Swing bed services include IV therapy, wound care, management of feeding tubes and tracheostomies, colostomy care, specialty beds, diabetes education, physical, occupational, speech and respiratory therapy. Medicare 2019 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Home Health Care Definitions HOME HEALTH AGENCIES – Refers to someone State licensed health care providers that offer both medical and non-medical assistance in the comfort of your home. Services include skilled nursing, occupational, physical and speech therapies, assistance with activities of daily living, homemaker services and transportation services, both local and long distance. PERSONAL CARE SERVICES – Services provided by a companion/sitter to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine activities such as cooking, cleaning, laundry, errands etc. Many agencies provide transportation to doctor visits, shopping etc, as well as recreational activities. They may not dispense medications. HOSPICE – a comprehensive program of comforting care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. Professional medical care is given, and sophisticated symptom relief provided. INTRAVENOUS THERAPY (IV THERAPY) – any medical treatments/medications that need to be introduced into the body via a patient’s veins including antibiotic and hydration therapy, pain management,

chemotherapy, and central line care. Numerous IV Therapies can be provided in a patient’s home under supervision of a M.D. RESPITE CARE – care that is shortterm or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the patient. SKILLED NURSING – a higher level of care where services are performed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver related to the disease process, treatment, health measures and medication management. SOCIAL WORKER – a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT) – A variety of services to assess, treat

Speech Therapy – services assess and treat speech, language, cognitive skills or swallowing. Common treatments may range from changes in food consistency, physical strengthening exercises, repetitive practice & the use of audio-visual aids. Physical Therapy – services for the treatment and the rehabilitation process to stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help

lessen disability, pain and improve overall function and quality of life. Occupational Therapy – services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Occupational therapists help clients to improve basic motor functions and reasoning abilities and compensate for permanent loss of function. WOUND CARE – diagnosis and treatment for ulcers, pressure soars and other wounds. Treatments may include but are not limited to irrigations & dressing. ~

SENIOR HOUSING DEFINTIONS

Senior Housing Definitions ASSISTED LIVING – Communities are typically apartment-style with kitchenettes and private baths. Also included in this category are residential area homes with an average of 5-8 residents. Some offer companion accommodations. Amenities vary. Services include meal plans, cable television, local phone, housekeeping, laundry, maintenance and transportation. Also provided is personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. CCRC / LIFE CARE – Continuing Care Retirement Community otherwise known as Life Care, offers a campus-style environment that may include villas, apartments/condos, assisted living and skilled nursing services. An endowment fee may be required that entitles residents to use assisted or skilled services when needed throughout their lifetime. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed ALFs are prohibited by law to promote these services without a license.

INDEPENDENT / RETIREMENT COMMUNITIES – Housing designed specifically for the elderly. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths. Services may include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; libraries, computer stations, and other social gathering spaces. Rates are monthly and based upon size/location of apartment. MEMORY CARE – These communities are either licensed assisted living facilities that specialize in delivering care to residents with dementia or a unit of a skilled nursing facility. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Staff is usually specially trained. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care. Payor sources usually accepted are private pay or some insurance plans.


RESOURCES FOR AGING WELL

SKILLED NURSING CARE – Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring

a high level of need and routine services. Residents can have a private or a shared room. Meals are served in a dining area, activities are scheduled, and therapies are available as prescribed by a physician. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Most facilities provide rehabilitation following a hospital stay, and accept Medicare, Medicaid, and private pay. ~

INDEPENDENT LIVING / RETIREMENT COMMUNITIES

RESPITE CARE – Provides a temporary break for caregivers by allowing a resident to have a short-term stay in a community that can meet their needs. Many assisted living communities and nursing homes have a respite care program. Residents typically stay from a week to a month, depending on the situation, and receive all the services of the community to meet their needs. Respite care services are usually private-pay.

Know the Difference in Inpatient You never know when you may be Rehabilitation for faced with the challenging task of Better Recovery being a caregiver for someone you love. Stroke, brain injury, hip fracture, Parkinson’s disease and many other conditions may require the need for inpatient rehabilitation. Knowing the difference between an inpatient rehabilitation hospital and a skilled nursing facility–or nursing home–can help you as a caregiver to make an informed decision in choosing the best rehabilitation program for your loved one. Making the right choice gives your loved one a greater chance of reclaiming independence for a higher quality of life and a return into the community. When comparing rehabilitation choices, it’s important to ask questions and compare notes from place to place. Questions like, how often is the patient seen by a physician? Do the nurses specialize in rehabilitative care? How advanced are the technologies that will be used? How much therapy will my loved one receive? Are rehabilitation teams experienced and experts in their field? By gaining this knowledge, you gain a clearer view of the benefits one place offers over another, boosting patient recovery to optimum levels. When you conduct your own research, you will know the difference before you choose a rehabilitation provider. Pay close attention to the following areas, key differences in many locations for treatment. SOUTHEAST MISSISSIPPI – WINTER-SUMMER 2019

Advanced technologies: New technologies and treatments consistently offer patients greater options for successful rehabilitation and recovery. Yet, some skilled living facilities do not offer these advancements, limiting progress and a successful return to independence. Frequent physician* visits: Patients are evaluated by a rehabilitation physician at least three times a week at a rehabilitation hospital, leading team meetings to discuss a patient’s progress in reaching goals of independence. 24/7 rehabilitation RNs: Patients have around-the-clock care from registered nurses that specialize in rehabilitative care. Three hours of therapy a day, five days a week: A rehabilitation hospital requires that patients receive at least three hours of therapy five days a week. Patients have their personalized therapies spread out throughout the day to improve appropriate skills including walking, swallowing, eating, cooking, grooming and dressing. A higher level of care in rehabilitation can make a proven difference for patient outcomes. ~ *The hospital provides access to independent physicians.

HELPING INDIVIDUALS WITH PARKINSON’S DISEASE

Helping Individuals With Parkinson’s The challenges faced by individuals Disease with Parkinson’s disease (PD), even within their own homes, can be many and varied. PD is a disease that develops and worsens slowly over time. Symptoms range widely from motor symptoms (like tremors, coordination, balance, walking deficits, lack of facial expression, decreased vocal volume and inflection) to non-motor symptoms (like depression, anxiety, constipation, lethargy). Symptoms can vary greatly from person to person, depending on how long they have had the disease, what sorts of treatments they employ, as well as other disease processes and other factors which may impact them. Treatments which can be used to successfully manage the symptoms and may assist with slowing the disease progression include medication management, evidence-based, PD-specific exercise programs and possibly surgery. Skilled home care agencies working with these individuals are in the unique position of seeing exactly what challenges patients with PD and their care partners face in their homes. Symptoms can fluctuate, sometimes in an unpredictable manner. Skilled home health staff can help to improve safety and function within the home, as well as help with the transition back to the community.

Within the home, individuals with Parkinson’s disease may struggle with slow, small movements which can affect their ability to dress, bathe, move in bed and prepare food efficiently and safely. Deficits in walking caused by PD may make it difficult to negotiate changes in flooring, small spaces and doorways, and may also increase an individual’s risk for falls. Individuals with PD and their care partners may require community resources and/or may need to find additional caregiver help in the home and information regarding finances and planning for the future. Lack of facial expression, deficits in speech and decreased breath support may impact communication and individuals may have cognitive deficits. Correct and timely medication regimens are also integral to safe and efficient management of symptoms. Home health clinicians with specialized training coordinate closely with all team members to target the deficits associated with Parkinson’s disease. RNs, PTs, OTs, STs, MSWs and CNAs enable their patients with PD to meet their goals and maximize their potential. ~ Editor’s Note: This article was submitted by Susie Lefebvre, PT.
