Can you imagine not being able to brush your own teeth? Perhaps it is due to arthritis, poor vision, or dementia. Your inability to perform routine and effective oral hygiene care leads to dental decay and likely periodontal (gum) disease. It can also contribute to an increased risk of heart disease, stroke, and aspiration pneumonia. Controlling your diabetes will be much more difficult. Oral hygiene care is more than routine grooming like brushing one’s hair, or shaving—it is infection control! Inflammation of the gums will negatively effect overall health. The mouth is connected to the body! The blood that is in the infected gum tissue is the same blood that is traveling to the heart. Many seniors have had the benefit of preventative dental care. They have their natural teeth! Who will take the responsibility for caring for them when they can no longer care for themselves? This responsibility falls on the family or caregiver. Basic steps can be taken to assist and encourage oral hygiene care. AT HOME Be prepared. Set up the toothbrush with a thin layer of toothpaste in the bathroom. Have a cup of water ready to rinse. (If rinsing is not possible, use a thin layer infant toothpaste gel—it is safe to swallow) Have the individual stand (if possible) or sit at the sink.

Hand the toothbrush to the individual and ask them to brush. If this is not possible, place the toothbrush in their dominant hand and have your hand over theirs and gently brush. Ask your dental professional (dentist or dental hygienist) what might work best for cleaning between the teeth. IN MEMORY CARE/ ASSISTED LIVING Ask who is providing oral hygiene care (brushing and between the teeth oral hygiene care) and how often. Once a day? Twice a day? What training did the caregiver receive? How is your loved one responding to this oral care? Are they cooperative? What steps are being taken to make it successful? One of the main reason seniors are hospitalized is aspiration pneumonia (AP.) AP is the inflammation of the lungs caused by breathing in foreign material. Where is that material coming from to “set up shop” in the lungs? Many times, the mouth! Hospitalization and antibiotics may be needed to prevent respiratory failure. One’s overall health depends on quality oral health! ~

to what you can learn and how you can grow at OLLI. Regular social events, lunch gatherings, a rotating art display in the Peck House gallery (Hattiesburg), and free public experiences help bring members from across the community together in a friendly atmosphere and at a cost that’s affordable for every budget. There’s an annual membership fee ($50 in Hattiesburg, $40 in Long Beach) that’s good for one year from the day you register. After that, you simply pay for the courses you want to take. Courses are as low as $8 and rarely exceed $50, though longer field trips can cost more depending on where they take you and for how long. Home Health Care Definitions HOME HEALTH AGENCIES – Refers to someone State licensed health care providers that offer both medical and non-medical assistance in the comfort of your home. Services include skilled nursing, occupational, physical and speech therapies, assistance with activities of daily living, homemaker services and transportation services, both local and long distance. PERSONAL CARE SERVICES – Services provided by a companion/sitter to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine activities such as cooking, cleaning, laundry, errands etc. Many agencies provide transportation to doctor visits, shopping etc, as well as recreational activities. They may not dispense medications. HOSPICE – a comprehensive program of comforting care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. Professional medical care is given, and sophisticated symptom relief provided. INTRAVENOUS THERAPY (IV THERAPY) – any medical treatments/medications that need to be introduced into the body via a patient’s veins including antibiotic and hydration therapy, pain management, chemotherapy, and central line care. Numerous IV Therapies can be provided in a patient’s home under supervision of a M.D.

RESPITE CARE – care that is shortterm or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the patient. SKILLED NURSING – a higher level of care where services are performed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver related to the disease process, treatment, health measures and medication management. SOCIAL WORKER – a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT) – A variety of services to assess, treat and rehabilitate patients to improve their readiness to perform daily living tasks. Speech Therapy – services assess and treat speech, language, cognitive skills or swallowing. Common treatments may range from changes in food consistency, physical strengthening exercises, repetitive practice & the use of audio-visual aids.


Physical Therapy – services for the treatment and the rehabilitation process to stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help lessen disability, pain and improve overall function and quality of life. Occupational Therapy – services to assess and treat patients to improve

Keeping the “Family Home” As Compared Many of us are in our 70’s, in to Living in a Retirefairly good health, have greatment Community grandchildren, and remain in our “family home” as we age gracefully. Yes, the mortgage is paid so we don’t have to pay that any longer, yet the cost of owning the home seems to keep going-up. In fact, in some cases the monthly cost of living your home is equal to what the mortgage note was. Not only is that so, but we incur physical and mental stress in paying the property taxes, the monthly exterminator and termite charges, the high utility bill, lawn maintenance, appliance repair and replacement, home security service, home repairs, . . . . did I leave-out anything? Perhaps the most concerning is the stress – that is, our lesser ability to withstand stress. Also, at our age, the natural resistance to change seems to thwart long-term decisions such as moving to a retirement community. Let’s look at the advantages of living in a retirement community. First of all, one monthly payment covers all of the aforementioned costs.

Secondly, the social dynamics are greatly improved as you interact with peers and experience new activities arranged by an activity director. Third, and perhaps most satisfying, you are in a safe, secure, comfortable environment, specifically designed for us as we age in place. When we need temporary assistance if perhaps we return from surgery, it’s available. When a higher level of care is needed, an assisted living community is on the same campus. When we compare the plusses and minuses of moving into a retirement community, the resistance to change is lowered. Non-nursing care is typically assisting individuals with personal grooming, bathing, meals, assistance with feeding, light housekeeping; as well as errands to pharmacy or grocery shopping. This non-nursing care is done by certified nursing assistants, home health aides or personal care attendants who can also assist with important mobility and functions which include ambulating, range of motion exercises, transfers in and out of bed or bathtub, and monitoring of vital signs. Private duty care can be for persons either on a temporary basis but are often more individualized and for those who require a continuous need. This type of care from non-nursing personnel has the ability to help with activities of daily living, work with people in a one-onone setting in their homes with the desire to improve care and maintain a professional relationship with the person and their family.

Who is responsible for payment of private duty services? Traditionally, private duty nursing was for Medicaid clients who require more individual and continuous care (lifelong). Most services are only on an intermittent basis, generally about two hour a day from once to three or more times a week, depending on the need. For Medicaid candidates, those individuals may be referred from private practice physicians, hospital discharge planners or can obtain information and approval from agencies such as the Area Agency on Aging in your local community. Most health care provider agencies will want to obtain as much information regarding the type and amount of care necessary to help with the day-to-day tasks. First, there is no age limit for private duty home care. Individuals may need assistance from a recent acute illness or surgery requiring several months of rehabilitation or someone who become physically unable to care for themselves. A private duty nursing agency will want to develop a service plan or plan of care with you or your loved one. That plan of care will revolve around what services is best for you. CCRC / LIFE CARE – Continuing Care Retirement Community otherwise known as Life Care, offers a campus-style environment that may include villas, apartments/condos, assisted living and skilled nursing services. An endowment fee may be required that entitles residents to use assisted or skilled services when needed throughout their lifetime. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed ALFs are prohibited by law to promote these services without a license.


RESOURCES FOR AGING WELL

MEMORY CARE – These communities are either licensed assisted living facilities that specialize in delivering care to residents with dementia or a unit of a skilled nursing facility. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Staff is usually specially trained. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care. Payor sources usually accepted are private pay or some insurance plans. RESPITE CARE – Provides a temporary break for caregivers by allowing a resident to have a short-term stay in a community that can meet their needs. Many assisted living communities and nursing homes have a respite care program. Residents typically stay from a week to a month, depending

on the situation, and receive all the services of the community to meet their needs. Respite care services are usually private-pay. SKILLED NURSING CARE – Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring a high level of need and routine services. Residents can have a private or a shared room. Meals are served in a dining area, activities are scheduled, and therapies are available as prescribed by a physician. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Most facilities provide rehabilitation following a hospital stay, and accept Medicare, Medicaid, and private pay. ~

5 Sneaky Signs You’re When your body’s fluid level gets Dehydrated low, these symptoms will tell you. You’re probably aware of the more obvious signs of dehydration: yellow urine, dry mouth, feeling thirsty. But the less-obvious signs are just as important and sometimes appear sooner, especially as you get older. The amount of water you need doesn’t increase as you age, but your body’s ability to conserve water is reduced and your sense of thirst weakens. So it’s important to drink even if you aren’t thirsty. “Sometimes my older patients just don’t feel like eating or drinking much during the day, so it’s crucial to encourage healthy eating and drinking patterns,” says Spencer Nadolsky, D.O., a family physician and author of The Fat Loss Prescription. “This is especially true in patients who are on diuretics for blood pressure or other conditions.” The importance of water can’t be overstated. It makes up roughly two-thirds of our body weight and is responsible for a variety of functions, including digestion, blood flow, and temperature regulation. It’s like oil to a machine. When your body is low on flu-

ids, all systems must work harder to function properly. This not only leaves you feeling fatigued, but if untreated, it can lead to dizziness when standing, kidney problems, or seizures due to electrolyte imbalances, Dr. Nadolsky says. That’s why it’s so important to prevent dehydration or catch it early. Here are five lesser-known signs you might be low on H2O. 1. YOU HAVE BAD BREATH When you’re dehydrated, your body secretes less fluid. You already know that means decreased urination, but it’s also true for tears and saliva. Saliva is antibacterial, so if you’re not producing enough, it can lead to bacteria overgrowth in your mouth. That means bad breath. It’s also why so many of us experience morning breath, as saliva flow almost stops completely while we sleep. If you suddenly have dragon breath for no apparent reason, try drinking more water regularly. That alone may clear it up. 2. YOU CRAVE SWEETS Dehydration can mask itself as hunger, especially in the form of sugar cravings. When you’re low

YOUR SKIN FEELS COLD AND DRY When you’re approaching severe dehydration, your body starts to limit blood flow to the skin, says Bruce Bushwick, M.D., a family physician and chair of the department of family medicine at Wellspan York Hospital. “We call it ‘shunting’ your blood from your skin to your internal organs,” he says. Your body is doing what it can to conserve whatever fluid is left—even stealing water from Peter to pay Paul. The skin is the first place to be robbed of water. 4. YOU FAIL THE PINCH TEST Reduced blood flow to the skin can also make it feel more doughlike and less elastic, Dr. Bushwick says. If you pinch the skin on the back of your hand and it doesn’t snap back as quickly as usual, it might mean you’re dehydrated. Just don’t expect your skin to rebound as quickly as it did in your twenties. 5. YOU FEEL LIGHTHEADED OR DIZZY Less water circulating in the body means less blood, too. This can lead

to lower blood pressure and cause you to feel lightheaded, faint, or dizzy. One of the key signs of dehydration-related dizziness is a sudden rush of lightheaded-ness when you stand up too quickly, a condition called orthostatic hypotension. Drinking plenty of fluids helps reduce these symptoms in most cases. But if a person is dehydrated to the point of mental confusion—or have experienced vomiting or diarrhea—it warrants immediate medical attention.

STAYING HYDRATED: YOUR ACTION PLAN There’s no set amount of water that every person needs. What’s best for you depends on the foods you’re eating, how much you exercise, and any health conditions you may have. Since thirst is less acute in older adults, try to drink before dehydration sets in. Keep a refillable water bottle with you whenever possible, and drink a full glass with every meal. Remember that fluid-filled foods (such as fruit and soup) as well as beverages (including tea and fruit juice) hydrate you, too. Listen to your body and watch for signs of dehydration—the five above plus more obvious ones like headache, fatigue, and yellow urine.
