“Parenting the Second “My friend Mary and her new Time Around” husband were on their honeymoon when social services called to tell her that her infant granddaughter needed immediate placement, unless “grandma was willing to step up, the child would be placed in foster care. That was 10 years ago, they are all doing well, but life changed overnight for this newly married couple who thought they would retire in a few years.”

Life is what happens when you are busy making other plans; for nearly 34,000 grandparents in Colorado that statement could not be more true. Nearly 40% of those households receive no parenting help from the biological parents and 16% of those households are living in poverty.

But; it is not all bleak; statistically most of the children in these (non-traditional) homes are thriving under the loving care of their grandparents.

Second time around parents, have experience but there are challenges to starting over. There is support for these “new families” and resources a mouse click away. Locally, Health and Human Service, Mental Health, Partners, Hilltop Community Resources, and the school district are all good

resources. If you are over the age of 55, the Area Agency on Aging can assist with local resources.

If you have friends or family members who are new to this role they need your support.

This new life will look different for everyone involved but make an effort to help your friends stay connected and maintain their identity. Perhaps you can offer the grandparents some respite by spending time with the children so they can have a break.

Local and internet Kinship support groups, The Colorado Kinship Connection, Generations United, and Grandparents Raising Grandchildren are just some of the resources that are available. If there is not a kinship group contact your Area Agency on Aging or school district about starting one.

Navigating the World Navigating the world of Medicof Medicaid aid can be a daunting prospect.

There are differing varieties of Medicaid with a variety of different names and an even wider variety of differing requirements.

One of the most useful of the Medicaid programs for those who are elderly or disabled is Long Term Care Medicaid. There are actually a variety of different programs that can be accessed if Long Term Care is being received.

These can vary from Home and Community Based Services that provides services to allow someone to stay in their home to skilled care in nursing facilities.

Medicaid and Long Term Care have different financial requirements. Under the most common form of Medicaid, your maximum income for an individual is $1,305 per month. Under Long Term Care, the maximum income is $2,199 per month. Generally, under Medicaid your assets will not be used to determine eligibility. Under Long Term Care, while a lived-in home and one car is exempt, there is otherwise an asset cap of $2,000 for a single person, $3,000 for a married couple with both asking for assistance, or $119,000 for a married couple where only one spouse is requesting assistance.

In addition, to receive Long Term

Care services, the person requesting services will need to functionally qualify. This means that a case manager must assess them to determine whether or not they meet nursing facility level of care. Part of this process includes a request for information from the person requesting services’ doctor in order to assist in the process of determining whether or not the person qualifies.

The purpose of Long Term Care is not to get Medicaid just for the health care coverage, although that is an added benefit, rather the purpose is to provide services that will allow an individual to stay safely in their homes as long possible. Examples of services that can be provided include fall buttons, electronic med minders, and caregivers to assist in the home with a variety of needed tasks.

The best way to get further questions answered regarding these services is to contact your local ADRC for a discussion of your options.

Food Insecurity: A Growing Concern The USDA defines food insecurity as for Older Adults a state in which “consistent access to adequate food is limited by a lack of money and other resources at times during the year.” Other ways to think of this can include being hungry, faced by the threat of hunger or struggling to avoid hunger.

In the span of 10 years (2001 to 2011), the percentage of older adults experiencing food insecurity increased by 88 percent according to a study by Bread for the World organization. That’s an alarming statistic for our country which will have approximately 79 million seniors by the year 2040.

Many older adults experience a variety of situations that put them at risk of being hungry. They may become frail or weak and therefore do not travel out of the home as often to the grocery store or market. Extended family may have moved away and therefore the older adult has fewer people in his or her support network who could help with providing food. Incomes have been affected by the rising costs of medical treatments and they choose to buy prescriptions over food. If the older adult is home bound, meaning they no longer can drive, they must depend upon friends, neighbors, family or outside services to provide them with meals.

Senior CommUnity Meals, a program of Volunteers of America, has been providing community meal

dining and home bound meals to area older adults for the last seven years. The program serves nutritious meals to eligible older adults at seven community dining sites in Cedaredge, Hotchkiss, Paonia, Delta, Olathe, Norwood and Nucla.

The program also delivers hot meals to eligible homebound individuals in the cities listed above, plus Montrose. Last year, Senior CommUnity Meals served 112,000 meals to more than 2,600 older adults, covering 4,600 square miles.

The program could not operate without the help of volunteers.

Delivering a hot lunch to a few homebound seniors takes less than an hour. The volunteer may be the only other person a participant sees during the week. Volunteers also help out at the community dining sites with meal preparation, meal service, record keeping, cleanup and socialization. At present, this risk statically spans an average of three years with the cost of care ranging from $92,000, to $276,000 annually!

For couples, this number rises to nine in ten that at least one person will be impacted by the need for custodial or skilled care, ranging from assistance with activities of daily living (bathing, dressing, toileting, eating, transferring, and incontinence) or some form of cognitive impairment such as dementia and Alzheimer’s. Medicare does not cover the costs of Long Term Care.

Greater longevity, advances in medicine and medical science, means that our society will continue to age. Baby Boomers are now turning 65 at the rate of 10,000 per day.

Many have already lived this nightmare with parents and other family members, often having to place beloved family members in facilities funded by Medicaid but only after having to spend down their assets to a poverty level and relinquish their independence.

More than ever, there is now a choice to be had; an insurance policy that guards against families losing their hard saved earnings, and individuals losing their independence,

dignity, and access to quality care.

The long term care industry has been around since 1974, and several of the major carriers are launching some very creative solutions that address this pandemic issue as well as providing the public with choices that include linking long term care insurance benefits with those of life insurance and annuities.

Since health qualification is a hurdle that confounds a good number of applicants, it is best to look at this coverage as early as age 40. Younger ages not only bode well with passing underwriting criteria, but also keeps premiums at a very modest level.

So ask yourself, “If I insure myself against the minimal risks affecting my home, my car, and my overall acute health needs issues, why haven’t I insured against this far greater risk of an eventual Long Term Care event?”

The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Living Stress Free Stress, you have it, I have it, we all have it and it doesn’t look the same for everyone.

Stress can look like emotional distress that can be seen by others: the agitation, anger, or tremors. Or maybe your stress is more subtle, maybe you don’t even think of it as stress, such as you can no longer handle certain types of foods, whether it’s nightshades, gluten, corn, or wheat, those are all signs of stress on the body.

More dramatic cases of stress can lead to diseases; heart disease, blood sugar disorders from lack of a proper diet, due to stress not eating properly, or other life threatening conditions.

So what can be done for stress?

There are options: meditation, relaxation, a vacation, exercise or more direct therapies such as Emotional Freedom Technique (aka Tapping), journaling, and biofeedback.

What we see too much of when dealing with stress is resorting to drugs. Some drugs lead to addictions and side effects, and more problems rather than solutions.

Most drugs only mask the real problem.

So why does it seem younger

people are more active, vital, and interested in life? What happens to us?

As we go through life we compound stress, one stressor added to another. Dysfunctional family life, maybe the break up in high school, unplanned pregnancy, marriage, raising a family, divorce, the death of family and friends.

All those things build up stress and create reactions in the body.

No wonder when we look at our aging population we see numerous ailments: emotional distress, grief, depression, and in many cases just overwhelmed by life. Old life traumas build up over time and create havoc in our lives.

If you were to look back on your life, whether young or old, and identify the stressors in your life, that will give you a great measuring tool for your current state of health.

Take action to clear old and current stress so that you can live life to the fullest!

The anti-gravity treadmill can be used in rehab programs for patients with a wide range of ailments and diagnoses including arthritis, joint replacement, spinal surgery, hip fracture, neurological injury including stroke and TIA, general deconditioning due to illness, and can also be used to assist in fall prevention.

The NASA-inspired Differential Air Pressure Technology allows patients to stand and ambulate in an “unweighted” environment up to 80% of their own body weigh which can then decrease discomfort, increase confidence, and improve early mobility for faster safe recovery of the patient. In particular, the treadmill allows the therapist to monitor and analyze a patient’s gait pattern while minimizing impact from ground as they walk. The goal is to create a fall-safe walking environment that facilitates im-

provement in neuromuscular control, balance, and stability. Therapists can objectively evaluate progress and set therapeutic goals that translate to improvement in functional ability. As a result, patients are able to increase their independence with daily tasks and improve their overall fitness level to be able to return to their prior level of living.

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job.

If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

Home Health Care Definitions 24 HOUR CARE – Refers to “around the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above.

Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

Atomic Workers Benefit from The Energy Employees Occupational Nuclear Care Illness Compensation Program Act (EEOICPA), first implemented by the Partners U.S. Department of Labor (DOL) in July 2001, provides compensation and medical benefits to former Department of Energy (DOE) employees who became ill as a result of working in the atomic weapons industry.

The Program compensates those suffering from more than 20 types of cancers or other ailments including beryllium and asbestos-related ailments and chronic obstructive pulmonary disease (COPD). Uranium miners, millers, and ore transporters are also eligible for benefits if they have developed an illness as a result of toxic exposure at a facility covered under Section 5 of the Radiation Exposure Compensation Act (RECA).

Nuclear Care Partners (NCP), an authorized provider of skilled home health care under the DOL, doesn’t file claims but can help chronically ill individuals navigate this process. NCP works exclusively with former DOE workers who have been diagnosed with work-related illnesses and qualified for no cost in-home medical benefits under the DOL EEOICPA, DCMWC and FECA Programs. They provide superior, patient-centered care and their goal is to support individuals in maintaining independence while living in the comfort of their own homes.

Burt Stephens, an NCP patient, has had to deal with exposure to

radiation. His story is both informative and inspiring. Burt joined the Army when he was 16 and was paid $50 per month. For an extra $50 he joined the paratroopers and started jumping out of planes. He was one of 2000 men who parachuted into the Korea conflict. Later, Burt worked in the Air Force where he worked on nuclear missiles and finished his career at the Nevada Test Site where he was an engineer technician who wired the bombs that were detonated at the site.

Burt began services with Nuclear Care Partners a little over a year ago and states, “If it wasn’t for this group (NCP) saying you’ve got a lot of years ahead of you…. and showing me how to live again, I don’t know where I’d be”. 7 Best Kept Secrets For Families need to understand Financing their options when it comes to paying for senior care, but Home Care first, it’s important to understand the level of care you need.

Senior care in the home can range from a couple of hours of help per week to 24-hour skilled nursing care.

You may think that Medicare is the only option for long-term care, but Medicare only covers nursing and clinical care for acute conditions, usually following a stay in a hospital or rehabilitation, or skilled nursing facility.

Rates vary on how much care will cost. Generally, 24-hour care in a nursing home will be much more expensive than private in-home care. However those rates also vary depending on how many hours a caregiver’s services are needed. Your family has many options to assist you in paying for long-term care.

In addition to Medicare, there are Medicaid, Veterans Administration benefits, long-term care insurance and money from savings and other assets. Comfort Keepers can help identify potenial pay resources that might be available to you.

Medicaid – This program covers a limited amount of long-term, in-home care for those who qualify.

Medicaid will only pay for in-home care if provided by a Medicaid-certified home care agency, not by an independent, paid caregiver or family member. To find out about Medicaid

PACE – The relatively new Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive home and community care for frail elders who would otherwise require nursing home care. PACE is only available in certain areas and eligibility is restricted to low-income seniors, usually those eligible for both Medicare and Medicaid.

Veteran’s and Surviving Spouses Benefits – Veterans and the surviving spouses of veterans may be eligible for some in-home care assistance from the Department of Veterans Affairs, either through health benefits offered to veterans or in the form of a monthly cash benefit. If your loved one is a housebound, these benefits may be even higher. Comfort Keepers can provide help and information in applying for these programs.

Long-term Care Insurance – If your loved one has a long-term care insurance policy, it may include coverage for in-home care.

the amount you owe, based on loan payouts and interest on the loan becomes due when you or the last borrower, usually the last remaining spouse, dies, sells or permanently moves out of the home.

Uranium workers’ benefits – If you or a family member incurred a lung illness, kidney cancer or fibrosis silicosis within the last 60 years (even if the family member is deceased) and they worked as a uranium miner, uranium miller, worked at a processing plant or transported uranium between 1942 and 1971, they may qualify for $150,000 to $275,000 tax free. This compensation can include in-home care.

Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths.

Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.

DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

LONG-TERM CARE – Chronic, custodial care provided on an on-going basis
