“What Are My Options for We hear this a lot when we begin the Long-term Care?” conversation about long term care planning.
We are not talking about planning for the assisted living or nursing home when we use terms like “Options Counseling” or long term services and supports. Options counseling is a person-centered process whereby individuals, family members and significant others are supported in developing a plan for addressing services and supports that align with their preferences, strengths, values and needs. Promoting informed decisions about long term services and supports is the primary goal of the ADRC (Aging and Disability Resources for Colorado). During the options counseling appointment the counselor will learn about the consumer’s preferences, values, current circumstances and potential supportive service needs. This conversation will help direct a plan that best meets the needs of the consumers and his/her caregivers. The counselor will provide public and private services and referrals as needed and assist the consumer in determining goals and a path for reaching them. We are blessed in Western Colorado with many resources to help older adults and adults with disabilities remain in their community, but people are often unaware of these

services until they are in a crisis. Options counseling allows us to develop a relationship and streamline access to the supports older adults need, the next step in the process is follow up with consumers to make sure the options they chose are working for them. Often times a minimal amount of support is enough to help someone stay in their home. Options counseling can also assist with advocacy, advance directives, caregiver support, estate planning, support groups, and benefits management. Understanding in advance the different care settings and when and why they may be appropriate will result in the best experience for all involved. The ADRC is an unbiased and trusted source of information for accessing long term care services and community resources. This is accomplished through an effective collaboration with many community partners. 70% of people turning 65 can expect to use some form of longterm-care during their lives, knowledge is power and having the necessary information will make important decisions a little easier. The western slope town of Grand Junction is home to not only 3 main hospitals, including a regional VA medical center, but also to the entire spectrum of care from support to pregnant mothers to an award winning Hospice organization and everything in between. This enables residents of all ages to benefit from the care not only for themselves but also for their aging parents. Being able to age in place has always been a priority to most people. Here in Grand Junction that can become a reality due to not only high tech advances but also to the many and varied services and agencies in the valley that can assure families of quality care in their own home. These services include the skilled (medical) and non-skilled (non-medical) agencies that can assist seniors from transportation, shopping and meal preparation, to the more intimate care such as assistance with dressing, bathing, skin care, and medication management. There are meals on wheels, home visits from the library, restaurant taxi service, bookkeeping and housekeeping services. All are focused on maintaining the senior’s

Special Needs Trusts Extra care and planning is needed to plan for disabled beneficiaries. Special Needs Trusts are Trusts which seek to keep Trust assets unavailable to a beneficiary for public benefits planning purposes. Typically there are three parties to any trust: 1) the Settlor also known sometimes as the Trustmaker - this person is the original owner of the property used to fund the trust and the creator of the trust 2) the Trustee - this person who manages the trust property and who must take care of the property (and the technical “owner” of the property) and finally 3) the Beneficiary - this is the person for whose benefit the trust is created. Public benefits programs seek to meet the needs of elderly and disabled individuals in our society. Common examples of public assistance programs include: Temporary Aid to Needy Families (TANF), medical assistance in the form of Medicaid, assistance with housing through the Housing Authority, and payments of certain cash benefits, including Supplemental Security Income (SSI), Aid to the Needy Disabled under state law, and Old Age Pension. All of these programs have one thing in common: means testing. Means testing comes in two forms. The first means test for most public assistance programs is to determine
whether or not the applicant has assets. An individual may not be eligible for public assistance if he or she has too many assets which are countable. In looking at eligibility for most forms of public assistance, the government will typically disregard certain assets, such as a primary residence and an automobile, and counts all of the remaining assets, such as cash and cash equivalents. This is the difference between disregarded assets and countable assets. Next, all means tested programs will consider the income of the applicant. Eligibility for a given program generally requires that the applicant’s income be below a set level. For instance, the income to receive long-term Medicaid services, of the Medicaid long-term care recipient cannot exceed $2,200.00 (approximately). Therefore, having assets in excess of the given resource limitations can pose a problem for continued eligibility. An individual can acquire assets based upon gifting from another person, or settling a personal injury claim, or receiving an inheritance.

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. Anti-Gravity Treadmill: Safe Option The Alter G Anti-Gravity Treadfor Rehabilitation mill is a unique tool that can be integrated into rehabilitation programs to encourage mobility in a safe and exciting way.
The anti-gravity treadmill can be used in rehab programs for patients with a wide range of ailments and diagnoses including arthritis, joint replacement, spinal surgery, hip fracture, neurological injury including stroke and TIA, general deconditioning due to illness, and can also be used to assist in fall prevention. The NASA-inspired Differential Air Pressure Technology allows patients to stand and ambulate in an “unweighted” environment up to 80% of their own body weigh which can than decrease discomfort, increase confidence, and improve early mobility for faster safe recovery of the patient. In particular, the treadmill allows the therapist to monitor and analyze a patient’s gait pattern while minimizing impact from ground as they walk. The goal is to create a fall-safe walking environment that facilitates im-

provement in neuromuscular control, balance, and stability. Therapists can objectively evaluate progress and set therapeutic goals that translate to improvement in functional ability. As a result, patients are able to increase their independence with daily tasks and improve their overall fitness level to be able to return to their prior level of living. With so many rehabilitation options available throughout the Western Slope, patients need to ask what technology and programs are available to best meet their needs. Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Inspiration and Community Caring for someone with Alzheifor Alzheimer’s mer’s or dementia can be one of the most challenging roles Caregivers any of us will ever face.
Alzheimer’s can be a very long course of illness. It is the only one of the ten top causes of death in America with no treatment, prevention or cure. It robs the person living with the disease of their memories and basic social skills, which can leave the caregiver feeling isolated even from the person they care for daily. In addition, studies have shown that caring for someone with Alzheimer’s can impact the caregiver’s health and cognitive function. To help address the unique challenges of Alzheimer’s and dementia caregivers, the nonprofit ClergyAgainstAlzheimer’s has produced a book of meditations, Seasons of Caring – available at www.seasonsofcaring.org where you can read excerpts of the book free of charge. Grouped by seasons of the year, the book’s interfaith meditations offer spiritual support and motivational insights to comfort and encourage caregivers. Edited by Dr. Daniel C. Potts, Seasons of Caring features watercolors by his father, Lester, who began painting after being diagnosed with Alzheimer’s. Dr. Potts writes, “Alzheimer’s and other dementias are no respecters of persons. In this cold, dark reality, how do we share the warm hope of spring? We share our stories of having been there before, of having found faith’s fire in our own cold reality.”
Another resource is the USAgainstAlzheimer’s Facebook Caregivers’ Community for those with Alzheimer’s, their family members and friends. Members share information and questions about caregiving and Alzheimer’s/dementia, share family updates and the stresses of caregiving, and provide advice, understanding and comfort to others. The group’s moderators have each been Alzheimer’s caregivers and maintain a warm, safe and supportive environment, offering advice from their personal experiences. In late 2014, Volunteers of America established a toll free, confidential referral help line in an effort to provide quick answers and become a trusted source for a variety of needs for our area’s older adults. The toll free number, 1-844-VOA4YOU is answered by an on call attendant or returned the same day if that operator is occupied. Callers can ask questions about health care including questions like: where to go for rehab, home health, adult day care, memory care, assisted living, or long term care. The caller can also inquire about senior meals programs, independent housing, family housing or veterans housing assistance. Questions about job openings or volunteering opportunities can also be inquired through the toll free number. If Volunteers of America is unable to provide the service, the attendant will attempt to connect you with another local service that can. The need arose due to the many services Volunteers of America operates being located in separate buildings throughout a two county
region. One easy to remember number provides individuals whom Volunteers of America serves with greater ease in locating exactly the help they are seeking. The organization has been in existence for 119 years and ever since it’s beginning has pledged to serve people and communities in need and create opportunities for people to experience the joy of serving others. Today, our services are delivered through a partnership of professional staff, volunteers and other community supporters. Volunteers of America measures its success in positive change in the lives of individuals and communities we serve. Our vision is to have a world where all people live in safety, with social, emotional and physical well-being, spiritual fulfillment, justice and hope.

ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers. MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button. MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications. TRANSPORTATION – Rides to shopping, appointments and other outings. WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work.

HOSPICE – Hospices provide individuals facing terminal illness
with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

Med-Alert – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance. Federal Benefits for Atomic Workers Ever since physicist Enrico Fermi Comes to Life with discovered the potential of nuclear fission in 1934, scientists and Local Company supporters have worked diligently to understand and harness the power. Illustrating the muscle and potential of this new technology, the United States carried out its first test of a nuclear bomb in the New Mexico desert in July 1945.
Many dedicated men and women since then have worked effortlessly to exploit this power in defense of the United States. Like any new discovery, we learn as we grow. Now, 70 years later, our nuclear program has produced many scientific advances thanks to the persistence of a generation of physicists, chemists, miners, bus drivers, welders, engineers, construction workers, and many other employees and subcontractors of the Department of Energy. Unfortunately, that growth spurt came at a high price for some. In 2000 President Bill Clinton signed into law the Energy Employees Occupational Illness Compensation Program Act (EEOICPA) which was designed to compensate individuals who worked in nuclear weapons production and as a result of occupational exposures contracted certain COLORADO’S WESTERN SLOPE - JULy 2015-2016
illnesses. Executive Order 13179 explains: “Since World War II hundreds of thousands of men and women have served their Nation in building its nuclear defense. In the course of their work, they overcame previously unimagined scientific and technical challenges. Local Media Alert for The very fear of your loved one with Lost Seniors: dementia wandering away from your home could become a reality. The Silver Alert “Experts report that 60 percent of people with Alzheimer’s disease will wander at some point during the progression of the disease, and half of those will become lost and separated from a loved one. Using the incident rate of one critical wanderer per year per 1,000 persons over the age of 65 (1998 Census figures), the expected total of critical wanderer incidents reported to local law enforcement comes to 31,000 cases a year. Research suggests that most people become lost within close proximity to their home,” as stated in the Report, LOST and… FOUND by the Alzheimer’s Foundation, June 2012. The State of Colorado, Department of Public Safety has recognized these staggering statistics and has established a Missing Senior Citizen and Person with Developmental Disabilities Alert Program (24-33.5-415.8) (individuals with dementia, Parkinsons, diabetes, etc.). This is very similar to the

Amber Alert Program, which alerts the public of any missing child. The programming procedure for The Silver Alert entails the family reporting the description of the missing senior along with the last know location to law enforcement. This data is relayed to participating radio and tv stations, and other media outlets in Colorado. The public notices will be placed along the media every 15 minutes for 2 hours thereafter on 30 min intervals until the individual is found. This alert program has been implemented in some variation nationwide to protect the most vulnerable elders. Keeping seniors safe and comfortable should be the number one priority for everyone!
