group therapy. During group therapy, licensed health professionals lead the group by setting appropriate topics for group discussion and encourage members of the group to participate. Group therapy is effective because a participant interacts with others in a structured setting that promotes a better understanding of geriatric behavioral health issues. Seniors share their thoughts and experiences, learning from each
other with the guidance of the group leader. Social interaction helps the senior to talk and socialize with other seniors who may have similar experiences. In addition to group therapy, the senior may also receive individual, family or marital therapy if ordered by the doctor. Sometimes these Outpatient Programs can meet a senior’s behavioral health needs without requiring more intensive care such as hospitalization. If a senior citizen’s behavioral health needs cannot be met on an
outpatient basis, more intensive services such as acute geriatric behavioral health treatment in a hospital are available. Each patient has a goal-oriented plan detailing how the hospital, patient and family will work together to reach treatment goals, resulting in discharge from the hospital. A senior may need this type of treatment due to thoughts of self-harm, not wanting to live, depression, anxiety, or medication management issues. One of the greatest benefits of acute in-patient care is the 24-hour controlled environment while behavioral health issues are addressed. How does someone go about requesting behavioral health care? The first step is a phone call to a geriatric behavioral health provider, and an assessment by a healthcare professional. Strict laws regarding rights to privacy protect all conversations and treatment by behavioral health professionals and health care facilities. Home Health Care Definitions HOME HEALTH AGENCIES – Refers to someone State licensed health care providers that offer both medical and non-medical assistance in the comfort of your home. Services include skilled nursing, occupational, physical and speech therapies, assistance with activities of daily living, homemaker services and transportation services, both local and long distance. PERSONAL CARE SERVICES – Services provided by a companion/sitter to be with your family member for a specified amount of time each day up to and including 24-hour supervi-

RESPITE CARE – care that is shortterm or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the patient. SKILLED NURSING – a higher level of care where services are performed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver related to the disease process, treatment, health measures and medication management. SOCIAL WORKER – a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family.
Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT) – A variety of services to assess, treat and rehabilitate patients to improve their readiness to perform daily living tasks. Speech Therapy – services assess and treat speech, language, cognitive skills or swallowing. Common treatments may range from changes in food consistency, physical strengthening exercises, repetitive practice & the use of audio-visual aids. Physical Therapy – services for the treatment and the rehabilitation process to stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help lessen disability, pain and improve overall function and quality of life.

Senior Housing Definitions ASSISTED LIVING – Communities are typically apartment-style with kitchenettes and private baths. Also included in this category are residential area homes with an average of 5-8 residents. Some offer companion accommodations. Amenities vary. Services include meal plans, cable television, local phone, housekeeping, laundry, maintenance and transportation. Also provided is personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. CCRC / LIFE CARE – Continuing Care Retirement Community otherwise known as Life Care, offers a campus-style environment that may include villas, apartments/ condos, assisted living and skilled nursing services. An endowment fee may be required that entitles residents to use assisted or skilled services when needed throughout their lifetime.
DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed ALFs are prohibited by law to promote these services without a license. INDEPENDENT / RETIREMENT COMMUNITIES – Housing designed specifically for the elderly. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths. Services may include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; libraries, computer stations, and other social gathering spaces. Rates are monthly and based upon size/location of apartment.

RESPITE CARE – Provides a temporary break for caregivers by allowing a resident to have a short-term stay in a community that can meet their needs. Many assisted living communities and nursing homes have a respite care program. Residents typically stay from a week to a month, depending on the situation,
and receive all the services of the community to meet their needs. Respite care services are usually private-pay. SKILLED NURSING CARE – Is provided by licensed nurses in nursing homes, 24-hrs, for residents with complex medical conditions requiring a high level of need and routine services. Residents can have a private or a shared room. Meals are served in a dining area, activities are scheduled, and therapies are available as prescribed by a physician. Some skilled nursing facilities have a separate secured area for residents with Alzheimer’s or other dementia conditions. “Long term care facilities” or “nursing centers” are synonymous terms for skilled nursing facilities. Most facilities provide rehabilitation following a hospital stay, and accept Medicare, Medicaid, and private pay. ~

No longer just a devoted daughter or son, you’re now what the professionals in the aging field call a “long-distance caregiver.” Thrust into what is often a new world of intricate responsibilities, you may find it hard to see the personal rewards ahead. But they are there, as is the help available to assist you on this caregiving journey. There is no one right way to be a caregiver; everyone’s situation is

Critical Information It will help you immensely if, before there is a crisis, your parent(s) provide you with information to locate their important records, phone numbers, email addresses and other essential contact information. If a crisis has already occurred, such as a stroke or traumatic brain injury, this information is still important to gather, but it may require more detective work on your part. n
Family Caregiver Alliance’s “Where to Find My Important Papers” will help you collect information which will simplify communication with government agencies such as Social Security or the Veterans Administration, help with banking and other financial transactions, and make speaking with your parent’s at-

torney, accountant and physician easier. Legal documents, such as Durable Power of Attorney for Health Care and Durable Power of Attorney for Asset Management can and should be prepared before a health condition makes it impossible for your parent to do so. Understanding Your Aging Parents: One of the keys to understanding our Seniors’ Top 10 parents as they age is to get a grasp Fears on the things that worry them. Our parents may not talk openly about these things, but you can be assured that they think about them—a lot. A recent study conducted for the Home Instead Senior Care network identified the top 10 things seniors fear most. 1. Loss of independence As parents age they begin to see this independence slowing slipping away as their cognitive or physical health deteriorates. It’s important they keep as much control over their lives as possible, so always try to ask if you can help or offer options instead of making major decisions for them. 2. Declining health Declining health of course means loss of independence and is often a tough subject to talk about. Your parents may reach a point where they need daily assistance with personal care or can no longer safely maintain their home but resist help because they fear they’ll lose their independence or home. 3. Running out of money Even those who have been prudent and have wisely put aside funds sometimes worry that they won’t have enough. They worry about what will happen to them—and the burden it could put on their loved ones.

It’s often hard for seniors to talk about money but you can ease into the conversation slowly. Make sure their basic expenses are covered then gradually open the discussion to their long-term financial plans. One conversation opener is to ask how they’d like their affairs handled if they become incapacitated to broach the subject. – 4. Not being able to live at home For most seniors, home is much more than the house they live in. It’s a place packed with memories. It’s familiar. It feels safe and it’s a huge part of their identity. Like most fears, talking about the subject will help alleviate the stress. Discuss future options such as hiring a live-in companion, downsizing, and senior housing and listen carefully to your parents’ opinions. 5. Death of a spouse or other family member Most seniors worry more about losing a loved one than they do their own death and fearing the loss of a caregiver can be extremely stressful. Discussing the possibility is the best answer because it allows your parent to look at the future more objectively.

7. Not being able to drive Giving up the car is a serious blow to seniors. It’s one more act of independence that they have to forfeit. They are no longer free to come and go as they please but have to depend on others. If it’s no longer safe for your parents to drive you should make sure they have reliable transportation, even if it’s just for a spur of the moment Sunday drive. 8. Isolation or loneliness Being alone increases feelings of being “unwanted.” Likewise, losing the ability to drive heightens the fear of isolation or loneliness and both can lead to depression. Whether it’s rotating family member visits or a network of neighbors it’s important that seniors get out and about and socialize regularly. 9. Strangers caring for them We all generally prefer to be around familiar faces. Having a stranger provide care (especially for intimate needs) is extremely uncomfortable. While having a family
member as the primary caregiver may be ideal it isn’t always feasible, so if your parent needs outside help you should vet the person carefully and make sure a family member is present until you’re sure your mom or dad is comfortable being alone with them. Listen carefully to any concerns your parent has and don’t discount their (or your) instincts if something feels “off.”

6. Inability to manage their own activities of daily living Perhaps nowhere is the loss of independence so acutely felt as in the inability to perform normal acts of daily living, such as eating, dressing, and bathing. Older adults fear losing control of their lives and requiring help in these areas is an unwelcome reminder of that. It’s important that your parent continue to do as much as possible even if it takes twice as long.

10. Fear of falling or getting hurt. Most seniors know that they are not as sure of foot as they once were. They know that if they fall or otherwise injure themselves that it will impact their ability to do things on their own. Many falls can be prevented so install hand rails, remove slipping hazards such as rugs, and take as many other precautions as possible. Check medications for side effects that may cause dizziness and keep on top of your parents’ health. It’s important to understand the fears our aging parents face. Often fears grow bigger when they’re not vocalized so it’s important to discuss them in an informal conversation.
