4 Adult Day Services (ADS)

ADS provides older adults with structured, stimulating programs in a supportive group setting, integrating memory support, physical activities and creative arts. Provides family caregivers a much needed respite. Private Pay, subsidies and some scholarships. ADS locations:

Adult Day Services, Evanston

Five-day-a-week program that considers the special needs of the individual. Programs are designed for persons who are frail, isolated or have memory loss. 847.492.1400

Adult Day Services, Deerfield

Seven-day-a-week, 23 hour-a-day program for persons with Alzheimer’s or related dementias. 847.374.0500

CJE at Sinai Downtown, Chicago For persons with early-stage memory loss. 847.492.1400

Benefits, Resources and Advocacy

Resource Specialists provide free information, referral and advocacy on benefits, services and community resources. SHIP-certified staff assess Social Security, Medicare, food stamps, Medicaid. Chicago north/ northwest suburbs. 773.508.1000

Center for Healthy Living (CHL)

Helps older adults remain independent, avoid disease and engage in society through health screenings, technology programs, lectures, activities and fitness programs. Encourages socialization, forging new relationships, and a healthy lifestyle. Fees can apply. 773.508.1073

Community-Based Care Transitions Program (CCTP) Federal Demonstration Project

Provides patients discharged from CJE partner hospitals with nursing coaches at no cost to help them manage their recovery and symptoms to avoid hospital readmissions. 773.508.1144

Home-Delivered Meals

Provides at-risk CCTP patients who have recently returned home from the hospital with care management and links to community resources. 773.508.1088

All meals are kosher (cRc approved). Five-course fresh or frozen meal. Includes a half pint of 2% milk, wheat bread and a piece of fresh fruit. Private Pay or subsidized for eligible seniors. 773.508.1000

Counseling

CJE’s licensed clinical social workers provide individual mental health counseling and facilitate support groups for older adults and family caregivers who are experiencing aging issues like depression and isolation. Available in Chicago, Deerfield and in one’s home, if homebound. Medicare, some insurance and private pay. Subsidized assistance available.

CJE delivers a delicious selection of freshly frozen cRc approved kosher entrees, breakfasts and soups directly to the door. Satisfying and nutritious, our chef-prepared meals are affordable and easy to serve. Kosher to Go is a meal provider for many area hospitals and organizations. Private Pay only.

Culture Bus™

CJE’s Culture Bus offers individuals with early-stage memory loss an opportunity to participate in day trips to various cultural sites and events in metropolitan Chicago and nearby suburbs. Gentle Home Services, Inc. (GHS)

We have a relationship with Gentle Home Services, and GHS provides CJE clients with hourly and live-in personal care services on a fee-for-service basis. 847.282.3883

Geriatric Care Management

Home Safety Assessments

CJE refers clients to a preferred provider for one-hour home safety assessments to identify possible safety hazards and discuss changes that may help people remain in their home longer. Private Pay. 773.508.1000

Leonard Schanfield Research Institute

Stimulates, fosters, and communicates new knowledge about aging. Its applied research has led to development of CJE programs, services and program models adopted by government and community agencies. 773.508.1076

Lieberman Center for Health and Rehabilitation

A free program for survivors and their families provides some financial assistance, socialization programs, support groups and assistance with reparations. Joint program of JF, JCFS and CJE.

240 private rooms in Skokie, IL. Skilled nursing and specialized care for Parkinson’s, cardiac disease, neurological and respiratory disorders. Specialized treatments: Dialysis, Infusion, Wound Care, Pain Management and Cancer Treatment. Planned activities, 3 kosher meals and snacks. Art therapists, social workers, wound specialist, dentist, rabbi/chaplain on staff. Private Pay. Medicare, Medicaid, some insurances.

144 private rooms for residents with complex medical needs who want personal attention and positive outcomes in a warm home-like setting. Respite care gives family caregivers a break. Palliative and hospice care also available. 847.929.3320

Haag Pavilion for Short-Term Rehabilitation

48 private rooms with physiatrist on staff. Rehabilitation therapy and medical care is available 7 days a week. Center for Heart Health provides cardiologist-guided after-hospital heart care. 847.929.3320

Linkages

Addresses the issues, needs and concerns of older adults and their family members with a disability. 773.508.1694

Managed Community Care Program (MCCP)

A program funded by the Illinois Department on Aging, provides home care for low-income elderly who otherwise would not be able to live independently. 773.508.1000

Parkinson’s Care

CJE provides persons with Parkinson’s disease research- and methods-based care. Most of our directcare staff at Weinberg Community, Adult Day Services, Lieberman Center and Haag Pavilion are trained in the management of Parkinson’s-related issues. 773.508.1000

12 Personal Care

For persons who are home from a hospital stay, recovering from an illness or frail and unable to live alone, CJE refers clients to Gentle Home Services (page 7), a privately owned personal care agency with whom we have a relationship. Government subsidized and private pay.

Religious Life

Offers spiritual and religious care and cultural programs in CJE’s residential settings, including pastoral care provided by a full time certified chaplain, Sabbath and holiday services, and cultural and educational opportunities. Religious Life Services works closely with community clergy to provide religious and spiritual care to our non-Jewish residents. 773.508.1695

Robineau Residence in Skokie has 24 recentlyrenovated furnished rooms with a private bedroom and bath. Kitchen and living room are shared among four residents. Housing Choice voucher accepted. 847.675.8580

Independent Subsidized Apartment Living

CJE SeniorLife operates five apartment buildings with subsidized rent. The residences, containing studio and one-bedroom units are: Farwell, Jarvis and Swartzberg House in Chicago and Krasnow Residence and Village Center in Skokie. CJE services are available to qualified residents. 773.508.1000

Levy House in Chicago is a renovated building in a desirable location near public transportation and the lake. Housing Choice voucher accepted. CJE services are available. 773.508.1000

Shalom Bus

Provides scheduled group trips to stores and other destinations in paratransit vehicles. Riders must reside in Evanston, Lincolnwood, Skokie or Chicago within strict boundaries. Affordable fees apply. 773.508.1000

Volunteer Services

Transportation Medicar

Non-emergency, medical transport to and from appointments: physicians, dentists, chemotherapy, dialysis, or physical therapy. Also provides rides to social events and adult day centers, airports, assisted living, synagogues, hospitals, nursing homes or rehab centers. Serves north side of Chicago and north and northwest suburbs. Private Pay. Medicaid (for healthcare visit only.)

16 Weinberg Community for Senior Living

Located in Deerfield, with dining and living space to promote socialization. Provides enriching programs, personal care services, laundry, fitness classes, health club, kosher dining and beauty shop. Respite care available. Private Pay and scholarships available. 847.374.0500

Gidwitz Place for Assisted Living

125 apartments located at the Weinberg Community for Senior Living in Deerfield for older adults who may need help with activities of daily living. Monthly rent includes three kosher meals/day, weekly housekeeping, some personal care, etc. Other services available for a fee.

Your Eldercare Consultants

Certified geriatric care managers, with advanced degrees in social work and special training in gerontology, provide advice, referrals and support for older adults and their families. They work to create a customized care plan and assist with future planning. They can evaluate a person’s ability to live independently and recommend short/long-term residential options. Services, on a fee-for-service basis, can include accompaniment to medical appointments, help with bill-paying and records, coordinating in-home services, emergencies and more. Friend Center for Memory Care

Licensed 40-room assisted-living special care for people with early- to mid-stage Alzheimer’s and related dementias. Monthly rental includes daily housekeeping, three meals plus snacks daily. 24-hour supervision and secured exit doors. 847.374.0500

In 2014, CJE provided almost 89,016 Home Delivered Meals to over 527 clients. This year, our Consumer Assistance specialists assisted 818 individuals in applying for public benefits, saving them an average annual total of $2,033 per household. 1,608 community members of all ages volunteered their time in 2014, for a total of 62,119 hours of volunteer service. 23,274 hours of Physical, Occupational and Speech Therapy were provided at Lieberman in 2014. This year, CJE’s Transportation Program provided over 48,817 trips for over 1,812 individuals.
