This edition hosts over 10 editorials written by local Florida senior professionals on a myriad of ideas and concepts beneficial to both seniors and caregivers. THE ORAL HEALTH LINK: SLEEP AND OVERALL HEART HEALTH Oral appliance as an FDA approved alternative to the CPAP PEOPLE WITH CARDIOVASCULAR PROBLEMS, SUCH AS HIGH blood pressure, congestive heart failure, and stroke have a high prevalence of sleep apnea. The term “apnea” means “no breath”. For some people, the relaxation of the muscles during sleep allows the base of the tongue to fall backward against the throat, which can constrict the airway. Obstructive Sleep Apnea (OSA) is the complete stoppage of breathing for short intervals due to this constriction of the upperair-way. Thus, oxygen is cut off from the lungs resulting in a significant oxygen deficit in the blood stream. This decrease in available oxygen to the body triggers a “flight-or-flight” response dramatically increasing heart rate and blood pressure. These two actions over time cause hypertension and wear out the heart - leading to Congestive Heat failure and even death! Snoring is also produced by constriction of the upper airway and often occurs intermittently with apnea. Fortunately, a small custom fabricated dental appliance can be equally as effective as a CPAP and much less intrusive. These FDA approved

devices treat OSA by positioning the lower jaw forward keeping the airway open thus allowing the patient to breathe easily and continuously. Therefore, oral appliances are also very effective at reducing or even eliminating snoring. Dr. Rich Gilbert is well versed in all the different sleep apnea oral appliances available. FLORIDA HAS HAD VERY STRINGENT REQUIREMENTS for creating a valid Power of Attorney. Changes were enacted into law in 2011 adding many more requirements for a valid Florida Power of Attorney. The law change in 2011 also added a provision that recognized Powers of Attorney that were validly signed in other states. Under the new statute, Powers of Attorney that were validly prepared in another state can now be used in the state of Florida. This is good news for many of our residents coming to Florida. These new provisions have not eliminated all of the traps and difficulties associated with out of state Powers of Attorney and therefore, you should seek to have

your out of state Powers of Attorney reviewed by a Florida Elder Law Attorney. Most of the remaining traps and difficulties for out of state Powers of Attorney are connected with Florida homestead and Florida real estate. So if you are a new resident to the state of Florida and have Powers of Attorney done in another state and you now own Florida real estate, it is critically important that you have your documents updated to deal with Florida law. These minor revisions can save significant dollars should you become incapacitated at a later date. Hopefully, these remaining traps will continue to be eliminated as time goes by, but until the legislature takes action, it is easy for you to do so.

ONE IN FOUR AMERICANS WILL BECOME A CAREGIVER AT some time in their lives, and with the rising number of “baby boomers” reaching age 65; caring for a loved one can become a tedious and overwhelming task. The demands of caregiving can be extremely stressful, especially if you feel you have little control over the situation, or you are the only one that is handling the responsibility. Identifying community resources, knowing one’s limits and finding help from others are key elements to help minimize the stress. Two-thirds of caregivers are employed full or part-time. This is the case for Susan Smith, who has her own business. Susan’s mother used to help her at work, but as her memory started to fail, finding alternative solutions became a necessity. That is when Susan discovered “The Living Room,” an adult day services program offered by the Friendship Centers in Fort Myers. The Living Room is where Susan’s mother Joyce enjoys fun and companionship. Each day there are a variety of therapeutic activities including games, singing, movies, fitness and live entertainment. While Joyce is able to interact with her peers, receive nutritious lunches and snacks planned by a dietician; the

program gives Susan the opportunity to go to her business with an ease of mind. “The fact is she is with her peers and caregivers who understand my Mom’s unique needs,” Susan said. “I have a secure feeling that she is with ffriends who have trained hands versus unfamiliar h people coming into the p home.” h The Living Room provvides respite for careggivers who can leave the home and have some h ttime to take care of themselves. Having a break s ffrom the 24/7 responsibilities of caregiving is b vital. Adult day services v can c provide this much needed time during the week while their loved one enjoys the opportunity for peer socialization and stimulating activities that may help with independence; lessening the need for a nursing home. The Friendship Centers has been providing senior services in Lee County for 35 years. The center’s hours of operation are Monday through Friday from 7:30 a.m. until 4:00 p.m. and there is flexibility with client schedules. A participant can come one or five days a week on a part/full day basis all depending on the caregiver’s needs. Limited funding sources may be available to assist with payment options. Living Independently at Home with Hope PACE AS WE AGE, WE HOPE TO STAY IN THE PLACE WE CALL HOME. But with each passing year, it becomes more difficult to coordinate multiple doctor’s appointments, medical tests and prescriptions. We often struggle to find safe transportation after our driving skills decline. Even day-to-day activities like cooking, cleaning and bathing can be exhausting. Frequent trips to the emergency room can be scary and unpleasant. Our friends and family members worry about us. At some point, we agree that we need support to stay safely at home. Hope PACE (a Program of AllInclusive Care for the Elderly) combines the best elements of medical care, home care and personal care to help you meet the challenges of aging at home. This innovative program addresses your medical and psychological needs while also helping you with practical matters. A professional team of doctors, nurses, personal care aides, activity coordinators and drivers provide you with comprehensive and coordinated care. As a Hope PACE participant, you receive all of your medical care through the program. Your prescriptions are included, along with careful medication management. You can visit with medical specialists – even dentists. Those who need extra help can benefit from occupational, physical and speech therapy. Hope PACE provides additional care in your own home. If you need assistance with personal care, light

housekeeping or equipment and supplies to keep you safe, it’s all available through this program. Many of your social and medical services will take place at a Hope Care Center. While at a Center, you can enjoy nutritious, warm meals along with fun activities like Bingo, art therapy, games and contests. Musicians and pet visitors frequently drop by for your entertainment. You can enjoy light exercise with interactive Wii games, simple stretches and outdoor activities. Don’t worry about how to get there – the Hope PACE program even provides free transportation! While you’re enjoying your day at the Care Center, your caregiver can relax for a morning or afternoon of respite. Your loved ones will receive expert guidance and support so they can also improve their quality of life. To become a participant, you should be 55 or older with a health issue that requires ongoing management of symptoms or medications. You must live in Charlotte, Collier or Lee counties and be able to live safely at home with help. If you are enrolled in Medicaid - or are enrolled in both Medicaid and Medicare, the Hope PACE program is free of charge. For everyone else, there is a fee to participate. Medicare 2014 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a "Medigap Plan." Medicare Part A HOSPITAL INSURANCE

UNITED RHEUMOR ARTHRITIS SOCIETY (URAS) is a non-profit 501 (c) (3) organization that’s purpose is to raise awareness, educate, provide help for research and studies, and to be an advocate to those in need on a local and global level. We provide education by giving free lectures and slideshows in schools, libraries, and other public venues as well as utilizing social media channels and the corporation’s website to provide facts, statistics, and other related data on causes, current efforts and solutions to treat this debilitating disease. “Our programs include sending out volunteers to raise awareness about rheumatoid arthritis and juvenile rheumatoid arthritis, and to hold fundraising events in order to provide funds for research and new medications and assistance to those suffering from rheumatoid arthritis regardless of their race, ethnicity, or religion. The strategies of URAS are to initiate a tactical research agenda that achieves major benefits for people with RA and JRA, to reduce arthritis-related inconsistencies in health and health care, and to influence policies and strategies to

NEW BUSINESS TO PROVIDE IN-HOME CARE and support services for area community members. For local Ft. Myers businessman Brian Anderson, acting as a primary healthcare provider for his aging parents was an eye-opening experience. Based on this experience, and with encouragement from his wife, Nancy, a registered nurse, Brian began to consider how to bring an enhanced level of support to area residents in need of home care. Health at Home - Definitions

PRIVATE PAY HOME HEALTH DEFINITIONS COMPANION/HOMEMAKER SERVICES: Refers to services providedby a companion to be with your family member for a specified amount of time each day up to and including 24-hour supervision. The companions may provide assistance with routine household activities such as cook-ing, cleaning, laundry, errands and shopping. Many agencies provide transportation and supervision to physician’s offices, grocery and clothing shopping as well as recreational activities. Companions are able to perform a wide variety of other non medical tasks and activities that are designed to assist the elderly to remain at home as long as possible. By law homemakers and companions cannot provide hands on personal care such as assistance with bathing and bathroom personal care needs, nor dispense medications for their clients. HOME HEALTH AIDES/ASSISTANCE ADL’S: Refers to persons specifically trained to help individuals with their personal care needs and activities of daily living (ADL’s). These services include assistance with bathing, showering, shampooing, dressing etc. Other tasks may be assigned to aides per individual agency and family negotiations/contract. Home Health Aides are available for just a few hours per week up to 24-hour care on a temporary or long term basis. SKILLED NURSING: Refers to services performed only by a licensed Registered Nurse or Licensed Practical Nurse that is registered with the State Board of Nursing in their respective State. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician. PHYSICAL THERAPY (PT): Refers to assessing and treating large motor function, utilizing rehabilitative techniques of exercise, gait training, prosthetics, and heat to restore the client to his/ her highest functional level of strength, range of motion and mobility. OCCUPATIONAL THERAPY (OT): Assesses and treats small motor function. Utilizes rehabilitative techniques of exercise, splinting and assistive devices to increase the client’s ability to carry out normal day to day activity such as feeding, dressing, grooming, and performing household tasks. Guides the client through specialized regimes and encourages sensory and muscle return. Exercises and activities are used to increase upper body extremity function. SPEECH THERAPY (ST): Assesses and treats speech and swallowing disorders. Provides individually designed treatment programs to maximize communicative effectiveness for the client, such as stroke, head injury, laryngectomy, and voice disorders, as well as cognitive deficits. LIVE IN SERVICES: Refers to providing care 24 hours a day for as long as the client requires this supervision. These services may be contracted through Companion Agencies or through Individual contracts. TRANSPORTATION: Refers to assistance both locally and Nationwide. The Senior Driver: Understanding Changing Abilities CONSIDER THE ACT OF DRIVING: IN ESSENCE, DRIVING is the process of navigating a fast moving, one ton piece of steel though a very difficult and ever changing obstacle course. Under the best of conditions it is an extremely complex task, requiring peak performance of both cognitive and physical abilities. In addition, the life threatening dangers associated with driving make it a serious responsibility. At the Lee Memorial Hospital Trauma Center we frequently see drivers involved in crashes that forget - or ignore - signs that their skills have diminished. This is why we need to carefully monitor our abilities, know our limitations, and be acutely aware of when we might be exceeding them. Research suggests that as we grow older (over age 65) our likelihood of being involved in a crash increases. Therefore, paying attention to changes in our bodies as we age can go a long way to helping us avoid a crash and is crucial to being a good driver. What are some of these changes? Well, because our vision tends to weaken as we grow older we recover from glare more slowly, have difficultly focusing on objects around us, and take longer to transition between objects that are near and far. Seeing at night also becomes more difficult since we require more light to see clearly and our

pupils react more slowly. Detecting noises like sirens and horns can be more of a struggle for seniors. Also, loss of muscle mass and strength may impair the ability to turn the steering wheel or depress pedals. Stiffness in the neck can make it difficult to look over your shoulder when in reverse or changing lanes. Focusing on multiple tasks simultaneously can be challenging for older adults. As we age our reaction time increases and our concentration decreases; diminishing our ability anticipate the actions of other vehicles. No single issue automatically indicates that an individual should stop driving. But, seniors should acknowledge the need to pay extra attention to staying alert, driving safely, and seeking the advice of qualified professionals if they are concerned about their driving. Lee Memorial Hospital’s Health and Wellness Center offers a driving evaluation program. It provides recommendations and training to improve skills, and helps to identify any adaptations that may be needed on your vehicle so you can continue to drive safely. Can Living Alone Lead to Dementia? A RECENT STUDY CONDUCTED IN FINLAND FOUND THAT persons living alone, divorced or widowed are more likely to develop some form of dementia versus those who are married or living with a partner. The objective of the study was to find whether mid-life marital status is related to cognitive function later in life. Two thousand participants were studied during their midlife (around the age of 50) and followed-up approximately 20 years later. You may find the results interesting. • Those who had a partner in midlife (approximately 50 years of age) were less likely than all other marital categories (single, separated, or widowed) to show a form of dementia later in life (ages 65-79) • People living without a partner at mid-life had around two times the risk of developing dementia in later life compared with those living with a partner • Those widowed or divorced in mid-life and still so at follow-up in later life had three times the risk of dementia compared with married or couples living together. Several studies have been conducted over the years to associate lifestyle and cognitive status. It has been found that education, physical activity, mentally demanding work, higher managerial positions, and various intellectual and leisure activities have produced a more favorable cognitive status later in life. Other research has

ffound fo und that a rich social life f can fe reduce risks for dementia. Studies also concluded that couples living in a positive relationship lead to good health and longevity. And now, we have learned that those living with a partner (married or otherwise) from mid life and beyond are less likely to experience cognitive impairments. Could this study possibly lead to the conclusion that, as humans, we are meant to have companions? Senior Housing - Definitions

SENIOR HOUSING DEFINITIONS ACTIVITIES OF DAILY LIVING (ADLs) – Define the personal care services available in assisted living and include bathing, dressing, grooming, bathroom assistance, eating and ambulation. Medication Administration and transferring are not ADLs but are factors in determining a person’s level of functioning. ALZHEIMER’S / DEMENTIA – These communities are either licensed assisted living facilities that specialize in delivering care to victims of dementia or a unit of a skilled nursing facility. The same licensure rules apply. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Staff is usually specially trained for the special needs of this population. Amenities vary. Services include meal plans, cable television, local phone calls, housekeeping, laundry maintenance and transportation. Personal assistance with ADLs available. Rates also vary from all-inclusive to base rates with levels of care/point system for care. Payor sources usually accepted are private pay, some insurance plans, Medicaid Waiver (AL) Medicaid (SNF) and Medicare (SNF). ASSISTED LIVING – Florida providers must be licensed with a Standard Assisted Living Facility License. Two additional licenses: the Limited Nursing Service (LNS) and the Extended Congregate Care (ECC) allow providers to deliver some skilled nursing services and more personal care. Communities are typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Amenities vary. Services include meal plans, cable television, local phone, housekeeping, laundry, maintenance and transportation. Also provided is personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. Payor sources usually accepted are private pay, some insurance plans, and some communities are Medicaid Waiver certified. DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Regulations require a special license for those who wish to promote these services. Many providers will provide day/night services for a daily or weekly rate. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed ALFs are prohibited by law to promote these services without a license. ECC – “Extended congregate care” means acts beyond those authorized in subsection (17) that may be performed pursuant to part I of chapter 464 by persons licensed thereunder while carrying out their professional duties, and other supportive services which may be specified by rule. services is to enable residents to age in place in a residential environment despite mental or physical limitations that might otherwise disqualify them from residency in a facility licensed under this part. INDEPENDENT LIVING – Housing designed specifically for the elderly. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths. Services include meal plans, transportation, wellness clinics, housekeeping, laundry and maintenance. Amenities are usually abundant and may include formal libraries, computer stations, swimming pools, gardens, indoor/outdoor bar areas and other social gathering spaces. Rates are monthly and typically based on size/location of apartment. Payor sources accepted are private pay and some insurance plans. In Florida, personal care services and nursing care must be provided by a third party at an Independent Living facility. These communities are NOT licensed to provide personal/nursing care. LNS – “Limited nursing services” means acts that may be performed pursuant to part I of chapter 464 by persons licensed thereunder while carrying out their professional duties but limited to those acts which the department specifies by rule. Acts which may be specified by rule as allowable limited nursing services shall be for persons who meet the admission criteria established by the department for assisted living facilities and shall not be complex enough to require 24-hour nursing supervision and may include such services as the application and care of routine dressings, and care of casts, braces, and splints. MEDICAID WAIVER – Medicaid Waiver in an Assisted Living Facility differs from Medicaid in a Nursing Home or Long Term Care Facility. The amount that Medicaid Waiver pays to a facility is dependent upon a resident’s income. In most cases, this stipend along with a resident’s monthly income combine a total of approximately $1550.00 per month. Families may supplement the difference. RESPITE CARE – otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Typically found only in rental communities. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities. SKILLED NURSING CARE – This level of care is typically found in a nursing facility or a transitional care unit in a hospital. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Dr. Loren and Barbara Rosenbach were impressed with The Carlisle Naples the moment they arrived. “Thom Braun (Director of Marketing) took us on a tour,” said Mrs. Rosenbach. “He knew residents by name and everyone was smiling. You can’t fake that”. She was also pleased with The Carlisle’s level of comfort as well as the community’s layout. “Everything is under one roof,” she explained. “You don’t need a golf cart or car to get around.” The couple has lived at The Carlisle now for nearly a year and has taken advantage of the community’s wealth of services and amenities since their move. They spend their mornings reading the paper and having coffee in their residence before heading to the community’s onsite restaurant for a complimentary breakfast. “It’s all made to order and you can have anything you want,” she added. “On Saturdays there are bagels and lox and visitors make it a point to come for this specialty.”

The Rosenbachs say the best part of living at The Carlisle is the opportunity to be active. From educational pursuits to health and wellness programs, the community offers a wealth of activities to pique everyone’s interests. The Carlisle’s recent talent show, for example, provided the perfect venue for Mrs. Rosenbach to showcase her vocal skills. “We are part of a family here,” said Dr. Rosenbach. “Recently, we were away on a trip and when we got back, we felt we had truly come home. People genuinely missed us. ”

CHRIS FENWICK REINVENTS HIMSELF WE’VE ALL LOOKED AT PICTURES OF OURSELVES THAT WERE LESS than flattering but how many of us made serious lifestyle changes because of an old photo or in this case a video. Let me tell you the story of one man who did just that. Once upon a time in a small town in Paradise (somewhere in Florida, of course), there lived a somewhat pudgy but nice policeman who liked donuts, it is in the job description you

pounds, just from running, which as it turns out, really wasn’t his favorite thing. So, he added swimming and cycling to his regimen. He didn’t think he was particularly good at any of the three but he did like the variety, so in time, he thought he would try a triathlon. There are a variety of triathlon events with different distances and levels of difficulty. Chris explains he enjoys the sprint triathlons which

One day this very nice police officer we’ll call him Chris, because after all, that is his name, was in a training class watching a video. The trouble began when he asked a friend who the “fat slob” was – his words, not mine, because you see the man in the video, he soon found out, was the same man he saw in the mirror every day. Chris Fenwick remembers very clearly that day because that was the day he changed his life. “That was a hard reality to face. I was in my 40s and I was tremendously out of shape. My weight had gotten up to 220 and exercise was not even on my mind. But I knew I looked like a slob, so I decided to start running.” He couldn’t run very far at first but with some perseverance of his own and encouragement from other deputies, his weight quickly dropped to 190

tend to have a general range of a quarter mile swim, 10 mile bike ride and a 5k run. Chris trains with a triathlon club called Geared Up which has members ranging from 8 to 76 and recommends triathlons to everyone regardless of their fitness level. “There is camaraderie among our people who do everything from the Ironman down to the sprints. You don’t have to be a super jock; you just have to have the intestinal fortitude to push through. That’s all it is, anybody can do it. I recommend it just for the exercise.” Chris entered his first triathlon in 1996 when he was 53 and he’s been a man in motion ever since. In 2012, he completed five triathlons. One of Chris’ favorite things about training for triathlons is the ability to enjoy the great outdoors. “I do weight training in the gym but then I like to

get ooutt and be in nat nature.” re ” According to Chris, training outside is especially important when it comes to swimming. “When you train in a pool, you go 25 yards, hit a wall and push off. In open water, of course, there are no walls to push from, so you swim 25 yards, then

aanother 25 yards, tthen another, until yyou’ve completed tthe swim.” When Chris and hhis wife Mary Ann bbegan exploring tthe possibility of themrrelieving sselves of some of tthe responsibility of o home ownership aand moving to a ffull service retirem ment community, t the lure of open w water was not an in ssignificant part of tthe draw to Shell P Point. As it has t turned out, Shell P Point’s close proxi imity to Lover’s Key w where he and Mary A Ann swim in open w water and the five m mile route around tthe community he m mapped out for hhimself for cycling aand running couldnn’t have suited them better. And t we won’t even go w iinto all the other aadvantages of living at Shell Point because this story is about a man of determination and perseverance who, like the Energizer bunny, just keeps on going and in his wake, he leaves inspiration for anyone who might need it.

ime Sure Does Fly. When Springwood Court opened over 24 years ago, guess who worked in the kitchen and bought the initial groceries? Donald Marshall from Albany, New York! So you’re asking “what’s so special about that?” Donald is now a happy resident of nine months at Springwood Court! Donald enjoys the pleasant residents as well as the friendly employees. “There is a lot of great entertainment and everyone keeps very busy and active.” Donald went on to say “I get involved with many of the residents by helping with the activities, sometimes I feel like I’m still working and tell the girls that “I’m taking today off!” After he retired in 1989, Donald moved to Fort Myers and worked for the Lee County School System. He enjoyed working in the Food and Beverage department at Cypress Lake High School for 13 years. Some of his accomplishments are something to be proud of: • Honorary Member of the 3rd & 4th Degree, Knights of Columbus • United States Army- Medical Technician during peacetime in 1958. Serving in Wurgbury, Germany for 3 years. • He worked many years at the Veterans Administration in Albany, NY as a Medical Technician It’s hard to believe that Donald will be celebrating his 80th birthday this October! He keeps very fit and active and enjoys the company of his pet cat. Yes… you can bring your pet along!
