Our guide is divided into 3 distinct sections and divided by color tabs.
COMMUNITY RESOURCES AND SERVICES
This section is alphabetical. You will find hundreds of different services and organizations that work with seniors and their families. This includes professional services, such as Adult Day, Care Management, Elder Law, Social Security, Senior Centers, etc...
2
HEALTH AT HOME –NON-MEDICAL, SKILLED HOME HEALTH, AND HOSPICE CARE
This section offers three different comparison grids; the first features Non-Medical and Personal Assistance at home, the second contains Skilled Home Healthcare, and the third grid showcases Hospice Services for end of life care. First familiarize yourself with the different services that are offered by each agency. You will find definitions about each service at the beginning of this section. Many options exist; we hope this will help you find a provider to meet your specific needs.
First you should familiarize yourself with the many options available and determine which one is best for your needs. You will find definitions of each type of housing throughout each section. Then turn to our easy to read comparison grids. The grids are divided into 4 categories, depending on the type of community: Independent Living and Retirement, Assisted Living, Memory Care and Skilled Nursing. The buildings are listed by city or county so you can zero in on the specific areas you are looking for.
You will also find many feature articles throughout the publication. These articles are written by local experts on a variety of topics that can aid you in making the right choices.

CARE MANAGEMENT
A care management agency is familiar with various fields of human services – social work, psychology, nursing, gerontology – and trained to assess, plan, coordinate, monitor and provide services for the elderly and their families. Advocacy for older adults is a primary function of the care manager. Agencies are typically either government based human services agencies or are members of national care management associations

Why Would I Consider a Trust For My Estate Planning In 2024?
In 2024, everyone needs an estate plan, regardless of their socio-economic circumstances.
A simple estate plan will typically contain at least three documents; a durable medical power of attorney and a durable financial power of attorney. These documents give the authority to a person of your choice, to speak on your behalf on your medical and financial issues if you’re not able to make necessary decisions due to health issues. The third document, a will, is a way to pass your property upon death to whom you wish.
A trust might be an option in your estate planning instead of or in addition to a will. Trusts are useful estate planning tools that can accomplish a variety of goals. They can help avoid probate, minimize taxes, and be used to give property to minor or disabled loved ones. Trusts can be created during a person’s lifetime (Living Trusts) or at the person’s death (Testamentary Trusts). Some different types of Trusts from both categories are:
Living Trust
A person can transfer their assets to a Living Trust and as a trustee continue using their assets as needed during their lifetime.
Tax Planning Trusts
Several different types of Living Trusts provide flexible alternatives for minimizing capital gains and es-
tate taxes, including the Charitable Remainder Trust, Irrevocable Life Insurance Trust, Qualified Personal Residence Trust, Grantor Retained Annuity Trust, and Grantor Retained Unitrust.
Testamentary Trusts
A person can create a Trust under a Will, called a Testamentary Trust, which does not take effect until they are deceased.
Disability Trusts (also known as Special Needs Trusts)
A Disability Trust is a type of Living Trust that allows a disabled person under the age of 65 to use his/her own assets, other than for food and shelter, for their special needs and keep public benefits, such as Medicaid and Supplemental Security Income (SSI).
3 reasons you may need a trust as part of your estate plan
n Are you concerned about protecting a gift from creditors or litigation?
n Does a loved one need nursing home care or Medicaid eligibility?
n Do you want to provide for children, grandchildren or charitable organizations?
The types of assets you own is another important consideration.
Three typical reasons you might need a trust:
1
A loved one cannot be trusted with a large financial gift or has special needs If you have concerns that a child does not have the financial skills to manage a financial gift, or the loved one receives government benefits.
2 You want to transfer complex assets in a thoughtful manner Trusts can be effective for keeping a vacation home or a closely held business in the family. For large charitable donations, a trust allows you to leave a vision for how you would like the gift used.

Limiting the potential for relationship-damaging fights is important When you have worked hard and been successful, a trust may be able to limit conflict and the legal fees associated with litigation.~

Many of us procrastinate when it comes to writing a will.
Many of us procrastinate when it comes to writing a will. Perhaps we don’t want to be reminded of our mortality. It may be a fear of the anticipated expense or a loss of control over property. But having a will extinguishes those fears. Want a few other reasons? A will contains your selection of who will receive what, when and how upon your death. You decide who will take care of your minor children. You facilitate the probate process (making it less expensive and quicker). You can disinherit individuals, make gifts or charitable donations and provide for your pets. You decide who administers your
estate to see that everything goes as you have directed. If no will exists, the state will determine how your property is divided.
This can be easily avoided with proper planning. By creating your will now, you can always add to the provisions or alter the document as your life evolves. It’s advisable to review your current will every five years to be sure that it’s up to date and still reflective of your future wishes.~

Medicare 2024 “In A Nutshell”
(Parts A, B & Medigap Plans)
Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”
Medicare Part A Hospital Insurance
Generally no cost (If you paid Medicare taxes while working)
$1,632 deductible (per benefit period)
It is possible to pay the $1632 deductible and other daily charges for multiple stays & extended care.
Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.
Providers are paid by Medicare assignment.
No coverage outside USA.
No out-of-pocket maximum.
Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.
Medicare Part B Medical Insurance
Most people pay monthly $174.90 (Premium could be higher based on income)
$240 annual deductible. (this can increase each year)
80/20 after deductible.
Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits
Most doctors accept Medicare.
No coverage outside the USA.
No out-of-pocket maximum.
Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.
A & B are designed for people over age 65 and those with certain disabilities
Supplemental Policy Plans A - N Medigap
Premium varies by company, age, location & other factors.
Guarantee Issue at initial enrollment (Other special enrollment periods may be available)
These plans generally pay the 20% not covered by Part B & other Medicare eligible benefits
When this coordinates with Medicare, many benefits are 100% covered
Can provide benefits outside USA
Plans are available through insurance companies.
Important to add Part D plan to cover prescription drugs & avoid penalties.
Must have Part A & B to enroll.
There is NO additional cost to use the services of a full time insurance professional. Please call us or a broker in your local area for additional assistance.

Medicare 2024 “In A Nutshell”
Med Advantage (Part C) with RX (Part D)
Medicare Advantage plans are governed by Medicare. They are only offered through insurance companies that have an annual contract with Medicare. They are NOT the same as Medigap policies. They must cover all Medicare approved services. Most Advantage plans include Part D prescription drug coverage.
Advantage Plans Part C INSTEAD of a Medigap Plan (A-N)
Premium varies by company and coverage. (Same premium for all ages).
Part B premium $174.70 is still paid (Premium can be higher depending on income)
There are many plan types available: PPO, HMO, HSA, etc.
Plans have co-pays and / or coinsurance for covered benefits & services.
Preventive care is covered 100%.
Plans have an annual out-of pocket maximum.
Plans may have additional services and discounts, including dental, vision, gym membership & more.
There is an Annual Election Period (AEP) which runs from October 15–December 7 of each year. This is the time to make changes in company or coverage.
Must have Part A & B to enroll.
Medicare Part D Drug Program
Premium varies by company and coverage. (Same premium for all ages).
Plans are generally a variation of the following:
Deductible (Phase I)
$0–$545 deductible
Initial Coverage (Phase II) co-pay coverage until the total cost of the drugs = $5,030
Coverage Gap / Doughnut Hole (Phase III)
The member now pays much more for covered drugs 25% generic / 25% brand. There is a drug manufacturer discount on certain drugs that can count toward the “True out-of-pocket” (TrOOP). You remain here until the $8,000 (TrOOP) has been met. It’s estimated you will pay about $3,300 of the $8,000.
Catastrophic Coverage (Phase IV)
$0 for covered drugs on the plan’s formulary for the rest of the calendar year Approved formulary insulins are no more than $35 per month.
Part D can be separate with Medigap & PFFS plans. There is a penalty for late enrollment into Part D.
Annual Election Period is Oct 15–Dec 7 each year
Must have Part A or B to enroll.

The Unseen Challenges of Caregiving
Being a caregiver is a role that millions undertake out of love, dedication and necessity. “Whether it’s an unpaid family caregiver or someone who has chosen a caregiving career, the role is critical to those who need help.”
Understanding some of the challenges caregivers face helps to appreciate their work.
Emotional Weight
- Caregivers often carry emotional burdens. They may experience feeling guilt, when they they’re not doing enough or when they take time for themselves. As a loved one’s—or a client’s—health fluctuates, the resulting emotional swings can cause stress and anxiety.
of their loved ones - compounded if the caregiver has to reduce hours or even quit their job to provide care.
Social Isolation - The time and energy demands often limit the caregiver’s ability to maintain social relationships, making them feel lonely and disconnected.
Juggling Act -
Many caregivers must balance the needs of the person they’re caring for, needs of the family while juggling employment.
Recognition and Support
- Caregiving is often not seen as “real work,” caregivers may struggle to find adequate resources, support and respite care.
Physical Toll - Physical demands of caregiving are often underestimated. Caregivers may do things like household tasks or keeping the care recipient comfortable. But if the care recipient has mobility issues, the caregiver may need to help them transfer from their bed to the toilet. Over time, these responsibilities can lead to physical strain and exhaustion.
Financial Strain - Often caregivers provide care without pay, they will incur expenses related to the care

Compassion Fatigue: What Family Caregivers Need to Know
Caring for others is often driven by motivations of love and understanding. It’s not uncommon for family caregivers to initially feel that their capacity for helping those they love is nearly limitless.
From taking care of finances and managing medications to preparing meals and cleaning, there’s certainly a sense of fulfillment that comes along with assisting a loved one. But many find themselves going through the motions of caring without taking time out to focus on their own needs – often to the point of physical and emotional exhaustion. As a result, family caregivers may feel something they’ve never experienced before: decreased empathy. This condition is known as compassion fatigue.
In decades past, compassion fatigue was seen primarily in healthcare professionals but as the country’s aging population continues to grow, there are many family caregivers who are left feeling much the same way.
In addition to the lower threshold of empathy, family caregivers experiencing compassion fatigue may feel the following:
n Exhaustion (physical/emotional)
n Feelings of dread or guilt
n Irritability, anxiety, or anger
n Difficulty sleeping
n Difficulty making decisions
n Feeling disconnected
n Trouble finding meaning in caregiving
n Self-isolation
The worst symptom of compassion fatigue is denial because it prohibits family caregivers from examining the feelings associated with their caregiving situation. And if ignored for an extended period of time, family caregivers may begin to feel resentment toward their loved one, ultimately leading to a communication breakdown and strained relationship.
Below are a few steps that family caregivers can take to combat compassion fatigue and get back to feeling positive about caring for their loved ones.
n Understand the common signs of compassion fatigue
n Practice a self-care of balanced diet, regular exercise, and consistent sleep.
n Find time to spend with friends and maintain social connections.
n Find a caregiver support program; community or online.
n Document your thoughts and feelings in a journal.
n Choose healthy activities during your downtime; a hobby, walking, meditating.
n Engage a counselor or therapist.

Hospice Care Definitions
vided at the individual’s residence, this can be their private home, a retirement community, an assisted living, or a skilled nursing facility.
RE-Respite Care – Provided when the family needs time to rest or go out of town for a short time. Ideally, this service is provided for five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.
GIP – General In-Patient Care –
Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.
CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs. Continuous care also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic
episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.
24/7 ADMISSIONS – Can a patient be admitted to the particular hospice agency 24/7? If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.
GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Each hospice agency will have a grief and bereavement support program. Many provide grief groups that meet and address issues of grief in a supportive group setting.
INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. Many Hospice agencies utilize social workers to help find resources for indigent families.
LICENSED MASSAGE THERAPY –Massage has become one of the

fastest growing trends in therapy for hospice patients. Used as a method for relaxation to alleviate stress, anxiety and pain with a light handson approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare; it is an added benefit that select hospice companies have decided to provide for their patients.
MUSIC
THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist is not just a music performer, but actually trained in counseling and music. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare; it is an added benefit that select hospice companies have decided to provide to their patients.
PAIN
PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their insurance.
PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.
PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. For some patients, contact with these gentle animals evokes memories of pets in their lives, past or present. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare, it is an added benefit that select hospice companies have decided to provide for their patients.
VA CONTRACT – If the patient is not a Medicare recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

Tips for Choosing a Senior Living Community
When shopping for an independent or assisted living community, there are several factors to consider to ensure a comfortable, safe, and enriching living environment.
Making the right choice involves evaluating location, services and amenities, quality of care, cost, safety, overall community atmosphere and personal rights.
Location - Proximity to family and friends can be important for maintaining social connections and receiving visitors. Additionally, consider the community’s access to medical facilities, shopping
centers, and other amenities. The surrounding neighborhood’s safety and the community’s accessibility to public transportation or other transit options are also significant factors to include.
Services and Amenities - For independent living, look for services that promote a maintenance-free lifestyle, such as housekeeping, laundry, and meal plans. Assisted living communities should provide personal care assistance, medication management, and emergency

response systems. Evaluate the recreational and social activities available, such as fitness classes, arts and crafts, cultural outings, and educational programs. Participation is optional based on personal preferences.
Quality of Care - Research the community’s staff-to-resident ratio, the qualifications and training of the caregivers, and the community’s approach to personalized care. Meet with the nursing director to ask questions about care levels and what services are included and what services are an additional cost.
in hallways and bathrooms. Inquire about the community’s emergency preparedness plans, main entryways security and how they handle medical emergencies.
Atmosphere - During visits, pay attention to the cleanliness and upkeep of the facilities. Observe how staff members interact with residents and whether they are friendly, respectful, and attentive. Look for a warm and welcoming environment where residents appear happy and engaged.
Costs - Independent and assisted living costs vary widely depending on location, services, and amenities. Ask for a detailed breakdown of all fees, including base rent, services, and any additional costs that may arise. Inquire about the financial policies, such as entrance fees, deposits, and refund policies. Inquire whether the community accepts long-term care insurance or Medicaid, which can help offset costs.
Safety - The community should be accessible for individuals with mobility issues, with features such as ramps, elevators, and grab bars
Personal privacy - Ask about the community’s policies on visiting hours, personal belongings, and the ability to personalize living spaces. Understand the extent to which residents can make decisions about their daily routines, social activities, and personal care.
By carefully evaluating these aspects, you can find a community that meets your needs and enhances your quality of life in your later years. ~

The Short on Rehab
hat is Post-Acute Rehabilitation? It is an inpatient center that provides therapies and/or Skilled Nursing.
Adults go there to recover from illness, injury, surgery, or other medical conditions following treatment in an Acute Care Hospital. Post-Acute Rehabilitation is a term used interchangeably with Short Term Rehab, Transitional Care Unit and Skilled Nursing Rehab Facility (SNF). These facilities offer similar services, such as: Physical Therapy, Occupational Therapy, Speech Therapy and Skilled Nursing Services. All centers have a Physician directing care and follow the same rules and regulations governed by Medicare. Not all Post-Acute Centers accept Medicaid and/or private insurance.
An individual’s Medicare benefits begin after a qualifying Acute Hospital stay if skilled services (Therapy and/or Nursing) are needed to regain physical health to go home safely. The first 20 days of PostAcute Rehab is paid for by Medicare without a co-payment. Starting on day 21 there is a co-payment, and, depending on the individual’s circumstance, the co-payment is that person’s responsibility and may be paid by secondary insurance, by Medicaid, or by themselves. It is important to remember that not all Post-Acute Rehabilitations Centers are licensed to take Medicaid. Bearing this in mind, when co-payments begin, they may have to transfer to another Rehab facility to continue
their skilled rehab services. (In extreme circumstances, Medicare stays can be as long as 100 days if the individual continues to qualify for skilled services.) Medicare Advantage plans must give pre-authorization for skilled services and will require re-authorization usually every 7 days throughout the Rehabilitation stay. One important consideration between traditional Medicare and Medicare Advantage Plans is the decision that a person no longer needs skilled services based on Medicare regulations and their Rehab benefit will end.Traditional Medicare, the Rehab facility and their therapy and nursing team makes the determination and Medicare Advantage Plans after consideration of chart review will determine when it is no longer necessary.
Questions to ask:
n Tour the facilities and take a family member and/or trusted friend with you. Look at the room, dining area, community rooms, and Therapy Facilities.

n Does the facility offer Physical, Occupational, and Speech Therapy? Do they have specialized equipment or services?
n Ask about dining services and whether the facility is able to meet your individualized preferences.
Ask to view a menu and you may even want to arrange to eat a meal.
n What is the facilities Medicare quality (CMS) star rating?
n Can the facility provide 1st medication dosing on site?
n Do they bill secondary insurance?
n What does the facility do to provide an individual plan of care that meets your discharge goals? ~
