The Lieberman +30 Transitional Nursing Program works with your patients to manage: • Medical conditions • Medication regimens • Follow-up care with primary care physicians and specialists Lieberman +30 Program employs a nurse coaching model that helps patients develop self-management skills and to understand their symptoms.

The Lieberman +30 Program 1. Detailed Discharge Planning.

Transitional Care Nurse (TCN) meets with patients prior to discharge and develops personalized healthcare plan for 30 days following discharge.

2. Home Transition Assistance.

For safer transitions to home, TCN visits the home within 72 hours of discharge to work with home health care providers in the community.

3. Sustained Monitoring.

TCN will contact home health care providers periodically for four weeks following discharge to monitor progress.
