“If It Sounds Too Good To Be True; Elder Abuse - Would You It Probably Is!” Recognize It If You Saw It? Helen is 83 years young and is proud to still be living on her own; so far she manages pretty well with most things except driving. She gave that up for fear of having an accident that would hurt someone else. Helen’s granddaughter Jesse lives a few blocks away and offered to take her places when she needed to go, it seemed like a good deal and worked well for a few months. Things changed when the granddaughter’s car broke down and she asked Helen if she could just borrow her car for a while; of course she would still help grandma whenever she needed a ride. Flash forward five weeks, granddaughter still has the car and has stopped calling Helen. Helen does not want to cause any problems in the family or make her granddaughter feel bad so she keeps quiet, but she is very upset about the situation. She has a Dr. Appointment so she calls Jesse to ask for a ride; Jesse is busy that day and suggests that grandma call her neighbor for a ride. Over the next few months the situation worsens when Jesse insists that Helen give her the car, since it is no good to Helen anyway. Helen’s health has deteriorated, she no longer gets out for groceries, medical appoints or her
weekly trips to the beauty shop. Her friends are worried but she has not shared her problem for fear of “looking like a foolish old woman, who never should have let this happen.” Scenarios like this are occurring everyday right under our noses. Each year hundreds of thousands of older persons are abused, exploited and neglected. Many victims are older, frail and vulnerable and depend on others to assist them. Abusers of older adults are both men and women, and may be family members, friends, or “trusted others.” Region 10 Community Living Services and our community partners are dedicated to curtailing Elder Abuse & Exploitation. The group will offer community education, and resources on this topic. As a Geriatric Care Manager, I have the honor of meeting so many interesting and different people! I always believe we can teach each other something new. One of my most recent lesson was on organization and humor. I was asked to help a couple with some organization of their legal and financial matters. This couple referred to their current filing system as their “Exit File”! This is a wonderful and to the point title for some very important documents. They were wise enough to have told their loved ones about what was in this file and where exactly it was located. This
is a very good first step in being organized while also injecting a flair of humor. So, what’s in YOUR Exit File? Here are some suggestions to what you will need to share with your loved ones. 1. 2. 3. 4.
Final arrangements/wishes Power of attorney papers Where the original Will is located What and where the financial documents are located 5. Safety Deposit information 6. Passwords for accounts/on-line account information

Has Anyone Seen My Exit File! continued while you are still healthy and well. This file will have some important details that they may need to attend to while you are incapacitated. For example, if you are unfortunately hospitalized in the middle of the night and your daughter/ Medical Power of Attorney is called and notified, she will need a file of your health history, a copy of the Power of Attorney, your Living will, phone numbers of neighbors that will take care of your cat, as well as phone number for your primary care physician, general medications (and Pharmacy choice) and allergies. A very thorough file would also include for your Power of Attorney, a copy of: your insurance card(s), Social Security Card, and Photo I.D. Having this information may seem

very over the top until your loved ones are put in the unfortunate position of finding this information without your help. Make it is easy for them because as you already know, they will be in a tough situation. Families are beginning to find it more normal to talk about these issues and to be proactive in their plan of care. Medicare 2017 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Traumas and events can create illness. How can that be? When we become distressed we can constrict good blood flow, we may choose to lie in bed, not eat nutritionally and the end result is poor health mentally as well as physically. What can we do about it? In the last edition I mentioned some general techniques such as meditation, yoga, exercise, as well as Tapping or EFT (Emotional Freedom Technique). This remarkable technique has been known to help people make major changes to their wellbeing, mentally, physically and emotionally. So why does this tapping thing work? It’s believed that rather than using needles like in acupuncture we are stimulating the same electromagnetic system (meridian system) that is used to get results with acupuncture. So YES it works amazingly well. The most amazing part is it is something YOU can do for yourself. Yes really. It’s easy and fast. You first identify what is causing you angst. Once you’ve identified the problem verbalize the problem out loud and tap on the edge of your hand with two fingers. Start your
statement with “Even though I have all this anger towards X. I totally and completely accept myself.” Say this several times while tapping the edge of your hand. Then say something similar to “All this anger, all this frustration, ….” And tap the following points at the beginning of the eyebrow, side of the eye, under the eye, under your nose, under your lip, near your collarbone point, your wrist, (see the picture.) You do not have to do it exact to get great results. This technique can be done by yourself. So why would you pay someone to go through the process with you? Because most of us cannot see the forest for the trees in our own lives. Many times we need someone else looking from the outside to help us get the most benefit from tapping. The Use and Mis-Use The long standing, overuse of of Antibiotics antibiotics has led to an alarming rate of prevalence of multidrug resistant bacteria, such as, (MRSA) Methylcillian Resistant Staph Aureus and (VRE) Vancomyacin Resistant Enterococcus. The Centers for Disease Control and Prevention (CDC) estimates that more than half of all antibiotics prescribed are not necessary. Antibiotics do not treat viruses, such as, colds and flu. Antibiotics treat bacterial infections. Antimicrobial stewardship programs promote the appropriate use of antibiotics, improves patient outcomes, reduces drug resistant organisms, and decreases the spread of infections caused by multidrug-resistant organisms. Antibiotic overuse contributes to the rapidly escalating problem of Clostridium difficile infection (C-DIFF) C.Difficile causes life-threatening diarrhea.. A 2015 CDC study found that C. difficile caused almost half a million infections among patients in the United
States in a single year. An estimated 15,000 deaths are directly attributable to C. difficile infections, making it a substantial cause of infectious disease death in the United States. Urinary Tract Infections are caused by bacteria and are treated with antibiotic medications. Asymptomatic bacteriuria is possibly cloudy or malodorous urine, without accompanying symptoms of a urinary tract infection such as frequent urination, painful urination and fever. Chronic asymptomatic bacteriuria occurs in as many as 50% of the population in long-term care. Bacteriuria is often misdiagnosed as an Urinary Tract Infection and inappropriately treated with antibiotics, putting the person at risk for developing multi-drug resistant organisms and/or C. difficile, among other unwanted side effects. The new Transitional Rehabilitation model is driving trends in post-acute care and providing a better recovery experience for patients.

Improve a Life At the new AdvantAge Health Resource Centers, an experienced case manager can perform a benefits checkup to match you with assistance programs. There, you may find out you are qualified for assistance through state or federal programs that can save you money on medications, health care, housing and other services. Assistance is often is accompanied by complicated forms and regulations. The AdvantAge Health Resource Center case manager can help decipher the form language, help you complete the forms and guide you to creating a plan for successful living. There are numerous challenges to successful aging. Many older adults often do not know where to turn when they experience a change to their normal routine like food insecurity, loss of physical mobility or suddenly needing to budget on a tight social security benefit. In late 2014, Volunteers of America established a toll free help line in an effort to provide quick answers and become a trusted source for a variety of needs for our area’s older adults. Today, the toll free number,

1-844-VOA-4YOU (1-844-8624968) is answered by the AdvantAge case manager. Callers can ask questions about health care including: where to go for rehab, home health, adult day care, memory care, assisted living or long term care. The caller can also inquire about sources for help with grocery and meals programs, durable medical equipment, independent housing, family housing or veterans housing assistance. Help is available to complete Medicaid and Medicare applications. The AdvantAge long term goal is to serve the need of any age group by providing a connection to a local service that can fill the need. Two walk-in locations will make it easy for customers to get help. The Montrose office is located inside the Proximity Space at 210 E. Colorado’s Independent Living Centers continued Independence is an Independent Living Center that provides solutions including paratransit, in-home services and environmental modifications including the installation of adaptations such as ramps and grab bars. ILCs also offer Transitional Placement services to facilitate the transition of one already living in a group, nursing or institutional setting back into an independent living situation in the community. This in turn saves residents, their families, and tax payers money otherwise spent on the high cost of institutional living. Benefits For Veterans And Have you or someone you love ever Family Members serve in the United States military? Are you the spouse widow, or widower of a former service member? If you’ve answered yes to either of these questions you may be eligible for benefits through the Department of Veterans Affairs. There are several programs through the VA that many veterans and family members are unaware of or have received incorrect information about. This brief article will provide a clear look into what veterans and their eligible family members have earned and are entitled to. First, let us define who is considered a veteran. If you or a loved one enlisted or was commissioned into any of the 5 recognized branches of the Armed Forces and fulfilled their obligation of service honorably then you or your loved one is a veteran. Now that we’ve established who a veteran is, what types of benefit programs are there for eligible veterans? Well there are three main categories; Veteran Heath care, Veterans Benefits, National Cemetery (burial). Veterans Health care is most commonly known, this is any care a veteran or family member receives at a VA Medical Center, eligibility for health care depends on income and service connection. If the veteran does not have a service connected rating of at least 0% for any one condition then the VA will base your enrollment into the VA

health care system on your annual income, unfortunately if your earned income is over the allowable limit you may be ineligible for VA health care. To receive a service connected rating the veteran will have to file a claim for service connected issues to obtain veterans benefits, you can do this by contacting you local County Veteran Services Officer or any other veteran service organization, (VFW, DAV, PVA, American Legion, e-benefits, etc). A claim for service connection is not based on income and is a tax free award for injuries and/or conditions that occurred in service, the rating may range from 0% to 100% depending on the severity for the issue/s. The veteran may apply for multiple issues that occurred in service to include PTSD, hearing loss, and exposure to chemicals or contaminated water. The third type of benefit is burial. If a veteran wished to be buried at a nationally accredited veteran’s cemetery, like the one we have on D road, then the burial is expense free. The family members may encounter additional costs from transportation or funeral home charges, the family member who pays for the additional charges may apply for a portion of reimbursement for those charges. Again, the family member will have to apply through the Dept. of Veterans Affairs with a service Continued on page 78

Follow these simple steps to find the correct equipment to keep you independent: Step 1: Start with your physician and discuss your concerns. They will diagnose your problem and may recommend visiting a specialized occupational therapist trained on adaptive mobility equipment called a Certified Driver Rehabilitation Specialist (CDRS).A CDRS evaluates your skills, restrictions and abilities to decide if you’ll be able to drive with vehicle modifications and adaptive equipment. Determining your unique needs allows for the correct equipment to be installed. Step 2: Find your nearest National Mobility Equipment Dealers Association (NMEDA) Certified Shop. This is very important. Modifying your vehicle will likely change safety features, functionality and structural makeup. Making sure you use a NMEDA shop ensures all safety and installation guidelines are followed and that
warranties will be upheld. ANY shop trying to install adaptive equipment without these certifications is in violation of National Transportation Regulations. This can put you at risk and liable for any injury caused to you or someone else.

Non Medical Home Care Definitions 24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. Medicaid Relative Care Program: If the parent, spouse, or other person you’re caring for is eligible for Medicaid, you can get paid as a relative caregiver. The Dept. of Human services case manager can qualify and set you up for this program to get paid up to 8 hours per week.

Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property. AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas

for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants. INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)
RESOURCES FOR AGING WELL
INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner. LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care. PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence.
PUBLIC HOUSING (PH) – Affordable apartments for low-income families, the elderly and persons with disabilities. REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients.
SENIOR HOUSING DEFINITIONS
INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/ location of apartment. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.
SHORT-TERM CARE – Acute care provided on a short term basis. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders. SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. ~

Crawford residents Ed & Pat Bennett had never been to the Delta Seniors Prom before but they love to dance so this was the year to check it out. The Seniors Prom was the brainchild of Leah Lewis of Delta and a group of high school students four years ago, the group discussed what a shame it was that the senior prom was so much work and enjoyed for such a short time. Leah wondered if there was a way for the Senior Citizens of the county to get in on the action and the “Delta Seniors Prom” came to life. The high school graduating class agreed to let the Seniors group use their decorations in the afternoon before the High School Prom in exchange for cleaning up the next day. The Seniors use the same theme but arrange for age appropriate music and refreshments. The Seniors Prom is made possible with the help of many community partners who want to ensure that our Seniors feel special. Guests all receive corsages
and a professional photo, this year the ADRC (Aging and Disability Resources for Colorado) hosted the event and nearly 30 area partners were there to set up, clean up, serve the seniors, pin corsages and dance with the guests. This year Ed & Pat were crowned

King & Queen, but Pat said “everyone was treated like royalty.” The Bennett’s live in Crawford, have been married for 33 years and between them they have 5 kids, 14 grandkids and 15 great grandkids. They love the outdoors, spend most of their time on horseback and enjoy square dancing. This event is a great partnership between local youth and senior citizens.
