Choose Your Neighborhood THE ISLAND features a waterfront lifestyle, with many of the residences overlooking the marina, lagoon and the Caloosahatchee River. PALM ACRES is a well-established single-family home neighborhood adjacent to the entrance of The Island. Featuring architecturally-unique designs, each home offers deep water access just minutes to the Gulf of Mexico. THE WOODLANDS offers luxurious residences and additional amenities in four elegant atrium-style buildings centered around the two-story Woodlands Commons building. Choose Your Contract

A

LIFECARE CONTRACT

This contract includes independent living with all of the lifestyle features and amenities available, plus assisted living, memory care and skilled nursing services, all covered by the same monthly maintenance fee.

OPTION

B

LIFECARE + 75% REFUNDABLE OPTION

This contract includes everything in the A Lifecare Contract, plus a 75% refund of the entrance fee upon death or contract cancellation.* *To qualify for the A-75 contract option, if a couple, one person in couple must be 85 years of age or younger. If single, you must be 85 years of age or younger. Pricing available upon request.

MODIFIED LIFECARE CONTRACT

This contract features a 15% reduction applied to the A Lifecare Contract entrance fee, while providing independent living with all of the lifestyle features and amenities available, plus assisted living services, including memory care in assisted living. This contract also includes guaranteed access to Shell Point skilled nursing services at private pay rates.

C

FEE FOR SERVICE CONTRACT

This contract offers a 100% refund of the entrance fee upon death or contract cancellation. This contract does not include assisted living, memory care or skilled nursing services, but provides guaranteed access to Shell Point assisted living, memory care and skilled nursing at private pay rates.

Rate schedule is based on a ■ minimum age of 60.

Prices, features and services subject to change.

Monthly fee includes all utilities ■ except phone and internet.
