CHD Rehabilitation operates two specialist rehabilitation centres, Bagshot Park Neurorehabilitation Centre and Kingston Rehabilitation Centre.
Each of the CHD Rehabilitation Centres are overseen by skilled clinical multidisciplinary teams, who are supported by GP’s and specialist consultants. The services are specialised in delivering neurorehabilitation, complex nursing, tracheostomy care and long-term disability management and are committed to providing the very highest levels of rehabilitation through individually tailored goal driven treatment programmes. Together our teams work to provide excellent treatment and rehabilitation for those in our care.
CHD Rehabilitation centres are located in Kingston-upon-Thames and Bagshot, Surrey and are purpose built to provide exceptional care and rehabilitation in comfortable surroundings whilst maintaining personal wellbeing. Therapy facilities at CHD Rehabilitation include fully equipped gyms and treatment rooms, a hydrotherapy pool and occupational therapy kitchen.

Personalised rehabilitation pathways are delivered through our expert multidisciplinary teams consisting of psychologists, nurses, occupational therapists, physiotherapists, speech and language therapists and rehabilitation assistants.

PATIENT SUCCESS STORIES: MRS R
In October 2021, Mrs R had an unwitnessed fall at work. CT head showed a subarachnoid haemorrhage, but with no aneurysm identified it was unclear if this was of traumatic or spontaneous nature. Mrs R was initially admitted to her local NHS Hospital for a short period of acute stroke rehabilitation. During this time Mrs R was showing some signs of progress and so after 8 weeks was transferred to Kingston Rehabilitation Centre (KRC) for a further period of specialist neurorehabilitation input.
On admission to KRC, her baseline presentation included:
Full hoist for all transfers Dependent upon assistance of two for all activities of daily living Not orientated to person, place or time Evident cognitive impairment Doubly incontinent Global Aphasia Communicating at 1-to-2word level Left sided weakness
Prior to her accident, Mrs R was fully independent, working as a domestic assistant in a hospital and lived in a first floor flat with her partner.
A full neurological therapy assessment was completed by the KRC therapy team who then discussed their assessment findings with Mrs R and set goals for placement together with a view to returning home and maximising her independence.
Goals identified included being able to mobilise using and aid with assistance, completing personal care tasks with minimal assistance, to communicate in full sentences and utilising cognitive strategies to support attention and memory n function.
Mrs R’s rehabilitation programme included functional task practice, upper limb rehabilitation using a forced use approach and repetitive task training, and daily cognitive rehabilitation including use of the O-log, process training, strategy training and functional retraining with the occupational therapists. She also engaged in lower limb strengthening through targeted gym-based exercise and gait re-education with the physiotherapists, as well as aphasia therapy conversation practice and word copying interventions with the speech and language therapist to support communication.
Over just a 12-week period, Mrs R made significant functional improvements in all areas of her rehabilitation and overall, there was a significant reduction in care dependency and a significant improvement in functional independence and quality of life.
At the time of discharge, Mrs R was able to mobilise with assistance and complete the stairs which enabled her to return home to living in her first floor flat with her partner and was able to have all care needs met with assistance of one, without the need for any specialist equipment in the community.
OUTCOMES: MEASURING PROGRESS
During a patient’s rehabilitation stay we collect information about care needs, mobility, cognition, communication, and engagement in daily living tasks on admission. This is reviewed regularly and again on discharge to identify and monitor progress and change as a result of rehabilitation. One such tool that we use is the Functional Independence and Functional Assessment Measure (FIM+FIM) and the FIM+FAM chart below depicts the progress made by Mrs R during her neurorehabilitation placement at KRC.
