A YEAR OF HIGHS AND LOWS Our 35th year of service to the community was one filled with highs and lows, both literally and figuratively. San Francisco Department of Public Health reported that the number of San Franciscans living with HIV who were virally -suppressed was higher than the national average (68% compared to 49%), and that there was an all-time low in the number of new HIV diagnoses (223–a 16% decrease over the previous year). These data further demonstrate that getting to zero is possible, and underscore that we must continue to focus on those who are disproportionately affected by HIV. In 2017, we saw a significant increase in the number of clients we serve and an expansion in our scope of work. On account of a second successful year of operations at Strut, improved data systems, the expansion of services and hours at the 6th Street Harm Reduction Center, and increased community outreach, we can now say that we provide care to 25,000 clients per year. Our medical care now includes hepatitis C treatment and anal health care. Our integrated longstanding programs continue to meet the complex needs of our clients, and deeper collaborations with partners in San Francisco and Alameda counties are meeting growing needs in the region. Constant attacks on our communities and movement did not hinder our progress. Our recently-expanded policy team led the resistance to seemingly daily federal threats, protecting federal funding and the Affordable Care Act. As challenging as the political environment was this year, we seized the opportunity to increase our engagement with socially minded individuals and institutions. In 2017, we developed, expanded and started relationships with funders, engaged a record number of volunteers in our work, and sparked dialogue through our media efforts. This annual report tells the story of our 2017 by sharing programmatic, resource development and administrative accomplishments. Most importantly, this report illustrates that without each individual donor, partner, volunteer, and participant, none of this would have been possible. We hope our work continues to inspire you to stay engaged in our life-saving work as we continue to pursue the end of new HIV infections, AIDS-related deaths, and HIV stigma. OUR VISION San Francisco can be the first U.S. city to end the local HIV epidemic. Since 1982, we’ve worked to radically reduce new infections and help people living with HIV achieve viral suppression because we refuse to accept that HIV is inevitable. Our work is guided by three ambitious, interconnected goals.

OUR WORK Our work in 2017 faced challenges like none in recent history as chaos in Washington, D.C. threatened access to care for countless people living with or vulnerable to HIV. Data released by the San Francisco Department of Public Health underscored local progress toward ending the HIV epidemic, but also showed growing health disparities related to new HIV infections and access to HIV treatment. Disparities are especially pronounced within communities of color, people experiencing poverty, transgender and gender non-binary individuals, and people who use drugs and alcohol. In 2017, more than 25,000 people accessed our programs and services that are intentionally built to serve these priority communities. Our work addresses these concerns through five interconnected program areas.

SEXUAL HEALTH Providing care specifically designed for our communities is what our sexual health services are known for. In 2017, we served more people than ever. We introduced a rapid sexual health screening service in the Castro and doubled the hours people can enroll in PrEP at our Mid-Market location. Clients who test positive for HIV can now receive confirmatory test results in 30 minutes (instead of 3-5 days) and can access treatment through our rapid HIV treatment initiative. Our mobile testing efforts successfully engaged communities most impacted by health disparities through strategic partnerships and outreach. Our navigation and benefits team reduced barriers to PrEP through same-day access, insurance and benefits enrollment, and ongoing client support.

SUBSTANCE & BEHAVIORAL HEALTH Substance use is one of many factors impacting HIV risk, and people living with HIV who inject drugs have the lowest viral suppression rate of any transmission category. Our substance and behavioral health services are based on principles of harm reduction. We work in partnership with clients who use crystal meth, cocaine, heroin, alcohol and other drugs to help them create the goals that make sense in their lives. In 2017, we expanded our group and one-on-one counseling work to include more options for people who inject drugs, people experiencing homelessness, and people of color. We expanded hours and services available at our 6th Street Harm Reduction Center and piloted hepatitis C treatment.

ENGAGEMENT & RETENTION A critical component of our work is ensuring people living with HIV have access to health care services, housing, and public benefits. Through these services, we are helping people achieve viral suppression, ensuring better health outcomes, and preventing new transmissions. Our teams ensure access to housing through the provision of rental subsidies to people living with HIV and coordinate access to other public benefits including drug assistance programs, insurance, and food services. Peer education, linkage to HIV care, and ongoing support for newly diagnosed and out-of-care people living with HIV help increase or maintain viral suppression. Our two Centers of Excellence work with clients who have co-occurring HIV, mental health, and/or substance use diagnoses to provide intensive medical case management and other supportive services to improve viral suppression.

SOCIAL SUPPORT We cannot end the HIV epidemic in San Francisco until all communities have equal access to health care, housing, education, and social support. In San Francisco, African-American and Latino men have the highest rates of new HIV diagnoses and the lowest rates of viral suppression of any racial/ethnic group. Transgender and gender non-binary individuals experience high rates of HIV transmission and have lower rates of viral suppression, and 63 percent of people living with HIV are over age 50. Our teams provide social support, health education, housing advocacy, and case management services in safe and supportive spaces that center Black and Latinx communities, transgender and gender non-binary people, and long-term HIV survivors over age 50. Our teams work across program areas to ensure that clients have access to the entire spectrum of services offered by the foundation.

POLICY & PUBLIC EDUCATION We champion evidence-based HIV/AIDS policy at the local, state and federal levels, fighting every day to protect the human rights of people at risk and living with HIV. 2017 brought both unprecedented challenges and windows of opportunity. Attacks on federal funding for HIV programs and the Affordable Care Act threatened millions of lives. Locally, a special task force examined the feasibility of launching safer consumption services, or medically supervised spaces to use drugs safely, to prevent deadly overdoses and decrease the harms of public drug consumption. Our public education initiatives brought this news, and information about viral suppression, long-term survivors, HIV stigma, PrEP, and sexual health, to audiences across the globe.

OUR LOCATIONS We operate at three main sites in SoMa, Mid-Market/Tenderloin, and the Castro neighborhoods, in homeless encampments and in several pop-up sites around San Francisco.

STRUT Strut, our health and wellness center in the Castro, co-locates sexual health services, substance & behavioral health services, and social support programs. In 2017, our teams provided quicker access to HIV and STI testing, continued enrolling people on PrEP, and linked them to benefits programs and insurance plans to cover costs. There are more opportunities than ever to access substance & behavioral health services in the Castro, and more people of color are accessing services thanks to our social support programs and cultural events.

6TH STREET HARM REDUCTION CENTER At the 6th Street Harm Reduction Center in the SoMa/Tenderloin neighborhood, community members can participate in drop-in programming and groups; health assessments and counseling; HIV/HCV testing and navigation as well as HCV treatment; and syringe access and disposal programs. In 2017, we expanded drop-in center hours, added groups specifically for medication adherence, and increased overdose reversal education efforts. Our teams also led community syringe clean-ups every week.

1035 MARKET STREET Our headquarters at 1035 Market Street is home to several social support activities, sexual health services, substance and behavioral health programming, as well as our engagement & retention, operations, finance, marketing, and fundraising teams.

MOBILE TESTING Mobile HIV/STI testing lowers barriers to sexual healthcare by providing services outside traditional business hours in locations not served by brick-and-mortar clinics. Our mobile testing unit visits parks, bars, and community centers around San Francisco, and in 2017, our team was extremely successful reaching communities most impacted by HIV disparities. Within our network of sites and locations, our mobile testing crew maximizes its time to best serve the community. In a given month, the crew operates 15 or more shifts aboard the mobile testing unit, 16 shifts at the 6th Street Harm Reduction Center, 12 shifts at 1035 Market Street, and 8 shifts for PrEP enrollment.

HOMELESS ENCAMPMENT & VAN-BASED OUTREACH San Francisco’s housing crisis and the HIV epidemic are intertwined because stable housing leads to better health outcomes. People living with HIV who are experiencing homelessness have the lowest rate of viral suppression (33%) compared to the population of people living with HIV overall (68%). During homeless encampment outreach, our teams link people to health services, basic needs, assistance programs, and social support. In order to truly meet people where they are, we also provide syringe access and disposal services through van-based outreach in locations throughout San Francisco.

PARTNER SPOTLIGHT In 2017, in collaboration with San Francisco Community Health Center (formerly API Wellness) and East Bay AIDS Center, we fortified efforts to connect members of our communities who are at risk for or living with HIV in San Francisco and Alameda counties to existing HIV testing, PrEP and health navigation services. Through this initiative, we continue to support San Francisco Community Health Center’s weekly trans health clinic in the Tenderloin. We are grateful to our partners and to the California Department of Public Health for the $1 million Strategic HIV Prevention Project contract that makes this work possible.

AIDS/LIFECYCLE AIDS/LifeCycle is a seven-day, 545-mile bike ride from San Francisco to Los Angeles. In 2017, AIDS/LifeCycle participants and donors raised $15.1 million for the HIV-related services of San Francisco AIDS Foundation and the Los Angeles LGBT Center.

VOLUNTEERS Volunteerism is at the heart of San Francisco AIDS Foundation and our volunteers provides a solid backbone for many of our programs and services. In 2017, more than 2,000 volunteers contributed close to 100,000 hours of service to the organization. From stuffing envelopes to providing HIV test counseling to distributing the lifesaving overdose reversal drug Narcan to supporting our AIDS/LifeCycle riders, volunteers are powerfully and positively impacting our community every day.
