Who is PAAA?
The Pueblo Area Agency on Aging is established under Pueblo County Department of Human Services. PAAA’s mission is to provide a coordinated, comprehensive service delivery system that assists older citizens in maintaining independence, health and personal well-being. PAAA is funded under the Older Americans Act and contracts with various agencies in Pueblo to provide services to the community. Services by SRDA, First Light Home Care, Colorado Legal Services, Center Toward Self Reliance and the Cooperative Care NOW Program are possible from funding provided by the Pueblo Area Agency on Aging. The partnerships between PAAA and other agencies provide options and assistance to older adults within Pueblo County.

Options to Make Aging in As our population’s life expectancy Place Possible extends, it can be difficult to adapt to the changes that are occurring both mentally and physically.
Learning to cope with a new set of physical challenges like limited mobility, diminished sight, or a new disease diagnosis can have a significant impact on how you see yourself aging. What many people fail to realize is that you don’t have to give up your dream of aging in place. Overcoming Stairs The stairs that you once ran up with a full laundry basket are now a significant challenge because of less flexibility or even vision decline. Many people tell me that they simply avoid going upstairs to the bedroom and choose to sleep downstairs because they are afraid of falling. Others have said that they crawl or scoot up and down the stairs. This is not ideal for aging in place and unsafe in some cases. Instead of avoiding the stairs in your home or “scooting”, consider installing a stair lift. Many homeowners don’t consider adding this option because they don’t think it will work or the cost is too high. Most brands offer both straight and curved stair chair configurations and can go in most homes. They are

an affordable alternative to buying a new home or moving into an assisted living community. How To Get Medicare Right Medicare can be confusing. Get the First Time your Medicare right the first time by avoiding these common mistakes.

Compare Both Medigap & Medicare Advantage Options – There are two unique options with Medicare and both systems have pros and cons. You should understand how both systems work, so you can make an informed decision. Don’t Just Choose the Same Plan Your Spouse or Friend Chose – Choosing your Medicare plan needs to be based on your individual needs such as your prescriptions, doctors, travel and Veterans Benefits. Understand In and Out of Network Coverage – It’s important to understand how claims are paid if you see a doctor that is out of network or need care outside of your local area. Claim All the Benefits You’re Entitled To – Apply for all local, state, federal and manufacturer programs that may reduce your premiums, eliminate the cost of Part B or reduce prescription and healthcare copays. Sign Up for Medicare When Eligible – Remember to talk to a local broker or Social Security 3 months prior to becoming Medicare eligible to determine if you should sign up for Medicare Part A, Part B or both. Missing deadlines can have

permanent penalties. Contest the High-Income Surcharge After Retiring – If you have been assessed an income related Part B or Part D surcharge and your income changes you may be able to appeal the premium surcharge. Do Not Contribute to an HSA Within 6 Months of Receiving Part A - You can’t contribute to a health savings account after you sign up for Medicare Part A and can incur a tax penalty for doing so. Seek Help - Contact a broker to go over your unique situation. Keep safety in mind. Guard your private information. Do not give out your Social Security or Medicare number over the phone. When meeting an agent meet at a public place or a local office rather than your home. Review Your Coverage Yearly – Medicare plans change benefits and coverage yearly. Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day. Pueblo Long-Term Ombudsman What is the Long-Term Care Program Ombudsman Program? The Pueblo Long-Term Care Ombudsman Program advocates for residents of skilled nursing homes, and licensed assisted living residences. The authority of the long-term care ombudsman program comes from the Older Americans Act, as well as from the Older Coloradans Act. The primary purpose of the Long-Term Care Ombudsman Program is to promote and protect the residents’ rights guaranteed these residents under federal and state law. We achieve this mission with a network of local offices across the state, which recruit, train, and manage teams of certified ombudsmen. Staff and volunteer ombudsmen visit long-term care facilities throughout the state to ensure residents’ rights are being upheld. Certified Long-Term Care Ombudsmen are trained to receive complaints and resolve problems in situations involving quality of care, use of restraints, transfer and discharge, abuse, and other aspects of resident dignity and rights. 24 HOUR CARE – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent
with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. MINIMUM HOURS PER VISIT – This refers to the minimum number of hours that an agency provides service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.
CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.
INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.
DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.
MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.

Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.
Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. espiratory Therapy – Refers to R services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is

provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting. INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their
on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare.

A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. As hospice providers we often hear this from families and patients. People are often reluctant to contact hospice for support because they feel like they’re giving up. Many times they call too late, so their loved ones miss out on all the benefits of hospice care. Did you know: n
Hospice gives you control. Calling hospice empowers you and your loved ones with choices regarding what you want, and what is most important to you, at the end of life. Together, we design the care you want, in the manner you choose, on your terms.

Hospice gives you choice. Your choices and goals for your plan of care may change over time. While you have to qualify for hospice, you can opt out at any time. You can opt back in later, as long as you qualify.
n
H ospice benefits the entire family. Hospice care benefits include a variety of support services for you and your loved ones, including: pain and symptom management, education, emotional and spiritual support, help with the patient’s personal care and hygiene, and respite care to give a family caregiver a break of up to 5 days.

M ost hospice care occurs at home. Hospice is not a place, but a philosophy of care that is provided in the place you call home, surrounded by the people and things you love. The care team comes to you — in your home, along with the equipment needed, medications and supplies, all covered under the hospice benefit. Inpatient care can be provided, if needed.

Hospice is a covered benefit. Hospice is covered by Medicare, Medicaid and most private insurance plans.

Who is eligible for hospice? Patients who choose to no longer receive treatments to cure their life-limiting illness, but instead choose care to improve their comfort and quality of life, may be eligible for hospice. Typical diagnoses may include lifelimiting illnesses such as cancer, end-stage heart, lung or renal disease, dementia, ALS, stroke, and neurological disorders, among others.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.
PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.
INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible. INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

Embrace, Prepare and Plan Aging in America has never been a popular subject. We know it happens, but we never really want to think or talk about it. When we do, the conversation usually revolves around how to hide the signs of aging instead of how we can embrace and prepare for the next phase of life. Believe me, life can be enjoyed through the end, I’ve seen joyful aging in action. However, education and preparation is pivotal to understanding how to age well and have the best care and quality of life.
With new technology and a growing older population, the industry is expanding and evolving by leaps and bounds. You need to be proactive, research and plan for the inevitable, not avoid it. The better the research the better the outcome.
For instance, do you know the difference between a Nursing facility (medical model of care), an Assisted Living facility (non-medical model of care, but can provide some healthcare services) and a Personal Care Home (non-medical model of care)? Are you aware that Therapy Services can be provided in your home? Did you know that Home Care, Home Health and Hospice are similar but offer distinctly different services that can be provided anywhere you call home?
Thankfully the days of the “Old Folks Home” are gone, replaced by vibrant senior living communities that focus on hospitality, wellness and aging gracefully with dignity and independence, but you need to make sure the community is the right fit for you. All communities are dynamic and have their own living culture regardless of whether they are owned by a large corporation or a small regional provider. You need to tour, ask questions of residents and staff, and attend a meal and/or activity. The same due diligence applies to researching home care agencies. Research and prepare now. Form a plan. Do not put off for tomorrow, what you need to do today. If you wait until you need the help your options may become significantly limited. ~
As an industry, Long Term Care has not done the best job educating our future consumers on what is available to them as they age.

Finding Your Home Away From Home The lives of our family, and in particular our mother (Josephine Giarratano), have been touched and impacted by the health care industry and the people that tirelessly toil in this profession. That being said, as we reflect on the experiences of the past two years, we would like to express our gratitude to The Center at Park West. My mother has had the opportunity to literally rehabilitate 6-7 times over the course of that time period. She considers this facility and the people that have worked with her a “home away from home”, and we agree. The doctors, nurses, therapy staff, and support personnel individually and collectively have had a direct impact on returning my mother to a quality of life and living each time. We have been impressed by the genuine commitment to services, the day to day care, and the passion and professionalism that encompasses this care. When our family, including my mother, began the process of investigating care/rehab facilities, our number one concern and priority was safety and quality of care, in order to facilitate and enhance my mother’s quality of life. TEAMWORK is extremely important to our family, and we can attest that the philosophy of the Center champions and embodies TEAMWORK. The environment is SAFE-EFFECTIVE-EFFICIENT and

has a number of care providers in multiple areas of the facility. Patient engagement and satisfaction foster a trust and the ability to speak freely and openly to staff (a BIG plus for us), education during the pandemic and access was unfailing, and empathy was paramount for both immediate family and friends. Facility and Staff will be the biggest plus when doing your research. I empower those individuals and family members seeking a “home away from home” to reach out if you so choose and I will gladly address anything you may want to know or questions you may carry – I will leave my contact information with the facility. Josephine’s comments from her stays: “I was treated with care and respect by nurses, CNAs, Physical and Occupational Therapy members”, “enjoyed having my own room and that is was always clean”, helped me when I needed help”, “when I had appointments they took me to and from promptly“, “breakfast, lunch and dinner were healthy and served timely” and “OT and PT staff were wonderful, helped me get better and out of the hospital and home each time”. ~ Editor’s Note: This article was submitted by the staff at The Center at Park West.
