Diagnosing stroke

A

stroke is a medical event that occurs when the blood supply to part of the brain is interrupted or reduced. When this occurs, brain tissue is deprived of oxygen and nutrients. Since brain cells can begin to die in minutes, damage from stroke can occur almost immediately. Some strokes are minor, leaving the individual with minimal lasting effects. However, a complicated stroke can cause lasting brain damage, long-term disability or even death, indicates the U.S. Centers for Disease Control and Prevention. That is why it is crucial to recognize stroke risk factors and figure out how to head off an event, if possible. Certain tests are used to determine the liklihood that a stroke may occur. It is important to note that not all people who will experience a stroke have noticeable symptoms. Imaging tests often are ordered after a stroke occurs to determine which type of stroke an individual suffered. Here’s a deeper look at some diagnostic tests for stroke. • Carotid ultrasound: This ultrasound is painless and uses sound waves to create pictures of the carotid arteries, which are responsible for supplying oxygen-rich blood to the brain. The images will indicate if plaque has narrowed or blocked the carotid arteries, a risk factor for stroke. • Doppler ultrasound: This is a special test that shows the speed and direction of blood moving through carotid arteries and other blood vessels.

• Carotid angiography: The Center for Neuro Skills® says a carotid angiography employs a dye and special X-rays to show the insides of the carotid arteries, again helping to diagnose potential blockages. • Electrocardiogram: An electrocardiogram, also called an ECG or EKG, measures the heart’s electrical activity. The test determines if the heart is beating regularly and the strength and timing of electrical signals that pass through the heart. An EKG can detect atrial fibrillation, which is a risk factor for stroke. • Magnetic resonance imaging: An MRI uses radio waves and a magnetic field to create a detailed view of the brain. An MRI may be used to detect brain tissue damage by an ischemic stroke (result of a blockage) and brain hemorrhages. A doctor may use a dye injected into blood vessels to highlight blood flow. • Computed tomography: A CT or CAT scan creates crosssectional pictures of the brain, says the Mayo Clinic. A CT scan can show if a stroke has occurred and also identify if the stroke was ischemic or hemorrhagic (the result of bleeding). • Blood tests: Doctors say that blood tests can measure various things, including platelets in the blood. Abnormal platelet levels could be a sign of a bleeding disorder (not too much or too much clotting). A stroke is a serious condition that can cause lasting brain damage. Testing can identify risk factors and determine if a stroke already occurred.

Spring also marks the return of a familiar foe: allergies. Seasonal allergies can put a damper on even the most welcoming spring day. After all, a day soaking up the sun isn’t so relaxing when it’s also spent sneezing and confronting allergy symptoms like congestion, runny nose and watery eyes. Thankfully, it’s possible for seasonal allergy sufferers to have their spring and enjoy it, too. • Pay attention to seasonal allergy trackers. Seasonal allergies are now easier to track than ever. For example, Pollen.com is an easily accessible and free site that allows visitors to type in their ZIP codes and access daily allergy reports for their towns and cities. Visitors also can see five-day forecasts that can help them plan trips and other outdoor excursions. Weather.com also offers free allergy reports and forecasts. Fibromyalgia is a clinical syndrome that presents as widespread body pain. If left untreated, fibromyalgia is a potentially debilitating condition. Middle age women are especially vulnerable to fibromyalgia, and early diagnosis is key so that treatments can begin. Treatment is likely to be more effective if introduced early, according to a 2017 study in the journal Australian Prescriber. The Mayo Clinic says doctors attempting to diagnose pain used to check 18 specific points on the body to determine if pain was linked to fibromyalgia. However, newer guidelines from the American College of Rheumatology no longer require a tender point exam for diagnosis. Instead, the condition is considered present when widespread pain throughout the body lasts for at least three months. Newly diagnosed fibromyalgia patients may have many questions about the illness and their treatment options. Fibromyalgia symptoms wax and wane, so Johns Hopkins Medicine says treatment strategies continually need to evolve and are tailored specifically to each patient’s needs. However, treatment strategies tend to fall into four broad categories: education; symptom management; cognitive behavior therapy; and complementary approaches. Symptom management tends to include a variety of medications. The Mayo Clinic Says doctors will recommend pain relievers such as acetaminophen, ibuprofen and naproxen sodium, which are available over-the-counter.

Opioid pain relievers usually are not recommended because of side effects and risk for dependence. Antidepressants are the most frequently used and best studied drugs for the treatment of fibromyalgia, indicates Johns Hopkins. They are effective in treating pain, sleep disturbances, fatigue, and depression. Anti-seizure drugs often are useful to reduce certain types of pain. In addition to medications, complementary approaches include physical therapy, acupressure, massage, acupuncture, hypnosis, osteopathic manipulation, and other techniques. Cognitive behavior therapy also may help those with fibromyalgia manage their condition by applying relaxation therapy and cognitive pain management, among other techniques. Speaking with a mental health counselor also can give individuals additional tools for dealing with painful and stressful situations that arise due to fibromyalgia.

Morris Hospital Orthopedic Spine Surgeon Provides Help for Neck and Back Pain If you’re reading this article, there’s a good chance you’re going to experience neck or back pain at some point in your life. Some estimates say upwards of 85 percent of people will have neck or back pain in their lifetime, whether the result of activity, trauma or age-related conditions. The likelihood only increases after age 40. “As we get older, on the other side of 40 for sure, most of us get to a point where we realize our neck or back is Dr. Mir Ali not what it was when we were 16,” says Dr. Mir Ali, board-certified orthopedic spine surgeon with Morris Hospital Orthopedics & Sports Medicine who received advanced training in spine surgery through a fellowship with Rush University Medical Center, Midwest Orthopedics. “I think most of us learn to live with it and manage it as best we can.” But the question that next arises is: At what point do neck and back symptoms require help from a primary care doctor? Or even an orthopedic spine surgeon? When to Seek Help The first sign medical help is needed for neck or back pain can be as simple as having lingering or reoccurring pain significant enough that traditional treatments or overthe-counter medications provide little to no relief. Other indications of needing to see a doctor include having to miss work due to the pain or experiencing numbness and tingling. Dr. Ali says more concerning red flags start with shooting pain down the arms or legs, weakness in the upper or lower extremities, bowel or bladder problems, or having trouble walking. “The areas of the spine are very delicate areas,” he says. “The spine obviously controls your whole body, so there are things that really worry us as doctors. If someone has any of those symptoms, they need to see their primary care doctor right away, their spine doctor right away, or they can go to the emergency room.” Advancements in Treatment While Dr. Ali hopes people seek a doctor before severe symptoms set in, he is concerned that fears or misconceptions

may keep them away. For instance, while his specialty is called orthopedic spine surgery, the vast majority of his patients are at least initially treated without surgical intervention. Depending on the patient’s condition, treatments can range from physical therapy, to medications for short-term pain relief, to cortisone injections, and as a last resort, surgery. Dr. Ali believes his approach to care delivers two essential components to his patients – options and education – so they feel they have evaluated all possibilities before surgery. “At least in the short run, 75 to 90 percent of our patients are able to avoid surgery,” Dr. Ali says. “For someone to come in to visit a spine specialist, obviously the pain has gotten to a point where it’s affecting their daily lives and their quality of life, but we really work up from the least invasive approach, saving surgery as a last resort.” While the least invasive approaches can help in many situations, there are some conditions that cannot be truly resolved without surgical intervention. But even that prospect shouldn’t be feared, according to Dr. Ali. A procedure like anterior cervical fusion for arthritic/ herniated discs in the neck is typically a one-hour, outpatient surgery that provides patients immediate relief and results. Another procedure -- lumbar fusion for severe deterioration of lumbar discs -- has advanced from being a 6-hour surgery with a week-long hospital stay to now being a one-hour surgery with the possibility of going home the same day. “Technology is always improving,” Dr. Ali says. “There are surgeries we do on 80- to 90-year-olds which used to take six hours 10 years ago. The fact is, we can do that surgery in an hour and a half today with less anesthesia so the patient can get up and go home the same day or the next day.
