book from a telephone booth, and let his finger randomly stop at a name. He would then meet said person and conduct a life interview with them, and my recall is that in every interview he uncovered a fascinating person with a fascinating story. It is with this approach that the staff at the Upper Arkansas Area Agency on Aging endeavor to work with our clients and their stories. Sometimes it is difficult, as time constraints, many tasks at hand, and client representation all need to be balanced. in a manner which reflects they’ve been heard, we really feel we are doing a good job.
